On January 28, 2026, I brought my vehicle to Kia of Fayetteville for brake pads, rotor replacement, and brake fluid service. I paid approximately $900 for these services and was provided a dealership loaner vehicle. Upon entering the loaner vehicle, I discovered a loaded firearm inside the vehicle as I was gathering my items. The firearm’s serial numbers appeared to be scratched off. Law enforce
... ment was immediately notified. No apology, acknowledgment, or remedial action was taken by dealership management. During this incident, the service manager, Alex Prat, acted in an aggressive, disrespectful, and unprofessional manner. Shortly after retrieving my vehicle, my husband who was driving the vehicle experienced a complete loss of braking ability. The vehicle was towed to another authorized Kia dealership, Southern Pines Kia, where technicians documented that multiple brake system components were improperly installed and/or unsecured, including missing or improperly fastened hardware. The vehicle was deemed unsafe to operate.Read more

During the last two years with my 2017 Ford F150 XLT 4X4 5.4L, I've replaced the brake pads 8 times. I've also replaced the rotors on all 4 wheels. Each time, with less than 30,000 miles between needing to replace the pads, the front pads have worn down the entire pad at an unreasonable rate. I do not brake hard, and I drive mostly on highways without much traffic, so having to replace the pads ev
... ery other month is excessive. With new rotors, still the pads continue wearing down to plate quickly. I've also had to replace the calipers on each wheel more than once because the caliper pins seize and will break when trying to adjust the caliper. The brakes and all the components are a major safety hazard as I'm always worrying if the brakes will function properly any give time I need to slow down. Also, the transmission shifts roughly and even downshifts without cause on occasion. It startles me when I'm driving and suddenly the transmission decides it wants to shift with a violent jolt as though briskly stepping on the brakes. I've had the transmission fluid changed twice in the last 2 years, which did not have any effect on the issue. Prior to each flush, the transmission has also developed a significant leak, which I led to the decision to flush the fluid and replace the pan and gasket. However, after doing so, the transmission continues downshifting and leaking. Once again, these issues have a significant impact on how safe I feel while driving my truck. I love my truck, but I hope my complaint helps Ford fix these little big issues and can develop a fix for future models.Read more

I was unaware that when I purchased this vehicle that when the oil is low, the breaks go out and the break power booster shatters. The beaks have gone out on me 2x already and thankfully I’ve only had to replace the break power booster only once. Now once the oil is changed the breaks go back to normal, there is no grinding of any sort heard, so now going forward I pay real close attention to my
... engine, I listen and pay attention to the feeling and sound of the breaks, once the oil begins to become low knowing it’s time for an oil change around 40% is when I start to feel a bit of the difference in the breaks and I can feel a little bit of grinding, no oil light has come on at this time nor has the oil light or any type of signal has ever come on at anytime to let me know that it was time for an oil change. I’ve checked for recalls, I’ve checked message boards, mechanics have no clue about this problem, but at least my mechanic does now!Read more

SEE NHTSA Safety Recall Report 25V-12- It appears it should apply to both 2025 FRONT WHEEL DRIVE Chevy Equinox EVs with ADAPTIVE CRUISE CONTROL (and without Super Cruise) in addition to 2025 ALL WHEEL DRIVE Equinox EVs with ACC (also without Super Cruise). This is very dangerous. Here's what happened: My 8 month old 2025 Chevrolet Equinox EV FRONT WHEEL DRIVE w/ACC(no Super Cruise) has had 2 insta
... nces where a very large "thunk" sound and vibration emanating from the rear of the car while stopped at an intersection, was immediately followed with sudden brake failure and sudden forward acceleration. The ACC was on in both instances. The first time this happened was alarming but since the was nothing in front of me there was no damage and the problem seemed to resolve itself thereafter. The second time was while waiting at a red light about 10 feet behind another vehicle. My car shot forward and into the other car, which was then pushed into the car ahead of it. Both my car and the car ahead of me had very expensive damages, but fortunately, no injuries. What I experienced sounds exactly like the recall problem described in NHTSA Safety Recall Report 25V-12 which has thus far only been applied to the AWD version of the 2025 Equinox EV, not the FWD version that I have. I believe this needs rapid consideration due to the danger of of sudden acceleration resulting in serious property damage, injury, or death. It appears to me that other instances of this problem have been described in various online forums and YouTubes.Read more

On the morning of January 28, 2026 I arrived to work and came to a complete stop in my works parking garage. I used my key card to raise the gate. Once the gate was fully raised I pressed the accelerator to drive up the ramp. Shortly after accelerating, my engine revved really loud and high rate, then my car automatically increased speed. I immediately pressed the brake pedal but it didn’t stop.
... It continued accelerating at a speed not controlled by me with my brakes not engaging even though I was pressing the brake as hard as could. I estimate I travel about 100’ with no control of the gas or brake. It lasted about 15-20 seconds. If a car were in front of me at that time, I would have crashed into them because I could not stop the car. I immediately called Davis Moore Mazda. I had the vehicle towed there because I didn’t feel safe driving it. Information from my cars computer program was sent to Mazda. Mazda didn’t see any issues and their diagnosis was a foot was likely on the accelerator causing the acceleration. The general manager called me to discuss. He said the only thing that registered was acceleration. I asked does the computer show my attempt to brake (stop) the car. His response was the computer wouldn’t read that because that was a normal driving process that wouldn’t be recorded. I asked why the acceleration would trigger an alert in the computer, since acceleration is also a normal driving process. He then said the computer picked it up because it accelerated quickly. There were no warnings present prior to this issue.Read more

I am reporting a safety-related defect involving unintended vehicle movement on my Hyundai Ioniq 5 (Model Year 2023, SEL AWD). Observed behavior: With the vehicle in Drive and the Auto Hold function active (indicated by the white “AUTO HOLD” indicator in the instrument cluster), I brought the vehicle to a complete stop by pressing the brake pedal. Auto Hold engaged (text now green), the vehic
... le remained stationary, and I removed my foot from the brake. After approximately three minutes of remaining stopped, a driver-assistance system error message appeared on the instrument cluster. At that point, Auto Hold disengaged (the “AUTO HOLD” indicator disappeared), and the vehicle began to creep forward on its own. I did not command acceleration, did not press the accelerator pedal, and did not intentionally release the brake. The movement was unexpected and created a safety risk. The first occurrence happened in a school drop-off line with the vehicle stopped near other vehicles and pedestrians. I have since reproduced the behavior and captured it on video. Related observations: Similar driver-assistance system errors have occurred while the vehicle was in Park and in Drive, with or without Auto Hold active. In those cases, the vehicle did not move because it was either mechanically locked or I had my foot on the brake. In all cases, Auto Hold deactivated. Safety concern: This behavior raises concerns about the reliability of Auto Hold and its interaction with braking and driver-assistance systems, particularly after the vehicle has been stationary for an extended period. Unintended forward movement presents a significant safety hazard. I am reporting this issue for investigation of potential defects in software, sensors, or control logic. Video evidence is available.Read more

On 12/18/2025 at approximately 73,097 miles, while driving on I-75 near Wildwood, Florida at about 75 mph, the vehicle experienced a sudden powertrain failure. The engine became seized/locked, the vehicle progressively lost power, and there were abnormal/loud mechanical noises. I was able to avoid a crash, but the event created a safety risk due to loss of propulsion at interstate speed. No accide

Dealer paperwork indicates completion on 01/07/2026, and the vehicle was returned to me on 02/02/2026. After the engine replacement, the vehicle displays safety-related warnings including: “Service Transmission Now – Unable to Shift Soon” “Service Electric Parking Brake” Because of these warnings, I am not driving the vehicle. The dealer plans to tow the vehicle in for inspection. In addition, the dealership has stated in writing that they typically clear diagnostic codes for test driving and that no post-repair/post-road-test diagnostic scan reports are saved or printed, meaning there may be limited retained documentation verifying that all modules were fault-free at release after the engine replacement. Component/System: Engine cooling and lubrication system. Coolant was found mixing with engine oil (milky oil), indicating internal engine failure (I was informed it could be oil cooler/housing, thermostat, head gasket, or related components). Vehicle is currently at an authorized Mercedes-Benz dealership and available for inspection. The front left inner lining of my front break pad also "failed"
... inspection and the other 3 front break pads say "caution" and I never received a notification or warning about my breaks. Safety Risk: Coolant contamination of engine oil can cause sudden engine failure or loss of power without warning, creating a serious safety risk while driving, especially at highway speeds. There was no indication that engine oil was mixing with coolant, check engine light failed to illuminate/ no diagnostic warning illuminated. The condition was discovered incidentally during routine maintenance, which prevented any opportunity for corrective action prior to the failure. Confirmation: The condition was confirmed by an authorized Mercedes-Benz dealership during routine maintenance. The dealership stated the failure was not caused by owner misuse.Inspection: Vehicle has been inspected by an authorized Mercedes-Benz dealership (manufacturer representative). No police or insurance inspection occurred, as there was no accident or external event. Warnings/Symptoms: No check engine light or diagnostic warning ever illuminated. The issue was discovered incidentally after I received a low battery voltage notification and a routine maintenance reminder indicating service was due in approximately 1,000 miles, which I arranged promptly. The manufacturer will not perform full diagnostic teardown to determine the root cause unless I pay approximately $3,000 out of pocket. On November 26, 2025, while traveling in stop-and-go freeway traffic, I experienced a severe braking failure that led to a rear-end collision. Despite applying maximum force to the brake pedal, the vehicle did not decelerate as expected. Objective technical evidence from the Bosch CDR report (Case #1105768) confirms a critical system failure: Extreme Driver Input: At -0.5 seconds before impact, D
... river Applied Brake Pedal Pressure reached 10,935 kPa, representing a near-maximum emergency braking effort. Inadequate Response: Despite this peak pressure, the Vehicle Indicated Speed decreased only from 24 MPH to 22 MPH in the final 0.5-second interval. Data Discrepancy: The vehicle decelerated by 12 MPH (from 36 to 24 MPH) between -1.0s and -0.5s under lower pressure (7,695 kPa). However, when brake pressure was increased to its peak at -0.5s, deceleration effectively stalled. Electronic Anomaly: The Traction Control System (TCS) was reported as "Active" throughout the braking event, indicating potential improper electronic intervention. This vehicle has a history of significant electrical failures, including a total power loss on 10/15/2024 that required a high-voltage battery replacement. The failure of the braking system to respond to maximum driver input poses a catastrophic safety risk. I am requesting an investigation into the 2024 Cadillac LYRIQ's brake-by-wire and electronic stability control integration.Read more

The incident involved a 2025 Chevrolet Trailblazer, purchased new in late February 2025. ... est. While driving in Cambria, New York, the dashboard illuminated with multiple warning indicators and the vehicle experienced a complete loss of power steering and braking. The brake pedal traveled fully to the floor with no resistance and no braking effect. Steering effort increased dramatically. The driver was only able to avoid a crash due to favorable road and traffic conditions and was able to coast to a stop and pull off the roadway. This failure posed an immediate and serious risk of injury or death to the driver and others. The problem had occurred previously in the form of repeated warning messages. Beginning within the first week of ownership, the vehicle displayed “Service the Charge System Vehicle May Stall” warnings and since then has appeared hundreds of times. The vehicle was previously towed to an authorized Chevrolet dealership, where we were informed that General Motors identified the issue as a known software-related problem and advised that the vehicle could continue to be driven while awaiting a future update. The catastrophic failure occurred after this guidance. The vehicle has been inspected by an authorized Chevrolet dealership both prior to and following this incident, and General Motors has been involved through dealership communication. No police or insurance inspection has occurred, as no collision resulted. Based on the repeated warnings, dealer statements, and nature of the failure, the best assessment is an electrical or software-related malfunction, though the precise root cause remains UNKNOWN and requires investigation.Read more

It went to the local Chrysler Dealership; they replaced the Master Cylinder. I picked it up 12/10 drove it 5 miles home and parked it. I got in the vehicle the next morning to go to work and on my way to work all the same dash lights came on and I had a complete loss of Brake function. It was towed back to the dealership again; this time they replaced the ABS module. I picked it up on 12/30 drove 5 miles home, parked it, tried to leave 3 hours later and couldn't push the brake pedal far enough to get the van started. When I did finally get it started all the same dash lights were on and the brakes were not functional. It was towed back to the dealership again and this time they replaced the wiring Harness. I picked it up 1/16. Drove it minimally over the weekend, on 1/19 the front-end collision warning lights came on and the check engine light came on while driving 70 mph. The brakes didn't lose function this time, however. It triggers to say someone or a vehicle is approaching from the side, but there is no one or nothing there. The issue that is most troubling is I almost hit someone in the rear end on 02-03-2026. I went to slow down for a traffic light, and it started slowing down, but then got a surge and would not allow me to stop any more. It kept going. I had to remove my foot from the brake and hit it again harder. It has happened a few times, but not in a situation like this. Thankfully I was following a safe distance or I may have hit the vehicle stopped in front of me. Similar situation happened a few times, one when I was pulling into an Applebees to eat, the brake did not want to stop the vehicle. It was like a surge of power again. This time was not as bad as no one or a vehicle was not in front of me. I have also noticed it a few times driving and when I go to stop the surge, but it seems to be escalating. I wasn't aware of this mechanism, and only learned of it after posting on numerous social media apps regarding this issue. This is a Major issue, as we are talking about a moving and unstoppable car! Tesla needs to build some mechanism that allows the car to be shut off by the driver or to override the screen and place the car in park. Pulling this firemans loop and unplugging the 12v battery, required me being in front of the vehicle, risking my life, as the vehicle could've easily moved if the concrete barriers didn't stop it, and also required me taking 30+ minutes to remove components from the Frunk, just to access these components!!Read more

Reporting multiple safety-related defects on my 2024 Mazda CX-90. Steering system: At approximately 5,000 miles, the vehicle developed abnormal steering behavior, including inconsistent steering response and abnormal feel during normal driving and turning. The condition affected my ability to steer the vehicle predictably and raised safety concerns, particularly during low-speed turns, parking ma
... neuvers, and city driving. The issue was confirmed by the dealer and required replacement of the steering rack, a major steering component. The vehicle remains available for inspection upon request. Hybrid system: At approximately 22,000 miles, the vehicle experienced a hybrid system malfunction that required the vehicle to be taken out of service for an extended period. The malfunction affected drivability and reliability and required dealer diagnosis and repair. The vehicle was inspected and repaired by an authorized Mazda dealer. Braking system: The vehicle has experienced ongoing abnormal brake squeaking during normal driving and low-speed braking. While braking function remains present, the abnormal noise raises concerns about brake system integrity, wear, and long-term braking performance. Electrical / infotainment system: The infotainment system intermittently shuts off or resets while driving without warning. When this occurs, the display goes blank and then restarts. This results in loss of access to navigation, rear camera display, vehicle settings, and driver-assistance information, creating driver distraction and a potential safety risk. These issues occurred on a relatively new vehicle and involve critical systems including steering, drivetrain/hybrid, brakes, and vehicle electronics. The combination of defects raises concerns about vehicle safety, reliability, and loss of driver confidence. Warning messages were present for the hybrid malfunction; other issues occurred without warning. The vehicle has been inspected and repaired by authorized Mazda dealRead more

The one pedal driving / regenerative braking would randomly deactivate. I have my car set on high one pedal driving. 99.9% of the time, the one pedal driving would work as expected with the car slowing down as the accelerator is released. However, the car would randomly deactivate one pedal driving, so when I release the accelerator, instead of slowing down or stopping, the car would continue goin
... g, some times it feels like there's even a slight surge. This has happened several times at slow parking lot speed and at fast highway speed. There's no warning. It's just random. It hasn't been inspected by the dealer yet.Read more

ABS module failed and showed the warning lights. It happened ? years ago and I took it to dealership but still can not get the part to have repaired. This has affected the anti lock brakes, traction control and disables the cruise control. I have lost control coming to a stop light during a rain but luckily didn't hit anything during the slide. Hard to believe that they don't have a fix for this y

Collision warning randomly activates when driving while no vehicles or objects are around. This has been ongoing, but this last time emergency braking activated extremely hard and would had caused a collision if there was a vehicle following as vehicle almost came to a complete stop on a 75 mph highway.

The vehicle experienced intermittent electrical failures, including screen blackouts/reboots during startup and while driving. These events affected visibility of vehicle information and settings. The drive mode selector malfunctioned, limiting the ability to select Eco, Normal, Sport, or Off-Road modes. Drive modes directly affect throttle response, braking behavior, and vehicle control. On mul
... tiple occasions, braking and acceleration behavior felt inconsistent with selected drive mode, creating safety concerns. The issue is intermittent and was reported to an authorized dealer. The dealer was unable to consistently reproduce the issue but did not dispute that the concern was reported. The vehicle remains available for inspection.Read more
