The second-row seat rails on my 2025 Toyota Sienna were manufactured with improper welds that can fail in a high-speed collision, potentially causing the seats to lose structural integrity and increasing the risk of severe injury. Safety Risk and Hardship: Due to this defect, Toyota has recommended that no passengers sit in the second row and no child restraints be used in these seats until a rem
... edy is performed. I have been awaiting a remedy for over 70 days, during which time the vehicle's primary utility as a high-capacity family transport has been compromised. This creates a severe accessibility and safety hardship, as I am unable to safely transport my family or use child safety seats in the designated locations. Forcing passengers into the third row, which does not accommodate three carseats side-by-side, is not a safe long-term alternative for our family, especially as at [XXX] pregnant, and soon to be postpartum with a XXX and two XXX in carseats. Reproducibility and Inspection: The safety defect was confirmed by the manufacturer (Recall numbers 25TB12 and 25TA12) and by my local authorized dealer upon notification. While the vehicle is available for inspection, the manufacturer has acknowledged the defect is due to incorrect weld machine settings at the factory. Symptoms and Warnings: There were no warning lamps or messages prior to the recall notification. The issue was discovered by the manufacturer during internal testing between July and August 2024 and publicly announced in October 2025. The color of my Prius 2010 starts with 070, blizzard white pearl. The body started peeling. I have visited Toyota dealer on 12/29/25 with this issue and got to know that this issue is well known but the service campaign was finished around June, 2025. However, it seems the issue is getting worse day by day. I did not receive any notice from Toyota about any campaign about this issue. My Toyota dealer recommended me to reach out to Toyota if any courtesy repair can be done on my vehicle. I reached out to Toyota, but seems to me they cannot do much. Date: December 28, 2025 Time: Approximately 3:25 PM Vehicle: 2026 Tesla Model Y Incident Location: Southbound on the I-35N feeder road, just north of Highway 114. Safe Harbor Location: Burger King (15925 N Hwy 114, Northlake, TX) Statement of Facts: I was driving my 2026 Tesla Model Y southbound on the I-35N feeder road at approximately 3:25 PM today. I was traveling at a steady speed of 55 MPH. W
... ithout any warning chime or dashboard notification, the front hood (frunk) suddenly and violently flew open. The hood slammed into the windshield, shattering the glass across my entire field of vision and completely obstructing the road ahead. I was forced to perform an emergency "blind" stop on the feeder road. Once the vehicle was under control, I drove at approximately 10 MPH with zero forward visibility to reach a safe location off the main road. I pulled into the Burger King parking lot located at the intersection of I-35 and Highway 114 to inspect the damage and call for assistance. Key Safety Failures: • Software Failure: At no point before the hood flew open did the vehicle alert me that the frunk was unlatched. • Mechanical Failure: The latching mechanism failed to hold at standard highway speeds, creating a life-threatening visibility hazard. • Structural Damage: The windshield is fully shattered, and the hood and hinges are visibly warped from the force of the impact. Conclusion: This incident was a catastrophic safety failure. Given Tesla's history of recalls for this exact issue (NHTSA Recall 24V-554), it is clear that this 2026 model either suffers from the same defect or the previous "software fix" is insufficient to protect drivers.Read more

With this 2025 Lexus NX350 Premium AWD, the front windshield glass is BADLY distorted. Looking at the road ahead while seated behind the steering wheel, objects aren't clear, they tend to be wobbly as there is a MAJOR safety issue here with the glass itself. On December 24, 2025, the contact's 2023 Chevrolet Malibu was involved in a severe side-impact collision. The vehicle was struck on the rear passenger side by a full-size pickup truck traveling at high speed (est. 60 mph) that failed to stop at a red light. The impact caused the contact's vehicle to spin approximately 180 degrees and strike a concrete median sideways, generating high lateral G-for
... ces. FAILURE 1: AIRBAG NON-DEPLOYMENT. Despite the severity of the impact and the violent spin into a curb (rollover trip point), the side curtain airbags and front seat-mounted side impact airbags failed to deploy. The failure of the restraint system caused the driver to be thrown against the driver-side door structure, striking their head on the window and B-pillar. The driver sustained a concussion, cervical strain, and lumbar strain. Medical records confirm the injuries are consistent with impact against the vehicle interior due to non-deployment. FAILURE 2: EMERGENCY TELEMATICS MALFUNCTION. Immediately post-collision, the driver attempted to manually activate the OnStar/SOS emergency buttons. The system failed to connect to an advisor. Instead, the vehicle's infotainment screen malfunctioned, opening a voice-to-text transcription application. The system then bypassed the vehicle's internal telematics module and attempted to route the emergency call to the owner's personal cell phone, indicating a complete failure of the vehicle's emergency communication hardware. This resulted in a 34-minute delay in coordinating professional assistance. Contact notes; while this vehicle currently shows 0 open recalls, the failure mechanism (sensor non-response during side impact) mimics the defect described in NHTSA Campaign 22V923000 (GM Recall N222392300), where improper welding of the front impact bar causes crash sensors to fail. The vehicle is available for inspection. Police and medical reports confirm the incident details. No warning lights were illuminated prior to crash.Read more

We are reporting two safety-related concerns involving a new 2026 INFINITI QX60 Sport. At delivery, a linear abrasion approximately 42 inches in length was present across the front roof panel and was not disclosed prior to sale. When this condition was raised, the dealership dismissed it by text message without performing a physical inspection. At a later date, the dealership applied a coating or
... protection product over the roof panel despite the presence of visible damage and potential surface contamination. Due to the length, location, and linear nature of the abrasion across a primary roof panel, we are concerned the damage may be consistent with transport-related strap or tie-down contact, raising questions about panel integrity, corrosion protection, and the vehicle’s condition as represented at sale. We are also reporting a child safety equipment concern. The dealership applied a chemical interior or fabric protection product while child safety seats and/or booster seats remained installed in the vehicle. The seats were not removed, and no warning, consent, or disclosure was provided. Child restraint systems are federally regulated safety devices, and chemical exposure — including overspray or vapor — may affect harness webbing, foam, or material performance in a manner not evaluated under FMVSS requirements. We submit these concerns in good faith for NHTSA’s safety review and tracking.Read more

I am submitting this complaint to report a dangerous and repeatable safety defect involving my Nissan Altima that caused me injury and resulted in vehicle damage. On December 16, 2025, I parked my vehicle on an incline and shifted it into Park. The gear selector and dashboard indicator both confirmed that the vehicle was in Park. I exited the vehicle and was not applying the brake. While the engin
... e was running, the vehicle remained stationary. However, immediately after I turned the engine off, the vehicle began to roll backward downhill at approximately 5–7 mph without any throttle input or action on my part. As the vehicle rolled, the passenger door struck me and pulled me down the hill, causing injury. My vehicle was also damaged and is currently at a repair facility. Later that same day, when a tow truck driver arrived to recover the vehicle, the exact same failure occurred again. The vehicle was placed in Park, and once the engine was turned off, it again rolled downhill unexpectedly. This occurred in front of the tow truck driver and confirms that this issue is repeatable and not an isolated incident. At no point did the vehicle roll while the engine was running, even though I was out of the vehicle and not applying the brake. The vehicle only moved after the engine was turned off, which indicates that it was not being held by a fully engaged Park mechanism. A vehicle placed in Park should remain stationary once turned off, regardless of incline. The failure of my vehicle to do so created an unattended roll-away hazard that directly caused my injury and posed a serious risk of severe injury or death to me and others.Read more

The factory-installed roof rails on my 2019 Nissan Rogue are severely corroding and structurally deteriorating. The metal is visibly oxidizing, flaking, and breaking apart. Portions of the roof rail appear to be separating and losing structural integrity. This corrosion is occurring under normal use and typical Midwest weather exposure, with no misuse, accidents, or modifications. The vehicle has
... not been exposed to extreme environments or chemicals that would reasonably cause this level of degradation. Safety Concern: The roof rails are a structural exterior component designed to secure roof racks and cargo. Due to the corrosion and material failure, there is a significant safety risk that: Roof-mounted cargo or crossbars could detach while driving Metal fragments could break off and become road debris The failure could occur at highway speeds, posing a danger to other vehicles and pedestrians This issue represents a potential design, material, or manufacturing defect rather than normal wear. Corrosion of this severity should not occur on a relatively modern vehicle. I am concerned this may affect other Nissan Rogue vehicles of similar model years and warrants investigation. Photos documenting the corrosion and deterioration are attached. Requested Action: I am requesting that NHTSA investigate this issue for potential safety defects related to roof rail corrosion and structural failure on the 2019 Nissan Rogue. While driving our 2022 Kia Telluride under normal operating conditions, the panoramic sunroof glass suddenly and spontaneously shattered outward with a loud explosion-like noise. The sunroof was fully closed at the time of the explosion. There was no impact with any object, no debris strike, no road hazard, and no external cause observed. Weather conditions were clear, traffic was normal, and the
... vehicle was traveling at a normal speed. The failure occurred without warning. There were no prior cracks, chips, noises, warning lights, or messages before the incident. A large hole immediately formed in the sunroof glass, and glass fragments fell into the vehicle and were caught by the shade. The sudden shattering and loud noise scared the driver and passengers and created an immediate safety hazard, requiring us to pull over right away. Occupants were placed at risk of injury from falling glass and sudden driver distraction. For safety reasons, loose and hanging glass was removed after stopping the vehicle. At the time of this report, the vehicle has not yet been inspected by a Kia dealer, manufacturer representative, or independent service center, as the incident occurred recently (we have an appointment in two days). The remaining sunroof components and vehicle are available for inspection upon request. This appears to be a sudden and spontaneous glass failure occurring during normal vehicle operation.Read more

Dear National Highway Traffic Safety Administration (NHTSA),I am submitting this report to express a serious safety concern with my 2015 Buick Verano, which I believe is unsafe for shorter drivers due to poor visibility and design limitations.I received this car as a gift from my father, but the moment I drove it off the lot, I realized I could not safely operate it. I am 4 feet 11 inches tall, an
... d this vehicle seems designed for much taller drivers. The windshield is too close, and there are several significant blind spots that make it extremely difficult to see vehicles, pedestrians, and obstacles around me.To try to fix this issue, we installed four extra mirrors and I used a booster seat, but these adjustments did not resolve the safety problems. I have experienced numerous close calls, including one incident where I nearly struck a pedestrian I could not see until he moved out of the way. Driving this car has become extremely stressful and frightening due to these visibility issues.I have spoken with several friends who are also under 5 feet 3 inches tall, and they reported the same difficulties when driving a Buick Verano. It appears that this model’s visibility design creates unsafe driving conditions for shorter individuals.I request that the NHTSA investigate this design concern to determine whether the 2015 Buick Verano meets safety standards for drivers of different heights, and whether design changes or warnings are necessary to protect shorter drivers.Thank you for your attention to this important safety issue.thank you.Read more

While traveling at approx 65 mph on a clear, debris-free Alaska highway in -5°F temperatures, the panoramic sunroof of my 2024 Kia Sportage spontaneously shattered with an explosive sound. There were no vehicles in the immediate vicinity (nearest car was maybe 50 to 75 ft away) and no observation of flying debris or falling objects from overpasses. The failure appears to be a spontaneous glass b
... reakage (SGB) incident potentially caused by thermal stress or manufacturing defect rather than external impact. Crucially, some of the remaining glass fragments and one large attached piece are displaced and flexed OUTWARD (upward toward the sky). There are also fracture lines in the remaining glass along and parallel to the sides of the vehicle as well as parallel to the front frame of the glass. They do not radiate out from a central point as one would expect from an object strike. This geometry to me indicates internal tension or cabin pressure release, as an external impact from a projectile would have forced the glass inward into the cabin. Additionally, there was very little glass in the vehicle cabin, certainly far less than one would expect given the size of the opening in the sunroof and the amount of glass missing. I have personally previously been in a vehicle whose side window was blown out from a vehicle kicked rock strike and in that case was showered with a lot of glass. Yesterdays catastrophic failure resulted in some glass intrusion into the passenger cabin at highway speeds, creating a severe distraction with sound was loud & sudden enough to potentially cause a loss of vehicle control. I am aware of similar reports regarding the 2023-2024 Sportage (and previous Kia panoramic sunroof class actions) regarding the use of ceramic glazing or assembly stresses that contribute to such failures. I request an investigation into whether the current model's panoramic roof assembly is capable of withstanding extreme thermal differentials and body flex.Read more
