Tailgate was dropping open while driving, and sometimes would not latch when shut without slamming it several times. Took it to Gupton Dodge Dealership 37172 and they performed the recall. A few months later the same problems started occurring again, then after a while longer the tailgate actually would not open at all anymore. I took it back to the same dealership to notify them that their rec
... all repair failed. They told me the recall had already been done and it was working when it left their shop, so I would need to pay for any further repairs which would require replacing the entire actuator assembly. I brought the truck back home and removed the tailgate latch cover to assess the problem myself. I found that the original actuator was still on and had never been replaced in the recall work. I also found that by cleaning up the metal actuator parts with carb cleaner spray, and lubing it with dry lube and white lithium grease at critical movement spots, the actuator and tailgate latches started working perfectly again. I now realize that they very likely did the same thing for the recall, just cleaned and lubed it, and no parts replaced or added. I expect it will now work for another few more months before dust and moisture will cause it to need cleaning and lubing again in order to keep it working. Serious safety hazard, not just from dropped cargo! My wife closed tailgate once while my 6 year old grandson was standing next to her. As soon as she stepped away the heavy tailgate fell open hard, missing my grandson's head by inches. Dealers are just doing a quick, cheap, temporary fix to say they met their obligation, and are perpetuating the danger. Even if the actuator assembly is replaced with the OEM part, it will also fail from normal dust & moisture over time. The recall needs to require the actuator assembly to be REPLACED with an IMPROVED DESIGN that will not fail from normal dust and moisture, since it is not in a sealed compartment.Read more

I am reporting a safety concern involving multiple loose windshield header bolts and cowl panel fasteners on my 2022 Mazda CX-9. The issue was discovered on 1/28/26 at approximately 44,000 miles. Several bolts securing the windshield header and cowl panel area were found loose. These components support parts located at the base of the windshield. Loose fasteners in this area raise concern about p
... otential windshield instability, movement of the cowl panel, and possible detachment or structural compromise over time. This condition could create a safety risk, particularly at highway speeds, including driver distraction from vibration or noise, potential windshield instability, and possible failure of components mounted near the base of the windshield. The Mazda dealer is aware of this issue. I was informed that they have repaired at least 15 similar vehicles, including those associated with a Roman Service member, indicating a possible pattern. Despite this, Mazda did not cover the repair cost. Given that multiple bolts were improperly secured and that the dealer has seen numerous similar cases, this appears to be a potential manufacturing or assembly defect affecting multiple vehicles. I am requesting that NHTSA review this issue for possible investigation and recall consideration.Read more

The liftgate has had issues from the moment I purchased this vehicle. It had 30,000 miles and we purchased all extended warranty options. The liftgate often fails to close, especially in cold weather. There have been times I have to manually push the liftgate closed certain instances. This would not even work, and I unknowingly started driving my vehicle while the liftgate was still open about an
... inch or two essentially, the liftgate seems to have a mind of its own sometimes it will work just fine more more often often than not it malfunctions. This is dangerous because it could easily cause a head injury or trap a child or a family member inside the car if it will not open or closed properly. We took the car to our dealership to have it repaired while under warranty. My car care technician reported that there was dust/dirt which voided my warranty coverage. I paid a lot for the warranties and for this woman to report dirt on my vehicle’s backend and that then causing my warranty coverage to not be valid was just ridiculous. We fought for an exception to be filed and they never spoke of this again. Dishonest and untrustworthy, this is how I would define my Toyota dealership. The issue was noted immediately to dealership/technicians. We had them provide us with a repair quote that was very costly and so we would not let them do the work. There is a massive amount of information for this problem and it is a common occurrence. Toyota is facing a class action lawsuit alleging that some of its “Highlander SUVs contain defective power-door components. This problem allegedly prevents the automatic open/close feature on the back door, also known as the “liftgate,” from working properly and has already cost some car owners thousands of dollars in repairs. According to the suit, Toyota knew about the defect and hid this information from the public.” Thus, the tech noted “dirt” to conceal a much bigger problem, make $$ off a loyal customer, revealing dishonest practices.Read more

Component or System Failed: Rear liftgate glass (rear window) shattered unexpectedly. The glass is available for inspection upon request. Description of Incident: Vehicle is a 2019 Acura RDX located in Massachusetts. I started the car from my remote. While the vehicle was parked, I closed the driver-side door and the rear liftgate glass immediately shattered without impact. There were no prior cr
... acks, chips, or visible damage to the glass. The shattering appeared spontaneous and was not caused by an object strike. Safety Risk: The sudden shattering of the rear glass created a safety hazard due to flying glass fragments inside and around the vehicle. If this had occurred while driving, it could have caused driver distraction, loss of visibility, or potential injury to occupants or nearby individuals. The failure appears to be spontaneous and without warning, which raises safety concerns. Problem Reproduced or Confirmed: UNKNOWN. The vehicle has not yet been inspected by a dealer at the time of this report. However, similar incidents involving 2019 Acura RDX rear glass shattering have been widely reported by other owners. Inspections Conducted: The vehicle has not yet been inspected by the manufacturer. Insurance will be inspecting the damage as part of a comprehensive glass claim. No police report was filed as there was no collision or vandalism suspected. Warning Lamps or Prior Symptoms: No warning lamps, messages, or symptoms appeared prior to the failure. There were no visible cracks or damage before the glass shattered. Additional Notes: The failure occurred immediately after closing the driver-side door. No rear defroster warning or malfunction was observed prior to the incident.Read more

It was again returned on March 13, 202
... 4, after water was discovered inside the trunk area of the vehicle. At the time of sale and shortly thereafter, water was present inside the trunk area, and the vehicle continued to exhibit water intrusion. The water intrusion appears to be associated with defective weatherstripping or sealing, which allowed moisture to enter the vehicle. On March 19, 2024, the dealership returned the vehicle to me with visible mold present inside the vehicle. The mold condition was observable at the time it was returned to consumer use. The vehicle was not professionally remediated for mold contaminated before being released back to consumer possession. A professional mold inspection later identified over 100,000 mold spores and at least ten (10) different types of mold inside the vehicle. Although the source of water intrusion was reportedly repaired on October 25, 2024, the vehicle remains inoperative and unsafe due to mold contamination. The mold condition persists despite repair attempts. I remain in possession of the vehicle, which cannot be safely driven or occupied. The manufacturer was notified of the water intrusion and mold contamination; however, the safety issue remains unresolved. I respectfully request that NHTSA review this matter to determine whether the water intrusion and resulting mold contamination present a broader safety concern and whether the handling and return of the vehicle to consumer use was appropriate given the visible mold condition.INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am writing to formally report a significant structural failure regarding the power liftgate system on my 2020 Toyota RAV4 Hybrid XLE. Recently, the trunk began failing to close properly. Upon a detailed inspection, I discovered that the sheet metal surrounding the hinges has suffered catastrophic failure. The left-side hinge mounting point is severely fractured, and the right side is exhibiting
... clear stress cracks. This is not a case of "wear and tear." This is a structural failure where the force exerted by the factory power struts appears to have overpowered the vehicle's body structure, leading to metal fatigue and tearing. I would like to emphasize the following points regarding this case: Maintenance & Care: This vehicle has been maintained strictly according to Toyota’s recommended intervals and has been handled with the utmost care. There has been no external impact or misuse of the liftgate. Safety Risk: The inability to securely latch the trunk poses a severe safety hazard. I am concerned about the liftgate's stability at highway speeds and the potential for exhaust fumes to enter the cabin. Consequently, I no longer feel safe using this vehicle for my daily commute. Request for Action: Given that this appears to be a known issue within the 2019–2021 RAV4 production years, I am requesting that Toyota stand behind its reputation for quality and safety. I ask that Toyota cover the full cost of the structural repairs and the replacement of the affected hinges and struts. I trust that Toyota will treat this as a priority safety matter. I look forward to your prompt response regarding a diagnostic appointment and a resolution for these repairs. Manufacturer Recall 18S20 (NHTSA 18V471) - Transmission Shifter Cable Bushing: The bushing is degraded or detached. Safety Risk: The rear door failure risks the door opening unexpectedly while driving, potentially causing ejection or injury to occupants/others. The shifter bushing risks the vehicle rolling if not properly in Park. The issues were confirmed by the authorized Ford dealership during the 10/15/2025 inspection. The vehicle has been inspected by the dealership; no other inspections noted. No warning lamps or prior symptoms before the door failure on 10/12/2025. Shifter bushing symptoms unknown. The manufacturer has denied our claim and we are tasked with getting home to permanently park the RV. The manufacturer communicated with the shop regarding their finds and the denial, but has not contacted me in any way. All recalls/TSBs have been performed. The rear TSB has not fixed the rear frame sagging and the front TSB was not 100% completed by a dealer, in that several lag bolts were not replaced and none of them had adhesive applied.
