My roof dents extremely easily and it seems after searching the internet this appears to be somewhat of a problem with this model and I am sure there is some type of pressure rating it should conform to to be safe? Vehicle: 2017 Dodge Grand Caravan Component/System: Sliding door lock actuator Is the part available for inspection upon request? Yes Description of the Issue: The sliding door lock actuator failed, causing the door to remain locked at all times. The door cannot be opened manually or using the power door controls. This renders the door completely inoperable. Safety Risk: This is a significant sa
... fety hazard. In the event of an emergency, passengers—especially children seated in the rear—cannot exit the vehicle through this door. It severely limits evacuation routes and could result in injury or death if occupants are trapped during an accident or fire. Has the issue been reproduced or confirmed by a dealer or service center? Not yet. Issue is ongoing and appointment is pending. Has the vehicle or component been inspected by the manufacturer, police, insurance, or others? No Were there any warning lamps, messages, or symptoms before the failure? No warning lamps or messages appeared. The door simply stopped responding to manual or power lock/unlock commands. When did the issue occur? June 2025. The door has remained inoperable since. Assessment: Based on consumer forums and other reports, this appears to be a known and widespread issue with this model, likely caused by a defective actuator design.Read more

Component/System Involved: Molded-in-Color (MIC) Hardtop – Composite Roof Panel What happened: The molded-in-color (MIC) hardtop on my 2021 Ford Bronco (2-door) has developed multiple visible stress cracks above the passenger-side window. These are not surface scratches — they are embedded, structural fractures in the composite roof material. The cracks have formed under normal, garage-kept d
... riving conditions at just over 10,400 miles and continue to worsen. Safety risk: I am concerned these cracks pose a potential safety risk, particularly involving water intrusion during rainfall or car washes. The affected area is directly above the front-row passenger seat and adjacent to electrical systems, including interior lighting and airbags. If water penetrates the roof structure, it could lead to mold, short circuits, degraded visibility, or electrical malfunction — especially under motion. Additionally, the structural integrity of the hardtop could be compromised in a rollover scenario. Inspection history: A Ford dealership in Winchester VA acknowledged this is a known issue and stated they had replaced multiple hardtops for the same defect. However, they declined to inspect my vehicle due to the warranty having recently expired. Ford Motor Company also reviewed the issue but denied coverage without inspecting the vehicle. Manufacturer acknowledgment: Ford delayed delivery of my Bronco by over a year due to widespread MIC hardtop defects. Despite that delay, the same failure has now occurred in my vehicle, just beyond the warranty period and with low mileage. Inspection availability: The vehicle and affected hardtop are available for inspection upon request. I have detailed photographs showing the extent and location of the cracks. Symptoms and warnings: There were no warning lights or error messages prior to the appearance of the defect. The cracks appeared gradually and became visibly obvious in June 2025.Read more

I am writing to file a formal safety complaint regarding my 2021 Jeep Renegade, which has been experiencing sudden, unprovoked automatic braking for almost a year. The issue began in Spring [Year], and over the past two months, it has become significantly more frequent. The vehicle brakes sharply on its own even when there are no obstacles, cars, or objects in front of us. This happens at normal
... driving speeds and creates a serious risk of rear-end collisions—especially dangerous on highways. We have a baby, and this malfunction has put our family in frightening situations multiple times. We’ve brought the vehicle to the dealer and service center several times, but they claim no error code shows up, so no repair is made. The car is currently at the service center again, and still no resolution. We’ve also requested assistance from our Jeep case manager, but no effective action has been taken. There are additional issues such as: •The door locks stop working after it rains, requiring us to use the physical key in the door. •The sunroof stopped working, even though it was replaced two years ago (this is currently being addressed, but remains part of the larger reliability concern). This automatic braking issue is the most urgent concern. The car is not safe to drive, and the lack of response from Jeep is deeply troubling. Please let me know if further information, service records, or documentation is needed.Read more

Subject: Urgent Complaint – Recall Repair Delayed Due to Unavailable Parts (2019 Explorer) To Whom It May Concern, I am writing to formally file a complaint regarding a safety-related recall/service campaign for my 2019 Ford Explorer (VIN: [XXX] ), specifically concerning the roof rack trim detachment issue (Customer Satisfaction Program / Recall #20N11 or 21S22). The plastic roof trim panel on my 2015 Audi A3 is peeling and deteriorating significantly. This is not painted metal—it’s a separate, non-paintable plastic component that runs along the sunroof and roofline. The surface began flaking and lifting over time, and multiple independent body shops confirmed the panel is failing structurally and cannot be refinished—it must be replaced. While this
... may appear cosmetic, it’s not. The panel helps protect the roof seam and substructure. Its failure opens the door to water intrusion, corrosion, and potential roof integrity compromise—a serious safety concern, particularly in the event of a rollover or high-impact collision. Other owners of vehicles built on the same MQB/8V platform—including the VW Golf and SEAT Leon (not sold in the U.S., but structurally similar)—have reported identical issues, often followed by cracking or detachment of the panel. Despite multiple attempts to resolve this, Audi of America has been largely uncooperative. Their warranty process requires a diagnosis from a dealership, but most Audi dealers do not perform bodywork and refer customers to third-party shops—whose assessments Audi then refuses to accept. I’ve been left in a loop with no meaningful support, despite the issue being widespread and well-documented. I believe this is a design or materials defect that should be reviewed before it leads to broader safety or structural failures. See the below URL's for similar issues. [XXX] [XXX] [XXX] [XXX] [XXX] [XXX] [XXX] Excuse the shortened URL's, I need to meet your character count. I also cannot seem to upload images (despite being under the file size). 2025 Land Cruiser bought 7 months ago constant rattling and tapping noise near passenger rear near pillar B and C. As recommendation by the service adviser the Land Cruiser needed to be bring to a car body shop to reweld parts from both pilar B and C that was creating the rattling noise. I believe the 2025 Land Cruiser have a manufacturer welding defects that creating the rattling and tapping noise which is needed to be thoroughly investigated. Pillars a, b, c, and d on cars which have welding defects can compromise the safety of the vehicles and its occupants. 1. It weakens the structure and reduce crashworthiness. 2.Failure of welded components will reduce strength and load bearing. 3.Risk of cabin collapse. 4.It impact on safety features this areas is where airbags and safety belts been supported. 5. Right after picking up my 2015 Ford Mustang from dealer service on July 23, 2025, the hood suddenly flew open while I was driving, completely blocking my view and creating a serious risk of collision. The dealership had performed multiple recalls and a multi-point inspection but did not flag any hood latch issue. The vehicle was released as “safe to drive,” yet this happened immediately after
... leaving their service. This was an extremely dangerous situation that could have caused a serious accident. The force of the hood flying up also caused major damage to the hood and surrounding components. The dealership took the car back and even offered me a rental, but they are refusing to take responsibility or repair the damage, claiming they are not at fault. I am reporting this because it is a serious safety issue and I want to ensure it is properly investigated to prevent similar incidents. Toyota should be forced to extend the extended warranty for the full duration of the court agreement of 10yr, Dec 2025, regardless of purchase date. My request for fuel door replacement in Jul 2025 was refused because Toyota claimed the warranty was only good until 10yr after date of purchase, not to the end of 2025. The warranty period applies to vehicles purchased Dec 2025, so the warranty program still exists, but Toyota National refuses to honor it for my vehicle. First off, we love the KIA Brand of vehicles. This is our fourth Kia. We also presently own a 2010 Optima with white paint and no peeling issues. Our Sorrento is currently showing (as of today) 133418 Miles. Sad to say that this is the only KIA we have ever owned with this very defective paint issue. Several months ago, I started to see several other 2014 Sorrento with the same peeling paint is
... sues, so it wasn't just our vehicle. Your company is in the business of producing sophisticated and desirable vehicles designed by engineers who can cover all the bases in the manufacturing process. With the exception of this faulty paint issue, KIA has achieved this goal. I can only hope that KIA recognized this paint failure early and will go on to the next design phase. 1n my humble opinion, if KIA wants to keep growing the company brand they need to work like crazy to get to the next level in the paint process. Buyers want to have confidence that things as simple as a paint job will have a long life. Interesting when you look at very old vehicles for sale and the owners say this is the original paint. Thus, my question is how will KIA remedy this issue with both current and future model owners? l await your reply. I hope KIA will share a disclosure that is fundamental and not try to hide or mislead.Read more

I am reporting a recurring issue with the front bumper misalignment on my 2025 Toyota Camry, which may indicate a widespread manufacturing defect. Shortly after purchasing the vehicle from Toyota Direct March 2025, I noticed that the front bumper was visibly misaligned, with uneven gaps near the headlights and fender panels. On June 9, 2025, the dealership replaced the front bumper with an OEM pa
... rt. However, the new bumper still does not fit properly and continues to protrude and separate at the corners creating the potential for the bumper to become loose during operation. This affects both the appearance, aerodynamic integrity of the vehicle and an potential for accident. Multiple attempts at repair have failed to resolve the issue. After researching, I found numerous reports from other 2025 Camry owners experiencing the same bumper misalignment. Some dealership representatives have acknowledged the bumper fitment as a “known issue,” yet no official recall or technical service bulletin has been issued. Given the widespread nature of this defect and Toyota’s lack of a permanent solution, I believe this issue warrants further investigation by the NHTSA to determine whether a manufacturing or design flaw exists that could affect vehicle safety, durability, or value.Read more

This report is on the paint defect issue on the 10th generation Honda Civic that can potentially impact the vehicle's structural integrity from the windshield damage. The white paint on my vehicle started peeling around the perimeter of the rear window with the rain groove. I am uncertain of the exact time it started, but the paint defect became noticeable to the naked eye just past the 5 years o
... f ownership. The size of the paint chip has now extended to the rear body of the vehicle. A section of bare steel body around the rear window is exposed to the elements. Based on the inspection and evaluation by the authorized Honda paint and body shop that was recommended by the dealership, I have been informed that the cause is commonly due to improper paint priming during the manufacturing process. According to the paint and body shop owner, such paint peeling is a widely known issue with certain color paint for many auto manufacturers that use the same paint primer. I was warned that paint peeling will likely worsen with the water being captured underneath the paint. I am reporting this incident as a safety concern because the rust on the bare metal chassis of the vehicle can cause corrosion under the windshield adhesive over time and lead to the window breaking. The windshield is critical to the vehicle's structural rigidity as it contributes to both the roof support and the airbag deployment. The Honda dealership service manager submitted goodwill on my vehicle past warranty for a paint defect from the manufacturing process, but American Honda Motor Company has denied the claim as they determined it as cosmetic damage only. There are multiple reports online on paint defects from other Honda Civic owners from various places. There was a lawsuit settlement on older generation Honda Civic vehicles, but it looks like the known issue is still present on the newer generation. Based on the location of the paint defect, it can become a safety concern.Read more

Request for Resolution -Peeling Paint on 2025 Toyota RAV 4 I am writing to formally file a complaint regarding a paint peeling issue with my 2025 Toyota RAV 4 and Toyota's refusal to address the defect, despite timely reporting and documentation. However, Toyota has since closed both cases without resolving the issue, stating that the paint repair program expired in November 2024. In reference to NHTSA Recall no. 23V-720 I have made several attempts to have the recall remedied at a certified dealer, dealership is 4hrs away, round trip. On 3 separate occasions I made appointments online for maintenance plus recall. In between 2nd and 3rd remedy attempt lower bumper detached on the highway into traffic. My own vehicle ran it over, several cars swerved on the roadway to avoid
... the obstacle, some successful and some hit it head on sending it across multiple lanes, it was lastly struck by a semi truck and unrecoverable. On the 3rd attempted I questioned why the recall was not being completed. I was told recalls need to be on a separate appointment. 4th attempt was for the only recall and was turned away because of damage on the upper bumper. Damage was chipped paint and a slight crease near the fog lamp hole. Damage was caused by the upper bumper flapping in the wind while driving since it was no longer braced by lower bumper. I was provided a flow chart by the maintenance manager stating that I needed to repair my bumper before the safety recall could be performed. I showed my recall notice that stated if damage was found on the upper bumper, both upper and lower would be replaced. I asked if my vehicle was going to be evaluated and was told no. A few days later I received a voicemail(missed the call) that my parts had arrived and an appointment had been made for me. I did not ask them to order the parts, I did not plan on returning to them because of their flippant attitude towards my safety. I felt obligated and trapped to return to them since they ordered the parts. I replaced my bumper but did not paint it yet. I figured since the chipped paint was a deciding factor to my safety it would be better not to have it in case other paint damage occurred. (why does paint matter on a safety recall?) Went to 5th attempt and was turned away because my upper bumper was unpainted. I have made appt with new dealer. This is not a reasonable time-frame.Read more

I am the owner of a 2020 Ford Escape experiencing two serious issues: 1.My vehicle is exhibiting the exact symptoms outlined in Ford CSP 23B56—specifically, the failure of the driver-side door check-arm welds. According to NHTSA, there is currently no remedy available. This adds another layer of safety concern, particularly considering the compounding risks and Ford’s delay in delivering a fix. Between the structural door failure and a pending recall involving potential engine fires, I am deeply concerned about the safety and reliability of my vehicle. On April 18th, 2025, I dropped off my 2022 Tesla Model Y at Tesla Collision Center (Paramus, NJ) after a minor accident. I later revoked consent for any repairs via email. Despite this, Tesla performed major body and paint work without my permission. No signed work order exists. The repairs were unauthorized and proceeded without my knowledge or approval until this day. These changes now risk alt
... ering my VIN history and vehicle safety records. I’m concerned this constitutes falsified documentation and could affect resale value, title accuracy, and liability in future accidents. I was never shown a repair summary or given the chance to approve or decline service. Even after I sent a formal cease-and-desist letter on June 6, 2025, Tesla towed my rental vehicle on June 4 (without warning), which I had been using through Tesla Insurance. My personal belongings were inside. The timing suggests retaliation for revoking consent and demanding documentation. There were no mechanical failures or warning lights before this, but my concern is that Tesla falsified records & carried out structural work under my VIN without my legal consent. That poses serious risk to my safety and consumer transparency. The work was done by Tesla’s own collision center. The vehicle is available for inspection. I request NHTSA to review Tesla’s repair authorization practices and investigate whether unauthorized repairs, falsified VIN records, or retaliation have occurred in my case and others. This incident reflects broader safety concerns around Tesla’s internal repair authorization system, which may allow collision centers to proceed without documented customer consent. If vehicle repairs are being logged under owner records without approval, this poses risk to accident liability, insurance disputes, and NHTSA transparency. I request a formal investigation into Tesla’s repair authorization practices and how they affect vehicle safety, ownership rights, and regulatory compliance. Thank you.Read more

The inside of the Solitude is a mess too with lights coming down from ceiling, doors are all scratch from rubbing while be transported, trim coming off, wall paper being torn apart, had lag bolts coming up thr
... ough the floor, leakage in master bedroom, floor warping and more. We took it in to have it fixed in November 2024 to only get her back in late march of 2025. They had to weld and reinforce frame but would not admit to frame flex or provide me with an updated weight placard so it's like they don't want evidence on it. They had to replace the whole floor which meant they had to take off slides to get everything out and now she is back up there with severe water intrusions from slides and other things again. Moisture reader is reading in places between 30 to 48% but Grand Design is denying any water intrusion even thought they took slides out. They only respond after an email is sent. This is unacceptable and hope that one day we can get these RV manufacturers to follow same rules as a vehicle. Why are they all in IN - they have the most laxed laws governing recreational vehicles. Our lives are in danger while driving these RV's on US highways which means we are putting others in danger too. Why aren't they held to a higher standard...where is the safety in all of this. I have pictures all everything if needed for documentation. Thank you for your time.Read more

The passenger sliding door pinch sensor did activate; but not before child’s hand was pinched in door and it almost drew blood. Door closes forcefully I have noticed compared to other minivan experiences I have with automatic doors. Quite a bit of force is needed to activate the sensor and cause the door to open back up. It needs to be more sensitive to avoid injury.

I discovered suspected non-OEM electrical components, including a twisted white wire pair and burgundy connector, routed through the passenger-side fuse box and terminating at a footwell speaker or unknown transducer. RF scans (TinySA) showed consistent emissions at 93–106.6 MHz in the cabin, even with the engine off. Symptoms include localized EM exposure targeting genital area, neurological di
... sorientation, and floorboard vibration. Vehicle appears modified to emit or transmit RF signals through concealed hardware. No such system is listed in OEM wiring diagrams. BMW dealership and manufacturer have been contacted to confirm authenticity. Components do not match any standard E92/E93 harness or speaker wiring. This poses a serious safety risk due to chronic RF exposure and unknown signal sources. Symptoms intensify when seated in the car, even stationary. Evidence includes RF spectrum logs, wiring photos, and physical inspection reports. Investigation is requested into vehicle tampering, electrical abuse, and shielding failure.Read more
