We experienced power steering failure. Our steering wheel control was taken over while backing out of our driveway and while driving on a highway. When this happens, you can’t turn the wheel. It becomes locked and then you become stuck. It is extremely unsafe and put our whole family at risk of getting in an accident. It is currently sitting at the dealership and has been there for weeks. We hav

We have been out of a car for weeks because the steering rack part is impossible to get and is in very high demand which leads us to believe we are not the only ones experiencing this. Again I stress, this was very scary for our family and it really puts people at risk. It is a new car and we should not have experienced this.Read more

Subject: Request for Re-Evaluation and Expansion of Recall 23V-770 – Steering Gear Failure I am submitting this letter to formally request that the National Highway Traffic Safety Administration (NHTSA) re-evaluate Recall 23V-770 concerning defective steering gear assemblies and investigate additional affected vehicles from the 2022 model year. Vehicles sold during the 2022 production year appe
... ar to experience the same steering gear defect identified in Recall 23V-770. The steering gear assemblies installed in these vehicles use the exact same part numbers as the components already covered under the existing recall. Due to COVID-era production and supply limitations, relatively few of these vehicles were imported and sold in the United States during 2022. As vehicle mileage accumulates, owners are now beginning to experience steering gear failures, many occurring at or before 20,000 miles. These failures indicate that the defect may not be limited to the currently recalled production range. The evidence strongly suggests that additional 2022 vehicles equipped with the same steering gear assemblies should be included in the recall population. This issue presents a serious safety concern due to the potential for loss of steering control and increased crash risk. I respectfully request that NHTSA: • Investigate 2022 vehicles equipped with the same steering gear part numbers covered under Recall 23V-770 • Review warranty claims, field reports, and dealership repair records related to premature steering gear failure • Determine whether the recall scope should be expanded to include these additional vehicles I appreciate your attention to this matter and your continued commitment to publicRead more

The vehicle experienced a sudden steering malfunction involving the electronic steering gear. While backing out of a parking structure, the steering wheel suddenly jerked and locked instead of naturally correcting, resulting in collision with a pillar. There were no warning lights, steering fault messages, or prior symptoms before the incident. Restarting the vehicle did not resolve the issue. Th
... e dealership confirmed the issue and documented “loss of communication with steering gear,” “internal steering gear failure,” and “no signs of damage causing failure.” The steering gear is being replaced under warranty. The failure created a safety risk because steering response suddenly changed without warning. I am concerned the incident could have resulted in a more serious crash had it occurred at higher speed or during active traffic instead of inside a parking structure. Multiple tow attempts through Ford Roadside Assistance reportedly struggled to move the vehicle afterward due to the steering issue. The failed steering gear component is currently being replaced by the dealership.Read more

My 2017 Tesla Model X (VIN: [XXX] ) is experiencing stiff and binding steering caused by corrosion at the lower steering column I-shaft universal joint. The steering intermittently binds and resists normal turning force, which has caused Tesla’s Full Self-Driving (FSD) system to disengage due to abnormal steering resistance — a direct safety concern. Tesla’s own Service Bulletin SB-20-32-004
... documents this exact condition — I-shaft U-joint corrosion causing stiff steering — on Model S and Model X vehicles. The bulletin covers 2015–2020 model years. However, Tesla service center stated that my VIN is not classified under this bulletin and that the $500 repair is an out-of-pocket expense not covered under warranty or goodwill. The issue has not yet been repaired. I am currently operating a vehicle with a known steering defect that Tesla has documented in its own service bulletin but has declined to cover for my specific VIN, despite my vehicle exhibiting the identical failure. I am requesting NHTSA investigate whether the VIN classification for SB-20-32-004 is appropriately capturing all affected vehicles, and whether this issue warrants escalation to a formal safety recall — particularly given the impact on automated driving system performance. While driving on the highway, I first noticed the vehicle becoming difficult to steer. The steering felt unusually heavy and sluggish, as if power steering assistance was reduced or failing. Shortly after this began, multiple warning lights illuminated across the dashboard. At that point, braking performance also became severely compromised. The brake pedal felt soft and traveled nearly to the fl
... oor with very limited stopping power. The vehicle required significantly increased distance to slow and stop, creating a serious safety hazard in active highway traffic. The combination of reduced steering assist and severely degraded braking performance made the vehicle difficult to control and presented a significant safety risk. I was able to bring the vehicle to a stop and it was later towed to a dealership. At the time of inspection, the battery was found to be dead. I had recently returned from a 5-day trip and the vehicle required a jump start before this driving event. The dealership replaced the battery and stated the vehicle was operating normally after repair. However, the incident involved progressive loss of steering assist followed by significant loss of braking performance at highway speed. I am reporting this as a serious safety concern because multiple critical vehicle control systems were affected while driving. I am concerned about potential recurrence of this issue and the risk it presents to driver and public safety. The vehicle is available for inspection.Read more

While driving on a high-speed interstate with two children in the vehicle, and without any prior warning signs or issues, the dashboard began displaying sequential warnings from multiple Advanced Driver Assistance Systems (ADAS) sensors. As the vehicle continued cycling through numerous dashboard warnings, the power steering failed, making the vehicle difficult to control. Additional electronic sy

After reaching the shoulder, the driver powered the vehicle off in an attempt to reset the electrical systems. However, once powered down, the engine would no longer start. When the tow truck arrived, the technician was required to jump-start the battery in order to shift the vehicle into neutral for towing. The technician also showed the driver a low-voltage reading on the jump-start device, indicating a possible alternator or charging-system failure. Hyundai dealership representatives later stated that diagnostic testing of the alternator and electrical system showed no faults and informed the customers that the battery was simply dead. After the dealership replaced the battery, the customers drove the vehicle approximately 15 miles to their residence. During that drive, the vehicle again began displaying the same sequence of sensor and system failures. The power steering failed a second time, and the same electrical malfunctions experienced during the initial incident were repeated.Read more

Incident date: Apr 27, 2026
Our 2012 Ford Fusion had a sudden and complete loss of power steering & “Power Steering Assist Fault’ flashed on dash. The steering immediately became extremely difficult to operate, requiring both hands and excessive physical force just to turn the wheel. It felt less like driving a passenger car and more like wrestling a fully loaded dump truck with flat tires. This created a serious safety
... concern because the failure occurred without warning and fundamentally affected our ability to steer the vehicle safely. Fortunately, my wife was not driving at the time. The amount of force suddenly required to turn the wheel could easily be overwhelming, particularly during an emergency maneuver, in traffic, or while entering an intersection. After the failure occurred, we discovered vehicle had previously been subject to recall 15V340000 and Ford Safety Recall 15S18, both involving the exact electric Power Steering Control Module (“PSCM”) that has now failed. Under that recall, a Ford dealer performed only a software update on 10/17/2016. We opened a case with Ford, brought the vehicle to a local dealer & paid for a diagnostic inspection. Using Ford IDS equipment, dealer confirmed the condition and documented ‘NO COMMUNICATION WITH PSCM’. The dealer report quoted “Steering Gear Assembly - Rack & Pinion Assembly Replace: $3,215.81.” The diagnosis was reported directly to Ford under the active case number. Despite the recall history and confirmed failure of the same steering system covered by the prior recall, Ford denied assistance because the earlier software reprogramming had already been completed. Our position is that this is clearly a hardware failure involving the recalled electric power steering system itself — the precise condition the recall was intended to address. Ford’s own recall communications dated May 17, 2019 specifically instructed owners to retain recall documentation in the event this exact situation later occurred.Read more

Nissan had a recall 2 years ago for a tie rod issue. When I brought the car in for that recall Nissan said that the tie rods showed no sign of bending so they didn't replace them. Two years later my car is not steering correctly and making noises. I find out that Nissan has a new tie rod recall (recall R23B3, NHTSA #23V-581) that requires the tie rods to be changed regardless if they show any sign
... of bending or damage. I bring my car in to have this completed (2 years from the first recall) and they are telling me the power steering rack has to be replaced as well as the tie rods. The issue is Nissan now knows that by not replacing the tie rods two years ago it's damaging the power sterring rack but they will not acknowledge it as they don't want to pay the additional cost for the power sterring rack. I spoke to Nissan USA consumer affairs and they are taking a hard line on this. It's dangerous as the car dealership service department was going to let me drive my car after the tie rod replacement knowing the power steering rack was also damaged until I pressed the actual technician that worked on the car and he said it's dangerous to drive. Nissan must be reponsible for replacing all the damage caused by not making it mandatory to replace the tie rods in the first recall two years ago. The cost to replace the power steering rack is $3000 and they don't want to pay for that.Read more

Component/System: Steering System (Electronic Power Steering / Steering Rack) What happened? I am experiencing a recurring and escalating failure of the steering system on my Mazda CX-90 PHEV. Currently, the steering wheel momentarily "locks up" or experiences a sudden, significant increase in resistance (stickiness) during left turns, especially when a large steering angle is applied. This occur
... s intermittently and without any warning lamps or error messages on the dashboard. Safety Risk: This failure poses a severe safety risk as it hinders my ability to complete turns smoothly and predictably. The sudden resistance requires unexpected physical effort to overcome, which could lead to a loss of vehicle control or a collision, especially in tight maneuvers or emergency situations. History & Inspection: Previous Symptoms: Approximately one year ago, the vehicle exhibited a "grinding/squeaking" noise when the steering wheel was turned to full lock. Prior Repair: A dealership inspected the vehicle and performed a recall repair (related to Recall 24V-022) by applying grease to the steering components. This temporarily resolved the noise. Recurrence: The noise has recently returned, and the new issue of "momentary locking" has now emerged. Inspection Status: The vehicle has been previously inspected for the related recall, and the current malfunctioning system is available for inspection upon request. Additional Context: I am reporting this because I believe the initial recall fix (grease application) was insufficient, and the vehicle is now demonstrating the "sticky steering" symptoms currently under NHTSA investigation (RQ26002).Read more

Motorcycle has exhibited unstable handling behavior since delivery. The defect is specifically a weaving and wobbling oscillation that occurs when the motorcycle is upright and traveling in a straight line, as if the bike is constantly riding on rippled or uneven pavement even on smooth road surfaces. The rear of the motorcycle feels disconnected from the chassis, with the rear tire feeling as tho
... ugh it is not tracking with the front. The instability is present at all speeds but becomes pronounced and consistent at approximately 40 mph and above. Critically, the defect occurs only when the motorcycle is upright. The instability is not present during cornering or while braking — the bike feels stable when leaned over or under deceleration. The weaving and wobbling are confined to straight-line, upright riding. The defect is present: •In all ride modes (Road, Rain, Dynamic, Dynamic Pro) •Both loaded and unloaded •With top case and panniers fitted — no difference observed •On and off throttle •On smooth pavement with no road-surface contribution The instability is not consistent with normal motorcycle dynamics and is not corrected by tire pressure adjustment, rider input, or ride mode selection. Dealer interaction: Original selling dealer was non-responsive to handling complaints. Motorcycle is scheduled for diagnostic evaluation at a second BMW Motorrad dealership on June 1, 2026. BMW Motorrad USA customer relations case has been opened [case number if available]. Safety concern: Sustained weaving and wobbling instability at highway speeds while traveling upright on a motorcycle creates a significant crash risk, particularly during straight-line highway riding, lane changes, or in crosswinds. The defect has been present since delivery and has not been resolved through normal service channels. No crash, injury, or fire has occurred to date. Report is filed to document a safety-relevant defect present since new on a current-production R 1300 platform motorcyclRead more

Vehicle: 2024 Honda Prologue Touring AWD The vehicle has an ongoing clicking/noise issue coming from the driver-side front CV joint/axle area, especially during turns, acceleration, and low-speed maneuvering. The clicking noise appears to be progressively worsening and raises concerns about premature CV joint failure and potential drivetrain safety issues. The issue has persisted despite service
... visits and replacement attempts. Additionally, the vehicle intermittently displays a “Charge Port Door Open” warning message even when the charge port door is fully closed and secured. This false warning can prevent charging functions from operating normally and creates concerns about electrical system reliability and sensor malfunction. Both issues occur intermittently but repeatedly, affecting confidence in the vehicle’s safety and reliability. The ongoing driver-side CV joint clicking noise has caused concerns about potential drivetrain failure while operating the vehicle. Due to the worsening nature of the issue and repeated occurrence, I no longer feel fully safe or confident driving the vehicle, especially during turns and normal road operation. The false charge port warning may also interfere with the vehicle’s charging capability and normal operation.Read more
