Subject: Safety Complaint – Loss of Drive Power Prior to Recall Resulting in Crash (2025 Honda Accord Hybrid) I am submitting a safety complaint regarding a 2025 Honda Accord Sport Hybrid (VIN: [XXX]) involved in a crash on [XXX], which we believe is related to a recall issued later in January 2026. At the time of the incident, the vehicle was being driven under normal conditions, below the sp
... eed limit, on an empty road. The driver suddenly experienced a loss of control, the vehicle became unresponsive, veered out of its lane, and did not respond to steering correction. The vehicle ultimately collided with an electric pole and was declared a total loss. At the time of the crash, no recall or warning had been issued. We reported the incident to Honda on October 23, 2025 (Case #[XXX]), describing the loss of control and unresponsiveness. However, no mention was made of any potential defect or safety concern. In January 2026, we received a recall notice stating that if the ICM CPU resets while the vehicle is in operation, it may cause loss of drive power and increase the risk of a crash. This description closely matches what occurred during our accident. We also informed the dealership, Scott Honda (Vineland, NJ), but our concerns were not investigated. Instead, the dealership attempted to sell us another vehicle without addressing the safety issue. We believe the defect existed prior to the recall and may have directly contributed to the crash. We are also concerned that the manufacturer did not warn us despite our earlier complaint describing similar symptoms. This incident resulted in the total loss of our vehicle and ongoing financial hardship, as we have been without a replacement vehicle for over six months. We respectfully request that NHTSA investigate the timing of the recall, the manufacturer’s awareness of the defect, and how pre-recall complaints like ours were handled. We are willing to provide all supporting documents. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5Read more

The power steering has stopped working. After starting the car it shows a "Steering Assist Fault" message and the steering doesn't work. Thankfully this happened when the car was stopped. My car battery is fine so this must be a failure in the Electric Power Assisted Steering (EPAS) system.

The SRS spiral cable assembly (clock spring), located directly behind the driver's instrument display in the steering column, is producing an intermittent crackling noise. The component is available for inspection upon request. Safety risk: The clock spring is a safety-critical component responsible for airbag deployment, horn operation, and steering wheel controls. A malfunctioning clock spring
... can cause airbag non-deployment in a collision or inadvertent airbag deployment while driving. The intermittent nature of the malfunction suggests internal ribbon cable contact or thermal expansion failure within the SRS circuit, which may worsen over time. The problem has not been confirmed by the dealer. The vehicle has been brought to Lexus of Jacksonville, a Lexus authorized service center, on 3 occasion(s). On each visit the dealer was unable to reproduce the noise and closed the repair order without repair. The noise is intermittent, occurring after approximately 60–90 minutes of continuous highway driving, lasting 10–15 seconds before stopping. It occurs with the steering wheel straight and during turns of varying degrees. The thermal onset pattern is consistent with internal SRS spiral cable component failure under heat soak conditions. The vehicle has been inspected by the Lexus authorized dealer only. Two audio recordings of the crackling sound were provided to the dealer with confirmation from the service foreman the crackling noise is heard. No independent inspection has been performed to date. There are no warning lamps or dashboard messages associated with this malfunction. The only symptom is the audible crackling noise originating from the steering column clock spring location. The noise first appeared at approximately 700 miles and has occurred on multiple occasions since.Read more

The vehicle experienced significant vibration at highway speeds and during braking within the first 6 months of ownership. The issue was confirmed by a dealership, and the rotors were resurfaced as a corrective action. The problem has now returned, and the rear brakes have worn down to metal-on-metal at approximately 35,000 miles. The rear brake pads are also unevenly worn, while the front brake
... pads remain in good condition. Multiple service representatives have stated they cannot explain this level of uneven rear brake wear. The brake defect causes strong vibration that impacts steering control and vehicle stability, especially at highway speeds and while slowing down. This creates a safety concern due to reduced braking performance and loss of consistent vehicle control. The issue has been confirmed by dealerships, but the root cause has not been identified or properly resolved. No warning lights or alerts were provided prior to the failure. The vehicle is currently unsafe to drive due to the condition of the brakes.Read more

I am reporting a safety-related electrical failure involving the heated steering wheel system in a 2023 Jeep Wrangler. While driving, the heated steering wheel malfunctioned and produced smoke from the steering wheel area. This created an immediate safety concern due to the presence of smoke and the potential for electrical overheating or fire in a critical driver control component. This was not a
... n isolated incident. The same system required a prior repair under warranty in late 2023. Despite that repair, the failure recurred in late 2025 with more severe symptoms, including overheating and damage to electrical components. The vehicle was taken out of service for repair, and the total time out of service related to this issue was approximately 115 days due to parts delays and repair time. The problem was confirmed and repaired by authorized dealerships, including replacement of the steering wheel and related electrical components. There were no meaningful warning indicators prior to the failure. The issue occurred during normal operations. The repeated failure of the same system, combined with smoke and electrical malfunction in the steering wheel, raises concern about a potential defect that could pose a safety risk to drivers and passengers.Read more

Vehicle steering wheel fails to return to center after sweeping turn. Found it’s a common issue and is incredibly dangerous and should be recalled!
Incident date: Mar 14, 2026
There's a very common issue with these vehicles where the steering rack breaks and causes a number of issues including a clunking noise when driving over bumps and a rattle in steering wheel when turning left and right. However the most important and dangerous issue is that the steering wheel stops returning to center after making a turn. The steering wheel will move about 1/2 of the way back to c

You can imagine how dangerous this could be if someone doesn't realize what's happening and the car continues turning and goes into oncoming traffic. GM can fix it by replacing the rack, but it's a $4k repair and will happen again after replacing it.Read more

Incident Date,March 07, 2026] Current Mileage, 31625 miles I am reporting a catastrophic structural failure of a safety-critical suspension component on my Tesla Model Y. Upon inspection, I discovered that one of the bolts securing the driver-side lower control arm has completely detached and fallen off. A second bolt is approximately 50% loose. This has caused a significant safety hazard
... , as the control arm is no longer properly secured to the subframe, leading to a high risk of total steering loss and wheel detachment while driving. Furthermore, when I contacted Tesla Service, they demonstrated extreme negligence by instructing me to drive the vehicle in this lethal condition to their service center. They also initially refused to cover the towing and repair costs, claiming it was not a factory defect despite the clear evidence of fastener failure (improper torque or thread locker application during manufacturing). This is a clear manufacturing defect that poses an immediate threat to the driver, passengers, and other road users. I am requesting NHTSA to investigate this batch of Model Y vehicles for suspension fastener integrity issues to prevent potential fatal accidents.Read more

The vehicle experienced a failure of the air suspension system, initially presenting with warnings indicating the vehicle was raising and advising not to drive to avoid damage. Shortly thereafter, the vehicle became visibly uneven, tilting approximately 2–3 inches to the driver side. I took the vehicle to Rivian service, where they confirmed the air suspension had failed. The repair was not cove
... red under warranty, so I initially declined service while attempting to escalate the issue. During this time, the condition worsened significantly, with the vehicle continuing to tilt and the suspension actively lowering while driving. On multiple occasions, the vehicle became unstable and began bouncing uncontrollably, making it difficult to steer and maintain control, creating a serious safety risk and potential for collision. The issue was ongoing and reproducible. I ultimately proceeded with the repair and had the suspension system replaced; however, following the repair, the vehicle is now tilting in the opposite direction (passenger side), indicating a continued or recurring failure. The vehicle is available for inspection, and a follow-up service appointment is scheduled. The cause appears to be a failure of the air suspension system and/or improper repair, though the exact root cause is unknown.Read more

I purchased a Certified Pre-Owned Mazda CX-70 that exhibits intermittent and unpredictable steering behavior, creating a significant safety concern. The vehicle also intermittently behaves as though lane-keeping assistance is active—auto-correcting relative to lane lines—despite all lane assist features being turned off. In multiple instances, the vehicle has tracked erratically along lane lines or navigated slight curves without driver input. The issue occurs unpredictably but is more pronounced on rural or uneven roads and during initial driving, though it can persist on highways as well. The issue caused several near miss accidents-twice it sent my vehicle careening into oncoming traffic; another time it tried to drive me back toward a pedestrian I was driving around intentionally. Driver has to be at constant heightened attention to avoid accident. Multiple repair attempts were made, including replacement of the steering column. The condition temporarily improved but returned and worsened. During the final repair attempt, dealership foreman, master technicians, and manufacturer technical support were involved, and the condition was ultimately deemed a “characteristic” with no repair available. The vehicle remains unrepaired and unsafe to operate. The dealer is refusing to supply the customer with any details of the repair order and diagnostic work that was done. The manufacturer has agreed to repurchase the vehicle under lemon law, confirming the defect could not be resolved.Read more

On March 20, 2026, while my 2022 Audi Q4 e-tron (VIN: [XXX] ) was parked in my private garage at approximately 98% battery charge, I attempted to start the vehicle and experienced a complete electrical system failure. All electronic systems failed simultaneously, including: brake system (Brake System Failure warning displayed), parking brake sensor, traffic/charging system, power windows, and side

The vehicle was towed to Audi Beverly Hills for inspection. The dealership diagnosed the fault as: Control Module Main Processor Performance, Symptom Code 20079, per Technical Service Bulletin TSB 2074484/3. Per that TSB, the dealer was instructed by Audi of America to create a PSS (Pending Service Solution) record, erase the fault memory, and release the vehicle to me with no repair performed. The stated guidance from Audi of America to the dealer was to inform me that this is "an extremely infrequent fault" that is "being reviewed." No fix exists. No timeline for a fix has been provided. The vehicle was released to me with the underlying defect unresolved. This is a known, manufacturer-acknowledged defect with no current remedy. As a fully electric vehicle, the Q4 e-tron's braking and all safety systems are electronically dependent. A recurrence of this fault while driving presents an unreasonable safety risk.
