Dear NHTSA, I am writing to formally file a complaint against Toyota regarding their failure to notify me of a recall related to the clear coat peeling from my vehicle’s paint. I was never informed of this issue through a recall notice, nor did it appear in my vehicle’s history. I only became aware of the recall when a friend, who owns the same make and model in the same color, experienced th
... e same problem and was able to have it repaired under the recall. When I contacted my local Toyota dealership, I was told that I am now outside the recall window and that the repair can no longer be completed under the recall. However, had I been properly notified in a timely manner, I would have taken my vehicle in for the necessary repairs. I believe Toyota’s failure to send me the recall notice has unfairly prevented me from receiving the repair I was entitled to. I am requesting NHTSA’s assistance in addressing this matter, as Toyota’s lack of proper notification has left me with an unresolved defect. Please advise on the next steps I can take to ensure Toyota takes responsibility for this issue. Thank you for your time and assistance. I suspect the issue may be related to plastic components, as I’ve read they can become brittle over time and lead to leaks Also, the these BMWs involves the water pump's electrical pump connector allowing water to drip from the positive crankcase ventilation system through the seal, potentially causing a short circuit.Read more

Upon completing a trip with the RV mentioned, an inspection was completed of the units suspension system where it was found to be compromised. Not knowing what compromised the suspension (leaf springs, shackles, hangars, etc), further investigation was done where the underbelly of the camper was removed. Upon gaining access to the actual RV frame, it was found to have at least two major cross me
... mbers completely broken off the I-beam that runs from the front to back of the camper. Not only were there two cross members where the welds had completely broken free, but there were other metal bracings between cross members and to the I-beams that have also completely broken off. With the welds completely broken, there is no support between the two I-beams on this camper leading to major safety concern not only to those towing the fifth wheel but other motorist around the unit while on the highway. No accident has occurred but instead the frame has completely failed, including multiple cross members, other structural steel bracing and even multiple outriggers compromised due to the movement of the main 12" I-beams. This is a MAJOR safety concern that Lippert (frame manufacturer) nor Jayco (RV brand) is taking any responsibility for but should be addressed as other units sitting on similar frames could be facing the same issues due to the low quality welds and bracing.Read more

I am [XXX]. I purchased this Honda Odyssey van after it was modified by BraunAbility. They put in an automatic underfloor ramp, and other modifications. When used without these modifications, there are three rows of seating. Passengers go in and out, and the passenger-side sliding door opens with an automatic button. There is a sensor that controls that door. It is never in danger of being hit o
... r disturbed in use by an ambulatory person. However, when it is used with a ramp, the second row of seats is removed and replaced with a passenger-side ramp. The ramp is 30 inches wide. The normal power wheelchair is 26 to 29 inches wide. This leaves only 2 inches MAXIMUM on either side of the ramp for the wheelchair. It is virtually impossible to NOT hit the sensor when going in or out by wheelchair!! I have had this van for about two years. I use it about once every two weeks. Most times I have an assistant driving. I have only used it about 50 times TOTAL, in two years. On [XXX], I had a doctor's appointment. I activated the automatic door and ramp. I rolled up the ramp, and parked in the passenger area. That seat has been removed for the wheelchair. My assistant pushed the button to activate the ramp to go back under-floor. It did so. BUT the sliding door on the passenger side did NOT close at all! Eventually he managed to manually slide the door shut. If I had been driving alone, I would have been stranded right there---at the doctor's office, shopping center, etc., as I do NOT have the strength to manually close that door! This is clearly a danger to disabled people using this modified Honda Odyssey. Honda and BraunAbility should be required to protect and reinforce the sensor area so users will NOT easily damage the sensor with light use of this van! 1. Complete Trailer brake system failure due to incorrect wire gauge used during assembly at Grand Design. 2. Frame failure at axle shackle weld on Lippert frame due to over-penetration. While towing the trailer there was a message on the tow vehicle information center "trailer Brakes disconnected". All trailer breaking was lost while going down the Siskiyous. At that time the trailer began to
... sway and was difficult to maintain control due to the loss of trailer brakes. During assembly Grand Design used 18-gauge wire to connect the 14-gauge wire from the brake magnets and axle tube to the wire running through the trailer underbelly up to the Vehicle plug. Lippert brake installation manuals for 4 brake systems state to use at least 14-gauge wiring. These manuals are readily available on the Lippert website. The photos we have provided clearly show 18 gauge was used and its insulation had melted exposing the center conductor. Repair documentation from the dealership to fix the brakes indicated that the right rear magnet was intermittently bad drawing high current which would draw all the braking system current to that one assembly. This maximum current to one set of wiring caused the smaller 18-gauge wire to melt off its insulation which resulted in a hard short that prevented all braking at a very inopportune time. Had 14 gauge been used we would not have lost all braking to the trailer. During the brake wire repair at the dealership the frame around the RR axle shackle failed when the trailer was jacked up. It appears that the shackle attachment weld over-penetrated the frame ultimately causing a weak joint and lead to the frame failure. The frame failing around the shackle weld happening at the dealership instead of on the road is beyond lucky and avoided another very unsafe incident on the road. These two items happening at the same time almost resulted in a multi vehicle crash on I-5 southbound.Read more

We were not sure what caused this rubbing of the furniture on the interior and were later informed that this is caused by extreme frame flex or failure. The pinch style moldings that cover the seams on the front cap of the rv have loosened while driving and flap in the wind while traveling down the highway, I have since been forced to apply silicone to keep these in place.
