What component failed: The rear plastic trim on my 2023 Toyota Prius cracked without any impact, accident, or external cause. The component is still on the vehicle and available for inspection upon request. Safety risk: While this is primarily a structural/cosmetic defect, cracking plastic trim that was not caused by the owner suggests a broader manufacturing quality issue. If this type of failure
... occurs in components closer to safety systems, it could pose a more serious risk. Confirmed by dealer: Yes. I brought the vehicle to a Toyota dealership and service center (Berkeley Toyota) for inspection. They confirmed the cracking. The vehicle was purchased at Livermore Toyota. Other inspections: The vehicle was inspected by the Berkeley Toyota dealership and service center. No other parties have inspected it at this time. Prior warning signs: None. There were no warning lights, sounds, or symptoms before the crack appeared. It was discovered suddenly with no preceding indicators.Read more

I am reporting two unresolved safety-related defects on my 2021 Tesla Model Y (VIN: [XXX] ) that occurred following paid service at Tesla's League City Service Center on 04/16/2026 DEFECT 1 — COMPUTER AND SENSOR SYSTEM FAILURE: The vehicle's onboard computer cuts in and out intermittently. Sensors fail during operation and the problem worsens significantly in high-temperature conditions. Tesla v
... ehicles rely on their computer and sensor systems for core safety functions including collision avoidance, autopilot, and driver assist features. Intermittent failure of these systems while operating the vehicle is a safety hazard to the driver and the public. DEFECT 2 — GLOVE BOX INOPERABLE: The glove box does not open following Tesla service. The glove box in Tesla vehicles houses key vehicle documentation and is integrated into the vehicle's locking and security system. I paid $2,497.50 for service on [06/16/2026]. Both defects were either caused or left unresolved by that service. I have notified Tesla service staff directly and followed up multiple times over more than four weeks. Tesla has not responded, has not scheduled any repair, and has not refunded my money. I am filing this complaint because (1) the sensor and computer failures present an active safety risk during operation, (2) Tesla has ignored all attempts to resolve this, and (3) public reviews indicate this is a pattern affecting multiple consumers. I request that NHTSA investigate and determine whether a safety defect investigation or recall is warranted. I am filing this complaint regarding a premature material failure on my 2024 Toyota Tundra. The factory-applied smoked overlay finish on the front grille is peeling and separating in large chunks, exposing the chrome underneath. This is occurring at approximately 46,000 miles and less than two years of ownership. This is not normal wear and tear, fading, or minor cosmetic deterioration. The finis

The vehicle has been properly maintained, has not been modified, and has not been subjected to misuse or environmental damage beyond normal driving conditions. Paint bubbling and aluminum body panel corrosion on hood and front doors of my 2021 Jeep Wrangler Unlimited (VIN [XXX] ), currently at 63,000 miles. This is a well-documented manufacturing defect affecting 2018-2021 JL Wrangler and 2020-2021 JT Gladiator vehicles with aluminum body panels. Stellantis has issued multiple Technical Service Bulletins addressing this exact condition: TSB 31-001-18 (Ma

The corrosion presents as paint bubbling along the leading edge of the hood and on the door panels. Filiform corrosion develops under the paint over many months before becoming visible. My corrosion warranty expired in January 2026, just one month before the latest TSB was issued. I am reporting this defect for federal documentation. The manufacturer has been aware of this defect for over 8 years (since 2018) and continues to issue revised TSBs rather than implementing a recall. A class action lawsuit ([XXX] et al. v. I received the first recall notice for this defect from Ford Motor Co. in February 2024 and took the vehicle to Sheehy Ford in Ashland, Virginia on August 7, 2025. This creates a significant safety concern because: * The driver window may not function during rain or emergencies. * The AC may fail during extremely hot weather. * The driver seat may not adjust properly, potentially affecting safe vehicle operation and driver visibility. * The intermittent nature of the failure makes the vehicle unpredictable and unreliable to operate safely. The issue has not been permanently resolved despite repeated service visits. I am concerned there may be a broader electrical or control system defect affecting vehicle safety.Read more

2020 Hyundai Kona, ~36,300 miles. Driver's side door latch assembly. On or around May 9, 2026, an abnormal mechanical noise began coming from inside the driver's door whenever the key fob lock/unlock button was pressed, consistent with a failing latch actuator. I booked a service appointment, and on May 15, 2026, an authorized Hyundai dealership confirmed the defect and recommended full replacemen
... t of the driver's door latch assembly. The vehicle is available for inspection. A latch that does not reliably operate is a serious safety risk. The driver's door is the primary egress point. An intermittent or inoperative latch could prevent emergency exit, block post-collision egress, or prevent first-responder access. The vehicle regularly transports children, increasing the consequence of any failure. No dashboard warning lights, alerts, or messages preceded the failure. This is a mechanical defect that does not trigger electronic warnings. Hyundai TSB 21-BD-006H (May 2021) acknowledges this exact failure mode — door handle inoperative or requiring repeated attempts to open — on the Elantra (AD/ADa) and Tucson (TL). The remedy is full latch assembly replacement, and Hyundai extended warranty coverage on the latch to 10 years / unlimited miles on those models. The 2020 Kona is not named in 21-BD-006H despite exhibiting the identical failure mode at low mileage on a 6-year-old vehicle. The defect appears to extend to the Kona platform and is not currently covered. Vehicle has not been inspected by Hyundai corporate, police, insurance, or independent shops. I am reporting a serious and repeated safety defect involving Hehr side windows installed on my 2018 Thor Palazzo 36.1 motorhome (VIN: [XXX] ). Multiple operable side windows on this RV have experienced adhesive bond failure between the scissor arm/operator mechanism and the vent glass pane. This has caused windows to partially detach and violently flap while driving at highway speeds. One rear b
... athroom window completely detached and flew off the RV during highway travel in 2022. Additional adhesive failures have continued occurring from 2023 through 2025 involving: - Kitchen window - Living room window - Dinette window - Rear bathroom window (including replacement assembly) In several incidents, I was forced to pull onto the highway shoulder to perform emergency temporary repairs using tape and adhesive to prevent windows from fully detaching from the vehicle. In December 2025, while driving near [XXX] , the rear bathroom vent window again separated from the operator arm while traveling at highway speed despite prior repair work. The window was violently flapping in the wind and appeared at risk of completely detaching from the RV. This defect creates a serious road hazard because detached glass/window components could strike nearby vehicles or motorists. I am aware of prior Hehr/Lippert recalls involving adhesive-related window failures, but I have not found a recall covering this specific failure point involving separation between the operator arm and vent glass. The defect appears widespread across multiple windows on the RV and has persisted despite repeated repair attempts. Component: Exterior Door Handle (Front Passenger) Incident Description: A latent manufacturing defect resulted in the structural delamination of the interior chrome plating on the front passenger door handle. This failure occurred after the last use of the handle, creating a razor-sharp metallic edge that was not visible during a normal approach to the vehicle. Upon contacting the handle to open
... the door, a passenger sustained a deep laceration from this jagged metallic blade. Safety Hazard: The defect transforms a primary vehicle touchpoint into a concealed cutting hazard. The chrome skin peels and curls into a sharp edge without warning or visible signs of wear, posing an ongoing risk of serious physical injury to any occupant. Manufacturer Response: Tesla Service (Los Gatos) was provided with photographic evidence of the razor edge and the resulting injury. While acknowledging the injury, Tesla refused a safety-based "goodwill" repair, citing the vehicle’s age (2019) and the absence of a formal recall. Tesla further attempted to attribute this structural material failure to cleaning products, despite the vehicle being garage-kept and maintained only with pH-neutral enthusiast-grade soaps. Tesla has since refused further discussion. Request: This defect poses an ongoing risk of serious physical injury to any occupant using the handles. I am reporting this to assist in identifying a trend of material failure in 2017–2020 Model 3 chrome door handles.Read more

The front grille and front bumper of my 2025 Nissan Pathfinder have melted twice on their own without any accident, impact, or unusual use. The vehicle is used exclusively for personal family transportation and is parked outdoors in Florida. Nissan opened a warranty case on April 1, 2026. However, their entire investigation was conducted based on photos only — no physical inspection of the vehic
... le was ever performed by Nissan or their representative. Despite this, Nissan denied the warranty claim stating it was not their responsibility, without identifying the root cause of the damage. During the claims process, a Nissan call center representative provided false information stating that our vehicle had been in service for 6 months, which is completely untrue. Multiple calls went unanswered or were disconnected. Written emails were ignored. It took over 6 weeks to receive a final decision. This is a known recurring issue affecting multiple Nissan Pathfinder and Nissan Armada owners dating back to 2021-2022 models. Other owners have reported identical melting of front grille plastic components under normal driving and parking conditions. In at least one documented case, Nissan sent an engineer for physical inspection and immediately confirmed warranty coverage — contradicting their standard practice of denying claims based on photos only. The melting of exterior plastic components on a brand new vehicle raises serious concerns about the quality and heat resistance of materials used by Nissan, and potential safety hazards if the root cause is internal heat rather than external factors. I am requesting that NHTSA investigate this matter as it may indicate a manufacturing defect affecting multiple 2021-2025 Nissan Pathfinder and Armada vehicles.Read more

The rubber trim in the pillar trim flaps in the wind and I was told to buy the parts on Amazon and that it isn’t hard to install.
