What component or system failed or malfunctioned, and is it available for inspection upon request? The passive keyless entry/anti-lockout system on my 2024 Chevrolet Equinox EV failed. The vehicle locked all doors with the key fob still inside the cabin. The doors appeared unlocked (handles popped out) but would not open from the outside. The key fob and vehicle are available for inspection. How
... was your safety or the safety of others put at risk? This malfunction created a severe child safety hazard. If my daughter had been inside during warm weather, she could have been trapped in a rapidly heating vehicle with no way to escape. This posed a life-threatening risk. Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet. The problem occurred once unexpectedly, but I intend to raise it with my Chevrolet dealer for confirmation. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not yet. The incident was resolved without emergency services, but I am reporting it immediately to GM and NHTSA for investigation. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? There were no warning lamps, chimes, or alerts. The vehicle gave no indication that the fob was inside or that the system had locked incorrectly.Read more

My driver front door latch assembly failed at 42,153 miles. I had it inspected and repaired at the dealership. I had to pay out of pockets due to no longer being covered by the 3/36 warranty. The symptoms were that the driver front door would not lock at all. It took a while to figure this out as I just thought maybe we left it unlocked. Also this vehicle has the perimeter key fob so it automatic
... ally unlocks when you have the fob with you and attempting to open. This had been going in fine moths u til we figured out the problem and took it to the dealership. They did offer a discount since I purchased it there but they said due to the design it messed an entire assembly and was over $1k before the discount. They also had parts in stock and I did confirm they had seen this failure before. While we did not have an actual incident happen with us, our vehicle was vulnerable and we had no idea, because the lock appeared to work and it still moved visually, it just wasn’t actually doing anything inside the door. This is my wife’s vehicle so my fear was either someone breaking into the vehicle or even worse, attempting to enter the vehicle while she was inside but she would be unable to lock the doors. As mentioned, the dealer did repair the vehicle and it is worked properly for now. Thought we did have to pay out of pocket.Read more

Paint peeling and chipping will cause rust.
Incident date: Mar 3, 2025
Indeed, I recently encountered a concerning issue with my 2021 Toyota Venza. The problem manifested in mid-July, during the process of preparing to enter my vehicle. To my dismay, the door refused to open. Despite my repeated attempts to use the keyless entry system, the door stubbornly remained closed. Consequently, I was compelled to resort to using my key fob, which presented its own challenges
... . While I would not typically express such frustration, the safety implications of this issue are significant and have become quite inconvenient for me. As a physically disabled individual, I rely on a scooter lift in the rear of my vehicle to facilitate my mobility. This lift allows me to walk to the front of the car while holding onto the side for stability, preventing potential falls. On the day in question, I slipped my hand behind the handle of the door to open it and it didn't work. When I attempted to activate the unlock button on my key fob, it failed to respond. Despite my repeated attempts to press the button at least ten times, it remained unresponsive. By this point, my legs had become increasingly weak, to the extent that I felt a slight loss of balance when using the key. As a physically disabled individual and of an advanced age, I am unable to stand for extended periods. Therefore, this issue has caused me considerable stress and emotional distress. Upon visiting my local Toyota dealer, I was informed that the repair would incur a cost of approximately $800, which I unfortunately lack the financial means to cover. Consequently, I contacted Toyota’s 800 number to inquire about any special financial hardship programs that might assist me in paying for this repair. Regrettably, I was informed that such programs were not available. This situation has become increasingly frustrating for me. I find it difficult to reconcile the fact that Toyota has a history of encountering numerous issues with various of its models.Read more

I was at the gas station, in line waiting for one of the three gas pumps to be available. I put my car in reverse to back up and pull around another car in order to go to the open pump in front. When my car was in reverse motion, I stepped on the brake pedal, but the car did not stop. I stepped on the brake pedal numerous times, but the brake did not function. My car backed into the front of
... the car in line behind me damaging both vehicles. The automatic emergency braking system did not activate, and there was no rear collision warning sound or message. I switched my car from Reverse into Park to stop its movement, but it was too late to avoid the collision. I had a passenger in my car who corroborates this description. Fortunately there were no pedestrians behind my car, so the only damage was to the vehicles. My vehicle has not been inspected by anyone since the accident, but will be soon. I have owned this car since April 2021 and previously had a few similar situations when moving in the forward direction. Previously, the brake problem occurred while moving forward at very low speed pulling into parking places. The brake failed to activate, and I had to throw the car into Park to stop it, but this was the first time I had experienced this problem traveling in reverse. When this situation occurs, I am pressing on the brake pedal, but the car is not stopping and the brake pedal seems to go to the floor. On March 5, 2025 I took the car to a Lexus dealer and reported the brake failure problems. They inspected, road tested, and checked for codes, but could not identify the problem. I believe that the collision back up system should have prevented this accident, but it did not function. I suspect that there may be some kind of computer issue that prevents the brake from actuating. The car will be going in for repair soon, and I have requested that a complete diagnostic review be performed of this brake problem.Read more

In early May 2025 while driving around 40mph, I heard a very loud pop which startled me. Pulled over and looked back and noticed my backseats were push up out of place. I attempted to put the seat back into place but it wouldn’t fit into the latch anymore. The framing underneath the seats looked pretty damaged so I took it to a local mechanic. After doing some research, he found information for
... a check valve/fuel tank recall (Service Campaign 9B1) and then advised me to go a Hyundai dealership. I attempted to take it to the dealership, whom sent me away after I advised them of what happened. I came back the next day asking them to take a look at while informing them about the recall that could be related. Later that evening, they called and said the issue was directly related to the recall and the repairs would be covered. My car was in their possession for from 5.12.25 to 7.25.25 without any updates or instructions on what is happening. I was told they’re waiting to hear back from Hyundai Manufacturer on how to proceed. They returned my car to me on 7.25.25 stating everything was repaired. On 7.31.25 they called me back stating my tank needs to be replaced and bring the car back asap. My car was again being repaired and was returned to me on 8.25.25. Now my vehicle will not start randomly sometimes. At this point, I’m under the impression my vehicle is no longer safe as all the issues I’ve been facing are fuel related.Read more

Vehicle experienced a sudden no-start condition in July 2025 after being parked for ~2 weeks. Engine cranked briefly, then shut off and displayed a red immobilizer warning. OBD-II scans showed BCM memory faults (B22A9), immobilizer lockout (P0513), CAN-C bus communication errors, and repeated low-voltage codes. Dealer replaced PCM (~$1,550) but vehicle still inoperative. Dealer then determined BC
... M (Body Control Module, part #68141881AJ) failed, but part has been discontinued by Mopar and is unavailable nationwide. Used BCMs are VIN-locked, leaving no authorized repair path. Vehicle is also subject to NHTSA Recall 23V338000 (power liftgate module electrical short / parasitic drain). My vehicle showed repeated low-voltage events consistent with this defect, which likely corrupted the BCM and immobilizer system. Dealer confirmed recall parts are still backordered. As a result, a 2014 Jeep Cherokee with only ~32,000 miles has been rendered permanently inoperable due to discontinued safety-critical electronics. The defect creates a substantial safety risk because electrical shorts can occur while parked or driving, and owners are left without remedy.Read more

Hello. From what I understand, Ford has not come up with a solution for the exterior a--pillar applique trim clip attachments. I fully believe they have abandoned this issue as its not a brand new vehicle. Well, its new to me so it fully counts and should not be abandoned. I was on the highway in St. Louis, Missouri (I live right across the river in Illinois) and out of nowhere, something hit my
... car. I swerved and almost hit a driver to my left and after that, I almost got the guy in front of me. Now this caused a massive panic attack and soon after anxiety set in. I have horrible anxiety issues and I have the prescriptions to prove it. Its so bad, I can't fall asleep and so I take another prescrption for help getting to sleep. Anyway, I got off the closest ramp ands parked to see what happened. Sure enough, the passenger side exterior a-pillar is hanging there. That's the "something that hit the car and almost caused a multi car accident. I tried to call Ford about this but I'm left with ongoing ringing. Live chat agent had no idea what I was talking about. That set me off. I had taken a screenshot of the Kelly Blue Book website stating there was a recall, which, by the way, has been removed. Now that is extra suspicious. The KBB website stated notices were sent sometime earlier this year but I got nothing. The registration is in my name and is 100% accurate with my address and phone number are all up to date. I found no other way to contact Ford. I didn't want to file a complaint, but here we are. This is a SAFETY issue which caused me to fall into a panic and anxiety attack. I have a witness as my husband was in that passenger seat when it happened. Like I said, I, without a single doubt, believe that Ford Motors has abandoned this problem and that they don't care. This issue needs addressed now. I can't afford to go get it repaired myself as I am between jobs. If this isn't resolved, I will call my attorney for guidance.Read more

Because they have not sealed the screws on the interior side of the roof that hold the rails on top of the Jeep it now leaks since we have attached a kayak rack to those Jeep factory placed rails. Jeep is not willing to repair it and it requires tearing out the complete interior headliner to repair. After it rains the water sits in the door frame and leaks into the floor of the backseat causing mo
... lding issues (last good rain we got I ended up with an inch of water sitting in the back floorboard). It also collects in the front interior light fixture so when you drive and come to a stop there is water coming down all over the dashboard and the electrical/computer components of the dash. I called Jeep customer service and they won't repair it even though it is a VERY COMMON problem (per google search to try and see what the cause and repair entails, this is how I found out that the rail screws are the issue). Since it is a common problem and Jeep doesn't want to repair it and it is not only ruining my headliner and the carpet, but it also poses a risk when driving that it distracts as you try to sop it up as it's leaking from the light fixture as you drive and there's no other way to get it out without taking the whole inside of the Jeep apart.Read more

While driving my 2022 Hyundai Tucson at highway speed, the panoramic sunroof spontaneously shattered with a loud “explosion” sound. The glass shows a central hole with spiderweb cracking radiating outward, which is consistent with a manufacturing defect in tempered glass panels, not an im

This matches numerous reports of Hyundai panoramic sunroof failures and is similar to the pattern documented in NHTSA Investigation PE13-017. The vehicle has not yet been inspected by Hyundai but is available for inspection. Incident occurred on [XXX], while driving from Monroe, Louisiana, to Atlanta, GA, shortly after departure. On 08/31/2025, I was involved in an accident while driving my 2025 Hyundai Tucson. While attempting to change lanes at highway speed, the driver behind me failed to see my turn signal and collided with my vehicle. I believe the cause was the unusually low placement of the rear bumper-mounted turn signals, which creates a direct visibility flaw. The Tucson’s turn signals are mounted low on the r
... ear bumper instead of at eye level within the taillight assembly. This design makes them especially difficult to see in two critical conditions: Bright Sunlight: The low-mounted signals are easily washed out in daytime conditions, reducing visibility to following drivers. Low Light/Traffic Conditions: At night or in heavy traffic, larger vehicles and brake light glare further obscure the signals. In my case, the driver behind me could not see my intent to switch lanes, and despite my efforts to avoid a collision, the lack of visible signaling directly led to a frontal driver’s side impact. This design presents an ongoing highway safety hazard: Poor Visibility: Low placement reduces signal recognition in both bright and low light. Accident Risk: My collision demonstrates the real-world consequences of this flaw. Durability Concerns: Bumper-level placement exposes wiring to water, debris, and road wear, raising the risk of malfunction. Although the accident resulted in minor damage, the potential for severe outcomes at highway speeds is significant. I respectfully request NHTSA to investigate this design flaw and evaluate whether corrective action (recall, retrofit, or advisory) is warranted. UNKNOWN if Hyundai is currently addressing this issue, but concerns about rear blinker visibility have been widely reported among Tucson owners and auto safety forums.Read more
