In early 2024, we learned our 2011 Ford Explorer was under open recall (NHTSA #24V031) for the A-pillar windshield trim, which could detach and create a road hazard. For two years, our Ford dealer said repairs could not be done due to parts availability. The A-pillar trim detached three separate times, including once at 75 mph on a highway, causing fear and danger. The vehicle’s elderly owner st

I am submitting this complaint regarding a possible airbag/SRS system failure involving a Hyundai vehicle. On May 26, 2026, the vehicle was involved in a severe front-end collision resulting in major structural damage to the front end, including the hood, bumper reinforcement, radiator support, and engine compartment. Despite the significant impact damage, none of the airbags deployed, including t
... he driver or passenger frontal airbags. The collision was serious enough that the occupants could have suffered severe injury or death. Based on the extent of the damage, I believe the airbag system, crash sensors, or airbag control module may have malfunctioned or failed to operate properly. Photos documenting the damage are available. I am requesting that NHTSA investigate this incident for potential safety defects involving: * Airbag non-deployment * Crash sensor failure * SRS system malfunction * Airbag control module defects I am also requesting review of whether similar complaints or investigations exist involving this Hyundai model or related vehicles.Read more

The message that appears when we try to automatically open the door is that we should first unlock the doors to which we already have. 1. The failed component was the panoramic sunroof glass. The glass spontaneously shattered and blew inward into the cabin while the vehicle was in normal operation. Yes — the vehicle and the remaining glass fragments are available for inspection upon request. 2.How was your safety or the safety of others put at risk? The failure created an immediate and severe safety hazard. When the sunroof exp
... loded inward, sharp glass fragments fell directly onto the driver and all passengers, including their heads, faces, and upper bodies. This posed a risk of: Lacerations Eye injuries Loss of driver control due to the sudden explosion Startle reaction that could have caused a crash with surrounding traffic Everyone in the vehicle was exposed to falling glass and the shock of the explosion. 3.Has the problem been reproduced or confirmed by a dealer or independent service center? At this time, the issue has not been reproduced, but the dealer inspection confirmed that the sunroof glass was shattered and had blown inward. The dealer acknowledged the failure but has not identified a cause. 4. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? There were no warning lights, messages, noises, cracks, or visible defects before the sunroof exploded. The failure occurred suddenly and without any prior symptoms.Read more

On May 26, 2026, my 2019 Ford Expedition suffered a total systemic safety failure that resulted in my toddler being locked inside the vehicle in direct sunlight, requiring an emergency extraction. I used the FordPass app to unlock the vehicle and opened the driver’s door and passenger door on the diver's side. I placed my bag containing the proximity key fob onto the driver’s seat floorboard w
... hile the vehicle was completely off. I closed the driver's door and walked to the rear door behind the driver's seat, which was still open, to buckle my child into his car seat. Upon closing his door, the vehicle immediately initialized a "ghost lock" command, sealing all doors with the key fob inside, directly violating the vehicle's "Smart Unlock" and Passive Entry Passive Start (PEPS) safety protocols. The FordPass remote application then completely failed, refusing to process any commands to unlock the vehicle or remote-start the engine to turn on the air conditioning for the child. Even after uninstalling and reinstalling the application via a good samaritan’s cellular hotspot, the app remained completely unresponsive due to a total communication crash with the vehicle. I called 911. Local police arrived on-site first and attempted an emergency manual override using a mechanical lockout tool down the window frame, which failed. Due to the rapidly increasing interior heat in the direct Florida sun, arriving firefighters determined the child was in critical danger and shattered the rear passenger door main window to perform an immediate extraction. This failure represents an unmitigated malfunction of the door latch systems documented under Ford TSB 20-2421, which overrode all safety failsafes without warning. An upcoming diagnostic testing appointment is being scheduled at an authorized Ford dealer to pull the module codes and initiate a manufacturer safety review.Read more

The power liftgate and pinch sensor system on my 2024 Toyota Sequoia Platinum continues to malfunction intermittently and creates an ongoing safety concern. The liftgate frequently fails to close using the electronic controls and instead emits two warning beeps before becoming completely unresponsive, forcing the liftgate to be manually pushed shut. The pinch sensor system appears to falsely dete
... ct obstructions when none are present, causing unreliable and inconsistent operation of the rear liftgate. Because this is a powered safety-related system, the intermittent failure raises serious concerns regarding occupant safety, system reliability, and the ability to properly secure the rear hatch while the vehicle is in operation. This issue has persisted despite dealership involvement and does not appear to be an isolated incident. Multiple owners have reported similar failures involving the Sequoia liftgate and pinch sensor system. Toyota has not provided a permanent corrective action. For a vehicle at this price point, repeated failures of a powered liftgate safety system are unacceptable. The defect creates the potential for injury, improper liftgate closure, and unsafe vehicle operation. I am requesting that NHTSA investigate this issue for a broader defect pattern and determine whether a recall or manufacturer-required corrective action is warranted.Read more

While driving at highway speeds, the factory sunroof glass panel of my brand-new Honda CRV spontaneously exploded outward with a loud bang, sounding like a gunshot. The vehicle was subsequently brought to an authorized Honda inspector (Farrish Auto Body) for a technical teardown and inspection. The certified estimator formally determined and documented that there was absolutely no evidence of an external impact point, rock strike, or road debris damage, confirming a spontaneous structural glass failure due to a manufacturing or assembly defect. The vehicle is currently un-drivable due to the missing roof panel and structural hazard, and the manufacturer (American Honda Corporate Case open) and purchasing dealership (DARCARS Honda of Fairfax) have delayed providing a resolution or safe temporary transportation. Vehicle: 2016 Toyota RAV4 Color: Factory Super White / Blizzard Pearl (whichever applies) I am filing a complaint regarding a well-known latent manufacturing defect involving peeling factory paint on my 2016 Toyota RAV4. The original factory-applied white paint is peeling and delaminating from the exterior metal body panels under normal use and maintenance conditions. The paint failure appears un
... related to any accident, impact, misuse, neglect, or environmental damage. The failed component is the factory-applied exterior paint and coating system on the vehicle’s metal body panels. The affected areas are available for inspection upon request. The paint is separating from the primer/base layer and peeling off in sheets, exposing underlying surfaces and creating risk of corrosion and reduced vehicle value. Toyota has publicly acknowledged widespread issues involving “Super White (040)” and “Blizzard Pearl (070)” paint on numerous Toyota models, including RAV4 vehicles, through Customer Support Program ZKG and multiple Technical Service Bulletins. Toyota documents specifically state the issue involves paint adhesion failure during the manufacturing process causing peeling on exterior metal body panels. This issue creates safety concerns because exposed metal surfaces may eventually corrode or rust, reducing long-term structural integrity and potentially creating visibility distractions while driving as paint continues to separate and peel unexpectedly. Large peeling sections can also detach while driving and create roadway debris hazards for other motorists. The condition has been widely reported by consumers across online forums, complaint databases, and dealerships for many years. Numerous owners of white Toyota vehicles have reported identical failures involving factory paint peeling and delamination. The problem has been visually confirmed by dealership personnel / independent body shop personnel (modify if applicable). And they have refused to cover it at allRead more
