The trunk of the vehicle was open. Without warning/reason, a support bar that helps hold up the open liftgate popped off and the liftgate swung closed rapidly with its full weight, not supported by anything. The support bar that had popped off obstructed the path of the falling liftgate, causing damage to the trunk panel. The speed at which the liftgate fell at full force could have caused severe

Safety concern related to the tail lights and trunk lid lights on a tesla model s. I have had to replace both right and left turn lights and the trunk brake lights on multiple occasions due to water intrusion into the light housings. Water seepage over time or after heavy rains starts shorting the LEDs, thus causing safety concerns due to non-functioning tail lights. The vehicle has not been in a

There is apparently a known issue on the WT "Work Truck" trim of the Silverado EV trucks where the factory wiring harness was assembled incorrectly. This prevents the truck from knowing when the rear doors are open vs closed. If only one (or even both) rear doors are open, all the doors may automatically lock (depending on auto-locking settings), potentially locking the driver out of the vehicle.
... As a first responder I immediately recognize this as a significant child safety issue: if a parent were to get out to check on their child in the back seat, finish what they were doing, then close the doors to get back in the front, they could find themselves locked out with their child locked inside. Because this would also mean the truck would be shut down, the climate controls would be turned off when this happens, and could easily result in heat injury or death of a child. This door issue also causes a safety issue as the dome lights do not come on when the rear doors are opened, and a door could potentially be partially or even wide open without the truck alerting the driver. The dash clearly shows and identifies the front driver and front passenger door open indicators, but does not show either of the rear doors. I have not taken my truck into the dealer for correction of this issue yet, but I have confirmed this is an issue with multiple other Silverado EV drivers who have the WT trim (both 3WT and 4WT packages). While I will be working with GM to address the issue on my vehicle, I feel the potential risks of this issue are significant enough that the NHTSA should intervene for all affected vehicles. To save a little time, here is a forum post on the matter. Problem Description Component/System Failed: The driver-side front door has been inoperable since April 2024, unable to open or close, linked to Ford’s Customer Satisfaction Program 23B56 (faulty door welds/hinges). Vehicle is available for inspection. Safety Risk: I must climb over the passenger seat to enter/exit, risking injury, especially in emergencies. My disabled veteran father, who sh
... ares the car, cannot drive it due to inaccessibility, severely limiting his mobility. An inoperable door could delay evacuation, endangering us both. Confirmed by Dealer: Empire Ford, New Bedford, MA, confirmed the issue in October 2024, tying it to 23B56. They quoted 2-3 weeks for parts but haven’t repaired it as of April 10, 2025. Inspections: Only Empire Ford inspected it in October 2024. They admitted to marking the work order as done without repairs, restarting the process. No other inspections by Ford, police, or insurers occurred. Symptoms/Warnings: No warning lights or messages. Around April 2024, the door became hard to open/close, then failed completely, remaining stuck closed. No prior noises or partial issues noted. Details: In October 2024, I took my Escape to Empire Ford for the 23B56 repair. They said parts would take 2-3 weeks. After months without updates, I learned they falsely marked the job complete, forcing a restart. Six months later, the door is still broken, and they only say parts are “ordered,” offering no timeline. This aligns with NHTSA’s PE23013 investigation into Escape door welds. The delay makes the car unsafe and unusable for my father. I request NHTSA’s help to push Ford for a prompt fix or loaner vehicle.Read more

Power sliding Door lock actuators failed. First started experiencing problems with the doors at 75,000mi clicking noises and then at 90,000 miles the drivers side door became stuck shut. ... 4/5/25 I received a warranty extension notice from Fiat Chrysler Automobiles stating a warranty extension on the part and repairs however my vehicle has passed the mileage upon receipt of their letter. When I contacted FCA US LLC Customer Care they refused to further extend the warranty for repair even after a supervisor agreed that this a safety concern and that he would not be comfortable with his family riding in a vehicle that risks them being trapped. Upon further research I have found that FCA has known about issues with the lock actuators dating back as far as model years 2015-2017 in which the warranty was extended to 15yrs/150,000 miles. This puts over 500,000 families at risk of being trapped in their vehicles in the event the lock actuators fail. This should be a safety recall with no mileage restriction as it puts millions of people at risk of death or serious injury if they were to be trapped in their vehicles.Read more

To Whom It May Concern, I am writing to formally report a recurring water intrusion issue with my vehicle, which poses a potential safety risk. Whenever it rains and the vehicle is parked outside, water consistently leaks into the passenger-side interior. While driving, especially during acceleration or braking, I can hear the sound of water moving within the passenger side area. Upon inspection,
... I have found water pooling in the front passenger footwell, where multiple electrical wires are located. Additionally, there is an electrical control box situated beneath the passenger seat that may also be exposed to moisture. I also discovered water leaking behind the rear seat area. This section contains two air vents, which may be contributing to the issue. The water intrusion raises serious concerns about electrical system integrity, especially given the presence of wiring and components in affected areas. Although my vehicle has not yet experienced electrical failure while driving, I am aware of multiple reports from other owners who have encountered similar leaks that led to sudden engine shutdowns, including while driving in traffic — a serious safety hazard. I brought my vehicle to the service center for evaluation, but as of now, I have not received any resolution or updates regarding a fix. I have several videos of the situation as well. Given the potential risk to both vehicle occupants and others on the road, I respectfully request that the NHTSA investigate this matter further to determine whether a recall or other safety action is warranted. Thank you for your attention to this serious issue.Read more

I am writing to bring to your attention a concerning issue regarding the instrument panel of the 2016-2020 Kia Sorento. My car - 2017 Kia Sorento. In approx. 2021, I noticed a small 2 cm crack resembling a razor blade cut on my instrument panel (Part Number: 84710-C5000WK, also 84710-C6000WK and 84710-C6001WK). At the time, I did not find this defect alarming as it was minor and located far from
... the airbag area. However, over time, the situation has deteriorated significantly. The small crack has now developed into three substantial cracks measuring between 6 to 8 inches, with some of them encroaching upon the airbag area. I am increasingly concerned that these cracks may impact the airbag deployment mechanism and potentially result in fragments from the panel flying out during a collision, an issue that has been documented in other vehicle brands in the past. After conducting some research, I found that this problem appears to be common among the Kia Sorento models manufactured between 2016 and 2020. Numerous sources online indicate that these dashboard cracks pose a safety hazard. I have made attempts to address this issue directly with Kia. However, I was informed that the initial inspection fee is $150, and the estimated cost for part replacement and installation is approximately $3,959. This amount is quite significant. I urge your organization to investigate this matter further and consider providing a remedy for affected customers. Thank you for your attention to this important matter.Read more

The rear passenger-side lower control arm mounting point on my 2010 Honda CR-V has rotted out due to severe corrosion. This has caused a loud clicking noise when driving over uneven surfaces and has compromised the structural integrity of the suspension system. The problem was confirmed by an independent mechanic, who stated that the control arm is no longer properly secured due to rust damage at
... the mounting point. The vehicle was also inspected by a Honda dealership, which noted frame perforation in and beyond the recall area related to NHTSA Recall No. 23V-228. Despite this, American Honda has refused to address the issue, stating that the corrosion does not fall within the defined recall area. This failure presents a significant safety risk, as the suspension could potentially detach while driving. The issue first became noticeable in mid-March 2025 when the clicking noise started and has persisted since. There were no prior warning lights or error messages. The corrosion is visible and the affected components are available for inspection upon request. The condition has been documented via dealer and mechanic reports, photographs, and video.Read more

After numerous attempts at getting Grand Design to fix this unit, I had to spend thousands of dollars to prove it was road worthy to tow it from Arizona to Indiana to their service center. They finally agreed to repair it. After 3 weeks we got it back with everything fixed or so we thought, less then 500 miles later the crack in the sidewall came back. After reaching out to Grand Design again they agreed to repair it again, 3 weeks later it was repair. Now the Wall the they repaired is moving, obviously something broke again. I purchased this vehicle on [XXX] from Sunnyvale Volkswagen and have experienced ongoing issues with the electrical system, infotainment, safety features, and OTA software updates. In early 2023, all error lights came on while driving, followed by a complete electrical system failure. The car would not restart until the next day. In 2024 and again in 2025, the center screen went completely black,
... disabling the rearview camera, hands-free navigation, climate control, and car settings. Restart attempts failed. On Feb 26, 2025, I accepted an OTA software update, which failed. I tried again on Feb 27. When I returned to the car later that day, the update was still running. The next day, the car would not unlock with either key. I tried the manual key over an hour and hundred times, which eventually worked, but the keys still weren’t recognized. I replaced both key batteries with new ones from the store (one key was spare hence never used), but the issue persisted. After several days of the same issue and hours of failed attempts, I was finally able to start the car and drive it to Sunnyvale VW on March 3rd. They acknowledged the issue and replaced both key batteries again with OEM ones, which temporarily resolved the problem. On March 25, I was contacted by Stevens Creek Volkswagen to address two open recalls. I scheduled an appointment for March 31 at 9am and called on March 30 to confirm. Despite the confirmation and their having a note for a loaner from the manager, I was told upon arrival—after waiting 15 minutes—that parts were unavailable and they couldn’t perform the repairs. I was told I’d be contacted when parts arrive, the same excuse I’ve heard since the recall opened a year ago. My safety recall remains unresolved.
