I am filing a safety complaint regarding a 2022 Forest River Salem Cruise Lite travel trailer, purchased July 21, 2022. At the time of sale, the unit had an open federal safety recall (NHTSA Recall 22V-243) involving a fire/explosion hazard. The recall was not remedied prior to sale. The vehicle was presented for service multiple times, and the recall was not identified or addressed until May 20
... 25. At that time, the manufacturer directed that the recall repair be completed; however, the issue has not been resolved. The manufacturer’s owner portal continues to show the recall as open, despite NHTSA’s system not showing an unrepaired recall for this VIN. The vehicle has been out of service for an extended period and is currently unusable. Request: Investigation into failure to remedy a safety recall within a reasonable time, potential reporting discrepancies regarding recall status, and the sale of a vehicle with an open safety recall.Read more

The bed is completely rusted out to the point every time I had it inspected i had to replace the struts. I was not being told the struts were NOT attached to the vehicle and thats why they needed replaced. Its available for inspection at ANY time. I asked for a Supervisor & was told it would be called within 24 hrs - no one has called. I called Toyota Customer Service explained the situation and was also told it was an Advisory. I asked for a Supervisor was told they would contact me in 24-48 & havent been contacted. Ex had me buy the truck from his bro-in-law who was Used Car Sales Manager who also didnt know about Advisory. My son's father remembers there being rust issued & was "going to take care of it" but didnt approx 2015. Issue - the frame could crumble or break in half any time IF IT was being driven & broke off on hwy causing wrecks. 2019 IT WAS ON WEBSITE AS A RECALL but now suddenly it was covered then downgraded to "advisory" current bf was "gonna handle it & didnt". 2017 & 2019 I had to replace struts to pass Inspection. My son wants to have it for his 1st truck. No representative will inspect it unless I will pay all costs to replace frame & i don't have that kinda money for an issue they have been aware of. NHTSA Recall Number 24V031. Exterior A-pillar applique trim flew off my vehicle while driving on I-95 in Florida in May 2023. This part is on my 2017 Ford Explorer and is listed on the NHTSA safety recall, 24V031, which was initiated in 2023. I did that, but the after market part was inferior. So, at this point in time, I am extremely frustrated and am filing this complaint to get an official response from Ford.Read more

I am writing to formally report a serious and ongoing issue with my 2022 Audi A5 involving multiple component failures. Most recently, both front windows suddenly stopped functioning and would not roll up, leaving my vehicle fully exposed. As a result, my car was subjected to rain, causing water intrusion and damage to the interior of the vehicle. This issue is especially concerning given my prior
... history with this vehicle. Since purchasing the car, the sunroof has already required repair twice within the past two years. Now, I am experiencing a complete failure of both front windows. This pattern of repeated issues raises serious concerns about the overall reliability and quality of the vehicle. I rely on my vehicle daily to transport my children and manage my responsibilities. Due to this failure, my vehicle is currently not secure or usable in normal conditions, which has placed me in a very difficult position. Additionally, I have been informed by an Audi technician that this is a known issue with Audi A5 models. Given that this defect has been acknowledged, it is extremely concerning that no preventative solution or recall has been implemented to protect consumers from this exact situation. This failure is unacceptable in a vehicle at this price point and presents a risk for further damage, including potential electrical issues due to water exposure. I am requesting that Audi of America take full responsibility for this matter and provide coverage for: Repair of the window system Any resulting interior or electrical damage caused by water intrusion A thorough inspection of the vehicle to ensure no additional damage has occurred I would also like this issue formally documented as part of any ongoing investigation into known defects affecting the Audi A5. Please provide a prompt response with next steps toward resolution. I can be reached at your earliest convenience.Read more

I respectfully submit the following additional information. In connection with this matter, the dealer in question asserted in writing that I "should have refused to participate" in the manufacturer-authorized "Z05 Service Campaign"—a measure designed to prevent potential damage—and characterized the program as nothing more than "free paint work." Service campaigns administered by manufacturers
... are conducted within the framework of federal safety regulations pursuant to Title 49, Chapter 301 of the U.S. Code; they are neither optional nor merely cosmetic in nature. Rather, they constitute systematic procedures intended to preserve vehicle integrity and support public safety. Statements suggesting that consumers should decline participation in such campaigns—or that they may bear financial responsibility for damage incurred during their execution—raise concerns regarding how these federally supervised programs are communicated and implemented and may discourage participation and undermine public confidence in the national vehicle safety campaign system. Although the dealership subsequently offered to replace the windshield at no cost, I have not received confirmation that the repair has been completed. This offer was made only after the vehicle remained at the facility for an extended period, during which I was repeatedly placed under pressure to assume financial responsibility for damage that occurred while the vehicle was under dealership custody. I was presented with two conditional options; however, in either scenario, I would ultimately be required to accept financial responsibility for the outcome. Ultimately, I was required to assume financial responsibility for issues arising during the work. On 2/23, I was told the loaner could be reported as "stolen," and I returned it immediately. After returning the vehicle, no alternative transportation was provided while my own vehicle remained unrepaired.Read more

I am writing to report a serious safety concern involving my 2024 Chevrolet Silverado that resulted in a child being locked inside the vehicle with the key fob present inside the car. On March 25 2026, after parking and turning off the vehicle, I exited the driver’s seat with my key fob inside my purse, which remained in the front of the vehicle. I then walked around to remove my daughter from
... the back seat. During that time, she climbed into the front seat and pressed the interior door lock button, locking all doors. My daughter has autism and is not able to follow instructions or unlock the vehicle on command. As a result, she was locked inside the vehicle with the key fob present inside, and I was unable to gain access from the outside. The vehicle did not prevent the lock from engaging, nor did it allow me to open the door despite the key being inside the vehicle. This situation required emergency assistance (911) to safely remove her from the vehicle. I understand that many vehicles have “lockout prevention” features; however, in this case, the system did not account for a vulnerable occupant inside the vehicle. Allowing the vehicle to be locked from the interior while the key fob is inside—and without any external override—creates a significant child safety risk. I am requesting that Chevrolet review this design behavior as a potential safety issue, specifically: - Why the vehicle allows interior locking when a key fob is detected inside - Why there is no external override to unlock the vehicle when the key is present - Whether a software update or safety feature enhancement can address this risk This is not just a convenience issue—it is a safety hazard that can lead to dangerous outcomes, especially for children or individuals with disabilities. Please confirm receipt of this concern and advise whether this issue has been previously identified or is under review.Read more

Immediately after the manufacturer recall requiring full frame replacement (completed November 2017 by the same dealership referenced below), the vehicle developed abnormal handling not present before the repair, including improper tracking and rear end instability. No warning lights/diagnostic messages were present. These issues were reported to the servicing dealership within weeks and repeatedl
... y over the following years. Despite multiple short initial inspections and two extended service periods—totaling approximately fourteen months in the dealership’s possession—the dealership repeatedly reported no defect found and continued to advise that the vehicle was safe to operate. Due to ongoing safety concerns, an independent inspection was recently obtained. It confirmed measurable rear axle offset and a thrust angle deviation outside acceptable tolerances. The defect is visually apparent: the rear wheels are not symmetrically aligned, and the vehicle exhibits a noticeable “crab walking” effect when viewed from behind. The root cause remains undetermined and would require further diagnostic work and significant disassembly. All documentation and inspection findings are available upon request. Safety risk: The vehicle does not track correctly and shows instability, especially in low traction conditions such as rain or snow, increasing the risk of loss of control. The manufacturer has been formally notified through repeated outreach, including ongoing contact with the Brand Engagement Center since December 2025 and multiple certified letters to Toyota North America Legal/Consumer Affairs between January and March 2026. The only response received was a generic statement from the Brand Engagement Center indicating that the vehicle is out of warranty and an offer to return it to the same dealership that performed the frame replacement and failed to identify the defect. No other response, investigation, or resolution has been provided by Toyota North America.Read more

I had ANOTHER door latch lock assembly fail on my 2016 Hyundai Tucson. There was a TSB for this issue that the dealership said expired for my vehicle three months ago. They confirmed it was another door latch lock assembly, although I’m not sure how because the door would not open from the inside OR the outside. When the
... rear passenger side assembly failed a week ago, my kids were stuck in the back of the vehicle. This is a potentially dangerous issue that Hyundai seems to be aware of. If the front driver’s side goes at the wrong time, and I am not able to open my door, it could have disastrous consequences. I could become trapped in my car in the event of an accident, fire, etc. Hyundai needs to recall all door latch assemblies and just replace them all free of charge. Water began accumulating in the front footwell area of the vehicle, resulting in a soaked carpet and persistent musty odor. The condition developed during normal operation of the air conditioning system, without any external water intrusion (no flooding or open windows). The issue appears to be caused by a failure of the A/C condensate drain system, which is not properly draining water outside th
... e vehicle. Instead, condensation is leaking into the cabin. This creates a risk of mold growth and potential damage to electrical components located beneath the floor, including wiring harnesses and control modules. The problem has been reproducible, especially when the air conditioning is in use. There were no warning lights or alerts prior to noticing the issue. Research indicates that BMW has identified this exact defect in Service Information Bulletin SIB 64 04 16, which states that the condensate drain hose may be improperly installed, kinked, blocked, or damaged during installation, leading to water ingress into the footwell area. This suggests the issue is due to a known design or manufacturing defect rather than normal wear and tear. This condition poses a safety concern due to possible electrical malfunction and reduced vehicle reliability. The affected components are available for inspection upon request.Read more

I am filing a complaint regarding the frame manufactured by Lippert Components on my 2021 Grand Design Solitude 380FL (VIN [XXX] ). Lippert is the OEM supplier of the frame and has been directly involved in inspection and repair of ongoing structural failures. The unit has experienced repeated structural failures, including frame flex, broken welds, cracking of the main I-beam rails above both ax
... les, torn steel upper sill plates and suspension failures. These issues began within the manufacturer’s warranty period and required extensive factory repairs from December 2024 to February 2025, including reinforcement of the upper deck, weld repairs, sill plate replacement, and added gussets. Despite these repairs, new failures developed shortly after. In September 2025, a Lippert technician (dispatched at Grand Design’s request) identified new frame cracks above both axles, a bent outrigger support, and repeated suspension failure. These were not pre-existing conditions but occurred after factory structural repairs. In December 2025, Lippert performed additional field repairs, including welding the cracked frame, adding fishplates, reinforcing outriggers, and installing cross-members. These repairs were performed under engineering direction, and Lippert confirmed no standard TSB applied to this condition. Despite multiple repair attempts, the unit continues to exhibit structural-related issues, including cargo doors opening during transit and recurring suspension failures. These conditions indicate ongoing and unsafe frame deflection and instability. Grand Design has now stated their repairs are done per a customer satisfaction campaign, and attributed responsibility for any ongoing frame issues to Lippert, and has abandoned any further assistance with cargo doors opening during transit. This represents a systemic structural defect affecting safety, including risk of debris or components entering roadways and loss of structural integrity during travel.
