On March 14, 2026, at 20:54:21 UTC, while the Autopilot system was engaged, the vehicle malfunctioned by initiating a sudden, unprompted right-hand steering command within 0.3 seconds, resulting in a collision with another vehicle traveling in the adjacent right lane. Telemetry data from the event confirms the system failed to maintain the lane despite the Primary Steering Torque Sensor measuring
... 0.00 Nm, proving no driver input caused the maneuver. This unintended steering event at cruising speed created an immediate safety risk to myself and the occupants of the other vehicle. To date, the manufacturer (Tesla) has declined to perform a technical or remote diagnostic review, redirecting the case to a collision center instead. No warning lamps, audio chimes, or system messages occurred prior to the failure; the vehicle is currently available for inspection and raw CSV telemetry logs can be provided to investigators upon request.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The steering gear in this vehicle began to fail around 40K miles. It clunks at low speed, doesn't roll out of turns, and recently has begun to oscillate during straight driving. I will keep the old part for inspection once replaced. How was your safety or the safety of others put at risk? The cars s
... teering does not return to center during a turn. Now, the steering is shaking violently during driving. It will likely seize if left unfixed. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. I also have video. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? This is a well documented issue with Chevy steering. The white paint on my 2016 Honda Civic keeps peeling, which looks bad and is leading to structural rust developing in several areas of my car. My 2019 accord gets stiff at speeds over 25-30mph and resists driver input, so much that you tend to over-compensate and make constant steering corrections every second so you don't crash. The car does not track straight at any speed, and does the same when the Lane Keeping Assist (LKA) is off or on. When LKA is active the car will not go straight on a straight smooth road. The car wanders dangero
... usly from the right to left line markers and often rides on the markers bringing you very close to other cars and then jerks the car back to the opposite side of the lane and continues to do the same back and forth like a drunk person is driving the car. Also the Driver Input Required amber light comes frequently on a straight road with highly visible lane markers. It is extremely tiresome to drive the car and try to keep it in its lane, even when I disable all the "driver-assistance" features. This is because the steering has a mind of its own and stiffens up. It resists the slightest driver input so you have to put more strength into it to make it go where you want to, but once the steering gives up fighting you and lets you do it, it results in overcompensating and constant steering corrections by the drive left-right, left-right, just going down in a straight smooth freshly paved road. It’s been over a year and the dealer has yet to detect anything wrong, they tell me my vehicle drives perfect, honda dealer after dealer.Read more

While performing a low-speed reverse turn into a driveway, a loud clunk was heard and the driver side front wheel completely separated from alignment. The vehicle became immediately undrivable. Upon inspection, the ball joint retaining nut was found to be absent from the front driver side suspension assembly. No torque verification mark was present on the fastener. The vehicle is currently at an a
... uthorized Tesla service center for repair and is available for inspection. Safety risk: Complete loss of wheel alignment and directional control. Failure occurred at low speed in a private driveway. Had this occurred at highway speed the consequences could have been catastrophic. Confirmed by dealer: An authorized Tesla service advisor confirmed that a Technical Service Bulletin exists in Tesla’s system for this exact condition. No owner notification was issued prior to failure. The advisor confirmed the retaining nut is installed by an automated robot assembly process with no human torque verification step and no verification mark applied. Prior warning symptoms: NONE. No warning lamps, no messages, no audible symptoms prior to catastrophic failure. The absence of a retaining nut on a safety-critical suspension fastener with no quality verification process represents a systemic assembly defect potentially affecting all vehicles produced under the same assembly process.Read more

After months of this I found out I did not have to take it to the dealer I bought it from. So, I took it to Monument Chevrolet. I explained my concerns related to the front-end wobble which was very intermittent. And the oil consumption issue which I was very concerned with due to the year model and mileage of the truck. Also, the 75,000.00 price tag was really has me extremely upset... Monument Chevrolet treated me well. I had actually brought it in and called about the steering which was greatly affected by the severe front end wobble more than one time. I was told at first the oil consumption was pretty normal. Until I told them how much I had to add between changes. I have attached two work orders to this complaint. These work orders show what was done at 18017 miles and 18323 miles. The later work order was when the PICO test was done. Also, an oil consumption test was started once I picked the truck up. After I had driven it for 2077 miles and was bringing the vehicle back to Monument Chevrolet, as instructed for the oil consumption results, when a loud squeeling, then hard knock, power loss and I pulled over. I called Monument and was instructed if I could get it to start and bring it. I drove it to them while knocking and rough running. It has been in the shop for over 2 months for an engine replacement. 2077 miles after recall work!
