This incident occurred on a public roadway while I was transporting my family, including two young children (ages [XXX] and [XXX] ). The uncommanded loss of steering assist created an immediate and serious safety hazard and could have resulted in a crash, serious injuries, or fatalities to vehicle occupants and others on the road. I was able to bring the vehicle to a stop and subsequently to the dealership, only with significant effort and risk. There were no prior warning lamps, noises, reduced steering assist, or other symptoms before the failure. The malfunction was abrupt and occurred without any advance indication. The vehicle had approximately 39,000 miles at the time, which is well below what would reasonably be expected for a power steering rack failure. The vehicle was not modified or abused and was driven under normal conditions. Audi dealership confirmed failure of the electric power steering system and diagnosed the issue as a power steering rack failure requiring full replacement. The repair was completed only after the vehicle became unsafe to operate. The removed component may be available through the servicing dealer or Audi of America for inspection upon request, and I have retained all repair documentation. Audi of America was notified of this incident after the repair but declined responsibility. Based on subsequent research, I am aware of multiple similar reports involving Audi Q5 power steering failures. I am submitting to assist NHTSA in determining whether this issue poses a broader risk. There were no warning indicators, messages, or prior signs of a problem before the incident. The driver was able to bring the vehicle to a stop in a nearby turn area and arranged for it to be towed. Months later, I have still not been contacted, and the recall issue remains unresolved. Based on my experience and publicly documented issues, this appears consistent with known defects associated with this model, including reported water pump failures leading to engine overheating, rear suspension issues affecting control, and concerns regarding exhaust fumes entering the cabin. This is not normal wear and tear, but a potential safety-related defect. For months my vehicle has had intermittent loss of power steering making it nearly impossible to control direction. On Jan. 29, 2026, I ran into the back of a vehicle that stopped suddenly and due to sudden power steering loss, I wasn't able to swerve and avoid collision. I was issued an infraction for this incident and had to file an insurance claim to repair the front end damage to my vehicle.

I was also notified that Ford issued Safety Recall Campaign/NHTSA #14S05/14V284 for this exact issue. I contacted the Ford dealership where I purchased my vehicle and they said my VIN is not included in that recall. It seems too coincidental that my 2011 Ford Escape is experiencing the very same issue that prompted a safety recall for thousands of 2008-2010 Ford Escapes but not mine. By denying this safety repair, Ford is forcing an unsafe vehicle to remain on the road or operate with manual steering, which is a major crash risk. The alignment could not be maintained, indicating an underlying mechanical issue. Tesla inspected the vehicle but stated that no suspension components required replacement and attributed the issue only to alignment. However, alignment does not go out of specification without a mechanical cause. Despite multiple requests, Tesla refused to provide detailed documentation regarding the inspection or repair of the suspension system. They replaced only one rear tire and did not address the root cause. The other rear tire had already worn completely bald. After approximately 5,386 miles, the replacement tire has again worn down close to the minimum safety limit, demonstrating that the underlying issue was not corrected and continues to affect vehicle safety. There were no warning lights or system messages prior to the failure. The first symptoms were abnormal tire wear, increased energy consumption, and unstable driving behavior at highway speeds, which began shortly after vehicle purchase.
