I own a 2025 Ford F-150 Lariat equipped with the factory Tow Package. The truck was sold to me and later serviced under the representation that the trailer brake controller was functional. Duvall Ford in Clayton, GA installed and programmed only a brake controller switch and assured me the trailer brakes were working, without performing a functional brake output test. The truck was later found to
... be missing the required Trailer Brake Control Module (TBCM), meaning no trailer brakes could ever function. As a result, I unknowingly towed a 30-foot travel trailer from North Carolina to California, across multiple states with zero trailer braking, creating multiple dangerous situations and close calls. Upon reaching California, Aaron Ford of Escondido inspected the vehicle and immediately identified the missing TBCM. They performed the proper repair, with an invoice of $1,043.00. The original dealer’s incomplete and incorrect service directly created this safety hazard. This issue may affect other F-150 trucks that were sold or serviced with incomplete brake controller systems due to Ford’s mislabeled parts catalog.Read more

After diagnostic troubleshooting from DTCs on a check engine light, it was discovered that the brake vacuum pump connected to the engine was completely nonfunctional. Previous symptoms included stiff brake, loss of brake power at random brake periods which could have cause and accident with other drivers, and finally malfunctioning sensors attributed to the exhaust system. This also led the vehi
... cle to be unable to pass state regulated Emissions testing because of the inability to maintain proper emissions levels. Brake Vacuum pump that was removed from vehicle to inspect and diagnose is still available for inspection. Recent findings show that Chevrolet/GM is aware of the issue in their vehicles but refuses to address or replace faulty brake vacuum pumps. It is a common problem amongst 2013-2022 Chevrolet Malibus. Vehicle that is in question was inspected by a certified mechanic shop with failed emissions test. Solid check engine light appeared once the brake had become harder to push and stop the car. OBD scans show DTC codes that were pertaining to MAF and MAP malfunctions in readouts and communications to the ECM. After further inspection, the ABS module had been acting as "backup" for hydrolic brakes system that had zero vacuum to power the brake booster, this could have caused the engine to stall, longer than normal stopping distances (stronger risk of rear ending other drivers), and multiple components and sensors to fail, requiring them to also be replaced.Read more

Beginning on April 18, 2025, my 2020 Toyota Corolla LE began experiencing repeated failures involving multiple safety systems while driving. Without warning, several lights illuminate at the same time, including: Brake System Malfunction, Pre-Collision System, Parking Brake System, Vehicle Stability Control, Check Engine, and Secondary Collision Brake System. These warnings appear suddenly during
... normal driving, then sometimes disappear, then return again unpredictably. These failures disable several integrated safety systems and create a serious safety risk due to changes in braking performance, loss of stability control, and disabled collision-avoidance functions. Diagnostic scans from both the dealership and an independent mechanic repeatedly show: P1578 — Brake System Malfunction P265B — Rocker Arm Actuator Position Sensor Circuit Range/Performance (Bank 1) Both codes reappear even after being cleared. The vehicle was with Loving Toyota Lufkin, TX multiple times over seven months. They received over $7,100 in insurance payments for repairs, $463 towing cost (due to the negligence of Loving Toyota) and almost $1,500 in rental cars (Paid by State Farm) but were unable to diagnose or fix the underlying failure. Loving Toyota also provided a courtesy rental for 7+ weeks while they did no repairs to the car. The safety warnings continue to occur. Toyota Corporate is now involved but the vehicle remains unsafe to drive. The same code P265B is present that was present back in April 2025. This car is not safe! The problem has been reproduced by me, the dealership, and an independent shop. The vehicle is available for inspection. All scans, photos, videos, and documentation are available upon request. The braking system on my 2015 Chevy Suburban LTZ failed at slow speeds in a parking lot. I have replicated the issue in both forward and reverse. The abs system is bad. I have 150000 on the truck. They are doing recalls on this system. But don't go back to 2014. There are other people having same issue. While driving on the highway at 70 MPH on cruise control the ABS and Traction Control lights came on and the cruise control stopped working. There was a warning message to have the ABS system serviced. This happened again two days later. The lights did not come on after restarting the truck. I took it into the dealer and the codes were C0020-1C and C0020-16. The shop said the ABS pump and ABS
... control module need to be replaced. The control module has been out of stock for 3 years according to the parts department. Reconditioning is an option but the chip to fix the module is no longer available. This one dealer has close to 20 trucks waiting for this part. The 2017 and 2018 Ram 2500, 3500, 4500, and 5500 were recalled for the ABS, but not the 1500. From my research it seems like the same parts were involved in that recall. I have been in contact with Ram Customer Service and they are attempting to find a part, which I appreciate, but they admit that many are looking for the part and some have been waiting years. They tell me there was an issue with the original manufacturer and they had to redesign and remanufacture the part. It seems to me that this was a defective part and similar parts to this were involved in a recall of other models by the same manufacturer. I am willing to pay for the repair just to have a reliable and safe vehicle, but I would think a defective brake part should be involved in a recall. The other problem is the lack of available parts for multiple years.Read more

SUMMARY OF PROBLEM: Vehicle experienced sudden uncontrolled acceleration and deceleration while driving on busy road with family as passengers on November 16 2025. Vehicle provided no warning before loss of control occurred. Created immediate risk of collision and serious injury. DETAILED DESCRIPTION: On November 16 2025 at approximately 5:25 PM ET, my 2025 Rivian R1S experienced sudden uncontr
... olled acceleration and deceleration while driving on a busy roadway. The vehicle began accelerating and decelerating erratically without driver input, creating immediate risk of rear-end collision or loss of control. No warning lights or messages appeared before the incident. The vehicle displayed "Thermal Fault Detected" and "Coolant Level Low" messages only after the erratic behavior began. This incident occurred with my family in the vehicle and created serious risk of injury or death. I was forced to have the vehicle towed. PRIOR RELATED INCIDENTS: This is not the first safety issue with this vehicle. Service Request SR-RAB3DU1AM documents "Emergency Braking Mistakenly Triggered on Regular Roadway" from August 29 2025 that remains unresolved in Rivian's system. The vehicle has required multiple service visits for thermal and cooling system failures at only 15000 miles. MANUFACTURER RESPONSE: Rivian service center claims rodent nesting caused thermal system failure leading to loss of vehicle control. If correct, this indicates critical design flaw - environmental factors should not be able to compromise vehicle control systems without warning or failsafe mechanisms. Vehicle lacks adequate: - Warning systems for thermal degradation before catastrophic failure - Failsafe mechanisms to prevent loss of control - Protection of critical systems from environmental factors SAFETY CONCERN: If thermal system damage can cause sudden loss of vehicle control without warning, this represents unreasonable safety risk. Other R1S vehicles may be at risk of similar failures.Read more

Unintended acceleration event. Entered vehicle, put my foot on the brake, cranked it, held in that position while seatbelting. Engine had a VERY loud revving sound. Within seconds vehicle moving backwards on its own. Foot still on the brake, had not touched gas pedal, had not touched gear shifter, and was still sitting straight forward with my foot on the brake and confirmed gear shifter in pa
... rk. The car quickly accelerated backwards faster and brake pedal had no effect on stopping or slowing vehicle. Went approx. 30’ across parking lot and only stopped because we hit truck parked behind us. Quickly turned off engine and removed keys. Shifter still shows Park. Careful driver. No history of accidents. I found there were several thousand reported events of unintended acceleration (Ford vehicles 2002 to 2010). Ford tried to explain many as driver error or floor mat touching the gas pedal. My mat was clipped down, as designed, with no all-weather mat on top of them, and my foot did not leave the brake pedal. AI says revving as described is "classic behavior of failing Electronic Throttle Body on 2010 Fords", "Surging without touching the pedal and known defect are extremely common". Says if "Worn or failing shifter linkage/bushings vibration form high revving could make it slip into Reverse". Ford has a 19S54 recall for AntiLock Brake Systems (not notified), where "A normally closed valve may be stuck open, which may result in extended brake pedal travel". Ford reports on similar vehicles "lunging" was due to deposits in the throttle body. Ford has installed brake override on vehicles after 2010 but that doesn't protect people driving older vehicles. Please look at combo of factors that could cause this. Disappointed to read Ford purged customer acceleration reports prior to hold 5 yrs and an expert they provided at trial was someone sheltered from all research Ford had on this issue so he pushed "driver error" as the case. Clearly not here.Read more

Dear NHTSA, I am submitting this complaint because my 2014 Jeep Grand Cherokee Limited is experiencing multiple safety-related failures that I believe are part of a wider defect pattern affecting this model year. Several of these issues involve systems that are critical for emergency safety, braking, and vehicle control. ⸻ 1. Telematics Control Unit (TCU) Failure Causing Loss of 9-1-1/SOS Emergency Function My Jeep is displaying repeated “Service Emergency Vehicle Phone” errors and the SOS/911 emergency button no longer functions. This is a major safety concern, especially in the event of an accident or breakdown. The TCU failure also causes: •Uconnect 8.4 system not booting •HVAC controls becoming non-functional or unsafe (blowing incorrect temperatures, failing to defrost, turning off unexpectedly) •Random system resets •Loss of ability to control climate during driving (creating visibility issues in cold/wet weather) These issues align with widespread failures of the Uconnect 8.4 / TCU hardware on 2013–2015 Grand Cherokees, which were not included in a recall despite many vehicles exhibiting the same dangerous symptoms. ⸻ 2. TCU-Related CANBUS Interference Causing Transmission and Drivability Safety Issues When the TCU fails, the vehicle experiences: •Transmission communication errors •Vehicle entering limp mode unexpectedly •“Service Transmission” and “Press Brake to Shift” warnings •Delayed or unpredictable shiRead more

I was approaching a 4-way intersection and had the green light ahead of me not knowing that a semi truck was entering the intersection due to other vehicles in left turn

GOD SAVED ME FROM WHAT COULD HAVE BEEN A TERRIBLE LIFE THREATENING CRASH DUE TO CATASTROPHIC FAILURE! Vehicle has been inspected by local service/repair shop prior to this event/failure for the same exact failure. They said nothing could be determined, located or seen that would indicate why such a failure occurs on and off. ABSOLUTELY ZERO warning indicators of any kind occurred before, during or after failure. Prior to this event and on multiple occasions the same exact failure type has happened, but during a rain storm or after one when the road is still wet thus the reason I took it to shop previously. I only have this one vehicle and rely on it heavily to get myself and kids to and from work and school etc daily. With a catastrophic failure such as this I would hope the manufacturer would step up to ensure these are looked at seriously and correct the issue so it doesn't continue to happen. My VIN is not part of a recall or anything service related to this failure type.Read more

While driving, suddenly all brake lights and warning lights went on, and brakes stopped working normally. My vehicle experienced multiple safety system malfunctions before a minor collision occurred. While driving, the car displayed warnings indicating that traction control had been disabled, followed immediately by a warning that the automatic vehicle hold/brake hold was disabled. These failures happened before any impact. At the same time, the vehicle’s cameras were not functioning correctly, and
... the vehicle did not provide any forward collision warning. The automatic emergency braking system also did not activate. Because these systems failed, the vehicle did not warn, slow down, or brake to avoid a collision. After the collision, the vehicle showed several additional safety system failures. These included automatic emergency braking unavailable, and repeated safety restraint system faults affecting both the front left and front right restraint systems. The airbags did not deploy even though conditions suggested they should have, and afterwards the vehicle displayed persistent airbag and seatbelt system error icons. The restraint system appears to have gone offline. The vehicle is currently unsafe to drive due to failures in braking, traction control, perception/camera systems, forward collision warning, automatic emergency braking, and the airbag/restraint systems. These failures occurred before the collision, contributed to the collision, and continued afterward. The manufacturer’s service center has declined to provide towing or a loaner or rental vehicle while the safety defects are being investigated, which will take about 30 days.Read more

I am reporting a serious safety defect involving the rear brake system of my 2021 Toyota Highlander, purchased from Penn Toyota (Glen Cove, NY). The parking brake cable was too tight as described in Ford SSM-51050. We travelled 200 miles each way to an RV dealer to repair and paid out of pocket. Ford instructed us to take the vehicle to a certified repair shop to fix since there is no Ford dealer in Washington state that would work on this chassis issue. The repair shop fixed the parking brake cable and it is now in working order. I re
... quested Ford reimburse us for this repair ($434.00) and they have ignored every request. This was an issue from the day purchased in June 2025 and we continually tried to find help to fix it. We were never informed of the SSM-51050 until I found it online after we had paid for the repair. This was an ongoing problem not one specific incident.Read more

In November 2024, I went to get into my 2019 Jeep Cherokee (17,000 miles). At that time, numerous lights started flashing on the vehicle dash (start-stop, stability control and check engine). I took the vehical to the nearest Jeep dealership, Valor Ford in Junction City. They indicated this was a known issue but unresolved by Jeep (received recall notice two weeks later). I was leaving for vac
... ation and would be back in two weeks, rather than leave the car sit at the dealership I offered to take it home and bring it back when I returned. I was told by the technician that was not advisable because my brakes could go out. I of course decided to leave at the dealership. Upon return, they indicated my jeep could be picked up. My husband spoke with the technician because we had received the recall notice by then and asked how they fixed the problem when Jeep had said there was no solution yet. He, the technician, indicated they had ordered the part which is the same part that went out previously so it wasn't really fixed. We have been waiting over a year, I will only drive locally because I am afraid of the ramifications of this part. Not only is it not fixed but they charged us $1,200 to not fix our car and refused reimbursement. This was our vehicle to travel in our retirement years and for the last year it has been an anchor.Read more

At stop signs, in a row of 3 or 4 cars, on the 3rd or 4th stop, I apply the brake but it accelerates as i am braking, moving forward 8 or 10 feet. Over the course of 1800 miles, this has occurred 3 or 4 times, narrowly avoiding collisions.
