While operating the vehicle on both interstate highways and local roadways, an intermittent malfunction occurs involving the All-Wheel Drive (AWD) system. The instrument panel displays a warning message stating, “Service All-Wheel Drive System.” When this warning appears, the vehicle automatically disengages from AWD and reverts to Two-Wheel Drive (2WD). Once this occurs, the system does not a
... llow the driver to manually return the vehicle to AWD. This issue has occurred on more than three separate occasions. Notably, there have been instances where the warning indicator illuminated immediately upon starting the vehicle, prior to any extended operation. When the malfunction is present, the vehicle exhibits abnormal driving behavior, including a noticeable jerking or hesitation that feels consistent with a transmission-related issue. The failure is unpredictable in nature, with no clear pattern or triggering condition identified. This creates a significant safety concern, particularly when operating the vehicle at highway speeds or in conditions where AWD capability may be necessary for proper traction and control. An appointment has been scheduled with an authorized dealership for diagnostic evaluation; however, at the time of this report, the vehicle has not yet been inspected or repaired, and the issue remains unresolved.Read more

I am reporting a safety-related defect with a 2025 Hyundai Palisade SEL. Within days of purchase (December 14, 2024), I began experiencing abnormal noise and vibration from the rear of the vehicle. The issue consistently occurs at highway speeds (45–55 mph) and over bumps. The problem has been present since the first week of ownership and has worsened over time. Multiple inspections confirmed
... the issue: •Selling dealership dismissed it as “normal tire noise” without proper diagnosis •Second Hyundai dealership confirmed severe abnormal wear (cupping) on all four tires •Independent inspection (Firestone) confirmed uneven tire wear, alignment out of specification, and ongoing vibration Despite early reporting, Hyundai Motor America has refused to provide a root cause diagnosis or take responsibility. They denied tire replacement, attributing the issue to tire wear. This explanation is not valid. Tires do not develop severe cupping shortly after purchase without an underlying defect such as improper alignment or suspension issues. This condition creates a safety risk: •vibration reduces vehicle stability at highway speeds •uneven wear increases risk of tire failure The issue affects all four tires and was present almost immediately after purchase, indicating a likely manufacturing or setup defect. The problem has been confirmed by multiple service providers but remains unresolved. The vehicle is not safe to operate under normal driving conditions. I request immediate investigation into a potential manufacturing defect affecting vehicle safety. Hyundai has been aware of this issue since the first week of ownership and failed to take corrective action. Despite multiple documented inspections confirming abnormal tire wear and vibration, no root cause diagnosis or repair has been provided. This raises serious concerns about a potential systemic defect affecting vehicle safety.Read more

After the car dealership completed the first tire rotations, at 30,000 miles I rotated the tires to find severe inner tread wear on the tires where the inside of the tire was wore to the sidewall while outside portion was intact. I took the vehicle to an independent shop and the right rear suspension/tire was out of specifications on camber causing said tire wear. Toyota did not manufacture the ca
... r so that camber could be adjusted. An aftermarket kit was purchased to be able to compensate for the lack of adjustment. This is a safety concern as if a person does not rotate every 5,000 miles it will lead to tire failure at highway speeds. This is a common problem while researching forums. I have contacted the manufacturer and was given a generic response. There are reports with documentation with tire failure as early as 11,000 miles. Toyota needs to be held accountable and make a remedy for this instead of offering consumers a run around and blame hitting pot holes or on tire manufacturers, when it is clearly a suspension limitation. Thank you.Read more

This vehicle safety complaint is regarding my 2019 Toyota Highlander XLE, VIN [XXX] . A Toyota-authorized dealership, Sansone Toyota in Avenel, New Jersey, documented that the front driver axle is prone to freezing in the differential. This is a serious safety concern because if the axle seizes while the vehicle is in motion, it could cause sudden loss of control. This vehicle is used regularly a
... s our family vehicle, including to transport our family of 6 with our young children. I am extremely concerned about continued operation of the vehicle while this documented condition remains unresolved. In addition to this safety-related axle issue, this vehicle has a broader history of serious mechanical problems. On May 13, 2022, after a recent service visit at DCH Brunswick Toyota, I reported that when I pressed the gas the vehicle was making an unusual noise like an old car, that the car delayed to speed when I pressed the gas, that I had to press the gas all the way in order for the car to move faster, and that it was not like this prior to service. At approximately 25,000 miles, I also asked that the brakes be checked because when we drove the vehicle and used the brakes, the car made a startling sound. In 2023, I also reported hard/heavy steering and jerking while driving. I reported transmission-related abnormal noises and drivability concerns while the vehicle was still within Toyota’s powertrain warranty period, but the condition was not properly diagnosed at that time. The transmission has since been confirmed by authorized Toyota dealerships to require full replacement. The vehicle also has a documented engine oil leak, leaking shock absorber, and prior recall history involving a stalling condition. I've filed this complaint with NHTSA due to the axle-related safety risk. Toyota is now on written notice of that condition. Q1: The steering and front suspension system malfunctioned. The vehicle experiences a severe “death wobble,” violent and uncontrolled shaking/oscillation of the steering wheel and front end after hitting bumps or uneven pavement, especially at higher speeds. The suspected affected components may include the steering linkage, track bar, steering damper/stabilizer, ball joints, tie rods, control
... arms, bushings, alignment/caster, or other front suspension/steering components. Q2: The defect causes violent steering wheel and front-end shaking while driving, creating a loss of stable steering control. When the wobble occurs, the vehicle becomes difficult to control and I have to slow down significantly to regain control. This creates a serious risk of a crash, especially at highway speeds, in traffic, on curves, or if another driver is nearby. The first time it happened I was on the highway near two tractor trailers who recognized the issue and slow down for me too. The condition puts me, my passengers, other motorists, pedestrians, and anyone near the roadway at risk because the vehicle can suddenly become unstable after normal road bumps or pavement irregularities. Q3: The problem has been brought to an local mechanic 3 times for repair or diagnosis, but the issue has not been resolved. The vehicle continues to experience the same severe steering/front-end wobble after those repair attempts. As of this complaint, the issue has not yet been formally diagnosed or resolved by an authorized Jeep dealer. I am filing this complaint because the condition remains unresolved and I believe it is a serious safety defect. Q4: The vehicle has been inspected and repaired by an local mechanic on 3 occasions related to this issue. The Healy dealership fixed the last recall right before this. I called them on the way home from the dealership when it first happened. Q4: There were no warning lamps or dashboard message before the wobble occurred. Became more aggressive in Jan 2026.Read more

My 2023 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting both Uconnect 5 (12” screen) and earlier Uconnect 4 units. These issues are widespread in 2021–2024 Ram TRX and Ram 1500 models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses
... all functions controlled through the display, including HVAC/defrost, backup camera, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including the inability to properly use the various vehicle drive-modes as driving conditions dictate. Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

While driving, the steering begins to bind and resist when turning, especially to the left. The wheel will “catch” and then suddenly release, causing a jerking motion and loss of smooth steering control. This creates a serious safety concern because the steering does not respond consistently. During the same event, the air suspension system lowered the vehicle into a safety mode without drive
... r input. The suspension would not raise back up, and the dashboard displayed suspension warnings. The vehicle lowered itself immediately after the steering bind occurred. These symptoms match the same failure pattern described in the recall for 2014–2017 Range Rover and Range Rover Sport models involving front suspension/steering knuckle issues and steering safety concerns. Even though my 2013 model year is not included in the recall, it is experiencing the same steering binding and suspension safety‑mode behavior. This issue creates a risk of loss of steering control and unsafe vehicle handling. I am requesting that NHTSA investigate whether the recall should be expanded to include the 2013 model year.Read more

I am experiencing multiple unresolved defects with my 2026 Chevrolet Equinox affecting drivability, braking behavior, and vehicle structure/fit and finish. The primary issue is a persistent vibration, shimmy, or wobble during moderate acceleration from a stop. This condition is felt through the steering wheel, seat, and floor, and occurs consistently during low‑speed launch and normal accelerati
... on. In addition, the vehicle produces a snapping or grunting noise during hard braking, as well as rough and loud movement noises from underneath the vehicle during normal operation, which raises concerns about braking and drivetrain integrity. There are also body and interior fitment defects. The front passenger door does not sit flush with the rear door panel and appears slightly open when closed. The dealership service manager stated that correcting this condition would require removing and pulling the rear door panel outward so both doors align. The glove box door also does not sit flush when closed, and the service manager advised that replacement might make the condition worse. These concerns were reported to an authorized Chevrolet dealer. The dealer confirmed that General Motors has acknowledged the acceleration vibration condition in a Preliminary Information service bulletin (PIT6466) affecting 2025–2026 Equinox vehicles and advised that GM engineering is investigating the issue and that no repair is currently available. The combination of drivability issues, abnormal noises during braking, underbody movement sensations, and body misalignment affects the use, value, and safety perception of the vehicle. The vehicle has very low mileage, and several of these issues have been present since early ownership. I am submitting this complaint to document these conditions and request that NHTSA monitor potential powertrain, braking, suspension, and vehicle structure concerns if a defect trend exists. The air suspension / Quadra-Lift system on my 2019 Jeep Grand Cherokee Overland with approximately 87,000 miles has failed and is available for inspection upon request. The vehicle is becoming stuck in Off Road 1 ride height and will not reliably return to normal height. When I am able to get it to lower, it raises itself back into Off Road 1 again without command. I also observed that one wheel/c
... orner of the vehicle was sitting higher than the others, indicating the suspension may be uneven or malfunctioning. This creates a serious safety risk because the vehicle height changes unexpectedly and remains in an elevated off-road mode not intended for normal road use. It can affect handling, steering stability, braking, visibility, headlight aim, tire wear, and rollover risk, especially at highway speeds or during emergency maneuvers. Uneven ride height from one side to another further increases the risk of loss of control. Prior to complete failure, symptoms included abnormal vehicle stance and the suspension not responding normally to ride height changes. Warning messages/lights: UNKNOWN at time of filing. The problem has not yet been confirmed by a dealer or independent repair facility, but online research shows many owners reporting similar Quadra-Lift air suspension failures around similar mileage. This appears to be a common premature failure involving air struts, sensors, compressor, or related components. The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives. I am filing this complaint because this suspension defect impacts safe operation of the vehicle and may affect many owners.Read more

Summary: I am reporting two significant defects on my 2025 Nissan Sentra that pose safety risks. 1. Suspension/Steering: During low-speed maneuvers (approx. 20mph) such as navigating roundabouts, there is a persistent mechanical creaking/squealing noise from the front passenger side. The dealership technician verified the noise but provided a physically impossible diagnosis, claiming "tires were
... slipping," despite the vehicle being brand new with tires in perfect condition. This suggests a defect in the control arm bushings or strut mounts which affects steering predictability. 2. Power Window/Electrical: The driver-side window produces a loud, piercing screech when operated. More critically, the window switch has experienced intermittent functional failure where it becomes unresponsive. This is a safety hazard as it could prevent emergency egress or cabin ventilation. Manufacturer Response: > Both issues are documented and have been reported to Nissan Consumer Affairs (Case #56515863). However, the manufacturer stated they have "no solution" and directed me to argue with the dealership. No technical specialist has been assigned to inspect these safety-related mechanical and electrical failures. The vehicle is available for inspection.Read more

These components are part of the load-bearing suspension system and may pose a safety risk if failure occurs. This level of corrosion is unusual given the vehicle has approximately 27,000 miles and has been operated in a region without exposure to road salt or severe winter conditions. Independent mechanics who inspected the vehicle also expressed concern about the severity of the corrosion. The problem was confirmed by both a dealership and an independent repair facility. The vehicle was not inspected directly by the manufacturer. No warning lights or messages were present prior to discovery of the issue.
