I am filing a safety defect complaint regarding my 2017 ford explorer. The B pillar has broke in half a couple of years ago and nearly struck a vehicle behind me and if the remaining trim piece detaches, it could become a serious risk of collision, injury or property damage. it states NHTSA works to prevent unsafe traveling which is why I'm writing this. ... dealerships multiple times with no luck of anything. I have had to continuously tape the area to door which is not ordeal nor a forever fix. The b pillar is held on by wires that are slowly breaking. The wires lead to a hole that is inside my door. the opening exposes from the outside, meaning if it rains or snows, it is getting inside the hole, into my car door. If the recall piece ever came in, my door has already experienced neglect and issues. I'm concerned this will lead to corrosion, electrical complications and additional mechanical failures. Also, if it is not taped up correctly, air blows directly into the car door, into the cabin while driving onto my feet causing extreme cold exposure and physical harm and making it hard to operate the vehicle. It's very painful and also not ideal, especially when I also have a full time job and 3 kids to worry about. I currently owe 12k still on the vehicle and want to replace it with a safer vehicle. I should not have to be miserable every single day and put me and my kids in danger and also let my vehicle deteriorate while i wait for this recall piece on top of having to pay the remaining balance of this vehicle. I respectfully request that nhtsa review this complaint and investigate before anymore physical, emotional, financially harm is done.Read more

The driver seatbelt was soaking wet after a rainstorm. The vehicle does not have a sunroof. Upon further inspection the airbag and its electronics were also soaked in water. ... e the FSD system was actively controlling the vehicle. Following the collision, I requested the vehicle operational and FSD engagement data from Tesla for the incident timeframe. Tesla provided a CSV dataset; however, the FSD engagement and autonomy decision-layer data were almost entirely missing, despite the vehicle being in motion and presumably under FSD control. On follow-up requests, Tesla stated they are unable to provide additional autonomy data and that “Tesla does not collect all your vehicle data,” despite marketing the system as Full Self-Driving and collecting extensive telemetry. This raises a safety concern because: 1.A collision occurred during FSD operation. 2.Tesla is not providing complete autonomy data for safety assessment. 3.There appears to be no transparency into FSD decision-making, object detection, or control authority at the time of impact. 4.Owners, insurers, and potentially regulators cannot review how FSD behaved during a safety-related event. I am submitting this complaint so NHTSA is aware that: (1) A collision occurred under FSD control, and (2) Tesla refused complete operational autonomy logs for evaluation.Read more

I have come across a forum where a person states: "So can confirm 1st row passenger door does not provide any warning as well. Looking at the manual, I see that there is supposed to be a warning sound when any door/hood/liftgate is open while driving, as quoted below. If someone else confirms this is normal behaviour, then I think we should file a safety defect complaint with the NHTSA and Transport Canada. “Door-ajar Warning Light/Liftgate-open Warning Light/Hood-open Warning Light The light turns on if any door/liftgate/hood is not closed securely. In addition, the warning sound is activated if any door/trunk lid/liftgate is not closed and the vehicle is driven. Close the door/liftgate/hood securely.” This was a very scary situation. The vehicle was sold with the wrong size battery that was not properly secured. This caused electrical instability, warning lights, and vehicle failure. The battery was not adequately supporting the vehicle’s electrical system. The airbag and restraint system showed warnings caused by third-party wiring, indicating potential airbag or seatbelt malfunction. The fuel system was defective. The gas cap was broken and required tools to refuel the vehicle. Structural and body components were unsafe. A bumper bracket was loose and flapped while driving, and the passenger seat was unstable. Accessibility systems were also defective, including lift and door malfunctions affecting safe operation. United Access was notified immediately but did not correct these safety defects. I reported the first incident to General Motors (GM) on May 15, 2024, but did not receive a response. On Sept 22, 2025, I reported the second incident that occurred on Sept 2, 2025, to GM, at which time they asked several questions and requested photos of my vehicle, which I provided. On Sept 22, 2025, GM requested I take my vehicle to my dealership, J.B.A. Chevrolet, for inspection. I took my vehicle to JBA on Sept 29, 2025, and was told that GM wanted photos of my vehicle, as well as photos of another 2024 Trax on the Lot for comparison. At that time, JBA had to wait for another vehicle to come in. Unknown if JBA or GM attempted to reproduce the issue during an inspection. On Oct 24, 2025, GM telephoned and issued a final determination, which I requested in writing and received on Nov 11, 2025. GM stated that because the design was consistent across Trax vehicles, and based on the facts provided, they were unable to assume responsibility for damages and suggested I resolve the issue with my insurance company. I did not ask for damages, I asked for corrective measures to avoid future injuries. I submitted a request for reconsideration on Dec 2, 2025. On Dec 16, 2025, a totally different GM office contacted me, which did not yield any results. Ford has a glue on heat shield under bed which fell off. Exhaust gets so hot it caught on fire. They should have put a metal fixed plate or something fixed no a cheap glue on piece of foil. The 2026 toyota suddenly accelerated when pulling into a handicap space. Suddenly the vehicle jumped the concrete barrier and sidewalk and hit a building. The airbags did not deploy and the seat belts came off as normal. The building was not damaged but my wife sustained a shoulder injury and whiplash. We decided to hire a person to download the black box and give us the results of the download. This is to occur on January 20, 2026. There were no warning indicators prior to the sudden acceleration. Body / Liftgate The rear hatch unexpectedly dropped and fell on my head while the vehicle was stationary. The right-side rear hatch gas strut separated from the body at the mounting point. ... ad and upper-body injury. The failure appears related to structural stress at the strut mounting locations, not misuse. Seat Belt – Rear Center Passenger The rear center passenger seat belt upper cover can be easily pulled off by passengers, which breaks the plastic retaining tab. When the tab breaks, plastic fragments fall into the seat belt mechanism, causing the belt to jam and fail to retract properly. This results in impaired seat belt function, which is a safety concern for rear passengers. The vehicle has not been modified, and the failure occurred during normal use. Vehicle mileage at time of incident was approximately 150,000 miles. The rear center passenger seat belt issue has been present since the vehicle was new. The rear center passenger seat belt mechanism was replaced twice by the dealer during the warranty period, with one replacement performed as a goodwill repair after Toyota Corporate declined coverage. Despite these repairs, the same issue reoccurred, indicating a persistent design-related vulnerability rather than isolated misuse. Seems to be an issue with the rear hatch and the locking system. Without any recent rain or snow the locking system will get frozen and not close correctly or open correctly. We though the hatch was closed but it popped open while backing up. We had taken the car to the dealership. The issue has occured twice.

All of my ADAS systems were disabled due to a water leak. The repair required the dealership to hire a "leak doctor" and then replace a module in the rear tailgate. ... ce purchasing the car, I have experienced consistent time consuming and serious issues that I believe indicate defects in manufacturing or design. First, the thermostat had failed during the first winter season I owned the Elantra, despite replacement the vehicle’s heating system still does not function properly. I’m not a fan of the cold. Also, the engine burns oil at an extreme rate. I am required to add oil literally every 2–3 days to prevent the engine from running dry, which presents a serious risk of engine failure and potential safety hazards while driving. And is also costly as oil is not cheap. In addition, the vehicle’s headlights developed spiderweb pattern cracking and shattered from the inside after approximately nine months of use, without any external damage. The paint has chipped across the vehicle, including in door jambs and other protected areas, suggesting poor paint quality. And recently, the lower engine underbody cover has completely broken down and disintegrated over time due to clips constantly falling out. Given the age of the vehicle and the normal use, the amount and severity of these issues are unacceptable. These defects have caused significant financial burden and raise serious concerns on vehicle safety and manufacturing quality. I am requesting that this complaint be reviewed for potential defects or recalls. The car has been taken care of in a manner that exceeds the manual recommendations including always flushing the underside of the chassis during washes; especially the winter season. The car was taken to a dealer to have the intake manifold gasket looked at. The dealer took the opportunity to raise the car in the air, conduct an inspection and reported that the front carriage assembly is rusting away and considered dangerous. The Service Manager indicated that the model years 2010 to 2014 exhibit a high-degree of corrosion to the supportive member which holds the engine and transmission in place and if it fails could cause an extremely dangerous situation. What if any information has NHTSA compiled regarding these issues and Subaru's use of an under-designed, defectively manufactured part that could potentially cause a serious accident if the support structure collapsed? And if the NHTSA has conducted any research into this issue is there any option for a consumer to recoup or require Subaru to cover the cost of replacement which according to two Subaru Dealers is approximately $4,200.
