On February 5, 2026, at 12:21 PM PST, my 2026 Tesla Model 3 (Hardware 4) collided with a 5-inch wooden pillar while using the Autopark feature. The system failed to provide any audio chimes or visual warnings prior to the impact. This is a follow-up to complaint #11714226 regarding an ongoing safety defect. The defect has now resulted in a crash. Warning indicators including Forward Collision-Avoidance Assist, Automatic Emergency Braking, Driver Attention Warning, Lane Assist, and Master Warning have illuminated without cause. This vehicle has been presented three times for repair attempts specifically for these braking and steering systems and has been out of service over 30 cumulative days. The manufacturer was unable to duplicate or repair the defect and returned the vehicle unrepaired. On March 12, 2026, Genesis Corporate (Case Manager Terrence) stated they were coordinating with Genesis of Selma to schedule a fourth repair attempt. However, on March 26, 2026, Mike at Selma Genesis stated via text: “We as a dealer won’t be working on your car.” This directly contradicts Genesis Corporate and has prevented further repair attempts. On [XXX], while traveling approximately 55 mph, the vehicle experienced a sudden loss of steering control combined with unintended braking, causing it to leave the roadway and collide with pistachio trees. Law enforcement responded. I sustained back and neck pain and sought medical care. The defect remains unresolved and poses a serious risk of injury or death. On the evening of April 8th 2026, my wife was driving our 2023 Tesla Model Y home in San Francisco, CA when she noticed unusual noise coming from the front left wheel area. The vehicle was otherwise functioning normally aside from the noise. The following morning, I inspected the vehicle and found a large hex bolt (Class 10.9 grade) on the ground directly beneath the car. The front left wheel was
... binding and the vehicle was completely undrivable — the wheel felt stuck and made grinding/scraping noises when attempting to move. Based on the bolt size, grade, and location, this appears to be a front suspension lateral link fastener that detached from the subframe. This is the same defect described in NHTSA recalls 21V-835, 22V-895, and 23V-235, involving under-torqued or improperly secured front suspension fasteners on Tesla Model Y vehicles. The vehicle had 28,482 miles at the time of the incident. There was no prior collision, impact, or road hazard event. No warning or alert appeared on the Tesla touchscreen at any point. I am filing this safety complaint regarding a documented engine wire harness chafing defect in my 2021 Chevrolet Silverado 1500 with the 2.7L L3B Turbo engine. GM issued TSB 21-NA-149 in June 2021 and a second bulletin September 17, 2021, both before my November 2021 purchase, identifying wire harness chafing in the L3B engine caused by a manufacturing error. A wire harness clip was removed during production creating an uncontrolled harness branch that contacts sharp edges on the ECM/TCM bracket, upper control arm, and shock tower during normal engine vibration. This defect was built into the vehicle at the factory and was never disclosed to me at point of sale. The defect manifested after warranty expiration causing cascading failures; wire harness, catalytic converter, and transmission, all within five weeks. Vehicle has been inoperable since January 8, 2026. Defect confirmed by independent mechanic and by Ancira-Winton Chevrolet (authorized GM dealer) in their own repair documentation referencing GM Bulletin 21-NA-149. Dealer documented catalytic converter failure as consequential damage from wire harness repair. GM created Customer Satisfaction Program N192219370 for this exact defect. My VIN was excluded despite experiencing identical symptoms. No safety recall has been issued. Loss of throttle control and stalling at highway speeds represents an imminent collision risk. I request NHTSA investigate a formal safety recall for 2019-2023 Silverado 1500 and Sierra 1500 vehicles equipped with the L3B engine.Read more

Also the Driver Input Required amber light comes frequently on a straight road with highly visible lane markers. It is extremely tiresome to drive the car and try to keep it in its lane, even when I disable all the "driver-assistance" features. This is because the steering has a mind of its own and stiffens up. It resists the slightest driver input so you have to put more strength into it to make it go where you want to, but once the steering gives up fighting you and lets you do it, it results in overcompensating and constant steering corrections by the drive left-right, left-right, just going down in a straight smooth freshly paved road. After months of this I found out I did not have to take it to the dealer I bought it from. So, I took it to Monument Chevrolet. I explained my concerns related to the front-end wobble which was very intermittent. And the oil consumption issue which I was very concerned with due to the year model and mileage of the truck. Also, the 75,000.00 price tag was really has me extremely upset... Monument Chevrolet treated me well. I had actually brought it in and called about the steering which was greatly affected by the severe front end wobble more than one time. I was told at first the oil consumption was pretty normal. Until I told them how much I had to add between changes. I have attached two work orders to this complaint. These work orders show what was done at 18017 miles and 18323 miles. The later work order was when the PICO test was done. Also, an oil consumption test was started once I picked the truck up. After I had driven it for 2077 miles and was bringing the vehicle back to Monument Chevrolet, as instructed for the oil consumption results, when a loud squeeling, then hard knock, power loss and I pulled over. I called Monument and was instructed if I could get it to start and bring it. I drove it to them while knocking and rough running. It has been in the shop for over 2 months for an engine replacement. 2077 miles after recall work! "REPAIR ORDER GCCS504610" I had to leave it in the shop for 30 days for a defective Exhaust System, Radio, Charging Port, Remote Key Fob not able to unlock the truck so i could get in. And some other warranty/recall issues regarding my A/C blower and Seat cushion blower motor and Trailer Brake Switch, Fog Lamps running while daytime running lights were on - All Safety issues, My truck was @ Beck & Masten GMC for 30 days during these repairs, recalls and warranty. Will attach INVOICE 123532 to show consistant sheduled maintenance per GMC guidelines. Waited while this service was performed. "REPAIR ORDER / INVOICE 140542" Again my truck was back at Beck & Masten GMC (19 days) for the Serial Gateway Module and Radio not working AGAIN! Again, no OnStar, No Internet, No GPS. And, I reported the oil consumption and Add OIL light coming on; which was dismissed as idling too often (. I had to add 2 qts of oil between scheduled oil changes(The truck had only 13754 miles at this time) The LAST time I had anything to do with Beck & Masten GMC South was when I spent over months waiting on a loaner vehicle because I needed them to please look into still having an oil light coming on.
