Despite multiple written requests, GM has not provided the following key event-data modules: – Brake System Control Module (EBCM/iBooster) export (brake pressure, pedal position, brake override flags) – Propulsion/Drive Control Module data (torque request vs actual torque and accelerator input) – Front Camera Module metadata (timestamps and event record IDs) Additionally, several data discrepancies remain unresolved — including mismatched timestamps between the ASCM and FCM reports and camera images that do not correspond to the actual crash scene. On October 29 2025, GM publicly announced Customer Satisfaction Program N252521980, covering certain 2023–24 Cadillac Lyriq and 2024 Chevy Blazer EV vehicles for a Brake System Control Module software update due to possible brake-control performance issues. Because my incident involved loss of braking response, this recall appears directly relevant. I have requested confirmation whether my vehicle is affected and whether this module was analyzed in GM’s investigation, but have not received a reply. I am requesting that NHTSA review GM’s handling of this case to determine whether the engineering review was complete and whether the vehicle’s BSCM or related systems could be implicated in unintended acceleration or brake override failures. At 5000-7900 miles I was told I needed brakes all around my vehicle. No warranty was applied I paid cash. Left the shop and the noise from the front continued and the squealing from the brakes was still very loud. I brought it back to the shop for treatment care. The rep stated it was a leaf in the front of my engine and he put some special oil on the brakes to stop the squealing but to no avail. The noise continued in both places. I brought the car back for my second treatment and then I was told they hear the noise in the front. They kept the car for a couple of days called me and stated the head dash board was burnt out. The same complaint I was making light flashing and noise. I went to pick the vehicle up stated my warranty had to cover almost $11,000 worth of work. I was devastated, depressed and then I asked is the car safe for me to drive the rep said yes. I was afraid but I had to take my car and leave. I left with tears in my eyes. I could not believe I saved my last of savings to buy me a reliable, dependable Luxury car due to my severe medical issues to make me feel better. I am very disappointed, confused, devastated and very saddened about my brand new vehicle I purchased for myself. I am reporting an ongoing safety issue involving the Skid Control ECU and loss of power break recall on my 2024 Toyota corolla hybrid. The first time, my vehicle experienced loss of power brake assist and displayed a “Hybrid System Malfunction” message on screen. I towed my car to Toyota, confirmed it was related to the Skid Control ECU recall and repaired it under the recall at no cost, the
... y did a software update. The second time, the exact same problem reoccurred. I had the vehicle towed again to a Toyota dealership, but this time, they refused to perform diagnostics unless I paid out of pocket for a 12-volt battery replacement. The dealership stated they cannot confirm if the issue is recall-related until diagnostics are complete, but they also refuse to complete diagnostics without me paying first. My vehicle is currently undrivable and unsafe, and I rely on it for daily transportation and work. So they made my pay for this 12v battery to do the diagnostics and now they tell me it is not recall related but one of the relays not working, the repair will be cover under warrant but i had to pay for battery and rental cars . I believe Toyota is in violation of its obligations under 49 U.S.C. §30120, which requires manufacturers to correct recall-related safety defects free of charge. This issue leaves me without a vehicle and unable to verify that the recall has been properly resolved. I have opened a case with toyota brand engagement. I am requesting that NHTSA review this case and ensure that Toyota complies with recall repair requirements so my vehicle can be safely diagnosed and repaired without charge.Read more

While driving out of my neighborhood at a low speed, the vehicle’s brakes suddenly stopped responding. I pressed the brake pedal multiple times, but it felt soft and failed to slow or stop the car as expected. I had to rely on the parking brake to bring the vehicle to a stop safely. Fortunately, there were no injuries or collisions, but this created a serious safety risk for myself and others.

After researching, I discovered that my vehicle model and year have been associated with a known braking issue, as described in a recall for certain 2018–2019 Toyota Camry vehicles related to a vacuum pump or braking system malfunction. I took the vehicle to an authorized Toyota dealer immediately after the incident. The dealer confirmed the brake vacuum pump had failed, matching the symptoms described in the existing recall for other vehicles of the same model and year. However, they stated that my specific VIN was not included in the recall and refused to cover the repair under warranty or recall service. ... ly engage the brakes. This results in rapidly and unexpectedly cutting speed by 10-15 mph before automatically resuming set speed 1-2 seconds later. This has resulted in trailing vehicles to think I am brake-checking them or otherwise caused them to abruptly brake or swerve out from behind me. It also can cause driver and occupants in the Odyssey to lurch forward and back unexpectedly - which has had a residual affect of unintended input on the steering wheel, cascading into unintended direction change and amplifying vehicle instability issues already introduced by the sudden speed change. There are multiple other owners of 2018+ Odysseys on the forum [XXX] which have reported similar situations. I have tried to encourage them to register their observations here. I believe this issue poses a significant risk to the operator and occupants, as well as surrounding motorists. Rental/fleet vehicles with drivers who are otherwise unfamiliar with the issue would be at exceptional risk. So the car stays, the car was returned a couple more days, theres still sounds and vibration coming from the brakes. Now they are telling me that the regenerative braking system that use to work when I first bought the car that it doesnt work and Sacramento cdjr is saying that it only charges 1% over where it's at. All false. (They also mention that there were a recall and that they needed to reprogram the car but we never got a letter or notice for it). Folsom CDJR also said they resurfaced the rotors, but when the car was returned to me they were all rusty rotors. Also the fuel meter gauge started jumping up and down, and fuel burning faster then before. Things were were not normal after the services. Dealer re-set the codes and sent the car home. This could have been a significant safety issue if the event had occurred under different circumstances. This same dealer did not mention this to me at the time of sale, that they had been subject to a recall, and since I did not have them inspect the car immediately after service (I had to miss work and my son drove me after he was off work and I feared they would close before I arrived so I had them park the car outside and the sales desk had the key). FCA issued recall C4B notification at the end of 2024, with estimated parts availability in the 2nd quarter of 2025. It is now the 4th quarter of 2025 (almost a year later!) with no update on parts availability. Considering that malfunction of the braking system is a critical safety hazard, it is unacceptable that they have and continue to delay a timely resolution for those impacted.

... told that GM had recommended several things that could be the cause of light coming on still could not find issue. Last thing GM and dealership recommended was to replace the battery. Cost $104.00 Check engine light came on 10/25/2025 while returning home from Tennessee to Michigan, light came on in Louisville, KY. Had it scanned, booster sensor was the cause. Called AAA had them replace battery light went off. (as you know that if you do that it reset the computer not surprised light went off drove rest of the way home to Michigan) Cost of replacement battery $295.00 Check engine light came on again 11/2/2025 my best bet is that it is booster sensor again. NHTSA and GM need to fix this issue. I have not lost breaking ability yet but the horror story that I've read on this site scars me. The airbags never opened.The car was totaled(the insurance company declared it a total lost and refused to repair and paid us a small amount , less than we paid for the car. This was the second repeat incident, the first one very similar was reported to NHTSA by my erhusband ,reported to NHTSA 0n 1/20/2025, 6.19 pm; complaint number 11637163 .after the first crash we tried to get Tesla to pick up the car . they refused. Instead they said they would check it remotely. We gave them the opportunity to inspect and repair the car between the first and second incident .
