On November 27th the accelerator on my Rivian became stuck causing the car to accelerate quickly and preventing me from immediately stopping the car. There was a message stating that the regenerative braking system was not available and the accelerator pedal was stuck in the down position. When I pressed on the brake there was a terrible grinding noise and smoke coming from the front tires. I quic
... kly realized that bringing the Rivian to a full stop would likely cause the car to catch fire. Since the road was clear I had time to discuss a plan with my family and I told them to prepare to get out of the car quickly. I pressed hard on the brakes which caused them to grind and smoke. I put the car in park and we all exited the car quickly. Flames were visible from the front tire. The car is available for inspection. If there had been other vehicles on the road or any pedestrians this would have been a life threatening incident. Rivian is claiming that I must have been pressing on both the accelerator and the brake at the same time. We had 2 adults and 3 children in the car and the incident lasted long enough for us to discuss a plan to exit the car. There is no chance that my foot was on both the accelerator and the brake. Rivian has conducted an inspection but has denied that there is an issue. There has not yet been an independent inspection. The car has been in Rivian's service center since the day after the incident. The only warning sign appeared during the incident stating that regenerative braking was not available but this did not explain why the accelerator pedal was stuck.Read more

On December 16, 2025 the check engine light and service stabiltrek warning light appeared on my 2016 Chevy Malibu Hybrid. On December 17, 2015 I took the car the Capital Cheverolet service dept who has been servicing my car regularly for the past five years. They performed a complete diagnostics on my car that determined I need to "replace the EBCM with brake pressure modulator valve" and said it
... would cost over $2800.00. Upon further research I discovered that the same make model and year was recalled under recall #39440. I also learned the my VIN number was not included in this recall. This is a safety defect that has rendered my car not drivable until I get it fixed. As a senior citizen on a fixed income I am unable to fixed. I would like to know why was my car not included in the recall and if its possible to get it on the recall list so it can be fixed.Read more

The brake system malfunctioned suddenly. While driving and while stopped, the brake pedal became very hard and braking behavior became unpredictable. The vehicle sometimes brakes aggressively and other times requires excessive pedal force to stop. The ABS and BRAKE warning lights illuminated. The vehicle no longer provides normal braking assistance. This issue is related to a failure of the brak
... e actuator / brake booster, which is a known issue affecting Toyota Prius models from 2010 to 2015. This defect creates a serious safety risk, as braking performance is inconsistent and may increase stopping distance or cause loss of control during emergency braking situations. The vehicle has not been repaired yet because the cost is very high and the manufacturer has declined assistance despite this being a known safety-related defect.Read more

The ABS warning light, Traction Control warning light, and Emergency Brake light are all lite up on the dashboard of my 2015 Jeep Wrangler Unlimited Sport. ABS is not working on my Jeep. The shop qouted me $1,500.00 for parts and labor and that was if they could find a new ABS control module somewhe
... re. If part isn't replaced, I'm told that brakes could fail or Jeep could seize up after being driven so many miles. Mechanic shop said ABS control module for Jeeps is on National backorder and they are hard to find, can find used ones at salvage yards but they are priced three and four times what they normally cost brand new and they may or may not be able to flash the module to match my VIN on a used one. In other words, can't get a new one because they are not being produced any longer, and there's no guarantee that a used one will work. Online forums have confirmed all of this information also, as there are thousands of people who are having this same issue over the past year or so. I have spoken with several online dealers who sell OEM Mopar parts and they have confirmed that this part isn't even being produced any longer and there is no replacement part for it. So all this to say, my Jeep is parked, undrivable because of a safety issue due to this part (ABS control module) not being produced by anyone any longer.Read more

While braking and attempting a left turn at an intersection, my vehicle completely shut down with no braking or steering control. I paid for the repair and approximately a month later, while slowing down for a red signal, the same thing occurred, only this time my vehicle impacted another vehicle, causing damage to both vehicles. I am writing to formally report a serious safety incident involving a Tesla vehicle and to document multiple critical system failures that occurred during an accident, resulting in physical injuries to me. On the 12/24/2025, while operating Tesla under rainy conditions, I experienced a sudden and dangerous malfunction of the vehicle. During the event, the steering system became unresponsive and
... appeared to lock, preventing me from maintaining control of the vehicle. Simultaneously, the braking system failed to respond as expected despite my attempt to apply the brakes. These failures significantly impaired my ability to avoid or mitigate the collision. Most concerning, the vehicle’s airbag system did not deploy during the impact. Given the severity of the collision and the resulting injuries, the lack of airbag deployment represents a serious failure of a critical safety system designed to protect occupants in such events. As a direct result of this incident and the apparent malfunction of multiple safety systems, I sustained physical injuries that required medical evaluation and treatment. Beyond the physical harm, the incident caused significant emotional distress and raised serious concerns about the safety and reliability of the vehicle. I am submitting this complaint to formally document these safety failures and to request a thorough investigation into the performance of the steering, braking, and airbag systems involved in this incident. I believe this matter presents a significant safety concern not only for myself but also for other drivers and passengers who rely on these systems to function properly in emergency situations. Please confirm receipt of this complaint and advise me of the next steps in your investigation process. I am prepared to provide additional documentation, including medical records, photographs, vehicle data, and incident details, upon request. While driving, the vehicle experienced a sudden brake system malfunction without prior warning or gradual degradation. The vehicle was being operated under normal driving, road, and weather conditions. No recent brake service, repairs, or aftermarket modifications had been performed prior to the failure. Multiple warning lights illuminated on the dashboard, including the ABS warning light, brake
... warning light, and traction control warning light. Shortly after the warning lights appeared, the vehicle began emitting continuous warning beeps. Audible buzzing or pumping noises were also present from the brake system during the malfunction. During this event, brake performance was reduced and braking felt inconsistent. The brake pedal became hard at times and spongy at other times, requiring increased effort to slow the vehicle and resulting in unpredictable braking response. This created a safety concern while the vehicle was in motion. The driver was forced to alter driving behavior and exercise extreme caution due to reduced braking confidence and unpredictable pedal feel. The malfunction has occurred intermittently during normal driving. The condition temporarily resolves on its own and later reoccurs without corrective action. The suspected failed component is the brake actuator / ABS actuator assembly. A diagnostic scan retrieved fault codes C1252 (Brake Booster Pump Motor ON Time Abnormally Long), C1253 (Pump Motor Relay Malfunction), and C1256 (Accumulator Low Pressure), which are consistent with brake actuator malfunction. The failure occurred at approximately 154,000 miles. This vehicle model and generation have a documented history of premature brake actuator failures with similar safety-related symptoms reported by other owners. The condition places the driver and others at risk due to unpredictable braking behavior, reduced braking effectiveness, and repeated warning alarms while driving. Warning lamps and audible alerts continue intermittently.Read more

While operating my Tesla Model Y in a commercial parking lot at a T-intersection controlled by a stop sign, the vehicle unexpectedly and automatically accelerated without driver intent. The incident occurred during a low-speed maneuver. Upon the unexpected acceleration, I immediately attempted to regain control by applying the brake pedal repeatedly and firmly. Despite clear manual brake applicat
... ion, the vehicle did not decelerate as expected and continued to move forward. I also attempted to stop the vehicle by pressing the Park button, but this action did not stop the vehicle. The vehicle continued uncontrollably for approximately 150+ yards, traveling over curbs, striking a fence, and ultimately colliding with the garage structure of a residential home. Throughout this sequence, the vehicle remained unresponsive to braking and stop inputs. Vehicle data later confirmed that brake pedal application was detected prior to impact, yet braking response was insufficient to prevent continued motion. At the time of the event, the vehicle recorded system fault states, and collision avoidance systems did not activate. Despite significant structural impact and property damage, airbags did not deploy. The incident raises serious safety concerns regarding: •Brake-by-wire system execution during fault conditions •Pedal input arbitration between accelerator and brake •Failure of collision avoidance systems to intervene •Failure of restraint systems to deploy during a severe impact •Vehicle behavior when control systems are in a fault state This event occurred without warning and despite appropriate driver response. Based on vehicle data and system behavior, this appears to be a system-level failure rather than driver error.Read more

After several visits to dealership, I have been told I am not able to get this recall remedied. I am stuck with a vehicle that is unsafe to drive, according to the Dodge dealership and unable to be traded in. I am now having a financial loss as I pay for rent on my RV and am unable to pull it due to the faulty part and safety concern. The ABS indicator and traction control light is lit on the

The staff at all dealerships I have visited have been of no assistance and virtually told me I am just out of luck. I commute to work and travel for pleasure. As an older individual, I do not have the financial means to take the loss on a vehicle and just buy another. My 2017 Jeep Cherokee rolled away three times in one day while in Park with the Electric Parking Brake engaged. My driveway has no flat area and slopes toward a public road, creating a serious hazard for my family and traffic. The rollaways occurred suddenly with no warning lights. I had to park the Jeep sideways in a snowbank for two weeks to prevent further rollaways, creating added risks for my
... family in winter conditions. The vehicle remains at the dealership. During the first two days of the evaluation, the dealer confirmed brake shoe thickness (4mm), verified the EPB software was current, and found no diagnostic codes. They could not recreate the rollaway or identify a cause. No mechanical or electronic faults were found. FCA communication has been inconsistent. I experienced dropped calls, long holds, contradictory statements, and repeated refusals to provide written documentation of the inspection authorization. FCA said they could not escalate the safety case unless the vehicle was already at the dealership, yet still refused written confirmation of the purpose of the inspection, and I was not willing to bring the vehicle in without documentation. After the evaluation, when I attempted to relay the required findings per the RO instructions, FCA informed me the safety case had already been closed, leaving no record of the defect. FCA said only the rental authorization remained open and they could no longer escalate my case. I then had to re-explain the entire rollaway incident. FCA and the dealer now disagree on whether the RO is open or closed, and FCA has not explained who is responsible for the ongoing loaner or what the next steps are. The defect remains unresolved and the Jeep is unsafe to operate or park at my home.Read more

2017 F150 Super Crew 4x4 brought into dealership for brake master cylinder recall. ( no lights on dash) Dealership changed both the master cylinder and brake booster at no charge. The vehicle was having issues with keeping straight on the highway and when I slowed down when coming to a full stop, the steering wheel turned co
... mpletely to 1 side on its own. These symptoms were not present when vehicle was initially dropped off. Dealership is now telling me that the ABS module is bad and that I need to pay for that repair. The local Ford dealership submitted documentation (Special Service Support Center Assistance Request) to Ford to cover related damage, but request was denied. I called Ford corporate to re-open case and appeal decision. Ford corporate would not re-open. I requested a field service engineer to review my situation, Ford said No. I asked for good will assistance, Ford said no. I asked if Ford could do anything to assist, Ford said no. It has now been over 45 days without my vehicle. The steering wheel rotating on its own after braking is a critical safety concern. The local dealership has changed the recall to complete and left me with a vehicle that is not safe to use. They claim the ABS module is bad and I have to pay out of pocket, but they have yet to provide any documentation or reports including any OBD2 codes that indicates this is not as a result of the recall work performed. In summary, I left my vehicle at dealership for recall on master cylinder and after repairs the vehicle is having issues with related components that were not present before dropping off. I am writing to request urgent assistance regarding a serious safety and service issue with my 2015 Dodge Ram, which has only 45,000 original miles. I recently took the vehicle to an authorized Dodge dealership after multiple dashboard alerts appeared, including: ABS Light On, Brake Light On, “Service Trailer Light,” “Service Antilock Brake System”, Service Electronic Braking”, “Servi
... ce Trailer Brake System” The dealership diagnosed the problem as a faulty ABS Module. I was informed that the part would cost approximately $1,400, plus the cost of ordering the part, and that the part may take up to four (4) years or more to arrive—if it ever becomes available at all. I contacted multiple Dodge dealerships and received the same response regarding the extreme delay and unavailability of the ABS Module. I was also told that the vehicle is unsafe to drive in its current condition. This is especially concerning, as this is my only vehicle, and I cannot use it due to the safety risks. Despite this, Dodge customer service informed me that no assistance can be provided unless I first pay for and order the part, even though the part may not arrive for years. Additionally, the dealership is unwilling to buy back the vehicle at a reasonable price due to the unresolved ABS issue, leaving me without a safe, drivable vehicle and without support or alternatives. I have confirmed that there is no recall listed for this issue, yet the ABS Module is a critical safety component, and the lack of part availability for years is unacceptable for a loyal customer who has maintained the vehicle properly and driven it minimally. I am respectfully requesting that Dodge Corporate provide A clear explanation for the extreme parts delay, A prompt solution—such as expedited part sourcing, coverage or assistance with repairs, or a reasonable buyback/exchange option, Guidance on how Dodge intends to support customers facing major safety issues caused by unavailable parts.Read more

I am concerned that requiring brake replacement at such low mileage indicates a potential design, material, or manufacturing issue with the braking system on this model year. I took it to First Class Halt and they found some code they had never seen. They cleared the code and 2 days later it started up with the same thing. I took it to Hendrick Honda and they also just cleared a code. A few days later it started all over making the car undriveable at least in a safe manner with full control over the brakes. It seems that neither an independent shop nor a Honda dealership can correct this dangerous problem.Read more

Brakes started seizing up while driving. Became extremely dangerous as the car would rock at speed and also would not start moving if I came to a stop. Went immediately to a shop, when I arrived into the shop, inspection found that the caliper was locked onto the wheel preventing the front wheel from moving. Had the caliper replaced but the caliper was still seizing, and then had to get the ABS

This issue had no warming lamps, messages or anything indicating this other than a feel that the car is accelerating more slowly. However, when the brakes started seizing, the situation felt extremely dangerous.Read more

I was driving to work last Wednesday, and I live on a hill. As I was driving, a "Brake failure" alert came over my dash and I lost all control of my brakes!!! I live right off of Main Street, and my options were only to go straight and into a house, or to turn onto Main Street and hope no one hits me or that I don't hit another vehicle. I turned and thankfully no one was coming. At the end of
... that street I was able to coast into an closed down gas station and park the vehicle. I called the dealership I had purchased this vehicle from 10 months prior, and they told me I had to use OnStar to have it towed to them. I did that, and they towed it to the dealership. The next day the dealership told me the cause of losing my brakes was due to a SOFTWARE UPDATE that was needed!!!!! They charged me $800 for the update & a battery that they said was not holding the charge, which I've never had an issue with. I had purchased an extended warranty with this vehicle and I was told it would be a bumper to bumper warranty AS IF I had purchased it new (it was 2 years old). The dealership said this was not a critical software update and was not covered under any warranty. I could have been KILLED or seriously injured or killed or hurt someone else due to my brakes going out due to a SOFTWARE UPDATE that was needed!! This is insane!Read more

This 2021 Toyota Supra, subject to Toyota’s own “Original Brake System Defect” recall, exhibited chronic braking noise from delivery. Despite multiple dealership visits, the core issue was never resolved. The defect escalated in late 2025, with the vehicle intermittently displaying the most critical “STOP DRIVING” brake warning. In mid-December 2025, it experienced a catastrophic “Sudd
... en Unintended Steering and Acceleration” event during a low-speed turn, where the steering wheel autonomously jerked side-to-side accompanied by uncommanded acceleration, causing near loss of control. This appears to be a dangerous progression of the factory brake defect into a complete failure of the vehicle’s dynamic control systems. On the afternoon of December 17, 2025, after 2:30 PM, I was returning from the grocery store. Approaching a T-intersection, I was the first car in the left-turn lane. After the light turned green, I initiated a left turn at a normal, controlled speed. At the precise moment I straightened the steering wheel to enter the through lane, the steering wheel suddenly and violently jerked left and right on its own. Despite my foot being completely off the accelerator, the vehicle autonomously accelerated in sync with the erratic steering movements. The situation was extremely abrupt, dangerous, and nearly impossible to control. I had no time to feel fear; my entire focus was on desperately trying to regain control to avoid a crash with pedestrians or other vehicles. Fortunately, through quick reflexes and rational judgment, I concentrated intensely on the actual conditions and surrounding traffic. With a final, forceful effort, I wrestled with the steering wheel and applied the brakes, managing to bring the vehicle under control after roughly ten seconds. I safely pulled over to the roadside. I cannot accept further repairs on a proven life-threatening product. I urgently request NHTSA to oversee an investigation into Toyota regarding this systemic failure .Read more

The brake booster vacuum hose attached to the intake manifold fitting swelled over time and was close to detaching from the intake manifold fitting. This condition could allow the hose to detach from the fitting during an engine backfire, resulting in loss of power brake assist and increased brake pedal effort. As a result, stopping distance may increase and potentially result in a crash.
