I had to take a detour one night coming home, the detour led to a bridge that connects to a highway. Mind you there is no shoulder currently during the active merging or on the bridge, but I didn’t think of what would happen next. As I merge onto the bridge at 35mph the car shuts off completely on me. My car automatically sets itself into Neutral mode and I was rolling on the bridge at 35mph, no shoulder and no brakes or gas to move properly. I called the police but they won’t dispatch a officer unless I am in a accident or about to cause on, but they heard I am rolling at 35mph and they said from her words herself “I can’t send a officer over for your reason ma’am, are you a threat to others or about to crash? Can’t you just brake?” My car system never turned back on, I stepped my foot on the brakes all the way praying something will work as I existed the bridge onto a small shoulder. The emergency brakes came on as I keep swerving onto the grass. As everyone was going 65mph I don’t know why I wouldn’t impose as a threat if I have no control over my car from oncoming traffic. This is a very dangerous situation, and my service shop said we try to retest the issue/diagnostics but couldn’t replicate the situation and told me that my car is fine. But this isn’t the first time that happened and it also wasn’t the last. The issue has been diagnosed by the Kansas City Land Rover Dealership as a faulty brake line rupture without warning. However, there are photo of the ruptured brake line, located where the aluminum line joins with the rubber line. A full independent inspection was completed. The vehicle has been involved in a police-reported accident, and insurance has been notified. The vehicle has also been transferred to a AAA or IAA facility, but is deemed totaled by my insurance. There was a visual inspection by the dealership in Kansas City, where they informed me that they found a break or rupture in the front passenger brake line. There were no warning lights, messages, or noticeable symptoms prior to the failure. The loss of braking occurred suddenly and without prior indication while the vehicle was in operation, causing a T-bone collision with another vehicle.Read more

Several other occasions the vehicle has had phantom emergency braking with no obstacles, vehicles or pedestrians near it but at slower speeds and did not cause as much trauma. While driving this vehicle, the brake line was cut after making contact with the A arm assembly, at the highest point on the line. I immediately lost my brakes as the brake pedal went to the floor. I am extremely lucky that I was able to stop within the parking lot that I had just entered and did not have to make an immediate stop.

About 3 weeks later, at a full stop at a stop sign, I was about to turn left onto another road. After confirming there was no cross traffic of any kind, and before I had completely released the brake, the collision assist engaged and suddenly applied the brakes (again with no screen display but an audible thump). Again, I was able to reapply the brake and then accelerate to make the turn. Since the same event had happened twice, I went directly to my local Hyundai Service Center and reported it. They kept the vehicle for a week and tested it thoroughly. Nothing showed as being faulty. When I picked up my vehicle on 5/22/26, they informed me that there's a known issue (NHTSA Campaign # 26V316) with other Hyundai models (not the Palisade) and that an update for the forward collision avoidance system in those models will be coming mid-summer. As the Palisade is not included in the Campaign list of makes/models, but has the exact same event/problem, I wanted to report this for inclusion in the safety recall. I am submitting this complaint regarding a serious and dangerous brake failure involving my 2020 GMC Terrain SLT with approximately 150,000 miles. While driving, the vehicle suddenly experienced a vacuum pump failure that caused a severe loss of braking ability with no warning beforehand. The brake pedal became extremely hard, and the vehicle would barely stop. This created a very dangerous situa
... tion and could have easily resulted in a serious accident. Thankfully, I was not driving at a high speed when the failure occurred, and there were no vehicles directly in front of me at the time. However, only minutes earlier I had been traveling approximately 65–70 mph on the highway. If this brake failure had occurred while driving at highway speeds or in heavy traffic, I truly believe it could have caused a major crash resulting in serious injury or death. What concerns me most is that there was no warning before the brakes failed. No warning lights and no issues with my brakes. The sudden loss of braking power was extremely frightening and created an immediate safety hazard. After researching this issue, I discovered that other GMC and GM vehicle owners have reported similar vacuum pump failures and brake assist problems. I believe this is a significant safety issue that should be investigated further. Drivers should not experience sudden brake failure without warning, especially on a vehicle that otherwise appeared to be operating normally beforehand. I am very thankful no one was injured in my situation, but I am concerned that another driver may not be as fortunate. My vehicle had to be towed to a nearby shop and they suspected it was the vacuum pump failure and the car started smoking badly. so I had it towed to the GMC dealership and they called me today to confirm that was the problem and it also caused damage to the camshaft. The vehicle was taken to the same dealer where it was diagnosed that the drivers door wire harness needed to be replaced due to an electrical issue that killed the battery. The vehicle was repaired but the failure reoccurred approximately four months later. The vehicle was towed to the same dealer where the vehicle has yet to be diagnosed as of yet. The component that failed/malfunctioned was the module that activates the EPB. It is available for inspection if requested. My safety was put at risk because the parking brake would engage every time the vehicle shut off leaving me stranded and could put other drivers at risk if my vehicle was stranded on the roadway. It would sometimes reset if the battery was disconnected and then reconnected,
... but eventually trying to reset it did not work. Within a week, the actuators completely seized and the vehicle was inoperable, requiring to be towed. The problem was confirmed by a Jeep dealership service center. They noted that water was leaking into the vehicle from the outside and pooling. The inside rear cargo carpets are moldy and need to be replaced and the module is completely water damaged along with other electrical components being rusted and corroded. The issue has been identified by a certified mechanic at the service center. I had planned to reach out to my auto insurance provider to file a claim, but was informed of an active recall on 2014-2020 Jeep Cherokees with similar symptoms to what I am experiencing. No warning lamps or messages were active prior to the failure. Both "Service Parking Brake" along with the red "brake" light were active to let me know my EPB was engaged. I was able to reset the message briefly to unlock the brakes by disconnecting the battery, but was only successful once. Then it needed to be towed to the dealership for further inspection.Read more

There has been an SSM release by Ford (SSM 51415) acknowledging that 2021-2022 Bronco vehicles flat towed in neutral may experience inconsistent or unintended ABS (anti-lock brake) module activation while being towed. The ABS module exits sleep mode unexpectedly due to road vibration input during towing, which can cause brake drag or lockup while the vehicle is being flat towed behind a motorhome.
... This is a safety concern as unexpected braking of the towed vehicle can affect the towing vehicle's stability and control at highway speeds. The vehicle is being operated exactly as documented in the Ford owner's manual, which explicitly supports neutral flat towing. Ford has developed a software update to address this defect but is declining to cover it under warranty or goodwill, instead classifying it as non-covered maintenance. The issue has been confirmed by an authorized Ford dealership who identified SSM 51415 as applicable. A community workaround requiring removal of fuses 28 and 29 to disable the ABS pump has emerged among owners, indicating this is a widespread known issue affecting safety during a manufacturer-approved towing method. Another time, I was driving my children to school and it happened about 10 seconds before crossing the worst intersection in our city. That could have resulted in a major crash based on the history of that intersection. A third time, I was traveling at about 40 miles an hour when warnings came on and the brake started to engage. Luckily, I had just gotten off a major highway and I was able to get to the side of the road. 3. The problem has been confirmed by a dealer, since it happened while I was in their service parking lot. However, they have been unable to recreate, find the source of, or fix the issue after 7 times of servicing it. 4. I am unsure whether the manufacturer has sent anyone outside of the dealer's service department to inspect the vehicle. We have been in contact with the manufacturer directly since December 26, 2025 in relation to this issue. Our extended warranty company may have had an inspector out on the first visit to the dealership for the issue, but no one since that I am aware of. 5. Emergency messages have included: Service Electronic Stability Control, Service Charging System, Service Hybrid Electronic Vehicle System, Auto Emergency Braking Unavailable Service Required. I was told to wait until the compenent failed by multiple dealers. My car was in motion and the brake boosters failed, fortunatly it did not cause an accident but this is a serious safety issue. Toyota should have done a compleat recall of the brake actuator and booster pump, rather than have drivers use known faulty components until failure. I am filing a safety complaint regarding a persistent soft brake pedal condition affecting my truck. Under normal driving conditions and especially during low-speed braking with no trailer or payload attached, the brake pedal feels excessively soft and requires additional pedal travel before adequate braking response occurs. After further evaluation, the dealer stated that their engineer determined the vehicle is operating within factory specifications and declined to pursue any additional diagnosis or repairs. I use this truck for towing an RV, and due to the abnormal brake feel and reduced confidence in braking performance, I do not feel safe towing with this vehicle. As a result, the truck remains parked at my residence because I believe continued operation may present a safety risk. I have driven other trucks of the exact same year and model that exhibited significantly firmer and more responsive brake pedal feel, which leads me to believe my vehicle is not operating normally despite the dealer’s claims. In addition, I have found numerous online reports from other owners describing similar soft brake pedal complaints, including reports of lemon law claims and unresolved brake concerns. I am requesting that NHTSA investigate this issue for potential brake system defects or inconsistent braking performance that could affect vehicle safety, particularly during towing or emergency braking situations.Read more

The accident caused the vehicle to flip, resulting in injuries and creating a dangerous situation for myself and surrounding drivers. 3. The issue itself was not fully reproduced prior to the accident, however the recall now issued by Hyundai appears to involve the same or similar malfunction connected to braking/safety system concerns. The vehicle was inspected after the accident through the insurance process and body shop estimates were completed before it was declared a total loss. 4. The vehicle has been inspected by insurance representatives, towing personnel, and others involved in the claims process following the accident. Police also responded to the accident scene and completed an accident report. 5. Prior to the failure/accident, there were no clear warnings that would have indicated the severity of the issue. If applicable, there may have been occasional warning indicators or abnormal vehicle behavior before the crash, but no information was provided to me at that time indicating the vehicle was subject to a safety recall. ... ruck was inspected by a mechanic who advised that the brake booster had essentially fallen apart and that I effectively had little to no braking capability. I was also informed that the condition and quality of the braking components were not acceptable for a vehicle of this age. This vehicle is only five years old and has been regularly maintained. For approximately the last three years, routine maintenance including oil changes, tire rotations, and inspections were performed at a Chevrolet dealership. At no time was I warned of any brake-related issue or safety concern. A major brake system failure should not occur suddenly on a properly maintained vehicle, especially without warning. This incident was extremely dangerous and could have resulted in a serious crash. I am filing this safety complaint regarding my 2025 Cadillac Optiq with approximately 6,500 miles. On May 25, 2026, during rainy/wet road conditions, the vehicle failed to stop normally on two separate occasions while I was approaching stop signs at low speed. ... e/indicator, but the vehicle did not decelerate or stop as expected. I had to continue attempting to brake, and the vehicle traveled farther than it should have before stopping. This happened twice in the same day under wet conditions. Fortunately, there were no other vehicles or pedestrians in the intersection at the time, so no crash or injury occurred. However, if another vehicle or person had been present, this could have resulted in a serious collision or injury. I checked the NHTSA and Cadillac/GM VIN recall lookup and did not see an active recall listed for my VIN. However, I am aware of public information regarding a GM/Cadillac service update involving the Brake System Control Module and reduced deceleration during low-speed ABS braking events. I am concerned that my vehicle may have a braking/ABS defect or brake control module software issue. I do not currently feel safe driving the vehicle until it is inspected. I am requesting that this issue be investigated because the vehicle displayed ABS activity but did not stop normally during low-speed braking in wet conditions.Read more

He was the driver. We started talking for several minutes with my back to the garage the entire time until neighbor hollered “[XXX] is going on?” The last thing I remember is my Audi Q7 slamming my body into one or both vehicles. I was thrown to the driveway with my right foot wedged under the Audi Q7’s driver’s side rear tire. I sought medical care first thing Monday morning, resulting in a diagnosis of acute neck strain and petechiae, as well as necessitating a specialized cardiovascular evaluation to check my pacemaker function due to localized swelling under the seatbelt line. While traveling on the highway in traffic, brakes became very stiff when depressed after attempting to slow down to exit. Was able to put full bodyweight into the brake pedal to slow enough to exit and navigate to the gas station right off the exit. ... he vehicle was released knowing the transmission was unrepaired and unsafe. The first failure occurred in 2025 on a highway with no prior warning. The vehicle was towed to the dealership, where the transmission was confirmed defective and repaired under warranty. The second failure occurred in January 2026, again on a highway with no warning. The dealership held the vehicle from January through May 2026 — four months — with no updates despite multiple owner inquiries. In April 2026 the dealership confirmed the transmission had failed again and was seeking Hyundai corporate authorization for the warranty repair. On May 21, 2026 I was told the vehicle was ready and the transmission had been fully repaired under warranty. This was false. On [XXX], within 15 minutes of leaving the dealership, the transmission failed a third time, completely shutting the vehicle down in an active turning lane with no warning. The general manager at Hyundai College Park later admitted in a recorded call that the transmission was never repaired and the vehicle was released only to recover the dealership's rental car, with full knowledge it was unsafe. All three failures were confirmed by the authorized dealer. Component is available for inspection. NHTSA investigation and recall consideration requested. ... behavior created a loss-of-control situation and significantly increased the risk of secondary collisions, injury to vehicle occupants, nearby motorists, and pedestrians. I was unable to immediately stop the vehicle despite attempting to do so. The exact failed component has not yet been definitively identified; however, the incident appears potentially related to malfunction or unintended operation of one or more of the following systems: * drivetrain/transmission engagement, * electronic throttle control, * braking system, * powertrain control module, * or Advanced Driver Assistance Systems (ADAS). The vehicle should be inspected for: * Event Data Recorder (EDR) data, * diagnostic trouble codes, * throttle/brake command activity, * drivetrain engagement data, * and post-collision electronic system behavior. The vehicle is available for inspection upon request. At this time, the issue has not yet been reproduced intentionally. The vehicle has not yet been fully evaluated or confirmed by a Kia dealer or independent repair facility for the specific unintended movement condition. The vehicle is expected to undergo inspection and diagnostic review as soon as the Progressive adjuster issues the acceptance/approval letter. The vehicle will then be observed by insurance representatives, and collision repair personnel, as part of the accident and claim process.
