Multiple safety-critical malfunctions have occurred on this vehicle involving unintended braking, unintended acceleration, and false activation of driver-assist systems. These events did not happen at the same time and occurred under normal driving conditions. At various points, the vehicle has suddenly applied the brakes without driver input, including hard braking events that felt like an automa
... tic emergency braking activation when no hazard was present. In separate incidents, the parking brake has engaged while the vehicle was in motion, causing abrupt deceleration. There have also been instances of unintended acceleration where the vehicle surged forward briefly without additional throttle input. These malfunctions appear to be related to electrical or ADAS system faults affecting braking logic and propulsion control. No warning lamps or messages reliably appeared before these events. The issues pose a serious risk to the safety of the driver and others because the vehicle’s behavior is unpredictable and can interfere with safe control. The dealership was unable to reproduce the conditions during inspection, but the incidents have occurred multiple times in real-world use.Read more

I rented the Toyota RAV4 from Fox Rent a Car (855 571-8414) at the Denver International Airport for a trip to Boulder, CO. After arriving at the hilltop, as I parked the vehicle, while applying very light pressure to the accelerator pedal with my right foot, I felt a gradual drop of several inches on the passenger side of the vehicle. Instinctively, I moved my right foot to the brake pedal and applied pressure. However, I immediately sensed that the vehicle was not decelerating; instead, it was accelerating forward. I pressed the brake pedal more firmly, and the vehicle accelerated further until it came to a complete stop after colliding with a rock on the driver's side approximately 15 feet from the point of initial acceleration. Once the situation stabilized, I exited the vehicle and observed a red, non-oily liquid of normal viscosity leaking from underneath the vehicle on the front driver's side, which I later identified as coolant. Upon opening the hood, I confirmed that approximately half of the coolant had been depleted. I Googled AI Mode: Question: For a Toyota RAV4 HEV, is it possible for the vehicle to accelerate when the brakes are applied forcefully and abruptly? The vehicle was taken to a GM dealership for diagnosis and repair. The dealership confirmed that the brake system had failed and there was inadequate stopping force being produced. The entire master cylinder assembly which includes the hydraulic valve block and electric motor were replaced under warranty. The vehicle had 24,638 miles at the time of failure. Prior to the failure, there were zero indications that there were any issues with the brake system. Fortunately when the brake system failed the vehicle had just backed away from a parking spot and pulled forward towards a stop sign at low speed. The potential concern was that pedestrians, fellow motorists or the driver of the vehicle could have experienced significant injury or death due to the vehicle’s brakes failing.Read more

To Whom It May Concern, I am filing a formal dispute and safety complaint regarding my 2022 Subaru Crosstrek Limited. This was a terrifying and dangerous event for me and my family. We could have been seriously injured or worse. The service department
... later claimed that the issue might be related to the battery, but I am not convinced that this explanation is accurate or acceptable. A vehicle with only around 41,000 miles should not suddenly lose power or stop at highway speed. This is a serious safety defect and raises strong concerns about the vehicle’s reliability, engineering, and overall safety. I request that Subaru of America open a formal investigation into this incident and document it as part of my ongoing complaints. I also ask that this matter be reviewed under the Florida Lemon Law and BBB Auto Line programs, since this issue demonstrates a clear and dangerous failure of the vehicle’s systems that should not occur under normal operation. This issue has happened to me twice now. Dec 19th 2024 I Driving on a highway at about 60mph, traffic slowed, I tried to stop and instantly had no brakes at all. On October 13th 2025, while driving, I began to hear a clunking and rattling sound from the front left wheel area when braking. Concerned for my safety, I brought the vehicle to a local independent brake specialist for inspection. Their technician found that one of the two bolts securing the brake caliper was completely missing, and the remaining bolt was approximately halfway backed out. The shop
... explained that this condition presented an immediate risk of caliper detachment and total brake failure, potentially leading to serious injury or death. After both bolts were replaced and properly torqued, the brake noise that Tesla had previously attributed to “pad burnishing” disappeared entirely. This indicates that the squeaking and vibration symptoms Tesla dismissed were, in fact, caused by an improperly secured caliper—a direct safety issue, not a normal wear condition. I notified Tesla legal of this problem (via certified mail) on October 13 2025 and requested a response within 14 days. I have received no such response, nor have I been contacted for further investigation into this dangerous situation with my vehicle.Read more

The brake servo control unit feel simulator which reduces the simulation force provided to the brake pedal during emergency or failed conditions and is a key component of realizing a Regenerative Braking System. It provides a good pedal feeling to a driver, improves energy recovery and ensures braking security. My safety has been put at risk as that part is integral to my brake system (including t
... he hybrid function) e.g. my car was in reverse and the brakes were not responding and had to leave my car. I towed my car to a local independent service provider and they confirmed via the OBD-II scanner that confirmed my break servo control feel simulator is bad and would need to be replaced. After doing research, the brake servo part is made in Japan by a company called Nissin. The part has been recalled in Japan and UK due to failures in similar hybrid models. The brake servo serves many functions including ensures consistency, mimics traditional braking, and provide the driver with a natural-feeling brake pedal, especially because the system relies on both regenerative braking and hydraulic friction brakes. For about 3 days I received a cluster of warning signs from my car such as traction control, brake system, brake, steering malfunction, and ACC. Given that this part directly affects braking capability and overall vehicle safety, I am deeply concerned that my safety has been compromised as the life of the part is supposed to last the life of the car (about 150,000 miles) . I respectfully request an immediate investigation, and would like to know whether this issue is covered under an existing or pending recall campaign. If not, I'd like to submit this to the recall campaign in the U.S. Please advise on the next steps for inspection, replacement, and ensuring my vehicle is safe to drive. Thank you.Read more

... a semi truck right behind my vehicle in moderate traffic! The semi slammed brakes on and after initial shock, I was able to accelerate the vehicle somewhat to avoid a collision. The dealership claims that "ALL" new vehicles do this when operated in wintery weather, snow, or mud kicked up from the road, or when sensors get iced over. They claim that there is NO reprogramming that can be done to fix this HUGE safety issue. I have NEVER heard of this or experienced this in any other vehicle other than this Hyundai Santa Fe! As a disabled veteran, this honestly scared the hell out of me and makes me very leery to operate the vehicle in the wintertime time and I am the primary driver for the family! At the time of the incident, it was not snowing, the temperature was approximately 36 degrees, and the roadways were clear of snow or ice! The only thing that could be noted was that the roadway was wet, and after stopping, there was sand or salt film in the front of the vehicle. As a disabled veteran with a lower back injury and radicular pain down both legs, I rely on a vehicle's cruise control when driving more than short distances, in order to not cause lower back or leg pain while driving! This issue with the cruise control stopping operation and not allowing the cruise to be restarted is both a HUGE safety issue for both the operator and all vehicles around/near the vehicle on/at highway speeds, and a violation of the ADA (Americans with Disabilities Act). With no way to reprogram or reset this awful feature, the Hyundai Santa Fe is unsafe for disabled individuals.Read more

My safety and the safety of others were put at risk because the brakes failed completely without warning while the vehicle was in motion. The issue has not yet been confirmed by the dealership for this most recent occurrence, but the same Ford dealer that repaired it in July handled the prior failure. The vehicle will be towed there again for inspection. No warning lamps or dashboard messages appeared before or during either failure. The only symptom was a sudden loss of brake assist and a hard, unresponsive brake pedal. What component or system failed or malfunctioned, and is it available for inspection upon request? I put the car in park, then turned off the vehicle, and removed my seatbelt. The car then began rolling back on its own. When I hit the break, it did not work to stop or slow the vehicle. Instead, the break hit the floor, and was "floppy". The car then crashed. The vehicle appears to have experience
... d a "roll-away event" despite being in Park and off. (a term I learned from the prior 2024 Telluride recall, my VIN was not included). • The brake pedal went to the floor and did not stop or slow the vehicle as expected, and felt “floppy” with no normal resistance. • The electronic parking brake / brake actuator did not appear to engage (no sound, no holding force). How was your safety or the safety of others put at risk? I had removed my seatbelt, so I sustained whiplash and other bodily harm from crashing into our house. Had I stepped out of the car, i could have been pulled underneath it. It also crashed/crushed into our grill which could have caused an explosion. And most importantly, it could have been fatal to my daughters (22 months and 4yr) who usually stand (95% of the time or more) where the car crashed, waiting for me to get out of the car. My dogs also roam the drive way and it could have runover them as well. Has the problem been reproduced or confirmed by a dealer or independent service center? I have notified KIA customer support and am waiting their response for further steps. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? NO, not yet. (see above) Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The brakes have malfunctioned a few times during reversing or driving. The auto stop has engaged itself when there was no danger of hitting anything, and has once stopped in the middle of an intersection, on its own.Read more

On November 10, 2025 while my 2025 Tesla Model Y was being driven, in Miami, Florida, around 2:55–3:05 PM, the brakes failed completely. Despite firm brake pedal pressure, the vehicle did not slow down or stop as expected and continued forward until a collision occurred. The brake pedal felt normal with no loss of resistance, but the car did not decelerate as expected. No forward collision warn
... ing (FCW) or automatic emergency braking (AEB) activated prior to impact. There were no audible or visual alerts displayed before or during braking. After the collision, the vehicle’s screen displayed messages stating “Airbags Deployed” and “Automatic Emergency Braking is unavailable. Feature may be restored on next drive.” However, the airbags did not deploy. A burst of white particulate dust, consistent with airbag propellant or deployment powder, was released inside the cabin. None of the vehicle’s cameras recorded the incident; there is no Dashcam or Sentry Mode footage from that time. At the same time, the cell phone, which was actively linked to the vehicle through Bluetooth and the Tesla app, stopped transmitting location data through the Life360 app for a few minutes. This may indicate a coinciding electrical or data interruption affecting both the vehicle and connected devices. This incident placed my families safety and that of others at serious risk. Multiple safety systems appear to have failed simultaneously, including the braking function, forward collision warning, automatic emergency braking, camera recording systems, and airbag deployment.Read more

On Th, Nov 6 ~1700, the 25 Equinox EV being driven encountered an electric drive failure and brake failure, including friction brakes. This Equinox EV is essentially still brand new with 3900 miles and no accident history. The vehicle was driven off the EB SR 528 exit ramp to Narcoossee Rd in Orlando and driven to the stop light at the WB on ramp. After the light changed and began driving, the
... vehicle console reported an exception with drive assist. At this time the driver attempted to slow down to take stock of the situation and the vehicle accelerated, at which time the driver took their foot off the brake pedal and attempted to identify a place to take the vehicle off the roadways. Thankfully a stoplight at McCoy Rd was green and the driver continued through that light and while attempting to brake, the vehicle accelerated instead. The driver then coasted, maneuvering the vehicle was into the Vista Palms commercial center. The vehicle, unresponsive to braking, propelled through a stop sign into the Kohl's parking lot at which time the driver began looping around the lot until the weight of the vehicle brought the motion down. While the vehicle was stopped, it rotated through warning messages indicating..forward collision, brake assist is reduced, steering assist is reduced, then finally Service Transmission Now Unable to Shift Soon. The vehicle was towed by AAA to AutoNation Chevrolet Airport in Orlando. Technicians found codes U1610 and U1639, and found K160 connecter unseated. It was reseated and greased. The concern is that this should have been locked and unable to be unseated. Is this an indication of manufacturing problem or operating problem? All braking was dysfunctional. How can we be certain that this was not a one off event?Read more

My daughter was driving the 2013 Toyota Prius on the interstate in Chattanooga, TN. The brakes did not feel right and the car was very difficult to stop. (It did not brake at a normal distance) My daughter was able to get someone safely, and she had the car towed to the dealer. My daughter's life was endangered by this brake booster failure, but there is no recall listed on the vehicle. Also, w
... hen I Googled the issue, I found that it is a well-known and common problem in this year Prius. The brake booster failure is due to a Toyota design flaw, and Toyota is apparently being sued for it. Why is this issue not listed as a recall? I would not have allowed my daughter to drive around in a car for which the brakes are known to fail. How many accidents and deaths will be caused by this problem before Toyota stands behind their product and recalls affected vehicles? I had to pay $3200 for this brake repair, even though it is caused by Toyota's design flaw and endangered my family's life. My family owns four Toyotas, and I do think they make the most reliable vehicles on the road. I cannot understand why there is not a recall, and they are not standing behind their product.Read more

The vehicle caught fire while it was in motion; there were loud bangs and a lot of noise, and smoke started coming out of the front, and the engine and the undercarriage caught fire.I managed to turn it off, but when they were towing it, the mechanics did the diagnostic and found wires were faulty and damaged the engine.

The vehicle will be in park, and a message will pop up saying "shift to Park check and advise". Took my car into the dealer shop to get this fixed their response was the cause of the condition may be that the park switch in the transmission control (shifter) assembly is not pulling the BCM signal low to electronically show that the vehicle is in the park position.

Rear brake pads and rotors worn out at 32000 miles. Rear brake pads originally worn out and replaced under warranty at 16800 miles. Dealership states that the issue is no longer covered under warranty. This excessive wear is a safety concern as it points to a design flaw in the braking system that is causing unexpected braking pressure to be applied to the rear brakes which may be causing exten

I took the vehicle in for its 80,000 service, which included a multiple point inspection on Oct. 24th, 2025. The dealer that serviced my vehicle for that specific service was the north park volkswagen in San Antonio. On October 30, 2025 at approximately 2:15 PM, I was driving down the highway when I felt something in the brakes crack. I had to use my whole body weight and a lot of force to stop th
... e vehicle, which could have ended in a fatal accident. I took the vehicle to the nearest Firestone with the parking brake close to my hand, where a certified technician inspected it and found that a brake line had been improperly reattached using glue and duct tape, posing a major safety hazard. There was visible glue residue and tape wrapped around the brake line. Because Firestone could not obtain the necessary part and did not feel confident repairing a European vehicle, I then took my car to Ancira Volkswagen in San Antonio. Their technicians confirmed that the brake line repair using glue and tape would not have lasted long had it been at the 70,000 service over 6 months go further proving that this unsafe and makeshift repair was done recently. When I reported this issue to North Park Volkswagen, the service manager, Albert Zapata, was rude, dismissive, and denied all responsibility, implying that I could have caused the damage myself. This situation reflects negligence and a failure to perform a proper inspection or maintain safety standards. The workmanship and lack of accountability from North Park Volkswagen directly put my life at risk.Read more

This is an ongoing safety issue with a 2025 Hyundai Santa Cruz Limited. The forward collision-avoidance system activates frequently and without cause during normal driving. The system will engage when another vehicle is simply turning, merging, or slowing down at a traffic light—normal, predictable situations where no emergency braking should occur. These unexpected activations create unsafe con
... ditions, including one incident where the truck was rear-ended due to sudden, unnecessary braking. The issue has occurred more than 50 times over approximately eight months of ownership. The manufacturer has been contacted and stated that this behavior is “by design.” They advised having the vehicle inspected by a dealership, but the local dealership requires leaving the vehicle for diagnostics for up to two days and does not provide loaner vehicles. Because I rely on the vehicle daily for work, this has made inspection difficult. Online reports from other owners appear to describe the same issue with the same model and year. No warning lights, error messages, or other indicators appear before the system activates. I have adjusted the collision-sensitivity settings from high to standard, but the issue persists. There is no option to disable the system entirely. The component is available for inspection upon request.Read more

While driving at low speeds in downtown traffic with the automatic stop/start system active, I stopped at a red light. The system engaged and shut off the engine as expected. However, when I attempted to accelerate, the truck stalled. The gear shift unexpectedly moved from Drive to Park, and the parking brake activated. I applied the brake and restarted the engine, but had to manually disengage th
... e parking brake before continuing. At the next red light, the stop/start system again shut off the engine. Upon attempting to drive, the truck stalled a second time. I restarted the engine, but this time the auto stop alert and battery warning lights illuminated. The truck moved forward very slowly, appeared to struggle shifting into gear, and then suddenly lurched forward. I had to quickly apply the brakes to regain control. The vehicle is currently at the dealership undergoing inspection for a faulty tire pressure sensor and this auto stop system problem. This is the third time the truck has experienced the auto stop issue in the past two years. The problem has not been able to be replicated at the dealership and no repairs have been made.Read more
