On April 15, 2026, while driving on the highway with my family in my 2025 BMW X5 (purchased new on September 25, 2025 from Principal BMW of Corpus Christi for approximately $75,000), the panoramic sunroof suddenly and violently exploded. The explosion sounded like a gunshot. Shards of glass sprayed throughout the cabin and got into our eyes. I was barely able to maintain control of the vehicle and
... prevent a serious accident that could have resulted in hospitalization or worse. Photos of the sunroof taken immediately after the explosion are attached to this email. These images clearly show the large shattered section, missing glass fragments, and debris inside the sunshade right after the failure. You can also see from the picture submitted from the front, the glass is pushed out from the car. This was an extremely dangerous safety failure that put my entire family at immediate risk. Because of this incident, I no longer feel safe driving the vehicle. I am now constantly on high alert, worried that the sunroof could explode again at any moment. This traumatizing experience has completely ruined my ability to enjoy what was supposed to be a luxury vehicle. Instead of the premium driving experience I paid for, I am left with fear and anxiety every time I get behind the wheel. This is not an isolated or “unheard of” incident. BMW has faced multiple reports and a class action lawsuit regarding exploding sunroofs dating back to at least 2017. Despite this known issue, when I contacted Principal BMW of Corpus Christi, the dealership claimed the problem was unheard of and showed absolutely no sense of urgency or willingness to address it properly. The service person took a quick look and told me that this incident has happened two other times within the last two months. Since the incident happened on 4/15/26 @ 6:35PM I was unable to speak to a service member.Service person told me to bring my car in for further evaluation on 4/20/26 since he’s only one working.Read more

I was driving merging onto the freeway and my sunroof completely shattered, no car was in front of me so, nor was I driving under a bridge, it just completely shattered. Looking as if someone grabbed a hammer or punched it, I know a rock wouldn’t shatter it, maybe crack but no it shattered. The glass looks like it lifted too , not caved in , so almost as if there was too much pressure as well.
... Have seen similar cases , not sure if any have reported but yes I have seen similar issues occurring. Thankfully I had my headliner closed , so the glass fell on there I had to pull over and figure it out, I did not touch the glass at all, as I’m currently having my Honda Dealership look at it. Just hope it doesn’t happen to othersRead more

I am filing a complaint regarding an unresolved safety recall for my 2025 Toyota Sienna involving the second-row seat welds. VIN: [XXX] NHTSA Recall ID: 25V668 Recall Date: October 7, 2025 Manufacturer/Dealer Recall Reference: 25TB12 This recall involves the structural integrity of the second-row seat welds. A failure of these welds could increase the risk of injury to occupants in the event of
... a crash or sudden stop. As of today, more than six months after the recall was issued, no remedy has been made available. I have not been provided with a repair, a timeline for when a fix will be available, or an adequate interim solution. On February 25, 2026, I brought my vehicle to the dealership, and it was placed out of service due to this recall. I was provided with a smaller loaner vehicle, which is not comparable to my Toyota Sienna and does not meet my transportation needs. This situation has left me without use of my vehicle for an extended period of time while also dealing with a known safety defect. I request that NHTSA review whether the delay in providing a remedy is reasonable and compliant with federal safety requirements, and whether additional action is necessary to protect vehicle occupants. All relevant vehicle details are included in this complaint. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Driver’s door electronic door handle failed and the door could not be opened. Interior handle should have a physical release but it took 6-8 attempts to open the door. The factory (OEM) roof rails on my 2023 Toyota Sequoia have become loose at the front and are popping up, no longer sitting flush or appearing securely attached to the roof. This issue occurs during normal driving and is visible without removing any trim or components. The front portion of the OEM roof rails appears to lift or separate, creating concern that the rails or associated hardware could
... detach while the vehicle is in motion. This presents a potential safety hazard due to the risk of parts coming loose at speed and becoming road debris, which could endanger other vehicles. I am aware of other owners reporting similar issues with the factory roof rails on this model, suggesting this may not be an isolated incident. The vehicle has approximately just over 40,000 miles. The dealer has inspected the vehicle and advised that the repair would require significant disassembly and replacement, with an estimated cost of approximately $3,400. Despite the issue involving OEM components and their secure attachment to the vehicle, the repair was denied under warranty due to mileage. Given that this involves factory-installed exterior structural components becoming unsecured, I believe this represents a potential safety-related defect rather than normal wear or damage.Read more

This complaint concerns a 2023 Hyundai Palisade subject to two active safety recalls that remain unresolved, creating an ongoing safety risk. The first involves noncompliance with FMVSS 226 (Ejection Mitigation), identified in a January 2026 recall. The manufacturer has acknowledged the vehicle may not meet federal occupant protection requirements but has provided no remedy and no timeline for re
... pair. The second involves defective seat belt buckle assemblies that may fail to properly latch. An authorized dealer confirmed the condition and advised that parts have been ordered; however, no estimated arrival date or repair timeline has been provided. As a result, the repair cannot presently be completed. The vehicle is used to transport young children in rear-facing car seats that rely on properly functioning seat belts for secure installation. The inability to ensure proper seat belt function, combined with the lack of compliance with FMVSS 226, presents an ongoing safety risk. Despite multiple written inquiries, the dealer has not provided any written confirmation regarding repair timing, instead offering only verbal and unverifiable updates. The manufacturer has been placed on written notice but has not provided any substantive response or identified an available remedy. The vehicle has been inspected by an authorized dealer, who confirmed the recalls and the current inability to complete repairs. These conditions have persisted for months without an available repair path, leaving the vehicle in continuous use despite known safety defects and federal noncompliance. The inability to obtain a verifiable repair timeline or confirm parts availability demonstrates that the defects cannot be remedied within a reasonable time. Supporting documentation is attached. The lack of available remedy, combined with the inability to complete or verify recall repairs for safety-critical restraint systems, leaves occupants exposed to preventable injury risks in crash scenarios.Read more

... en. Recently, after taking the vehicle in for an oil change, I was informed of multiple additional problems (three separate issues) affecting the vehicle. In addition, the check engine light remains on continuously, even after attempted repairs, indicating unresolved underlying problems. I have also previously attempted to have a recall repair completed, but was told that the necessary part was unavailable and that I would be contacted when it became available. Months later, I have still not been contacted, and the recall issue remains unresolved. Based on my experience and publicly documented issues, this appears consistent with known defects associated with this model, including reported water pump failures leading to engine overheating, rear suspension issues affecting control, and concerns regarding exhaust fumes entering the cabin. This is not normal wear and tear, but a potential safety-related defect. It is a serious concern that known recalls and defects cannot be repaired due to lack of available parts, leaving the vehicle in an unsafe condition. I am also concerned about the risk of continuing to operate this vehicle and potentially causing an accident that could injure myself, my family, or others on the road. This risk is heightened by the lack of timely communication and resolution regarding known defects and recalls. I am requesting investigation into these issues to prevent harm to drivers and passengersRead more

I am submitting a vehicle safety complaint regarding a vehicle purchased on March 5, 2026. Within days of purchase, the vehicle exhibited multiple safety-related issues despite reportedly passing a Virginia safety inspection on February 27, 2026. The safety concerns identified include: - Driver-side headlight partially inoperable, which resulted in a traffic stop on March 15, 2026 - Front hood not
... properly secured/latching, creating a risk of opening while driving - No engine oil present upon inspection shortly after purchase - Coolant reservoir completely empty - Check engine light illuminated shortly after purchase Diagnostic testing following the check engine light revealed code P0420 (Catalyst System Efficiency Below Threshold). A mechanic advised that this condition is not a new issue and would have developed over time, indicating that it likely existed prior to the sale of the vehicle. At the time of purchase, the vehicle had an outstanding manufacturer safety recall identified as NHTSA Recall No. 23V651000 (Recall #251), dated April 3, 2024. This recall had not been completed prior to sale and was only addressed on March 24, 2026, after I took the vehicle to a Hyundai dealership. Additionally, the vehicle was subject to a manufacturer campaign identified as Campaign #P33, dated December 15, 2025. This further indicates that the vehicle required manufacturer-related service that had not been fully addressed prior to sale. An independent inspection conducted on March 16, 2026 confirmed conditions inconsistent with a vehicle that had recently passed a safety inspection. These issues raise serious concerns about the safety, inspection integrity, and roadworthiness of the vehicle at the time it was sold. I am requesting that this matter be reviewed for potential safety-related concerns and any related violations. I am available to provide supporting documentation, including inspection reports, diagnostic results, service records, and photographic evidence.Read more

I am asking that the NHTSA please look into reissuing the NHTSA Campaign 09V444000 to have all 2000-2003 Toyota Tundras still registered for road use in severe cold weather states be inspected by a certified Toyota dealership for repairs, frame replacement, or vehicle buybacks to prevent unnecessary accidents due to a manufacturer defect that was never rectified properly.Read more

The factory‑applied exterior paint on the roof panel of my 2016 Acura MDX has experienced clear‑coat and base‑coat delamination, resulting in complete separation of paint layers and exposure of bare metal. This is not a cosmetic blemish or paint discoloration. The failure involves loss of adhesion between factory coating layers, causing large sections of paint to peel away unexpectedly. The
... exposed metal surface is at risk of corrosion, which may compromise the long‑term structural integrity of the vehicle’s roof panel. In addition, peeling paint has detached in rigid fragments that could become roadway debris, creating a hazard to other motorists if detachment occurs while driving. This condition matches a known manufacturing defect previously acknowledged by the manufacturer in a technical service bulletin addressing paint delamination on similarly built vehicles.
