On several occasions the Toyota Mirai would slam on the brakes on its own while at a considerable speed with no other cars around. It was very scary and concerning traveling at high speeds and the brakes being applied abruptly automatically.

We only learned of the recall recently through the Toyota app, not from the dealership. This is unacceptable given that brake recalls pose a serious safety risk. It felt as though only the regenerative braking was (slowly) working and that my manual brake application had zero effect. The vehicle continued to glide forward with no significant braking power, and I was unable to stop the car in time, resulting in a collision with the rear of the vehicle ahead. Summary of Defect: The primary safety failure was the complete lack of response from the manual brake pedal. My attempt to override the vehicle's automated systems in an emergency was unsuccessful, as the brake pedal was unresponsive. Reporting History: I have already reported this incident to my auto insurance, my local Chevrolet dealership, and the Chevrolet Customer Assistance Center. 2022 Toyota Venza TOTAL BRAKE FAILURE. On 11/8/2025 brakes did not engage while exiting a major highway. There was no warning lights on the dashboard, simply the brakes failed us to come to a stop until we hit the concrete curb to come t a stop. Once we hit the concrete curb, we had to tow the vehicle to a mechanic to change the brakes, rotors and the lower control arm. After the impact from hitting the curb, the control arm was damaged and pushing rotors on the driver side, where the vehicle was undrivable from the control arm pushing and grinding into the rotors. See attached pictures. This is the second time it happened where the brakes did not get engage. First time braking system engaged after multiple attempts to stop the car, this time braked totally failed to stop the car. This is a 100% a safety issue where people life could be lost in matter of seconds from no fault of their own whatsoever, where brakes are supposed to engage at ALL the time no Exceptions. This accident was fully due to braking system failure on 2022 Toyota Venza. We also have 2024 Toyota Prius where sometime brakes make a clinging noise and do not engage until brakes are pumped multiple times, which could also cause a major accident and fatalities. I searched online and it seems other drivers to report same problem on Venza and other Toyotas, this could very well be a major Toyota manufacturing problem with braking system across their vehicle and an investigation and a recall must be issued before peoples lives are lost if no actions are taken and negligence prevails.Read more

... issue with my brake booster. To make sure this was the issue I followed up and went to a Toyota dealership for a diagnostic. They initially told me that "all brake systems components were functioning normally at this time" and that all I need to do was to change the break fluid. I did this, but the problem persisted. I then returned to the Toyota dealership for a second diagnostic. This time they noted that the brake master cylinder and brake booster are loosing pressure internally and need to be replaced. When I googled this I saw that some 2019 Corolla (and other hybrid) vehicles were included in a 2019 recall for a brake booster pump that could fail, leading to a loss of braking assist, similar to what I was experiencing. I brought this up with the Toyota brand engagement center. They said there was no recall at this time on file, but that they would put me in touch with a representative who would manage my case (case # 251015000598). The representative would not pick up my phone calls. I was not able to speak with him. After many calls, a different representative spoke with me and told me that the case was already closed and that the assigned representative said he had explained to me why. This is not true as I had not had the chance to speak with him. I am concerned that Toyota has not given my break booster pump due diligence and is dismissing my legitimate safety concern and I would like to report this safety problem to the NHTSA.Read more

The person would not allow me to file for reimbursemenent and after going back and forth finally told me file a claim.I did. Another weeks past and I called again, went through the whole explanation again. Asked to speak to supervisor and after a conversation told me go to dealer to file papers.I did and service manager at first started saying the same thing but eventually put in the claim. I waited several weeks, then check with dealer who told me Ford had to give a code. Called Ford was told no code needed.Called dealer and told them. Several weeks later check dealer and was told still needed code. I called Ford, and asked for a code I was told again about the warranty expiring .I spoke with supervisor same problem. Is 4 months. I told him to check to see if there are any recalls active, he said I do see a recall, but this doesn’t have anything to do with your car. The next day I heard from the service manager told me I have an update. I have attached the diagnosis of my car. This is what they say needs to be repaired in my vehicle. I asked my service advisor questions about how related the recalled part was to the issue. You can see the conversation in the pictures. At this point, I created a case with you all when he started to argue over terminology. I had a conversation on the phone with Jose and my service advisor later where they then told me that there is no recall that is showing up. I said I need to look over this because I saw a recall there. I verified this and messaged the advisor back. There were no further conversation conversations over the phone. Everything was done over messages and you can see them below. I was being threatened to be charged storage fees. And they said that they performed the recall, but the car was not confirmed to be in drivable condition. I did not go by to pick the car up. I called corporate numerous times. I have an ongoing safety concern on my vehicle, seemingly having to do with the brakes or ABS malfunction. There is a delay in the breaking of the vehicle occasionally in specific circumstances. Typically in a parking lot or stop sign, if there is a buckling of the road or potholes or uneven surfaces between the tires, the vehicle will skid over the surfaces and braking is delayed. Because this typically happens at a stop sign and the car has driven through the stop sign the issue could absolutely cause a multi exhale accident. Because this issue is surface specific, it is difficult to reproduce and the dealership is unable to find the root cause and fix the vehicle. During the two service visits, the dealer has tried to find the issue by doing a diagnostic scan and did a road test on each of the visits as well. In the four months that I’ve owned this vehicle and 10,000 miles that I have driven. This is happened less than a dozen times – most of the incidents are personally documented. No visible warning lights engage when the events have happened. I have contacted Hyundai consumer affairs and am working with them on a resolution of which neither party is sure of at this point. While going 80mph on the highway at 2:00am on or around 10/6/2023, my power braking system went out without warning. I was able to get it off the road and to a shop where they told me it was the vacuum pump and cam shaft. Repair took a week. 7 months later on 5/31/2024, it went out again, luckily in town going 35mph, and involved the vacuum pump, cam shaft, and this time the oil pump as well.

While driving my 2025 Jeep Wagoneer S, I experienced a sudden loss of regenerative braking, which caused the vehicle to take much longer to stop than normal. On initial brake application, deceleration was greatly reduced and required substantially more pedal travel and pressure to bring the vehicle to a stop. A “Regenerative Braking Failure” warning appeared in the instrument cluster. At the
... same time, the vehicle’s power output dropped. The display showed multiple warnings, including Electrical Vehicle System Fault (wrench-and-bolt icon) and Reduced Power (“turtle”). A message appeared stating that the vehicle would shut down soon. Additional messages indicated that the Forward Collision Warning (FCW) and Automatic Emergency Braking (AEB) systems were disabled, further reducing braking support and collision avoidance capabilities. I was able to stop safely, shifted to Park, and powered off the vehicle. After opening and closing the driver’s door, I waited for all displays and indicator lights to turn off completely. A distinct mechanical clunk was then heard, consistent with the sound of high-voltage contactors disengaging. When I restarted the vehicle, all warnings cleared, and it drove normally again. This event created a potential safety risk due to the unexpected loss of regenerative braking, reduced propulsion power, and temporary disabling of FCW and AEB systems. These symptoms match those described in Audi’s recall 46P6, which involves improper assembly of the bolt connection between the brake-pedal input rod and brake-booster actuator rod.
