While driving my vehicle at normal highway and city speeds, I noticed a serious steering issue where the steering wheel feels excessively loose and unresponsive. The vehicle does not track straight and constantly drifts left and right, requiring continuous steering corrections to keep it within the lane. The steering feels vague and unstable, as if there is excessive play in the steering system. A
... t times, small steering inputs result in delayed or inconsistent vehicle response, making it difficult to maintain a straight path. This issue occurs on smooth, dry roads and is not related to road conditions, wind, or tire pressure. This condition creates a significant safety concern, especially at higher speeds, as it feels like I could lose control of the vehicle or unintentionally drift into another lane or off the road. The problem has been persistent and appears to be worsening over time.Read more

The defect involves the front suspension and/or steering system of a new 2026 Tesla Model Y Long Range AWD. The vehicle is available for inspection upon request. Symptoms—persistent front-end clunking/thunking (primarily driver-side), steering instability, and vibration—began immediately upon delivery in late 2025, with Tesla notified same afternoon as delivery. The vibration now begins aroun
... d 60 mph and intensifies with speed. Steering feel is excessively vague on-center (requiring constant correction to maintain lane position) yet overly twitchy and sensitive to inputs, creating inconsistent and unpredictable handling. The condition has progressively worsened and now renders the vehicle unsafe at highway speeds due to risk of loss of control. No warning lamps, messages, or other symptoms have appeared prior to or since onset. Tesla service centers have had multiple opportunities to evaluate the vehicle under warranty. The problem has not been confirmed objectively by Tesla; during at least one evaluation, a technician drove only at speeds below complaint threshold (despite requests to test at the speeds where symptoms occur), stated he did not feel the issue, and performed no further diagnosis. Multiple appointments were canceled or rescheduled by Tesla (including one after I arrived). The vehicle was repeatedly returned without documented objective testing or repair addressing the complaint. During one visit, the Tesla app indicated an active “visual quality check” while the vehicle remained parked outside and not being worked on for an extended period (documented). To rule out tires, I installed a brand-new set; the vibration worsened afterward. Safety risk: Unresolved steering/suspension instability increases the risk of loss of vehicle control at highway speeds, endangering occupants and other road users. A critical front suspension component failed on my 2023 Tesla Model Y without any accident, misuse, or external damage. While backing out of my garage at low speed, a loud “BAM” occurred — the front lateral link bolt fell off, detaching the lateral link and causing the front body to drop onto the tire. My child was inside the vehicle. Tesla-certified technicians inspected the vehicle and se
... rvice records confirm the lateral link bolt was loose and missing, directly causing the failure. Multiple suspension components were replaced under warranty, confirming a manufacturing defect, not customer-caused damage. This is a serious safety-critical failure. At driving or highway speed, it could have caused loss of steering, severe injury, or death. The day before, my spouse drove the vehicle on the highway with our younger child — by luck, a catastrophic event did not occur. Before and after the wheel detached, the Tesla monitor did not display any warnings. Even after the lateral link and wheel partially detached, the vehicle remained in driving mode and ready to operate, despite a wheel being unsecured — a basic condition that should trigger an immediate alert. This demonstrates a critical failure in Tesla’s safety systems. There were no recalls listed under my VIN. However, NHTSA records show similar failures, with recall notifications issued under NHTSA Recall 21V-835 in 2021 for 2021 and earlier Tesla Model Y and Model 3 vehicles. The same failure in my 2023 Model Y at 23,641 miles shows the defect was not fully corrected and continues to pose a public safety risk. I am reporting this to NHTSA due to the extreme safety risk and request a full investigation.Read more

While driving my Tesla, the front suspension abruptly failed. Two bolts from a critical suspension/control arm component fell out while the car was in normal motion. There was no impact, no debris, no road hazard, and no prior service that would explain missing bolts. The vehicle suddenly dropped, and the chassis collapsed onto the wheel, causing grinding and severe loss of drivability. This failu
... re occurred without warning. Tesla Service inspected the vehicle and confirmed the bolts were missing but could not explain how they came loose or fell out. The bolts showed no damage, and there was no undercarriage damage, indicating a structural or manufacturing defect. This was a life-threatening incident. This type of suspension/separation issue appears to be a known problem with Tesla vehicles. I request that NHTSA review this failure as it poses a significant risk to vehicle occupants and the public.Read more

The car has been taken care of in a manner that exceeds the manual recommendations including always flushing the underside of the chassis during washes; especially the winter season. The car was taken to a dealer to have the intake manifold gasket looked at. The dealer took the opportunity to raise the car in the air, conduct an inspection and reported that the front carriage assembly is rusting away and considered dangerous. The Service Manager indicated that the model years 2010 to 2014 exhibit a high-degree of corrosion to the supportive member which holds the engine and transmission in place and if it fails could cause an extremely dangerous situation. What if any information has NHTSA compiled regarding these issues and Subaru's use of an under-designed, defectively manufactured part that could potentially cause a serious accident if the support structure collapsed? And if the NHTSA has conducted any research into this issue is there any option for a consumer to recoup or require Subaru to cover the cost of replacement which according to two Subaru Dealers is approximately $4,200. I am writing to formally report an incident involving the service department at Lakeland Hyundai (1500 W Memorial Blvd, Lakeland, FL 33815). My 2007 Hyundai Santa Fe was brought in on 12/31/25 for an active engine recall involving the Cylinder Head Cover Oil Leak Repair (Recall Code 976 / Campaign 24-01-026H). The vehicle was driven into the dealership in proper working condition. I was later inf
... ormed that allegedly stripped bolts for the cover would require an out-of-pocket cost of $483.36, despite this being a recall repair. There was also a required Upper Timing Chain Guide repair quoted at $58.38 to complete the recall work. When I was called in on 01/12/26 to pick up the vehicle for completed work, I was charged $29.99 for a Multi-Point Inspection related to the recall, yet no documentation or billing was provided for the timing chain guide repair I was told was completed. Upon attempting to leave the lot, the check engine light was on and the vehicle immediately began shaking violently and producing a loud noise. The service representative could not identify the cause and later advised there was nothing further they could do, citing a P0300 code, and instructed me to return the loaner and retrieve my now-undrivable vehicle. My vehicle remains at Lakeland Hyundai and is undrivable due to issues that arose immediately following recall service. I respectfully request guidance and resolution regarding responsibility for the damage, loaner coverage, and loss of transportation.Read more

The contact owns a 2013 Ford Explorer. The contact stated that the vehicle was taken to the dealer for a recall repair related to a possible rear suspension toe link fracture. The contact stated that while his son was driving 55 MPH, a month and half after the recall repair, the rear driver's side tire had a blowout, causing the vehicle to swerve and to drift into another lane. The contact stated
... that there was no oncoming traffic during the failure. The driver was able to pull over to the right side of the road. The contact then met his son on the side of the road, and the contact replaced the rear driver's side tire with the spare tire. The contact then drove to the residence and inspected the tire. The contact noticed that the inner side edge of both rear tires were prematurely worn. The contact related the failure to the recall repair. The dealer was notified of the failure, but no assistance was provided. The vehicle was not diagnosed or repaired. Landmark Ford, Inc. (2401 Prairie Crossing Dr, Springfield, IL 62711); was contacted and the contact was informed that the previous dealer did not complete the alignment during the recall repair, causing the premature wear of the tires. All four tires on my 2025 Toyota Tundra experienced severe and abnormal outside edge wear at approximately 13,000 miles. An authorized Toyota dealership inspected the vehicle, documented the abnormal tire wear, and performed alignment

Alignment measurements showed values outside specification. The tire manufacturer reviewed photos and documentation and confirmed the wear pattern is caused by vehicle alignment and not a tire manufacturing defect. The condition affected all four tires and required premature replacement, creating a potential safety concern due to reduced tread depth and handling stability. The issue appears to be related to steering or suspension geometry rather than normal tire wear.Read more

Hi. I noticed my vehicle was randomly making a clunking noise in the rear end. Also, on the highway, it would drift and was difficult to steer. Because they claimed outside influence, they did not cover it under warranty. I took it to my mechanic. He believed the bolt had come loose on its own. He thought this because the trailing arm bolt on the passenger side was very loose and about to fall out. The mechanic replaced the missing bolt and tightened the loose bolt. I would like to document that this happened and see if any other people had reported this problem. Lexus denied that there was an issue.Read more

While performing routine maintenance on my 2018 Acura TLX, I discovered a critical structural failure in the front right (passenger side) lower control arm. The factory weld connecting the stamped steel control arm to the compliance bushing housing has failed and separated. Visual inspection reveals a clear manufacturing defect: there is a significant crater and void in the weld bead, indicative o
... f incomplete fusion or a "start/stop" error during the manufacturing process. This is a latent structural defect, not a stress fracture from mileage or impact. The weld has begun to separate from the housing, leaving a visible gap. Safety Risk: This failure compromises the suspension geometry and structural integrity of the vehicle. If the weld completes its separation while driving, it would result in the control arm detaching from the subframe, leading to immediate loss of steering control, wheel collapse, and a high probability of a collision. Status: No warning lights were illuminated prior to discovery. The vehicle has not been involved in any collisions that would affect this component. The defective part is currently on the vehicle but is available for inspection by NHTSA or manufacturer representatives upon request.Read more

Subject: Catastrophic Suspension Failure Resulting in Total Loss of Trailer Braking Vehicle: 2017 Keystone Montana On January 5, 2026, while traveling at highway speeds, the rear driver and passenger-side leaf springs suffered a catastrophic "brittle fracture" failure. This resulted in the axle shifting 6+ inches, wedging into the chassis I-beam. The violence of the axle displacement severed the
... electrical brake wiring harnesses, causing a 100% loss of trailer braking. We were forced to perform an emergency stop of a 13,000lb+ load using only the tow vehicle’s brakes. Defect History & Pattern: This is the third suspension failure on this chassis. Previous repairs (documented) revealed factory-installed oversized hardware (screws) obstructing suspension travel. The failure mode matches the metallurgical defects described in NHTSA Recall 23V-822 (Hydrogen Embrittlement) and Safety Advisories 24-453 and 22-420. Safety Risk: The failure of the primary structural suspension led directly to the failure of the primary safety system (brakes). This defect renders the vehicle unroadworthy and constitutes an extreme risk of a "runaway trailer" event, multi-vehicle collision, and potential loss of life. The manufacturer (Keystone) has been on notice of these defects for 9 years but has failed to provide a permanent remedy, dismissing structural collapses as "normal wear and tear." There was absolutely no warning given prior to the incident taking place. When we first took the exit ramp for the I25 we heard a loud pop, then in our sideview mirrors began to see smoke coming from the side of the RV. Unable to immediately stop due to being on the top of the exit ramp with absolutely no extra space for other vehicles to safely get by, we continued down the ramp with just the use of our truck breaks until we reached a safe stopping point at the bottom of the ramp.
