Unresolved Safety Defects & Request for Buyback/Replacement - 2024 Dodge Hornet GT Dear NHTSA I am writing to formally demand a resolution regarding a persistent and life-threatening safety defect in my vehicle, purchased from Gary Barbera Autoland. Since the time of purchase, this vehicle has experienced repeated electrical malfunctions causing a total loss of power, including the failu
... re of steering and braking systems while in motion. To date, the vehicle has been returned for service seven times over the past 2.5 years for this identical issue. The dealership has failed to rectify the defect and is now citing an expired mileage warranty. However, the records clearly indicate that this safety defect was reported and remained unresolved while the vehicle was well within the warranty period. Under the Pennsylvania Lemon Law and the federal Magnuson-Moss Warranty Act, a manufacturer is required to repair a vehicle within a reasonable number of attempts (typically three). This vehicle is currently unsafe for myself, my disabled mother, and the public. I am requesting that Stellantis the Manufacturer initiate a buyback or replacement of this vehicle immediately. Please provide a written response within [5-7] business days outlining your intent to resolve this matter. Thank you Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The power steering system in our 2024 Mazda CX-90 intermittently "sticks" while steering, requiring noticeably increased effort to turn the wheel before it suddenly releases. The symptom matches the defect described in NHTSA Recall 24V-022 (sticky steering due to worm gear friction in the power steering assembly) and the ongoing NHTSA Recall Query into vehicles where the 24V-022 remedy has failed
... to resolve the issue. Component: Power steering gear / worm gear assembly. The vehicle is available for inspection upon request. Safety risk: The intermittent increase in steering effort followed by sudden release creates a real risk of overcorrection, particularly during lane changes, highway driving, and turns from a stop. The driver cannot predict when the wheel will resist or release, which compromises the ability to maintain a consistent lane position and respond to road conditions. Dealer/service confirmation: UNKNOWN at this time — we are scheduling a dealer inspection. We will update if the dealer reproduces or documents the condition. Manufacturer/police/insurance inspection: None to date. No crash has occurred. Warning lamps or messages: None. No dashboard warnings, no error codes, and no messages have appeared before or during the sticking events. The condition is purely tactile through the steering wheel and is intermittent. The vehicle's recall status shows no active open recalls, suggesting any prior remedy under 24V-022 was already performed, yet the sticky steering symptom is still present — consistent with the complaints driving the current NHTSA investigation.Read more

The front left lower control arm (driver's side) snapped off (broke) while turning a corner. There was no reason for the breakage other than turning. It was repaired through insurance. The pole had very l

While entering a roundabout at a slow speed (~5MPH), the electronic power steering on my wife's car failed, almost resulting in an accident with another vehicle, as it suddenly became very difficult to steer. The vehicle's power steering warning indicator turned on, and then the following warning messages appeared on the dashboard: Check Forward/Side Safety system, Check Lane Safety system, Check
... Lane Following Assist system, and Check Inattentive Driving Warning system. After turning into a nearby parking lot and parking, the vehicle was turned off and then back on, and all warning messages and the power steering warning indicator turned off. My wife was approximately 1 mile from home and decided to try to drive home. After getting back on the road, the same issue occurred again with all indicators and warnings repopulating on the dashboard this time with the vehicle traveling at approximately 35 MPH as she crossed a major intersection. My wife was able to get the car home safely without power steering, and after turning the car off and back on again, all warning messages and indicators turned off again. The next day, 4/30/2026, we contacted our local Hyundai dealer and Hyundai roadside assistance. The vehicle was towed to the dealer, where it is currently being serviced for this issue. We have not yet received an update on what caused the issue, but we felt it was important to report it to NHTSA because, if it had happened at a higher speed, it could have resulted in a serious accident and possibly serious injury or death of my pregnant wife and child, who was with her in the car at the time. Prior Action: I visited two Honda dealers who told me they can only follow Honda recommendation. I contacted Honda Customer Service to request a repair under the current steering gearbox safety investigation, but my request was denied. Honda Case Number: 16300136. Case Status: Honda closed this case on 4/30/26 without providing a remedy, citing that my VIN is not currently part of the 2023–2025 recall population. I am reporting a safety-related defect with a 2025 Hyundai Palisade SEL. Within days of purchase (December 14, 2024), I began experiencing abnormal noise and vibration from the rear of the vehicle. The issue consistently occurs at highway speeds (45–55 mph) and over bumps. The problem has been present since the first week of ownership and has worsened over time. Multiple inspections confirmed
... the issue: •Selling dealership dismissed it as “normal tire noise” without proper diagnosis •Second Hyundai dealership confirmed severe abnormal wear (cupping) on all four tires •Independent inspection (Firestone) confirmed uneven tire wear, alignment out of specification, and ongoing vibration Despite early reporting, Hyundai Motor America has refused to provide a root cause diagnosis or take responsibility. They denied tire replacement, attributing the issue to tire wear. This explanation is not valid. Tires do not develop severe cupping shortly after purchase without an underlying defect such as improper alignment or suspension issues. This condition creates a safety risk: •vibration reduces vehicle stability at highway speeds •uneven wear increases risk of tire failure The issue affects all four tires and was present almost immediately after purchase, indicating a likely manufacturing or setup defect. The problem has been confirmed by multiple service providers but remains unresolved. The vehicle is not safe to operate under normal driving conditions. I request immediate investigation into a potential manufacturing defect affecting vehicle safety. Hyundai has been aware of this issue since the first week of ownership and failed to take corrective action. Despite multiple documented inspections confirming abnormal tire wear and vibration, no root cause diagnosis or repair has been provided. While driving my 2021 Honda CR-V at highway speeds, the steering intermittently binds and becomes difficult to move, creating a dangerous “sticky steering” condition. The steering will momentarily resist normal input, and when I apply additional force to correct it, the wheel suddenly releases, causing the vehicle to jerk or swerve within the lane. This makes it difficult to maintain smooth, c
... ontrolled steering and creates a serious safety concern, especially at higher speeds where precise steering is critical. The condition is unpredictable, unnerving, and significantly affects my confidence in safely operating the vehicle. This defect creates an especially concerning hazard because I frequently transport my two young grandchildren in this vehicle. When the steering binds and then abruptly releases, it compromises my ability to maintain stable lane control and increases the risk of losing control or being involved in a collision. This is not merely an inconvenience—it is a clear safety issue that makes the vehicle feel unsafe to drive. The symptoms my vehicle is experiencing are identical to those described in Recall 24V-744 involving sticky steering in Honda vehicles. Although my VIN is not currently included in that recall, this same defect has been widely reported by other 2021 Honda CR-V owners. I understand the defective steering gearboxes were identified in vehicles produced beginning in February 2021, and my vehicle was manufactured in August 2021, which falls within that timeframe. Based on the identical symptoms and known defect history, I believe my vehicle is affected by the same steering gearbox issue and should be included in the investigation and recall population.Read more

While driving my Genesis G70 on the highway in rainy conditions, the vehicle repeatedly loses traction and hydroplanes at normal highway speeds. Re: Vehicle Safety Defect Complaint – 2023 Chevrolet Blazer To Whom It May Concern, I am submitting a formal safety defect complaint regarding my 2023 Chevrolet Blazer. This vehicle has exhibited multiple system failures that present a serious risk to driver safety and public roadway safety. The vehicle was purchased new on July 20, 2023. Beginning on April 21, 2025, at approximately 27,610 mile
... s, the vehicle developed a progressive defect initially presenting as oil-related issues. Despite multiple repair attempts, the condition worsened and required a full engine replacement in January 2026. Following the engine replacement, the vehicle continued to experience critical safety failures, including engine stalling, reduced engine power warnings, and electronic stability control (ESC) alerts. On April 26, 2026, the vehicle experienced a driver assistance system malfunction in which it attempted to steer off the roadway. I was forced to manually disable the system to maintain control of the vehicle. On April 27, 2026, the vehicle exhibited a fuel system failure, including fuel leakage and fuel odor, along with multiple simultaneous warning indicators: • Reduced Engine Power • Service ESC • Steering Assist Reduced / Drive with Care • Power assist driver system disabled This combination of failures presents both a fire hazard and a loss-of-control risk. Additionally, I was informed by the dealership that a high-pressure fuel line, a one-time-use component, was not replaced during the engine installation. This raises concerns regarding improper repair and continued unsafe operation of the vehicle. The vehicle has undergone multiple repair attempts and a major powertrain replacement, yet continues to present serious safety concerns. Due to these ongoing issues, I do not feel safe operating this vehicle. I respectfully request that NHTSA review this matter for potential safety defects that may impact other vehicles of the same make and model.Read more

The Power Conversion System (PCS) in the Tesla Cybertruck is experiencing a high rate of hardware-level failure (specifically the PCS2 module) that poses three distinct safety risks beyond the loss of AC charging. Loss of Steer-by-Wire Integrity: The Cybertruck utilizes a full steer-by-wire system and a 48V low-voltage architecture. The PCS is responsible for managing the 48V bus and charging the
... LV battery. A hardware failure in the PCS (common alerts: PCS2_a094, PCS2_a137) threatens the stability of the 48V system. Any sudden collapse of the 48V bus while in motion could result in a total loss of steering control and vehicle maneuverability, creating an immediate risk of a high-speed collision. Loss of Propulsion and Stranding: PCS failure frequently leads to "limp mode" or a "no-start" condition where the vehicle cannot engage Drive or Reverse. Experiencing a loss of propulsion while in active traffic or being stranded on a highway shoulder without the ability to move the vehicle creates a severe life-safety hazard for occupants and other motorists. Failure of Visibility Systems: Critical safety components, including the primary windshield wiper and exterior lighting, are powered via the 48V system managed by the PCS. Hardware instability has led to reports of these systems failing during inclement weather, severely compromising driver visibility. Tesla has implemented software bypasses (OTA updates) to allow DC Supercharging when the AC hardware fails, but this is a "band-aid" that does not address the underlying hardware defect—likely MOSFET degradation. With replacement parts on backorder for 8+ weeks, owners are forced to operate vehicles with compromised electrical architectures. I am reporting this as a systemic hardware defect that requires an official safety recall.Read more

... crash. This also posed a danger to other drivers on the road. The condition was severe enough that I believe I narrowly avoided a major accident. Confirmation of Problem: The issue has been confirmed by a Mercedes-Benz dealership service department, who identified the subframe as rusted and structurally compromised. Inspection by Other Parties: The vehicle has been inspected by a Mercedes-Benz dealership. It has not yet been inspected by the manufacturer directly, police, or insurance representatives. Warning Signs Prior to Failure: There were no clear warning lights or dashboard messages indicating this issue prior to the failure. The problem appears to have progressed without visible warning until it became severe. Additional Context / History: Mercedes-Benz has acknowledged subframe corrosion defects in certain vehicles from approximately 2010–2020, but my specific vehicle was not included in the extended warranty coverage. This is the second Mercedes-Benz vehicle I have owned (previously a 2012 model) that has experienced the same subframe failure. I was previously told that this issue had been resolved in newer models, which influenced my decision to purchase this vehicle. Additionally, this vehicle was purchased as a Certified Pre-Owned Mercedes-Benz, which implies it passed a detailed inspection process. Despite this, a major structural component failed. I am filing a formal safety complaint regarding a repeated dangerous defect in my 2018 Buick Encore. The vehicle intermittently locks up while driving without warning. When this happens: brakes become unresponsive or lock steering becomes difficult or locks gas pedal does not respond dashboard warning lights activate vehicle systems malfunction vehicle may not shut off normally This defect has
... occurred multiple times and creates a serious crash risk. The vehicle has now undergone three repair attempts at an authorized Buick dealership. During the second repair attempt, dealership staff personally experienced the same malfunction while test driving the vehicle. This vehicle remains unsafe to drive, and the defect has not been corrected despite multiple repair attempts. I believe this may involve a serious electrical, braking, steering, or control system failure that warrants investigation before someone is injured.Read more
