Subject: Safety Complaint – Power Sliding Door Failure Injuring Child (2026 Kia Carnival) I am submitting a safety complaint regarding a 2026 Kia Carnival. During normal use, the vehicle’s automatic power sliding door closed while my [XXX] child was partially outside the vehicle near the door track area. The door continued to close and did not stop or reverse when it encountered my child, re
... sulting in the child being pinned between the sliding door and the vehicle body. I physically intervened by placing my body in the path of the door to stop it and remove my child. The child sustained bruising to the legs and torso. The concern is a potential failure of the anti-pinch / obstruction detection system on the power sliding door, which is intended to prevent this type of injury. I am requesting that this incident be documented and reviewed as a potential safety defect. The plastic upper rail glass guide frame on the rear sliding window assembly of my 2023 GMC Sierra developed structural micro-cracks during normal vehicle operation. This manufacturing defect is a known issue tracked internally by the manufacturer under Technical Service Bulletin (TSB) #18-NA-383.This structural failure allows severe water intrusion into the passenger cabin. The leaking water runs
... straight down the inside back wall of the cab, completely flooding and corroding sensitive, factory-installed electronic safety modules mounted directly beneath the leak path. This continuous water intrusion caused the catastrophic electrical failure and corrosion of the video processing control module located behind the rear seat. The sudden failure of this module completely disabled the rearview digital mirror camera and the 360-degree perimeter driver assistance cameras, creating immediate blind spots and an active driving safety hazard while operating the vehicle. The vehicle was inspected and repaired by an authorized dealership, confirming the rear window leak as the direct cause of the electronic destruction. The out-of-pocket cost to replace the water-damaged safety components was $2,747.00. Because the manufacturer refused to cover the replacement of the defective rear glass assembly, the structural leak remains unresolved. This active water intrusion continues to flood the cabin, creating a health hazard via mold growth and guaranteeing that the newly replaced safety cameras will inevitably short-circuit and fail again.Read more

The front camera behind the rear view mirror comes up with code and the cruise control will not work and the lane departure system and the forward Collision system and adaptive cruise control. The front camera issue can only be repaired by replacing the camera between 800 and 1500 cost to purchase the camera plus a 5 to $700 Adas aiming at the dealership and I don't feel that that's correct being
... that almost every single Honda Odyssey I've touched in my repair shop from 2018 till current has this code coming up. Sometimes I can put a scan tool on the vehicle and clear out the code and do a dynamic aim and the vehicle will correct itself but it will come back relatively quickly and sometimes it's impossible and the camera needs to be changed before anything can be done and all of those systems are deactivated until the front forward facing camera has been replaced. I am a mechanic I service a lot of these Honda Odysseys I have a list of items that I find almost every single one has a problem with this particular vehicle I'm sending in about is my personal vehicle and it has had numerous issues that had nothing to do with the operators of the vehicle solely to do with quality of the vehicle front camera is one the sliding door cable housing collapsing too the rear shocks snapping apart at under 50,000 miles front strut mounts going bad at 60,000 miles. There's also an issue with the driver's seat belts fraying in my car in particular frayed in under 12,000 miles More troubling is that every customer I send to the dealer to get things repaired under warranty comes back weather repaired or not without a single item being recorded by the dealer invariably what ends up happening is the dealer will reset some parameter and make the message or error go away. They will then return the car without any documentation stating anything. Once the customer brings it back after warranty is up the dealer charges to replace a repair because the customer has no proof of prior complaintRead more

My vehicle previously received Safety Recall 20V-437 related to the sliding door handle cable assembly. After the recall repair was completed, the driver's side power sliding door has again begun exhibiting abnormal behavior. The driver's side sliding door does not consistently open fully and has failed to close properly on more than 20 occasions. The problem is reproducible and has become increa
... singly frequent. On multiple occasions, the power sliding door has also begun closing while children were entering the vehicle, requiring intervention to prevent contact. This condition creates a safety concern because occupants may believe the door has opened or closed properly when it has not. It also creates a risk of injury when the door unexpectedly changes direction or begins closing during entry or exit. The affected component appears to be the driver's side power sliding door system and related latching/closing mechanism. The vehicle has not yet been inspected for this recurrence because Honda advised that a dealer diagnosis is required to determine whether the current condition is related to the prior recall repair. There were no warning lights or dashboard messages associated with the condition. Because this vehicle previously received a safety recall repair addressing sliding door operation and latching concerns, I am reporting the recurrence of similar symptoms for review.Read more

On XXX, driving at a relatively low rate of speed coming off the on-ramp from [XXX] onto [XXX], the hood of our Thrifty rental vehicle, a 2024 Ford Bronco, suddenly and without warning, flew up and completely obstructed the windshield. As we had just entered the highway we were quite fortunate to have been in the right hand lane and able to safely pull into the breakdown lane. And even more fortun
... ate that there were not cars immediately ahead or behind us that we could have hit. And third, very fortunate that the force of the hood did not shatter the windshield. The hood flew up with such force that it essentially "wedged" itself into the side edges of the car and had to be manually dislodged to be closed. Some tube or coil-like structure on the right side of the car had been split in two by the force (later learned this was the windshield wiper fluid hose). Two other points of importance - it was very windy that day. And there was absolutely no warning on the dashboard system that the hood was ajar or anything to that effect. We drove at a very low rate of speed to the next exit to find a service station where the car could be inspected before driving the 3 hours on to [XXX]. The mechanic stated the problem had been with the hood latch, but it appeared safe for us to drive. On returning the car to Thrifty, a verbal report was immediately given, but it seemed strange to me that no paperwork was filled out. The woman processing cars wrote down "windshield wiper hose" and left to move some vehicles. I waited for about 10 minutes, but she didn't return. Research upon my return home revealed similar problems reported with other Ford Broncos - both with the hood opening unexpectedly while driving and with faulty dashboard warning systems. [XXX] I was contacted by Thrifty's 3rd party vendor handling "damage incidents" - PurCo and related these events. They claim Thrifty sees no problem on their end. I am filing a safety complaint regarding my new Honda CR-V which is 6 months old. The next day, the vehicle became unresponsive and the battery went dead. This was alarming because the warning lights appeared du
... ring highway driving, where a sudden electrical or system failure could create a crash risk. The vehicle had been parked and closed when the water intrusion occurred, so I am concerned there may be a defect related to water sealing, electrical systems, or battery failure. The dealer confirmed that the seal was broken and might have been a manufacturing defect. This caused water seepage. But they were not sure if the water seepage was related to battery giving electrical warnings and going dead. This uncertainty caused more insecurity as we do not know the reason for battery failure exactly when water leaked into the vehicle. Please investigate whether this issue is related to a safety defect. I request that this complaint be reviewed for any pattern involving water intrusion, dashboard warnings, electrical failure, or battery drain in this model. I am submitting a safety complaint regarding the panoramic sunroof assembly on my 2021 Hyundai Palisade. The vehicle was purchased as a Hyundai Certified Pre-Owned vehicle in September 2023 with approximately 27,000 miles. The panoramic sunroof had only been opened approximately two or three times since purchase when the failure occurred. During normal operation, the sunroof wind deflector/visor
... malfunctioned and jammed inside the mechanism. This caused the pulleys and track assembly to bind and pull the glass panel out of position, damaging and breaking the sunroof track/mechanism. The dealer diagnosed the failure as a broken sunroof track/mechanism assembly requiring extensive repair estimated at approximately $4,000. This failure appears to directly match Hyundai Technical Service Bulletin 23-BD-003H-1, which acknowledges that the sunroof wind deflector may malfunction and damage the sunroof mechanism on 2020–2022 Hyundai Palisade vehicles. Hyundai issued this bulletin before I purchased the vehicle as Certified Pre-Owned. My concern is that this is a known recurring mechanical defect involving the panoramic roof assembly that could create a safety hazard if the mechanism jams, detaches components, shifts the glass panel unexpectedly, or causes the roof to fail while driving. The failure also caused the panoramic sunroof to become stuck open while driving. This created excessive rattling and vibration noise at highway speeds, which was distracting during vehicle operation and raised concerns about the stability of the sunroof assembly while in motion. Because the roof could not be properly closed or secured, the defect also exposed the interior of the vehicle to possible rain intrusion and additional electrical or interior damage. The sunroof was not abused or damaged externally. The failure occurred during ordinary use and after very limited operation of the sunroof since purchase.Read more

Dealer replaced the BCM, repaired the leak, and replaced the starter. Total repair cost approximately $3,200. Failed components are available for inspection at the dealership. The control arm detached from the front subframe due to massive rust on a car that was well cared for and only 10 years old. This happened as we were driving: the front passenger-side wheel became loose and began flopping back and forth, forcing me to immediately pull over into a gas station. I do not want to imagine what would have happened if this had detached on the highway instead of on a town
... side road. There were NO warnings or symptoms before this catastrophic failure. I had it towed to my regular mechanic, not realizing the extent of the damage—they wouldn't even put it on a lift. I then had it towed to the dealership, where they confirmed the damage was so extensive that (1) both control arms and the front subframe would need to be replaced due to rust ($5,800), and (2) the rear subframe was also extensively rusted, unsafe, and in need of imminent replacement, quoted at an additional $7,000. This being an exorbitant amount for a 10-year-old Nissan Pathfinder, I was forced to sell it to a salvage yard for a pittance just to buy a vehicle I could drive to work—and I can only hope it lasts until I can afford something better. I purchased this Pathfinder about 14 months ago. It was "inspected" by the dealer as well as by my regular mechanic at that time, with no indication that rust was a concern or even worth a comment. I am horrified by how much worse this could have been. We were on a trip more than an hour from home, heading toward the highway to return, when this occurred. No one should have to worry about something like this. Especially because, when I began researching, I found that this is a KNOWN defect on this model and others like it. It is NOT okay to put people at risk of serious or deadly injury when manufacturers KNOW water pools in places that can't be seen or checked, rotting the frame and subframe from the INSIDE out.Read more
