This is a rental vehicle- I had several incidents when I could not stop the vehicle. Above the brake is a structural bar about 1/2" in diameter. I wear a size 13 shoe. At several red lights I tried to apply the brake but the car would not stop. (I did stop before hitting anyone) Upon investigation I discovered that my toe was pressing on the bar, not on the brake. Only when my shoe's heel was on
... the floor was I able to clear the bar and step on the brake. I normally keep my foot somewhat higher than the floor when braking my own car, so this was a dangerous surprise to me.Read more

The 2024 Subaru Crosstrek brakes engaged TWICE without the driver pushing on the brakes, slowing the car down significantly. The initial vehicle speed was around 65 mph on the highway, and slowed to around 45 mph after the second automatic braking incident occurred. Adaptive cruise control was NOT on. The car in front of the Crosstrek was 8-10 car lengths away. Then while the Crosstrek was still
... moving, a truck hit the back right bumper of the Crosstrek and pushed it into the next lane to the left. (I), the driver of the Crosstrek, was able to steer back into the original lane of travel to the right. After the Crosstrek was steered back to the original lane of travel, it came to a stop due to right rear axle damage. The Crosstrek driver was able to creep to the shoulder to wait for emergency personnel to arrive. The safety of myself and others, due to the Crosstrek (Eyesight?) malfunction, is extremely important to investigate. This safety feature when engaged unexpectedly is dangerous. When drivers do not have control over the braking system of the car, sudden and unexpected slow-downs can lead to rear-end accidents, and even the loss of life. When I was pushed to another lane of travel, a secondary accident could have occurred, or a chain-reaction with many cars. I did not see or hear any warnings, messages, or experience any unusual symptoms before the car braked on its own and I was rear-ended. After I crept to the right shoulder of the interstate, the Crosstrek was still running and there were no warnings or messages. Since the Nov 11 accident, Dean Team Dealership in Ballwin MO has reported the issue. State Farm Insurance has totaled the 24 Subaru Crosstrek, recovered data from the Event Date Recorder (EDR), and is discussing the vehicle losses with Subaru Corporation. I was driving my 2014 Kia Soul on Friday 12/12 when without any warning the brakes went out. Thankfully I was able to get it to a parking lot without incident and requested a tow. After having it repaired, the mechanics stated the brake lines completely popped due to brake line corrosion. After researching this issue, I found that my Kia Soul had a Voluntary Service Campaign recall regarding the e
... xact corrosion on the brake lines that would likely be responsible for this catastrophic failure. I was never notified of this possibility despite having my car routinely serviced in NJ and VA. This is a huge oversight on Kia’s behalf – other people with this make and model need to be notified that there could be unseen corrosion happening. Fortunately, I wasn’t on the highway when the brakes went out and thanks to my careful navigation I was able to avoid getting into an accident with others on the road at the time my brakes suddenly failed. Afterwards, I called Kia and requested reimbursement as this was a recall that I was never notified for but they refused as my VIN was not listed under the VSC. Due to the catastrophic nature of the failure and the potential for damage to property, severe injury, or loss of life, I am notifying NHTSA in hopes of escalating this issue and to raise awareness for other Kia Soul drivers. Prior to this incident, NO lights, NO audible warning, OR other signs of impending failure occurred. Had I been notified of this issue through the inclusion in the VSC, I would have been able to take steps to preemptively address this issue and avoid the associated risk of operating such a potentially compromised vehicle.Read more

I am experiencing a recurring battery/charging system detection warning with my vehicle that has not been resolved despite multiple service visits. I have made repeated attempts to work with GM and the dealership, but meaningful assistance or escalation has not occurred. This ongoing electrical issue raises concerns about the reliability and safety of the vehicle, as well as GM’s failure to correct a known, documented problem. I am requesting appropriate escalation and resolution, including further diagnostic intervention by a regional service manager or field engineer, or consideration of alternative remedies if the issue cannot be permanently repaired.Read more

There are no lights indicating problems on my dashboard. The weather was clear and sunny in the morning. There was no rain. There was no snow. All the roads were dry. I took the car to my dealership and they could not find anything wrong. The brake pads were well within the normal safety range at 8 mm. There was no computer system malfunction. They could not give me an explanation. The Audi dealership’s head mechanic said that because my front brake pads and rotor, and my back brake pads were replaced at Pep Boys two years ago that this could be a reason for the brake failure. although he was not 100% sure of this. He suggested that I pay $3500 for Audi to replace the current rotors and brake pads despite the original determination that there were nothing wrong with them. The transmission is slow to change gears in lower gears and the van jerks a little bit. Next my rotors were warped. I was informed by the dealership that there was nothing wrong with my transmission and the auto idle down was working properly. NO one could explain the rotors being warped. They reshaped my rotors and replaced the battery and told me they hoped that fixed the problem. Well indeed it did not fix the problem. As of 12/27/25 the van completely shut off at a stop sign and again on 12/28/25. The issue is only getting worse (24,000 miles) and now I am scared to death that the van will stop while i am driving. To Whom It May Concern, I am submitting this complaint to report a serious vehicle safety issue involving a Honda vehicle that was under a lease agreement. From the start of the lease, the vehicle suffered from repeated malfunctions of its safety and proximity sensors. These issues were reported, and the vehicle was taken multiple times to an authorized Honda dealership. Despite these efforts, t

I later discovered that the vehicle had an active safety recall that was never disclosed to me, even though the vehicle had been serviced at the dealership on several occasions. This failure to inform me of a recall represents a significant safety concern. Subsequently, the vehicle was involved in an accident that resulted in a total loss. I firmly believe that the unresolved sensor failures and the lack of recall disclosure played a role in this incident. I am submitting this report so that NHTSA may investigate this matter and determine whether there is a broader safety issue affecting other vehicles. Thank you for your attention. Incident date: Dec 1, 2025
CRITICAL SAFETY DEFECT: HIGH-SPEED BRAKING SURGE & ELECTRICAL FAILURE Life safety concerning ! 1. Braking Failure at Highway Speeds: The vehicle exhibits a severe safety defect involving unintended acceleration/surging while braking. While driving in 'D' (Drive) mode at approximately 50 MPH, when applying the brakes to stop for traffic/red lights, the vehicle fails to decelerate immediately. Inst
... ead of slowing down, the vehicle surges forward against the braking input, feeling like a loss of regenerative braking authority. This results in extended stopping distances and near-miss collision risks at high speeds. 2. Charging & Electrical System Failure (Suspected Recalls): The vehicle suffers from persistent electrical failures that the dealer dismissed as "user error" or a "bad plug," but which match known Audi safety recalls: (A)AC & On-Board Charger Failure: The vehicle threw "AC System" error codes and fails to charge reliably, shutting off after short periods. This mirrors symptoms of Recall 93FR (High-Voltage Battery/On-Board Charger), risking a 12V battery stall. My car model was among those impacted recalled car dearies even now it is not listed as one of the recall Model by VIN#, I strongly suspected my car has same defect. Many websites have address this life threatening issue would [XXX] ." (B):Charging Cable Overheating: The included 220V charging cable is suspected to be defective (matching Recall 93U6), which poses a fire risk at the outlet. (C)Interior Voltage Instability: Wired USB-C ports cut power/disconnect devices less than 5 minutes of use. 3. Dealer Negligence: The dealership (Audi Tysons Corner) failed to diagnose these issues ("Could Not Replicate"), refused to replace the recalled charging cable, and released the vehicle with active "Inspection Due" warning lights. Front Passenger Brake Hose Failure by Crimp
Incident date: Dec 26, 2025
The failure involves the engine/powertrain system, specifically the engine oil pressure system, resulting in loss of engine power and a subsequent loss of effective braking assistance. The vehicle displayed a low engine oil pressure warning and a “powertrain system fault” message prior to the loss of braking effectiveness. The vehicle is currently not being driven and is available for inspecti
... on upon request. The engine was previously replaced in April 2025 as part of a recall-related interim remedy following an engine failure, yet the same symptoms have since reoccurred. My safety and the safety of others were placed at immediate and severe risk. On December 26, 2025, while driving at highway speed in heavy rain, the vehicle rapidly decelerated from approximately 79 mph to 60 mph, creating a dangerous situation in active freeway traffic. After exiting the freeway, the risk escalated when the vehicle experienced a loss of effective braking. While entering my apartment complex, I was forced to engage in defensive driving to avoid a collision. I narrowly avoided striking a pedestrian who was exiting their vehicle and then had to defensively steer away from the front of my apartment building to avoid crashing into the structure because the vehicle would not stop. A serious injury or fatality to myself or others was narrowly avoided. Yes. In March 2025, after experiencing a serious loss of power while driving, the vehicle was towed to a Ford dealership. The vehicle remained there for approximately one month, during which time the engine failure was confirmed and attributed to an active recall. As a result, the engine was replaced under the recall. This confirms that the issue has been previously verified by an authorized Ford dealership. Despite this repair, the same failure symptoms have reoccurred. The vehicle has been inspected and serviced by an authorized Ford dealership in connection with the recall-related engine failure and replacement in March–April 2025.Read more

My brakes went out completely while driving my 2015 kia soul with my daughter in the car. It was traumatic and stressful for us both. They inspected my car and said they would replace the rear brake lines under the recall. A couple weeks went by and I was told
... they couldn't find the parts to fix it and they are going to cover the front break lines as well under the recall. I didn't receive a loaner car and transportation was a daily struggle. Altogether my car was at the dealership for around 35 days. I received an invoice through email and didn't get a call or any notification that my car was ready for pickup. I called the dealership 2 days later and the person said it was ready. I get to my car and it was dead and I had to jump my car in the parking lot. The cover for the under carriage was not on my car where is goes it was in my back seat and it was dirty. My gas light was on and I had a quarter of a tank when I paid to tow it to the dealership. My receipt said my car was fixed and in working order and they completed a check point inspection and besides the battery they said it was all good. I drove my car to my house and by the time I got to my house I noticed my breaks were going all the way to the floor. My car still has no breaks just as it did when I had it towed to the dealership. It feels like no work has been done. I've already been without my car for over a month to fix this problem under a recall. The dealership was rude and not professional and I notified Kia America through email but because the Christmas holiday I haven't heard back yet. I'm scared to drive it now and it has created a fear I can't shake. Im not sure what to do next but I'm going to call Kia corporate in the morning.Read more

While coming to a stop on a slight incline, the brakes failed to stop the car, brakes were applied and initially slowed the car but when the slight incline was reached the brakes would not engage and the idle of the car move it forward and still the brakes would not engage. I had to put the car into neutral then park to stop. After the incident the brakes seemed to work fine. Nonetheless this is
... a major issue. There were no warning lights or any indication that something was malfunctioning. There were no indications after the incident either that anything malfunctioned. I will be taking the car to the dealer ASAP to have checked. We lightly tapped a truck that was in front of us on the slight incline - no obvious damage to either vehicle ( need to verify in daylight) - no injuries to anyone in the incident. If we were traveling faster there could have been serious injury or if there was a pedestrian there could have been serious injury.Read more

Vehicle Breaking on Its Own While Driving I am writing to file a formal safety complaint and to demand immediate corporate intervention regarding an extremely dangerous mechanical/ADAS defect. My Nissan Altima has serious, ongoing safety issues where the vehicle will unexpectedly and aggressively apply the brakes on its own while I am driving, including on highways and regular roads. This defect h
... appens without my input, without warning, and creates a major risk of being rear-ended, losing control of the vehicle, or causing a serious accident. This issue is Not minor. It is a life-threatening malfunction that makes the vehicle unsafe to operate. I have taken the vehicle to authorized Nissan dealerships for diagnosis and repair on multiple occasions & originally they told me it should be under warranty & when they spoke to the dealership they said it did not qualify under the warranty nor the extra insurance purchased & i'd have to come out of pocket for repairs. A vehicle that breaks on its own, especially at highway speeds – is not safe, not reliable, and not roadworthy. This situation has gone on for years and the vehicle is now a clear danger to me and the public and my three minor children who ride in the car with me. I am requesting immediate action and a good-faith resolution.Read more

I believe there was an issue with the ABS system that kept me from stopping the vehicle, as speed was increasing while going downhill with the brake pedal pushed hard. I was forced to make the choice to steer the vehicle into a mountainside to stop it before I ended up over the side of the mountain road. The vehicle then tipped over, coming to a stop.

I own a 2014 Jeep Wrangler, Rubicon. I have not been able to drive my Jeep for 3 years because the ABS light is on and I am unable to get the part to replace it. I have heard from several other Jeep owners that they are experiencing the same ABS failure. The ABS light has been on since 2022 and there are no MOPAR ABS replacement parts. I would like to officially report this failure because I hav

Toyota’s part number 44050-48190 brake actuator assembly failed, as it has in other models and is very commonly doing in the Highlander model. It is triggering other warning lights issues in the vehicle. Vehicle safety is at risk. Many others have reported the same issue.

My 2012 Toyota Prius plug in hybrid had just passed state inspections and was even looked at by a dealership a year ago with no reported issues. I brought up a whirring issue at the dealership when breaking but the dealership said that’s normal for a hybrid. In a matter of minutes while driving, the traction light, brake light, and ABS light came on as well as a groaning, whirring sound when bre
... aking. Over time, the breaks failed and I could not stop the car, almost causing a wreck because I could not stop without putting all my body weight on the break pedal. When taking it to the Toyota dealership for a whole diagnostic, they said it was a break actuator and break booster failure that is known to happen with Prius’s. They quoted me $3000 for a repair and then marked my breaks as yellow not red on their sheet implying I could still drive it despite the dangerous situation of driving without functioning breaks. When looking into it, this seems to be a known and common problem with Prius’s due to a faulty manufactured part. Codes were c1391, wc1256, and c1252. I believe this part should be recalled and Toyota should fix these faulty parts, especially considering they inspected my car and did not mention it as an issue despite it being a known issue. I got a second opinion from Mavis Tires and they concurred with the diagnosis but could not fix it.Read more

The brakes are loud, grinding, and sounds like water sloshing around. Upon research, VW knows this issues since their 22 model and has yet to address it but rather keeps using bad material brakes. The braking results in shaking when breaking especially in the colder months. This is not safe and on a brand new car, should not be happening. The dealerships acknowledged that this is a known issue wit

The vehicle is a 2016 Ford Fusion with 70,000 miles when this happened happened the first time. I pushed on the brakes and the pedal went to the floor. I had just enough braking power to stop the vehicle. The danger is obvious, I had no brakes but I was going slow on local roads. I had the vehicle towed to an independent repair shop where they diagnosed failure of the right rear brake caliper.
... They replaced the caliper, hoses and ceramic brake pads at a cost of about $600. All was good for about 1500 miles when the exact same thing happened, except now it was the driver's side caliper. Fortunately, once again, I was driving on local roads at low speeds and by holding the pedal totally against the floor, I was able to drive over the curb onto a lawn without hitting anyone. if I had been on an Interstate, I don't see how it wouldn't have resulted in a serious accident. I have no parts or pictures from the first failure, only the receipt for work done, but the car is in the same repair shop as I write this and I will have the failed parts and pictures when I pick up the vehicle on Monday, 12/29/25. Two brake caliper failures, with no warning, at 70,000 and 71,500 miles is a scary scenario. I have no confidence in the vehicle's safety and will be disposing it as soon as it's fixed. Your system won't allow me to put in two dates for the two failures but the first was on 01/24/25 at approximately 70,000 miles and the second was today 12/26/25 at approximately 71,500 miles. The vehicle is registered to me at the address listed on this form but I am a snowbird and the vehicle is in Naples Florida at my winter residence.Read more

Emergency braking occurs at random, even if I'm the only vehicle / object on the roadway. This has been ongoing and has been to the dealership three times. It starts back up each time the vehicle gets an update. Adaptive cruise will brake hard due to false emergency braking, but does not always stop accelerating at the set point (has gone as high as 20 mph over the set point previously and brakes
... would not respond during). Vehicle was not starting and "press brake to start" warning was showing even though brake pedal was pressed. Brake master cylinder had to be replaced along with brake module. When picking the vehicle up afterward, warning light came while vehicle was warming up "service brake assist" and then shortly after the yellow-circled ABS dashlight came on. Dealership said this was a new problem and separate from the brake master cylinder and brake module.Read more
