The sunroof of the vehicle spontaneously shattered while driver was driving on I-5 South in Orange County, California. Driver’s safety was compromised due to the potential for glass to have entered the vehicle while vehicle was in motion. The safety of other motorists on the road was also compromised, as pieces of glass appear to have flown off the sunroof into traffic. The sudden shattering was
... also extremely loud and startling, which was also a safety hazard because driver could have panicked and lost control of vehicle. After calling to report the incident to Acura, owner was advised the vehicle’s sunroof has not been recalled nor is it under an extended warranty, despite evidence of a class action lawsuit against American Honda Motor Co., Inc. for related incidents in 2015 to 2021 vehicles. Per the lawsuit, which is a case out of California, the alleged defect in 2015 to 2021 model year Honda and Acura vehicles, referred to in the complaint as the “class vehicles,” puts drivers and their passengers in “serious danger,” as the sunroof explosions can cause “shards of glass to fly through their vehicles, sometimes while driving at high speeds,” and “the loud explosion and flying glass distract drivers and create a hazard to the people in the Class Vehicles and those around them.” The complaint alleges that the sunroofs in Honda and Acura vehicles are made with tempered glass that, because of the pressure created during the tempering process, has been known to “explode suddenly” if the outer layer is compromised. The lawsuit also claims Honda’s use of thin tempered glass for its sunroofs is “substandard, dangerous, and inadequate” given the material’s propensity to shatter or explode.Read more

Front Bumper Fitment Issue: The front bumper clips fail to secure the bumper properly, causing a noticeable gap between the bumper and the body, particularly around the headlight and fender. Despite visiting my local dealer today (10-11-2025), they have confirmed that this is a known issue, but no permanent solution has been provided. The bumper misalignment negatively impacts the aesthetic qualit
... y and structural integrity of the vehicle. This problem has been reported by multiple 2025 Camry owners and persists even after the bumper is replaced. It is evident upon delivery and has not been fixed by Toyota or the dealership to date. Rear Seatbelt Warning Malfunction: I experienced a malfunction with the rear seatbelt warning system. The warning alert activated while the vehicle is in motion, falsely indicating that an unbuckled passenger is in the backseat, even when the rear seats are empty. At my dealership visit in August, I was told that this was a normal characteristic of the vehicle, triggered when rear doors are opened or closed without the seatbelt being clicked in. However, this explanation does not account for the behavior I am experiencing, where the warning is erroneously triggered while the vehicle is moving. This malfunction undermines the reliability of Toyota's safety systems, and I request that Toyota investigate it thoroughly. I would also appreciate guidance on how to disable or adjust the rear seatbelt reminder in the meantime, if that is a viable solution.Read more

Both B-pillar camera housings on my 2023 Tesla Model Y Performance become extremely hot even when the vehicle is parked in a closed garage and not in use. On the morning of the incident, the garage temperature was approximately 78°F. The car had been parked overnight for over 12 hours, plugged in, and charging to 80%. While the vehicle was preconditioning, no defrost features were active. Despite
... this, both pillar surfaces measured 156°F with an infrared thermometer—hot enough to cause skin burns within seconds according to standard burn-threshold data for human skin (~155°F). Both sides heating to this temperature simultaneously suggests an electrical or control malfunction keeping the pillar camera assemblies energized. These components sit near the seatbelt anchor and side-curtain airbag area, raising concern of fire, electrical, or occupant-safety risk if the condition persists. Continuous heating while parked could also degrade wiring insulation or camera electronics used by the vehicle’s driver-assistance systems. The vehicle and affected components are available for inspection upon request. The issue has not yet been inspected or confirmed by Tesla, but a service appointment request was submitted immediately after the event. Tesla repeatedly rescheduled the appointment, delaying it by more than six weeks despite the described safety concern. No diagnostic alerts, warning messages, or notifications appeared in the vehicle or Tesla app. This abnormal heating has occurred more than once under similar conditions and does not appear to be caused by ambient temperature or sunlight exposure.Read more

The sunroof on my 2018 Ford Expedition is leaking at multiple points and has become stuck and inoperable. The dealership advised that the brackets are broken and the entire sunroof assembly must be replaced. The leak occurs in several areas across the Vista Roof, causing water to enter the cabin. This appears to be related to known design defects already referenced in multiple Ford Technical Servi
... ce Bulletins (TSB 18-2374, TSB 19-2374, and TSB 21-2292). Details: I purchased this 2018 Ford Expedition on August 17, 2024, from Battlefield Ford in Culpeper, Virginia, where it underwent a Ford Blue Warranty inspection before sale. The vehicle had approximately 98,000 miles at purchase and now has around 109,000 miles. Shortly after purchase, I noticed water intrusion from the sunroof area. The vehicle was inspected at Sheridan Ford in Smyrna, Delaware, where technicians confirmed leaks at four separate points, broken sunroof brackets, and recommended complete sunroof replacement. In addition to the leaks, the sunroof is stuck and will not operate properly. Ford customer service was contacted, but they stated that no assistance or goodwill coverage would be provided. I find this unacceptable given the severity of the issue, the existence of related TSBs, and the potential safety hazards associated with electrical and visibility impairment due to water intrusion. The vehicle also required brake replacement shortly after purchase, despite being reported as low 3 mm during the Blue Warranty inspection, which raises additional concerns about the adequacy of Ford’s inspection process and potential oversight of critical components. Safety Concerns: Water intrusion may affect electrical components, airbags, or roof-mounted sensors, increasing the risk of malfunction or short circuits. The sunroof’s inability to close or seal properly could result in driver distraction, loss of visibility, or mold and corrosion inside the cabin.Read more

I am writing to formally report a critical safety incident involving my 2020 Tesla Model 3, where the bumper detached while driving through a puddle of water on Oct 9th, 2025. This occurred on a busy street, and fortunately, another driver alerted me just in time, averting a potentially serious accident. Following the incident, I arranged for my vehicle to be towed to a Tesla-authorized collisio
... n center and opened a claim with my insurance company. I have since learned that this issue is recognized internally by Tesla, as they have created a service incident regarding it, but have failed to communicate this to customers effectively. Tesla's lack of accountability is concerning. Despite repeated attempts to address this with their support and service teams, they have deflected responsibility and continue to downplay the seriousness of this matter, insisting I must rely on insurance to cover the costs. This situation raises significant safety concerns, not just for myself but for all Tesla Model 3 drivers. I urge NHTSA to take the following actions: 1. Investigate this safety issue regarding the bumper detachment on Tesla Model 3 vehicles. 2. Pressure Tesla to publicly acknowledge this known issue and take immediate corrective action to prevent future occurrences. 3. Ensure that affected customers are compensated for damages and repairs due to this defect. I appreciate your attention to this serious safety issue and look forward to seeing NHTSA hold Tesla accountable to protect all consumers and ensure vehicle safety. I have attached proof of diligent efforts on my part to get this resolved: - Claim number with Progressive - Communication with Tesla giving them proof of this issue - Earlier NHTSA bulletin acknowledging the issue and the Tesla internal service incident created for this issue. While driving, the check engine light came on. Soon after, the rear floor of my 2021 Kia K5 lifted upward, and I noticed the fuel tank underneath had swollen. The car is now unsafe to drive, and the rear seat cannot be used because the floor is deformed. I
... discovered that Kia has a Voluntary Emissions Service Campaign SC288 addressing this exact issue (fuel tank swelling and excessive pressure due to a faulty PCSV valve and EVAP system), but this is clearly a safety hazard, not just an emissions problem. concerns regarding my 2010 Ford Escape. Within thirty (30) days of purchase, the transmission completely failed, rendering the vehicle inoperable. Additionally, upon further inspection, it was discovered that the entire underbody of the vehicle is severely rusted and corroded. This corrosion appears extensive enough to compromise the structural integrity of the frame and essential safety component
... s. While operating the vehicle prior to transmission failure, I also experienced significant shaking and vibration during both driving and braking. These issues raise major safety concerns, as they could indicate underlying brake, suspension, or frame instability. Given the combination of a failed transmission, extensive underbody rust, and unsafe driving behavior, I believe this vehicle may present a serious risk to myself and others on the road. These types of defects and corrosion levels are unusual for a vehicle of this make and model and could point to a broader safety or manufacturing issue. I am requesting that Ford review this matter urgently and provide direction on possible inspection, repair, or compensation options. The first critical component was reported in my complaint tracking number 11459461 submitted on 04/03/2022. The component in that filing was the engine pan that supported the engine. The passenger-side front bumper and fender liner detached during a 15 mph swerve to avoid a traffic cone on day 3 of ownership. There was a light graze with no significant impact, but approximately 2 miles later, a noise prompted a safe pull-over. The component (bumper assembly and liner clips) failed completely, with clean detachment and a missing OEM clip. The vehicle and component are available

Their report falsely claimed the vehicle was taken before inspection. No manufacturer, or insurance inspection has occurred. No warning lamps, messages, or symptoms appeared prior to the failure.
