Nature of Defect: While operating Full Self-Driving (FSD) at highway speeds, my vehicle exhibits involuntary lateral swerving — sudden, uncontrolled movement to the left and right without driver input. This behavior occurs repeatedly and unpredictably, posing a direct safety risk to the occupants of my vehicle and to other drivers. Tesla Service diagnosed the root cause as a failed FSD compute
... r and recommended full replacement at a cost exceeding $3,000. Crashes or Injuries: None to date, though the behavior described above creates a significant risk of collision at highway speeds. Frequency: Recurring during FSD engagement at highway speeds. Additional Context: This defect was present during my vehicle's original warranty period. Tesla has been aware of this issue since my service visit and has not completed the repair. I have an active goodwill warranty request pending with Tesla that has gone unacknowledged for over two weeks. I am submitting this complaint because I believe this defect may affect other Tesla vehicles equipped with the same FSD hardware, and I want NHTSA to have a record of this failure mode.Read more

Persistent steering wheel vibration at highway speeds (60-80 mph), present from the first drive within two hours of taking delivery of this Certified Pre-Owned vehicle on April 14, 2026 at 21,075 miles. No warning lamps or messages. The vibration is constant at highway speed, not intermittent, causing driver fatigue and distraction during sustained highway driving. This is a known defect documente
... d in Volvo Technical Journal TJ 36752.3.0, classified under function group 4661 (drive shaft). Kline Volvo of Maplewood, MN confirmed the vibration and performed the TJ-prescribed repairs: tire balancing, tire replacement, and driver's airbag module replacement containing an oscillation damper. Volvo's own TJ notes this repair only "lessens" the vibration. The vibration improved but in my assessment has not been fully resolved. A second Volvo dealer's service manager independently acknowledged the issue and consulted Volvo's regional service representative, who confirmed no further fix is currently available beyond the TJ repair. I have escalated to Volvo executive leadership in writing and am awaiting response. The vehicle is available for inspection.Read more

Motorcycle has exhibited unstable handling behavior since delivery. The defect is specifically a weaving and wobbling oscillation that occurs when the motorcycle is upright and traveling in a straight line, as if the bike is constantly riding on rippled or uneven pavement even on smooth road surfaces. The rear of the motorcycle feels disconnected from the chassis, with the rear tire feeling as tho
... ugh it is not tracking with the front. The instability is present at all speeds but becomes pronounced and consistent at approximately 40 mph and above. Critically, the defect occurs only when the motorcycle is upright. The instability is not present during cornering or while braking — the bike feels stable when leaned over or under deceleration. The weaving and wobbling are confined to straight-line, upright riding. The defect is present: •In all ride modes (Road, Rain, Dynamic, Dynamic Pro) •Both loaded and unloaded •With top case and panniers fitted — no difference observed •On and off throttle •On smooth pavement with no road-surface contribution The instability is not consistent with normal motorcycle dynamics and is not corrected by tire pressure adjustment, rider input, or ride mode selection. Dealer interaction: Original selling dealer was non-responsive to handling complaints. Motorcycle is scheduled for diagnostic evaluation at a second BMW Motorrad dealership on June 1, 2026. BMW Motorrad USA customer relations case has been opened [case number if available]. Safety concern: Sustained weaving and wobbling instability at highway speeds while traveling upright on a motorcycle creates a significant crash risk, particularly during straight-line highway riding, lane changes, or in crosswinds. The defect has been present since delivery and has not been resolved through normal service channels. No crash, injury, or fire has occurred to date. Vehicle: 2024 Honda Prologue Touring AWD The vehicle has an ongoing clicking/noise issue coming from the driver-side front CV joint/axle area, especially during turns, acceleration, and low-speed maneuvering. The clicking noise appears to be progressively worsening and raises concerns about premature CV joint failure and potential drivetrain safety issues. The issue has persisted despite service
... visits and replacement attempts. Additionally, the vehicle intermittently displays a “Charge Port Door Open” warning message even when the charge port door is fully closed and secured. This false warning can prevent charging functions from operating normally and creates concerns about electrical system reliability and sensor malfunction. Both issues occur intermittently but repeatedly, affecting confidence in the vehicle’s safety and reliability. The ongoing driver-side CV joint clicking noise has caused concerns about potential drivetrain failure while operating the vehicle. Due to the worsening nature of the issue and repeated occurrence, I no longer feel fully safe or confident driving the vehicle, especially during turns and normal road operation. The false charge port warning may also interfere with the vehicle’s charging capability and normal operation.Read more

I am submitting this complaint regarding my 2021 Kia Telluride experiencing repeated and dangerous electronic system failures while driving. Kia of East Syracuse (NY) diagnosed the issue as related to the backup camera assembly and communication/network failures. While driving, the vehicle experienced loss of power steering assist, instrument cluster failure, speedometer malfunction, incorrect gea
... r indication, backup camera activation while driving forward, blind spot warnings activating incorrectly, flashing/blank temperature display, and multiple driver assistance system failures. The speedometer displayed 0 mph while the vehicle was in motion and later displayed approximately 40 mph while stopped at a traffic light. The vehicle also became stuck in reverse while backing out of my driveway and would not shift properly, leaving the vehicle stopped in the roadway. Multiple safety systems malfunctioned simultaneously, including Blind Spot Collision Avoidance Assist, Lane Following Assist, Rear Cross Traffic Collision Avoidance Assist, Smart Cruise Control, Parking Distance Warning, and Forward Collision Warning systems. The dealership documented multiple fault codes and stated the dashboard lit up “like a Christmas tree” while the instrument cluster stopped functioning entirely. These failures create a serious safety risk because critical driving information and driver assistance systems become unreliable or inoperative while the vehicle is moving. The failures occur unexpectedly and without warning. Photos and video evidence have been retained, including video of the backup camera activating while driving forward and the speedometer malfunctioning while the vehicle was in motion. I am asking NHTSA investigate these failures as a potential safety defect involving the camera system, instrument cluster communication systems, and CAN network communication failures in the Kia Telluride.Read more

Subject: Formal Safety Complaint – Structural Manufacturing Defect – 2021 VW Atlas Cross Sport (VIN: [XXX] ) To the NHTSA and Volkswagen Customer CARE: I am formally reporting a significant safety hazard regarding my 2021 Volkswagen Atlas Cross Sport. Incident Details: Incident Date: 05/07/2026 Current Mileage: 76,305 miles VIN: [XXX] Supporting Documentation: See attached files image.png
... and image_2.png. Description of the Problem (Per Certified Technician): As documented in the attached invoice from Vacaville Volkswagen (image.png), the vehicle was brought in for a "steering concern" involving clunking noises and swaying at freeway speeds. After a test drive and inspection, the technician found: The left front control arm front bolt was loose. Three other bolts were not tightened to specification. The noise and instability were caused by the resulting movement of the control arm. The Issue: While the dealership identified the improperly secured hardware and torqued all bolts and nuts to specification, they declined to cover the repair under warranty or recall, resulting in a labor charge of $230.00. Loose and under-torqued structural bolts on a control arm are not maintenance or "wear and tear" items; they represent a critical manufacturing and assembly failure. This component is vital to the vehicle's steering and suspension geometry. Evidence of a Systematic Issue: This defect directly mirrors several existing Volkswagen safety recalls for the Atlas line involving improperly torqued chassis and suspension hardware, such as Recall 42HX and Recall 40R7. It is clear that my 2021 model was impacted by the same production lapses identified in these adjacent recalls. Requested Action: For the NHTSA: I request that this incident be documented to investigate whether the scope of current suspension recalls should be expanded to include the 2021 model year. For Volkswagen USA: I am requesting a goodwill reimbursement of $230.00. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5Read more

Tesla just sold me a car with defective front half shafts axles. The car makes a reproducible and very concerning accelerator pedal shudder that makes the entire vehicle front vibrate, including the steering wheel and brake pedals while the car is accelerating between 30-60mph. This interferes with safety features like, lane departure assistance and blind spot steering wheel feedback, Confusing th
... e driver and creating a distracting scenario where an accident can happen. Upon bringing this to the attention of the service center in Fort Myers Florida, the service center manager took a drive test with me, and was able to replicate the issue, but immediately started dismissing it as "normal" and within Tesla specs. This is becoming a regular practice at that service center, where they deem warrantable issues as normal and within (their tolerance). I insisted that this issue is not to be taken likely by Tesla, because it is something that involves a safety hazard. Upon further research, I was able to find that this issue is happening to many Teslas model S and X, where the front axles bend due to the weight and high torque of the vehicles. The issue falls completely under warranty and the half shafts should be replaced, but apparently Tesla has instructed their service centers to dismiss and disregard the symptoms, and just tell the customers that no warranty replacement will occur, because it is not necessary, we just have to drive the cars like this, until the car completely fails, or an accident happens because one of the bent axle shafts breaks on a highway during normal drive. I recording this report as evidence that the issue has been reported to the highest vehicle safety authorities, and I expect a corrective action so Tesla can fix my car under the existing unexpired warranty.Read more

The vehicle was brought to a Hyundai dealership for inspection. The dealer pulled diagnostic fault codes, found none, performed a 20-minute test drive, and returned the vehicle stating they could not find anything wrong. No physical inspection of the steering components was performed. The failure condition — a slow full-lock turn from a complete stop — was never specifically replicated during the dealer's test drive. I am reporting a serious safety issue involving my 2019 Subaru Forester related to failure of the Electric Power Steering (EPS) system. Since April 21, 2026, I have been in communication with Subaru of America regarding whether this issue is related to an existing safety recall (WUC-88). During this time, I provided detailed information about multiple occurrences of EPS warning light activation a

A local repair shop also inspected the vehicle and confirmed EPS-related diagnostic trouble codes (including B2811 and C2512), which are consistent with known EPS issues. This information was shared with Subaru of America; however, I did not receive a clear or actionable response, and the case has been delayed without resolution. Initially, the issue was intermittent, with the EPS warning light turning on and off. However, since May 9, 2026, the EPS warning light has remained continuously ON, indicating that the condition has worsened. On May 11, 2026, I visited an authorized Subaru service center while the EPS warning light was actively ON. Despite the issue being clearly present, the dealership only took photos confirming the EPS warning light and did not perform any additional diagnostic testing or repair. They advised that the EPS rack needs replacement at an estimated cost of approximately $2,500, with the part currently on backorder and no clear repair timeline. I have already reported this visit and the current condition to Subaru of America and am still awaiting a response. This issue involves intermittent and now persistent loss of steering assist while driving, causing the steering wheel to become extremely stiff and difficult to control. This poses a significant safety risk. Given that the issue has been ongoing since April, has been confirmed by both a repair shop and a Subaru dealer, and remains unresolved, I believe this represents a safety defect that is not being properly addressed. When the steering wheel momentarily resists input or feels stuck, it can require extra effort to correct. This could increase the risk of delayed steering input, overcorrection, or difficulty maintaining the intended path of travel, especially at highway speeds or on curved roads. Mazda Safety Recall 6524A / NHTSA Campaign 24V-022 was previously performed on this vehicle. Despite the recall repair being completed, the sticky/notchy steering condition is still occurring intermittently. The problem was evaluated by a Mazda service department. During the service visit, Mazda service personnel also felt issues with the steering. I was informed they are looking for Mazda to follow up, including any additional guidance related to NHTSA Recall Query RQ26002, which I understand is reviewing sticky steering/increased steering effort after the original 24V-022 recall remedy. I am submitting this complaint because the vehicle is experiencing steering symptoms similar to Mazda Safety Recall 6524A / NHTSA Campaign 24V-022, even though the original recall remedy was already performed. The continued sticky/notchy steering behavior appears consistent with the type of post-remedy concern being reviewed under NHTSA Recall Query RQ26002 and presents a safety concern.Read more

I immediately hit the brakes and tried to veer to the right. Luckily, I was in the right lane and was able to get to the edge of the shoulder. My son and I sat there in shock. A few seconds later, I hit the ignitio

For the past year anytime I drive my vehicle above 40mph I notice the steering feels “sticky” and often you have to use additional effort to turn the wheel which cause the wheel to jerk to whatever side you attempting to turn the wheel. Apparently this is common on the same year civic but the dealership stated my vehicle VIN is not included on the list of vehicles that need the repair. The veh

While driving at highway speeds, steering wheel sticks at 11 and 1 o'clock position requiring extra input from the driver to return to zero or 12 o'clock position. This causes a jerky motion when the wheel finally unsticks or an oversteer condition which is unsafe when driving in busy windy roads with opposing traffic.

At speeds between 40 and 70 my 2021 Honda Civic EX Hatchback steering feels sticky when making small adjustments. There is a slight resistant in steering when a slight correction is needed. Once that resistant is overcome, the steering wheel jerks in the direction of the correction potentially causing an over correction. This is a safety problem and can cause an accident. I am not seeing any warni

After having the airbag recall for my Mercedes done, the Electronic Steering Lock failed and my car would not start and the steering wheel is locked. They said my car was ready to pick up and when I arrived the car was completely inoperable. The MB Dealership in Lynnfield, MA wants $2700 to fix it. If you google Mercedes W204 ESLfailure you will see thousand of owners on facebook, message boards
... and forums that have this part fail, leaving people stranded in sometime dangerous situations. There should be a recall to permanently fix this issue as owners are getting stranded. Its really an issue of "when" the ESL will fail on older models and not "if". The kicker is, the dealership said only they can fix the problem as it is an anti-theft issue. This seems very wrong, owners shouldn't be forced to pay Mercedes to fix their defective, cheap electric motors in the ESL that fail on almost every model that its in. These defective motors are what causes the steering lock failure in almost every instance. MB acts like its normal to be driving properly one day and then next your steering is completely locked and your car will not start because a $15 electronic motor in the ESL failed. Furthermore, these are All wheel drive or rear wheel drive, in addition to the locked steering wheel which makes towing very, very difficult and unsafe. Thank youRead more
