SUPPLEMENTAL INFORMATION FOR ODI#11708898. I am providing additional documentation to this ODI case. I had to create a new complaint since there appears to be no way on-line to add this information. Gunther Volvo has not provided ANY status updates of my vehicle which has been in their possession since Sunday, January 4, 2026. The Director of Customer Experience, Nyam Smith had not responded
... to my email sent on 1/4/2026. Volvo Customer Care had assigned a case # for towing and for the safety incident but both the dealership and Volvo Customer Care have been unresponsive. I did not receive responses to my texts to the dealership asking for any updates, and Nyam Smith said in an email that I needed to get any updates from Volvo Customer Care. I called them to get an update but Pablo had no update and said he had to call the dealer! The Service Tech I had talked to on 1/5 about the car left me a voice mail at 5:05 PM on 1/5 stating that the foreman has been in contact with Volvo Engineering and that they had a few questions -- how many times had this issue occurred, how did I get the car to stop running and they requested a photo of the shoes I was wearing. I sent that photo that evening but I never received any response from numerous texts asking for a status. They have not provided a courtesy loaner for the past 15 days so I have had to incur incidental rental charges. 1/9/26, I spoke to Jocelyn at Volvo - she was unable to give any updates but she sent me a form (one of the questions asked where the vehicle was!) -- I emailed the completed form that same day but received no acknowledgement from them until 1/15. They sent me a form authorizing an investigation of my car. I have not received any further communication from Volvo Cust. Care or the dealer. They are withholding diagnostic. data (BCM/ECU logs following the unintended surge event on 1/4/26. I am calling the dealer 1/19 to speak with Vasil Blagoev, Service Mgr. for a statusRead more

While driving on interstate 20 and then on I65 my cruise control dropped off causing the vehicle to sudden decrease in speed. Rear end collision narrow avoided due to commercial vehicles, following, quick action saved a disaster. This loss of cruise control, is/was caused by the warning ⚠️ of recall C4B/NHTSA 24V-896. I am reporting a safety concern involving delayed brake engagement on my 2024 Acura ZDX RWD. During normal braking, the brakes do not respond immediately. The vehicle slips and launches forward several times before braking force applies, and the traction control system activates prior to adequate deceleration. It feels like the brakes aren't catching properly. This condition occurs consistently an
... d repeatedly, including on dry pavement, and is worse on wet pavement. It is not intermittent and does not require hard or emergency braking to occur. The delayed braking response creates a loss of braking confidence and presents a safety risk, particularly in traffic or during routine stops. I am concerned this may involve the brake control system, regenerative braking transition, or stability/traction control integration.Read more

While driving my 2022 Lexus GX460, I experienced a sudden and unexpected loss of braking capability while stopped. After bringing the vehicle to a complete stop, the brake pedal pressure gradually decreased while my foot remained firmly on the brake. As pressure lessened, I applied additional force, but the brake pedal continued to travel downward until it reached the floor. At that point, the veh
... icle began to move forward at idle despite the brake pedal being fully depressed. I rolled towards the intersection (at a stop light). I took my foot off the brake and applied again. It stopped, and then this repeated. I then put the car in park while I waited for the light to turn green. My destination was only 1km away. When I got there, I turned off the car, and have since been unable to replicate the issue. I took it to a dealership (Lexus on the Park) and they charged me for an investigation and told me the car is good to drive. I need Toyota/Lexus to be informed. Note that I am in Canada but a friend advised for me to submit here. I posted my issue on a Lexus forum and a person named Patrick said he had the same issue in September and was told by the dealership everything is fine. I've asked him to submit to NHTSA as well. Approximate Mileage on Car: 26500 Make/Model/Trim: Honda CRV Hybrid Sport-L Location: Manheim and Lancaster Counties, Pennsylvania Date of Incident: 1/17/26 Weather: Light snow, under 3", mostly plowed/salted surfaces While driving in light snowy road conditions, the vehicle failed to decelerate appropriately when the brake pedal was firmly applied. The driver was operating the vehicle cautiously
... and at reduced speeds under 20mph, appropriate for winter conditions. Snow accumulation was less than approximately 3 inches, and the roads were mostly plowed and treated with salt. Despite firm brake application, there was no noticeable ABS pulsing or vibration, and the vehicle continued to slide forward with minimal reduction in speed, right through a stop sign and then again at a 4-way traffic light intersection. Both times, the braking response felt delayed or ineffective, resulting in a significantly extended stopping distance. Steering control while braking was limited, leading to fishtailing and spinning across the road. This did not feel like normal loss of traction or typical ABS engagement. The behavior was unexpected given the relatively moderate road conditions and the cautious driving behavior. No warning lights were immediately displayed during the incident. The experience felt like a loss of effective braking capability in winter conditions, creating a serious safety concern. This condition increases the risk of collision due to the inability to stop the vehicle as expected, even when driven cautiously in moderately snowy, plowed, and salted road conditions. A driver has no reliable way to compensate when braking response is inconsistent or delayed. The vehicle is marketed as AWD and suitable for adverse weather conditions. The braking behavior was inconsistent with expectations and with prior vehicles driven by the driver under similar winter conditions for over 40 years.Read more

On or December 5th, 2025 (4 days after taking possession of the vehicle, I experienced a sudden and severe steering system failure while driving. When turning the steering wheel (even slightly), the vehicle produced a loud buzzing/grinding noise and the steering effort increased significantly, creating an immediate safety hazard. Because the steering issue worsened quickly and made the vehicle dif
... ficult to control, I stopped driving and brought the vehicle to an authorized Audi dealership for inspection. The Audi dealership confirmed the steering failure was caused by a major power steering system defect, including active power steering fluid leakage from the steering rack area consistent with internal seal failure, and the power steering pump being faulty due to fluid loss. The dealership advised replacement of the power steering rack and pump and stated the vehicle was unsafe to test drive due to its condition. During the same inspection, the Audi dealership also identified additional serious safety-related defects, including a brake system condition where the brake pedal slowly sinks toward the floor when pressure is applied (indicating a braking system performance failure requiring brake fluid exchange and further diagnosis if unresolved). The inspection also documented multiple additional mechanical and safety concerns requiring extensive repairs, including suspension and drivetrain defects and other conditions indicating the vehicle was not roadworthy as sold. The vehicle remains available for inspection. This incident involved a sudden loss of steering assist and braking abnormalities that could lead to loss of control, crash risk, and injury to the driver, passengers, and other motorists.Read more

The failure involves the Integrated Brake System, which is subject to a manufacturer issued safety recall, and related braking components including a wheel speed sensor that is part of the brake and stability control system. The vehicle is currently back in my possession and is available for inspection upon request. My safety and the safety of others were put at risk due to sudden and unpredictabl
... e loss of braking capability while driving, including abrupt deceleration and stopping in traffic, creating near collision situations. The issue has been reproduced multiple times and confirmed through repeated service visits, with the first and second recall related repairs performed at BMW of West Houston and a third repair attempt performed at BMW of Houston Midtown, following which the vehicle was returned to me. The vehicle has been inspected by authorized BMW dealerships and BMW North America has been notified, though I have not received guidance from corporate BMW regarding a plan moving forward or instructions on how to proceed should a fourth brake failure occur. Brake related warning lamps and system messages appeared following both the first and second recall repairs, and after the October 30, 2025 recall repair the vehicle experienced a brake system failure within approximately 22 miles of driving despite the presence of warning indicators. Given the repeated failures after recall repairs, the prior warning messages, and the lack of direction from BMW North America, I am concerned about operating the vehicle, particularly for longer distances, due to the unpredictable nature of the brake failures and the unresolved safety risk.Read more

No accident has occurred as of yet (for my case). Ongoing issues with Hyundai left brake bulb and center brake bulb burning out due to over-amperage. It appears that these two bulbs are possibly receiving 15A and therefore burning out prematurely (3 months or less). I recently replaced these two bul
... bs at the same time, and now my battery is shorted. To keep my battery charged, I need to pull my 15A Brake Lamp fuse while the car is parked. This has now caused my ABS light to come on, as that system relies on the signal from the 15A brake lamp circuit. I suspect that some 15A components (10A+5A) received 20A (15A+5A) and welded shut. I am still looking for it to trace it down, resources are limited and I'm intent on just buying the $35 diagnostics book. Hyundai dealership did not inform me that this fuse was pulled while I drove home from the shop yesterday, creating an unsafe condition. They quoted me $500 to replace a brake lamp relay and the ABS (I'm assuming the whole ABS, but maybe just the control module), I declined. My plan for now, as this car isn't driven much, is to trace it down and find the shorted component. From my research, this is clearly affecting many years of Hyundais and Kias, not just my Elantra and not just my year. In fact, it's SO common that there are facebook groups (such as this one [XXX] ) dedicated to helping owners remedy this defect. I think a recall service, and a full investigation of what is electrically wrong with these cars, and what the actual remedy should be, is necessary based on the amount of cars that could be impacted. Hyundai & Kia technicians I am SURE are intimately knowledgeable about this issue by now and us owners are on the hook, having only 80,000 miles of warranty on anything purchased used. Incident Description (ABS Module Failure) The ABS control module is failing intermittently. The vehicle is available for inspection upon request. The issue is a known manufacturer defect with an established repair; however, the manufacturer states that replacement ABS modules are currently unavailable due to parts backorder, with no repair timeline provided. The issue has been acknowledged by the manufacturer and dealer. No inspection by police or insurance has occurred. Based on available information, my best assessment is an intermittent ABS module failure that renders the vehicle effectively unusable. On December 22, 2025, I purchased a brand-new Chevrolet Blazer EV SS from a dealership located approximately 1 hour and 10 minutes away from my home. Immediately after leaving the dealership lot, I noticed a grinding / grunting noise coming from the vehicle. This was not a minor rattle or normal “new car” sound—this was an alarming mechanical noise in a vehicle that had just been sold to me
... as new. Because the selling dealer is so far away, and because I was concerned about driving a potentially unsafe vehicle long distance, I took it to a closer Chevrolet dealer for evaluation. I first brought the vehicle to Ciocca Chevrolet in West Chester, PA. After assessing the issue, they advised me to open a ticket with General Motors (GM) because they believed there was a legitimate defect that required GM involvement. Shortly after that, I left for the Christmas holidays, but the issue did not go away and I remained extremely concerned that I had just bought a defective vehicle. When I returned, I took the Blazer EV to Del Chevrolet in Paoli, PA for a second opinion and a thorough evaluation. After keeping the car and spending an entire day assessing it, Del Chevrolet acknowledged the problem. Despite two separate dealers identifying and acknowledging the issue, I have been unable to get timely action toward a repair. I have been told that GM is requiring additional testing and, most importantly, that GM is not authorizing Del Chevrolet to order the necessary parts. The dealership has indicated they are essentially stuck waiting for GM approval, which means my brand-new vehicle is in limbo with no clear repair timeline. As of now, the vehicle has only about 140 miles on it. This is a brand-new car that developed a serious noise immediately upon delivery, and I have now been forced into a loaner vehicle. It is Jan 17th. The vehicle is still at Del Chevrolet service center. Del Chevrolet, Fred Beans Chevrolet and GM corporate are involved in resolving this issue.Read more

Safety risk Degraded rear braking performance increases stopping distanc

Confirmation or reproduction of the problem The issue has been confirmed by a dealership service department, which recommended replacement of the rear rotors and rear brake pads. The vehicle has approximately 16,000 miles and is just over 4 years old. During the service visit, dealership staff stated that this type of rear brake wear occurs frequently with these vehicles. The dealership also indicated that Volkswagen previously replaced rear rotors and pads under warranty for similar issues at approximately 24,000 miles or 2 years, but that this coverage is no longer available. Because this vehicle was driven significantly less than average, it did not reach those mileage thresholds within that earlier timeframe. Inspections performed The vehicle has been inspected by a dealership service center. It has not been inspected by the manufacturer, police, or insurance representatives at this time. Additional details / driving conditions The vehicle has been driven very infrequently, with low-mileage trips on flat roads in Charleston, South Carolina. There has been no aggressive driving, towing, mountainous driving, or heavy stop-and-go use. The front brakes remain in normal condition. Given the low mileage, light usage, and prior warranty replacements for similar conditions, rear rotor and pad replacement at this mileage appears premature and abnormal.Read more

Since we bought the vehicle new in 2022, it has had an ongoing brake noise at low speeds. It sounds like a screeching or grinding, and I only hear it when I press the brake pedal. It is most noticeable right after startup when driving slowly. The main issue I am reporting is uneven rear brake pad wear, which may indicate the rear brake system is not applying evenly or not functioning correctly. T
... he component involved is the rear brake system (rear brake pads and related parts). The vehicle is available for inspection upon request. A Volkswagen dealership confirmed an uneven wear condition. On 12/31/2025, the dealership performed an inspection and recorded that the left rear brake pad measured 3 mm while the right rear measured 8 mm. They recommended replacing the rear brake pads but did not diagnose or repair the cause of why one rear side is wearing much faster than the other. In prior visits, the dealership stated the noise was related to the type/quality of brake pads used. When I contacted the dealership again about the ongoing noise and the uneven wear, they stated they did not have a solution to correct the underlying problem and referred me to Volkswagen Customer Care, indicating they cannot get the brakes functioning beyond replacing pads. Volkswagen Customer Care then advised me I would need to work through the dealership. I believe this creates a safety risk because uneven rear brake wear may mean the brakes are not working the same on both sides, which could affect stopping distance or braking stability, and could lead to brake problems if one side wears out much sooner than the other. There have been no warning lights or messages. The symptoms began soon after purchase in 2022 and have continued over time.Read more

Vehicle: 2022 Ford Mustang Mach-E Mileage at failure: ~36,800 miles Complaint Description: The left rear electronic parking brake (EPB) wiring harness/connector failed, resulting in a parking brake fault and abnormal braking behavior. This component is not a wear-and-tear item and is located in a fixed, protected area of the vehicle. It is not subject to routine maintenance or driver interaction
... . The failed wiring harness was inspected by an authorized Ford dealership and is available for further inspection upon request. This failure put safety at risk because the electronic parking brake is a safety-critical braking system. The fault created uncertainty regarding proper engagement and release of the parking brake, increasing the risk of unintended vehicle movement when parked and reduced confidence in braking reliability while driving. The problem was confirmed and diagnosed by a Ford dealership, which determined the wiring failure developed over time due to moisture intrusion and corrosion, not misuse, accident damage, or normal wear. The vehicle and component were inspected by an authorized Ford dealership. Ford Motor Company reviewed the repair documentation through Ford Customer Care but did not dispute the diagnosis. The component has not been inspected by police or insurance representatives. A parking brake fault warning message appeared shortly before diagnosis, accompanied by abnormal braking behavior. There were no prior accidents, vehicle modifications, or external damage related to the failure. This failure is identical to a known defect already acknowledged by Ford in Customer Satisfaction Program 22P31, which covers EPB wiring harness connector failures on other 2021–2022 Mustang Mach-E vehicles. Although my VIN was excluded, the same non-wear safety defect occurred within the same model year, indicating a broader safety concern beyond the original VIN population.Read more

Incident date: Apr 18, 2023
Over the past two days, the vehicle has intermittently displayed an Electronic Stability Control (ESC) system error message while driving. At the same time, the brake pedal becomes extremely hard to press, as if the brake booster or power assist is not functioning. When this occurs, it requires significantly increased force to slow or stop the vehicle, creating a serious safety hazard. The issue a

I purchased a vehicle a year ago whenever my vehicle was still under the tenure and the 10 year and it’s still under 150,000 mile range. My car is very dangerous and doesn’t have any brakes and it’s still very new from the dealership. What it’s very interesting is that it’s not throwing any codes at all, though it beeps the entire time that I’m in the vehicle with the ABS light on and I know that it’s that problem why is it not showing any codes I think that for sure, the dealership would’ve had to done something to prevent that in order to sell the car unsafelyRead more

Description On 12-15-2025, in 18°F weather, the vehicle was parked on a residential sloped driveway with the manual parking brake fully engaged. While the driver was outside the vehicle, the parking brake system failed to hold the car’s weight. The vehicle rolled backward, crashed through a wooden railing, teetering on a 4-foot retaining wall, and came to rest over a culvert. Two young children
... were inside the vehicle at the time of the failure. Component Failure & Availability: The manual parking brake system failed to provide sufficient holding force to secure the vehicle. The rear brake calipers are currently available for inspection. Safety Risk: The safety of the two children inside the vehicle was put at extreme risk due to the unexpected movement of the car. This failure matches the exact conditions and consequences described in Safety Recall 1217F (NHTSA Campaign 17V-393), which states that "unexpected movement of the vehicle if parked on a slope" increases the risk of a collision. Reproducibility & Confirmation: The failure occurred under the specific environmental triggers (cold temperature and slope) identified in the manufacturer's own safety recall documentation. However, the manufacturer has officially refused to authorize the internal teardown of the actuator shafts—the only diagnostic method capable of verifying the rust and sliding resistance that causes this loss of holding force. Inspections: The vehicle has been visually inspected by an authorized dealership and insurance representatives. The manufacturer has declined the necessary internal mechanical diagnostics to verify the internal defect. Warning Symptoms: There were no warning lamps, messages, or symptoms prior to the failure. The parking brake felt fully engaged at the time the vehicle was parked.Read more

On November 12, 2025 the transmission, power train, and/or other components on my Porsche Macan S failed. My Macan was parked (gear selector in "Park") on a level surface, the ignition turned off and the key removed from the ignition when it was tapped from behind by another vehicle that was exiting a residential garage. The Macan proceeded to roll over two hundred feet from the impact, jumped tw
... o curbs, crossed a street with active traffic, and finally stopped when it hit a tree. There was extensive damage to my vehicle but fortunately there were no injuries because the Macan was unmanned at the time of impact. If there were pedestrians and/or other vehicles crossing the path of the Macan they would have been hit. There were no warning signs prior to the failure. This incident has been reported to Porsche Cars North America, Inc. ("PCNA"). I requested an inspection of my vehicle at an authorized Porsche repair dealership but was told: "PCNA has determined that our involvement would not be required in this matter therefore, we are unable to investigate it further". PCNA has disavowed any responsibility for the manufacturer's defect that endangers other drivers, pedestrians and me, given the risk of reoccurrence.Read more

Vehicle brake pedal is not fully depressing to normal state after use. This is causing the break lights to remain on until you pull the pedal itself back to resting state. If you do not do this the cruise control will not work. The ESC error light and Service Trailer Brake System error continue to come on.
