Electronic parking brake engaged randomly when getting gas and would not disengage letting me leave the pump
Incident date: May 24, 2026
The ABS module, which controls the brake drying feature, is malfunctioning. This BMW feature is designed to lightly pulse the brakes when the windshield wipers are active to wipe water off the rotors for better braking performance. The intervention is supposed to be imperceptible. Instead, the system intermittently applies hard, sudden brake pressure while the windshield wipers are operating. This
... causes abrupt, strong deceleration that feels like the brakes are slamming on by themselves. The problem occurs in wet conditions when wipers are on. It has happened multiple times and has nearly caused rear-end collisions or loss of control on multiple occasions. This unexpected hard braking at speed in wet conditions creates a significant risk of rear-end collision, loss of vehicle control, or accident, especially on highways or in traffic. The only workaround is to drive without wipers in the rain, which is also dangerous due to reduced visibility. I have not yet brought the car into the dealership to have it inspected but I do intend on getting that done within the week. There are no warning lights or messages to suggest there even is a problem.Read more

A 2015 Toyota Sienna wheelchair-accessible conversion van has two open, unremedied safety recalls related to its braking system: a Braun Corporation recall for incorrect tube nut fittings on the rear brake lines that can cause a pressure drop in the braking system, and a Vantage Mobility International recall for rear brake hoses that may be too short. There is no indication either recall has been
... completed on this vehicle. The vehicle has been operated as a paid passenger-transport service carrying elderly and disabled passengers, including persons with cognitive impairment, to and from medical and rehabilitation facilities. This commercial use places vulnerable passengers at safety risk given the unresolved brake-system recalls. The vehicle has not been inspected by a dealer, manufacturer, police, or insurance representative to the reporter's knowledge. No warning lamps or other symptoms are reported — this submission is based on the open-recall status visible in the NHTSA VIN lookup. Supporting documentation is available on request.Read more

The vehicle's Automatic Emergency Braking (AEB) / Smart Brake Support system intermittently triggers "false positive" emergency braking events during critical driving maneuvers, specifically while accelerating to merge into highway traffic. During these events, the system violently applies the brakes with no external hazard present. Crucially, the vehicle fails to acknowledge driver override input
... . Depressing the accelerator pedal fully does not disengage the automatic braking, stripping the driver of vehicle control and exposing the vehicle to a severe risk of being rear-ended at high speed. Simultaneously, the vehicle experiences a persistent "In-Unit Telematics Malfunction" error on the dashboard, accompanied by rapid-fire mobile app notifications (14 alerts in a single day) stating "Critical Vehicle Malfunction: Vehicle cannot be driven." These communication module errors have persisted despite the dealership applying Mazda's latest software updates, indicating a critical network failure or hardware defect that compromises active safety systems.Read more

I have written several times before about my car's phantom braking in cruise control. When the Santa Fe Tesla dealer, where I bought the car, told me recently that the problem had been fixed, I was disappointed they didn't bother to tell me, but thrilled to hear the news. On May 12-13 I drove over 900 miles. I tried cruise control for significant portions of both drives and did not experience o
... ne problem. Finally, I thought, they fixed the problem...and how nice it was to have cruise control for extended drives. Not so soon: on May 19th I was driving 2 friends up to volunteer at the Wildlife refuge on I-25 in northern New Mexico. The phantom braking happened again. My friends were dismayed...one thought I had seen something in the road that she didn't see. The other, in the back seat, tried to understand what happened. Please, this is beyond the pale! Of course I will call and take the car in to the dealer...an hour's drive for me. I will be surprised if they fix it, and if they do it under warranty. I did just buy new tires (see previous report about original tires with multiple flats and not even making it to their 40,000 mile warranty) so I'm no longer worried about getting a flat in the rural areas where I live and drive.Read more

I’ve been dealing with some intermittent problems with my 2022 transit for a couple years now. Basically I’ll be driving on the highway and all the sudden I’ll lose gas control, and power braking and the dash displays a message that says “shift to neutral to restart vehicle”, battery icon flashes red and I have to coast to the side of the highway. The electronics in the car will still be
... working when I pull over but the battery is dead. If I shift to park my engine doesn’t turn over or make any clicks at all. The few times it’s happened it eventually will turn back on after 10 min or so after using a jumper pack. I’ve taken it to dealers and they all say the computer isn’t throwing any codes so they can’t help. I’ve nearly been in 2 accidents now on the highway trying to get to a shoulder/lack of shoulder and the fact that I lose my brakes makes this an undrivable vehicle. My vehicle is still under manufacturer warranty but the ford dealers have not been able to help and keep sending me on my way with a very dangerous vehicle. I need help, I’m concerned about being involved in a a dangerous accident that could hurt myself or others when I lose brakes and gas. This has happened 5times now over the last 25k miles. I have a long video of the last incident that I would love to share if a video upload link is made available.Read more

While driving my 2024 Hyundai Kona on the freeway on May 22, 2026, at approximately 9:30 AM, traveling around 75 mph, my vehicle suddenly displayed a forward collision warning on the dashboard even though there was no vehicle, object, or road hazard in front of me that would justify emergency braking. Immediately after the warning appeared, the vehicle’s automatic emergency braking / forward col
... lision avoidance system activated on its own, causing the car to brake abruptly and rapidly decelerate in active freeway traffic. This created an extremely dangerous situation because vehicles behind me were traveling at freeway speeds and could have rear-ended me, potentially causing a serious multi-vehicle collision resulting in serious injury or death. I had no warning prior to this event, no dashboard warning lights, service messages, or other symptoms indicating a problem with the system. The suspected failed component is the forward collision avoidance / automatic emergency braking system, and the vehicle is available for inspection upon request. The issue has not yet been reproduced or confirmed by a dealership or independent repair facility, and the vehicle has not yet been inspected by the manufacturer, police, insurance representatives, or others. My best assessment is that the system falsely detected a collision risk and activated emergency braking without cause, creating a severe highway safety hazard.Read more

This vehicle suffered a sudden; catastrophic hydraulic brake fluid leak and complete system failure shortly after a service visit with only 700 miles total on the new vehicle. The vehicle experienced a sudden unprovoked, catastrophic, and total electronic and mechanical failure of the primary braking and ADAS. Driving at expressway speeds, the vehicle immediately lost all power brake assistance, threw immediate multi-system critical alerts on the instrument cluster, and became instantly undrivable. The vehicle is available for inspection at a local licensed Lincoln deal
... ership service center. The failure occurred completely without warning while navigating heavy traffic at active highway speed on the expressway. The instant loss of power braking functionality dramatically extended the vehicle's stopping distance and significantly reduced control of a SUV in a high-speed environment. This sudden, unmanaged deceleration created an immediate, life-threatening risk of a high-speed rear-end or multi-vehicle pileup, placing the occupant of the vehicle and all surrounding motorists in extreme danger of severe injury or death. The driver was able to bring the vehicle to a stop in a precariously exposed location on a very narrow shoulder when one became accessible. Miraculously, the vehicle was not struck by fast-moving trucks while waiting for a tow-truck. Yes. The local authorized Lincoln dealership service technician team has confirmed the catastrophic code failures. Due to the severity and complexity of the total system shutdown, the local dealership escalated the vehicle to Lincoln factory corporate engineers. However, the root cause of the catastrophic failure was never identified and the technicians and engineers have given up further attempts. There were absolutely zero pre-existing symptoms, warning lamps, soft codes, or performance degradation prior to the exact moment of the failure. The system failure was instant. The instrument cluster simultaneously illuminated multiple critical alerts, including the primary braking system warning, ABS fault, and ESC failure alerts, accompanied by an immediate audible safety chime.Read more

Incident date: May 21, 2026
The vehicle experienced a sudden, catastrophic brake-by-wire system failure while operating in one-pedal driving (Regeneration Boost mode). While decelerating at a low speed behind an 18-wheeler semi-truck, the vehicle failed to continue slowing down. I immediately depressed the physical brake pedal, but the vehicle completely ignored the foot pressure. The pedal and braking system became complete
... ly unresponsive, and the vehicle kept rolling forward uncommanded. Despite keeping the brake pedal fully depressed, the vehicle did not decelerate, resulting in a front-end low-speed collision with the metal rear bumper of the semi-truck. Because the collision occurred at a very low speed, the operator of the commercial truck was unaware of the impact and safely continued driving forward without stopping. The vehicle's brakes entirely refused to re-engage until I frantically pumped the physical brake pedal and cycled/pressed vehicle control power switches. This action seemingly forced the electronic braking modules to reboot or re-establish hydraulic pressure, allowing the vehicle to finally come to a stop. The vehicle was subsequently removed from the active roadway. This is a severe, life-threatening safety defect involving a total loss of electronic brake-by-wire and friction braking application without any mechanical override intervention, presenting an extreme hazard. The vehicle has been towed directly to an authorized Toyota dealership for a comprehensive electronic diagnostic pre-scan of the Skid Control ECU, Electronic Parking Brake system, and 12-volt auxiliary power systems to retrieve the stored DTC fault logs and freeze-frame data.Read more

My truck has used automatic emergency braking multiple times while driving at variable speeds with no obstacles or vehicles in front of me causing whiplash and further damage to my neck that has 5 budging discs. Since I’m only allowed to add a specific mileage and speed below, I’ll note here that it’s happened since I purchased the truck with roughly 35000 miles on it to the current mileag

My 2022 Chrysler Pacifica Hybrid has developed a dangerous rear brake system malfunction. During a dealer inspection on 4/27/26, the certified technician confirmed that the rear brake pads are only contacting approximately 50% of the rotor surface, leaving the remaining half completely unused and heavily corroded. This is not normal corrosion or wear. It is a mechanical failure in which the pads a
... re not engaging the rotor correctly. The technician also stated that the pads were sticking in the caliper slides, which prevented full engagement. This is a functional defect in the brake hardware, not environmental corrosion. Corrosion does not selectively prevent half of a brake pad from engaging the rotor unless the caliper mechanism is not operating properly. This defect results in: Reduced rear braking force Increased stopping distance Excessive load on the front brakes Potential loss of stability during emergency braking Possible degradation of the parking brake The vehicle was serviced on 9/23/25 (tire rotation + NYS inspection) and 12/10/24 (four new tires + NYS inspection). No rear brake issues were identified at either visit. If this were normal corrosion, it would have been visible during those inspections. The sudden onset of severe partial engagement by 4/27/26 indicates a latent mechanical defect, not gradual wear. Chrysler/Stellantis has refused to classify this as a defect and instead labels it “corrosion,” despite the technician’s findings of pad binding in the slides and improper brake engagement. The manufacturer’s internal system blocks any warranty or engineering review because it incorrectly categorizes the failure as routine maintenance. This is a premature and unsafe brake system failure at only 33,000 miles. The braking system is not operating as designed, and the defect could lead to reduced braking performance or loss of control. I am requesting NHTSA’s review of this issue as a potential safety defect affecting the rear brake systemRead more

While driving normally, my 2025 Honda Prologue experienced an immediate, catastrophic mechanical and electronic failure in both the steering and braking systems. The steering wheel completely locked up/failed to respond, and the brakes became completely non-functional simultaneously, causing an absolute loss of control of the vehicle. Due to this sudden system failure, the vehicle veered off cours
... e and crashed, resulting in severe structural damage, complete destruction of the front passenger-side suspension, and rendering the vehicle a total loss. American Honda Motor Co. Corporate was immediately notified of this severe safety defect (Case No. 16372717. However, Honda Corporate has explicitly refused to send an engineer or inspector to evaluate the vehicle's electronic or mechanical logs, and they also refused to provide their denial statement in writing. The vehicle remains un-repaired as evidence of this dangerous factory defect.Read more

Brakes completely failed. Foot went to the floorboard and car did not stop. Drove until is was slowed down enough to throw it into park. Engine light stated low on brake fluid. Filled it up and it went about a mile before brakes failed again. Took to a mechanic who determined that brake cylinder and booster had failed. Called Ford who said my 2018 Ford F-150, built in Kansas City was not in
... the Ford April 14, 2025 recall (25S37) as the build date on my vehicle was a few months after the May 19, 2017 cut off. However it is clear that later built models also had the same problem with the defective brake cylinder as the earlier builds. I suspect they used the same part. Ford should expand the date of brake failure to cover repair of later models with failed brake booster and master cylinder.Read more
