While the vehicle was parked and no one was pressing the brake pedal, the ABS actuator pump continued to run and made a constant buzzing/whirring noise. This suggests that the actuator is cycling excessively to maintain brake pressure. I am concerned this could eventually lead to loss of ABS function or reduced brake assist, increasing the risk of an accident. No dashboard warning lights are curre
... ntly illuminated, but the problem happens repeatedly and can be reproduced. The vehicle has not yet been inspected by the dealer, but I am aware other Toyota/Lexus hybrid models have had similar brake actuator issues in the past.Read more

The reason that I'm currently writing you this letter is to report an incident that happened on Sep. 16th, 2025. This is the date that I was very excited because I got my first EV - Tesla Model 3 new car. Me and my wife went over to the delivery plant in Fefe, Washington. The address is 3701 20th St. E., Fife, WA 98424. We took the car and I myself drove it back home around 2:45ish (-3 :00 pm the
... latest). It was just two blocks away from the plant outbound to the eastward trying to get to the entry of the highway. I stopped my car on my brake, and suddenly my car went forward by itself. This unintended acceleration caused me to hit the car in front of me. We had to stop in the next parking lot on my left side as soon as we passed the traffic light. This intersection happened right at the intersection of 54th Ave. E (Northbound) and Pacific Highway E. I hit the car in front of me. My front bumper got some damage. The car is still drivable and I have put the photos and maps for your understanding. Read more

This particular model 2009 Lexus LS460 apparently has an issue with the ABS system module (ABS Brake pump actuator) and when it fails it is like a domino effect not to mention having to reprogram the cars whole computer system all over again, I researched this issue and found that the issue is rampant with this year and model, not only is it expensive to fix (4k) the part alone is over 1k. If the
... system was to fail while driving it would pose a significant danger to myself and others as I would not be able to brake, or if I drive in wet weather the system could fail on me again. Everyone that owns this model has the same issue. once it happens it is said that it will happen every 30000 miles or so. I bought the car in 2024; I hadn't even had it a year before the system failed and I have not been able to drive the car since March 2025. I think issues involving the cars ABS should automatically be a recall. Now I feel like I am stuck with a car payment for a car I can't even drive because the brake system failed, and will continue to fail over and over, even if I could drive it I would be scared to. If the car senses and accident or spinout and the abs fails and locks up that's dangerous. I can't afford to fix this issue multiple times no one should have to go through this when buying a car as luxury and expensive as Lexus is. The car is too old for the lemon law but I bought a lemon, as big as this issue is this should have been a recall. Please help...Read more

Subject: Safety Concern – Mercedes-Benz E350 4MATIC (2011) Subframe and Brake Line Rust Dear [Mercedes-Benz Customer Care / NHTSA], I am writing to report a serious safety concern regarding my 2011 Mercedes-Benz E350 4MATIC. I pulled over safely and turned the car off and on again but it wo
... uldn't go away. The brakes were also noticeably more firm compared to before as it was quite squishy but now it was solid and all regen braking went away and it appeared I had no more ABS which could lead to a collision. I immediately took it into the dealer to have it diagnosed. I did some reading and if I kept driving with this problem, eventually the brakes would fail and collapse completely and leave me unable to stop the vehicle. At the service center, they were able to read the code DTC C1246 and clearing the codes did not work. The part that was replaced was the brake booster and the brake master cylinder and the brake pedal stroke sensor. It appears that a warranty program exists for this issue but it is not a recall. I am also not sure if the part that was replaced is a revised part that will no longer have this issue or there is no change in the part design. I believe this is a massive safety issue and should be a recall and not a warranty replacement program as every prius should be recalled for this defect. I was very close to being out of warranty and if I had taken any longer to bring it into the dealer I would have been out of the extended CSP warranty. Even the dealer initially thought I would be out of warranty but I had to argue that I was in warranty and should be eligible for a warranty replacement and that I should not need to pay for a diagnostic fee despite it not being a recall.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The defects involve multiple electronic control systems, including steering, braking, and airbag modules, which continue to generate active and stored Diagnostic Trouble Codes (DTCs) even after the May 2, 2023 recall repair. The vehicle remains in our possession and is available for inspection upon r
... equest. 2. How was your safety or the safety of others put at risk? Because the vehicle fails to illuminate dashboard warnings, we were unaware of critical malfunctions occurring in real time while driving. This creates the risk of sudden failure in steering, braking, or airbag deployment, directly endangering both occupants and the public. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Independent diagnostic scans repeatedly confirmed active and stored DTCs following the recall repair. These findings show the defects are reproducible and ongoing. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle was inspected by Kia’s authorized dealer, Spradley Motors in Pueblo, CO, for the recall repair. Since then, diagnostic evaluations have been performed by independent service providers. Documentation of these inspections is attached. 5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? No dashboard warning lamps illuminated, even though faults were recorded internally by the vehicle’s control modules. The absence of warnings despite active codes is itself the central safety issue. 2012 Toyota Prius. The brake system malfunctioned and my daughter was almost in a wreck. The ABS, traction control, and brake warning lights on the bashboard all illuminated at the same time. The brake pedal feels soft and is less responsive requiring more effort to stop the vehicle. The brake actuator assembly is making a hissing noise. Ran a ABS test using a scan tool and received a C1391
... error code. The Code points to a leak within the brake actuator system. Upon research I have found that this is a very common and costly repair that Toyota has known about for many years. Toyota issued a customer support program related to this issue that ran for 10 years or 150,000 miles from the vehicle's date of first use. This car, being a 2012, is beyond the support program. I contacted Toyota, hoping they would do the right thing and cover the repair. I was told that they would not cover the repair or supply the parts to cover the repair. I told the Rep. for Toyota that I did not feel like it was fair for Toyota to continue to put millions of drivers at risk for a product that they clearly realize for years now is defective and could hurt or kill someone. The code and the braking issue did not appear until after the customer support program ended and therefore we had no warning or way to anticipate this braking issue or the danger to my daughter and anyone else riding with her in the car or on the roadway near her travel routes. I traveled 300 miles, rented a Uhaul car tow trailer and brought the car back home. My mechanic confirmed the code C1391 and the fact that the brake actuator has failed. The mechanic has ordered the parts (the same defective parts) to replace the broken ones. Toyota is still selling the same part as a replacement part clearly knowing that it is not safe and no warning came with the replacement part explaining the potential for a braking issue. Based on internet research this same part is in many 2005 to 2013 Toyota and LexusRead more

ABS system failing
Incident date: Sep 13, 2025
Upon inspection, our mechanic determined that the master cylinder assembly and the actuator assembly require replacement (error code C1391). It is currently available for inspection, but will need to be repaired soon. Our safety and the safety of others is at great risk, because there is impending brake failure if driven and the end result could be catastrophic. On 09/13/25, the BRAKE, TRACTION
... CONTROL, ABS and ((!)) lights came on about 10 minutes from home. We drove home, parked the car and did not drive it again until we took it to our mechanic on 09/15/25. During this incident, the brakes failed to respond properly, which could have led to a serious accident. We took the car to a local mechanic and repairs are currently pending. The estimated cost for the necessary parts is approx. $3,000 and the labor costs are estimated to be between $1,100 and $2,000. Our local Toyota dealership was also contacted and they confirmed the presence of code C1391. Please reference Toyota Service Bulletin POL18-03, which addresses issues related to the braking system in certain Toyota models, including the Prius V. This bulletin indicates that various brake system-related warning lamps may illuminate due to internal malfunctions within the brake system, and it outlines affected parts that may require replacement. Additionally, please reference Toyota Customer Support Program 20TE07, which provides extended coverage for certain brake system repairs. It is concerning that, despite this well-known, very serious problem, Toyota has not issued a recall. While our vehicle currently has 116,975 miles, we are aware that the warranty period ended 10 years from the first date of use or 150,000 miles. However, this malfunction not only endangers our safety, but also that of other drivers on the road. We urge the NHTSA to investigate this matter thoroughly and consider the implications for a potential recall for this model, because Toyota is well-aware of this issue.Read more
