On November 18, 2025, I was driving a 2025 BMW i4 eDrive, VIN [VIN], at approximately 10–15 mph in slow traffic. I applied the brake pedal firmly when the vehicle in front of me slowed, but the vehicle did not decelerate as expected. The vehicle continued moving forward, resulting in a rear-end collision with the car in front of me, which in turn struck another vehicle. Prior to this incident,
... the vehicle occasionally exhibited brief reductions in forward drive power that were subtle and initially attributed to drive mode (e.g., comfort/eco or battery status). At the time of the incident, the vehicle was subject to Safety Recall 25V-395, but I had not received any recall notification prior to reporting the incident to BMW. This appears to be a potential vehicle safety defect involving braking performance or interaction of propulsion and braking systems, as the vehicle did not stop when brake input was applied under normal conditions. I am reporting this to NHTSA for investigation of a possible safety defect. While driving the car just shut-off in the middle of the road. If you tried to restart the car it would turn on rev the engine and shut off. The dash display displayed a message " precollision system malfunction" and "secondary brake collision malfunction". Safety wise if this had happened on the highway could have been very dangerous or worse. The car was inoperable and required a tow truck. Luck

... garage in Newark, DE- Littles Garage told me that their system said there was a recall on my vehicle and that I'd need to have service done at a Chevrolet Dealership at no cost. I went to 2 Dealerships who stated my Truck's VIN did not show up in their system as having any active recalls. Reading the Recall notice, I see that GM contacted the Dealerships directly (I guess not to make the recall public) who have/had sold this vehicle with instructions to contact owners directly; however, I purchased the Vehicle in SC from a Ford Dealership in 2017. Since RECALL # N192261050 is stated as a safety problem that could cause my truck to be a dangerous safety hazard that could cause serious damage and even death to occupants of the vehicle. I understand from my garage that the issue is intermittent once discovered, even as I have not experienced anything for about a week, and it can re-appear at any time. You can understand that I need the NHTSA to help me resolve this issue and if I am deserving of a Recall Fix, to help me with Dealerships in DE. NuCar Chevrolet of New Castle, DE has been recommended to me by Littles Garage as one of the best Chevy Dealerships. NOTE I am visiting DE and need to get back to Virginia shortly (I'm hesitant to drive this vehicle at highway speeds). Chrysler/Stellantis has acknowledged this brake booster defect by issuing a warranty extension; however, they have not issued a safety recall. I was denied repair coverage solely because my vehicle is now outside the extended warranty period, despite the fact that the defect is known and directly affects braking performance and vehicle control. This is not normal wear and tear. It is a failure of the brake assist system that compromises safe operation of the vehicle, whether driving at speed or stopped in traffic. The condition presents a clear safety risk to occupants and others on the road. I am submitting this complaint so NHTSA can investigate whether this defect warrants a formal safety recall.Read more

Sudden and unexpected loss of braking ability significantly increases the risk of a collision, serious injury, or death to the driver, passengers, pedestrians, and other motorists. The vehicle was deemed unsafe to operate due to the risk of brake failure. The problem has been confirmed by the authorized Chevrolet dealership. The service department diagnosed the issue as a defective master brake cylinder and/or related brake system components. The dealer advised that the condition presents a safety risk and that the vehicle cannot be safely operated until the issue is addressed. The vehicle has been inspected by an authorized Chevrolet dealership acting as the manufacturer’s representative. The manufacturer has been made aware of the problem. No inspection by police or an insurance representative has occurred, as no crash took place. The vehicle remains unrepaired as of February 9 due to parts unavailability. There was no adequate warning that would have allowed the driver to anticipate the loss of braking performance. The defect results in sudden brake failure without sufficient advance warning, which is a significant safety concern. The issue was identified in January 2026, at which time the vehicle was taken to the dealership. On January 13, 2026, the vehicle was removed from service and has remained at the dealership continuously since that date. It was snowing and I was driving slow. About 8:40am, Feb 6th, 2026, I got a "One pedal driving disabled" alert just for a moment, then tried to stop before an intersection because a signal was turning to red. However, break didn't work and I had to enter the intersection under a red signal. Fortunately, there was no car in front of me, also a car coming from the side didn't notice signal changed.

I was informed that only a “temporary fix” could be performed, and I was required to pay approximately $500 for this repair in order to regain use of the vehicle. The vehicle has not received a permanent repair, and the dealership has taken several months without completing a permanent fix. I have also been waiting several months for reimbursement of the almost $500 paid for the temporary repair, with no resolution. I am concerned that I was charged for a temporary repair related to a serious safety failure and that continued delays in both reimbursement and permanent repair place me at ongoing risk. I am filing this complaint due to the dangerous nature of the brake failure, the lack of a permanent repair, the extended delay in resolving a critical safety issue, and the financial burden placed on me for a temporary solution.Read more

I had stopped uphill, with my foot on the brake, heading East on the left lane of Pomona Road, at the approach to the Traffic Light Junction with Route 45, around 3.50 PM on Friday 1/23/2026. There were about 5-6 cars also stopped, ahead of me, at the Red Traffic Light and several cars behind me, when I heard my car engine rev up loudly. I kept applying the brake. ... ng with a smell of burning rubber from the tires as they spun on the road, as my car lurched forward, and hit the rear of the stopped car ~ 5-7 feet ahead of me. I felt that I had no control of my car and turned the engine off after the collision. I called 911 twice at 3.55 PM and 4.12 PM and waited as advised, for the Police Officer who arrived about 30 mins later. I filed the report and the officer was able to retrieve my auto license plate which had fallen off from the road where I could clearly see the tire marks on the asphalt from the spinning tires at the time of the accident. The only message I recall receiving on the display was one that informed me that I had experienced a crash after the collision. I had never experienced this problem before. Later ,after the Police Officer had arrived and documented the accident, I was able to drive the car home. The burning smell from the tires persisted for some time. The next day I called the BMW dealer in the AM, and with great trepidation, drove the car to the BMW dealership. I explained to the Service Advisor on duty on the Saturday, what exactly had transpired during the accident. I also explained this later to the BMW Body Shop manager when I spoke with him. I requested a full inspection and investigation by BMW. I personally do not feel safe driving the car again after this most unnerving and frightening episode.Read more

-I had just exited from the highway and was driving into my town. While doing so, I had both hands on the steering wheel and suddenly the car started beeping and flashing warnings/ alerts indicating that my hands were not on the steering wheel, which they were. Within seconds the car began to deaccelerate and pull me off the road to my right hand side. I initially had no control over the vehicle.
... Then again- without explanation it began to register hands on the steering wheel and I was able to resume control of the vehicle. I believe the lane assist feature was on at the time. It all happened very quickly. Had I still been on the highway as I was moments before, there was enough traffic that I would have been pulled into another vehicle or caused an accident if people saw me suddenly rapidly deaccelerating/braking in front of them because they would have no warning and a very tiny amount of time to react. Then, in an other instance that followed shortly thereafter, my boyfriend was driving with one of the lane assist on. While driving, the weather changed and it began snowing and freezing rain that was freezing when it hit the windshield. The lane assist began to have trouble seeing the lines on the road since the snow and ice were obscuring them somewhat. It began to beep and gently correct the car- which was not appropriate in this case (we understand that the road conditions were beyond its abilities to safely discern things accurately). My boyfriend who had two hands on the wheel trying to carefully navigate this sudden burst of weather so at not to slide. He tried turning it off, but it wasn't turning off, and couldn't troubleshoot it without taking his hands off the wheel. He instructed me to turn it off through the settings menu in the infotainment system, which seemed to disable it. Prior to these dangerous problems, I had experienced some seemingly sensor- related malfunctions with the tire pressure monitoring system. I'm out of room to write more.Read more

I am aware of some Jeep owners who have left their Jeeps at a Jeep dealership for over a year, waiting for a replacement part! In some states, such as Hawaii, where my Jeep is located, an annual safety inspection must be passed to drive the car on Hawaii's roads. With ABS and traction control warning lights, my Jeep won't pass. At this point, I cannot drive my Jeep legally or safely. The NSHTA has been made aware of the safety issues of Jeep Wranglers driving with faulty ABS modules or, in some cases, disconnected ABS systems. The vehicle has been brought to an authorized dealership for numerous service visits related to electrical system issues that ultimately affected the braking system. During one service visit, the vehicle was returned to me with multiple active dashboard warning lights illuminated related to the braking system, including Electronic Stability Control and Automatic Emergency Braking. ... e presence of these warning at pickup and was advised by the dealer that the vehicle was safe to drive and that the warnings would clear on their own. After driving the vehicle for a short period, I became uncomfortable continuing to operate it with brake-related warning lights still active and returned it to the dealer. Upon further diagnosis by a service technician, it was discovered that the vehicle has significant brake system defects requiring replacement of major components, including the brake control module and master cylinder. During diagnosis, the brakes locked and the parking brake would not disengage, rendering the vehicle undrivable. No collision or injury occurred. However, the vehicle was released to me with active brake system warnings and later found to have serious brake defects. Due to the safety-critical nature of the braking system and the repeated electrical and brake-related service history, I am reporting this for documentation and review.Read more

The car came in with the mental to metal break wear on the driver's side, while the passenger's side brake pads still at 50% life. On 01/16/2026, Quirk Chevrolet completed product safety recall N242482680 for the Brake Pressure Switch Connector Fire on my vehicle. Shortly after, my vehicle would come to an abrupt stop on its own while driving. This happened twice on my way home and then the third time at home at which point, it would no longer move forward or backward. The vehicle needed to be towed to the dealership that
... did the recall. After researching online, I discovered this is a known issue after the recall because the recall component is faulty. One source reports the cause is an improperly manufactured jumper harness which is missing protective tape or a faulty internal C-clip in the cylinder. Another source states the post-service emergency brake issues often stem from a software update meant to enhance safety or an incomplete repair of the brake pressure switch wiring harness. The update automatically engages the parking brake. I absolutely know that the catastrophic failure of my vehicle's braking system was a direct result of the recall work; however, the dealership made me pay for the repairs out of pocket until such time as GM or Chevy makes an "official" recall on the faulty recall. My safety and the safety of others was put at risk by way of my vehicle stopping abruptly on its own. The problem has been reproduced by many sources online - just do an online search "2021 Chevy 5500 emergency brake issue after brake recall completed" and see all the results that come up corroborating my personal experience. Quirks Chevrolet in Manchester, NH inspected the vehicle. No warning lamps or messages appeared on the dash. I have been driving the car in normal condition in southern ca. It seems that the break pads were defective and degraded at an alarming/unsafe rate or that a different component on the car caused them the continuously rub and wear down. The dealership blamed it on owner operation even though they supposedly checked them 2 months before and didn't see abnormal signs of wear. I am very concerned with the QUALITY, RELIABILITY & SAFETY of the front breaking system due to the fact that the breaks wore out so fast. I am worried that the problem has not been properly identified and that that breaks will continue to have this problem if it wasn't caused by faulty pads. I have never had a break wear problem like this before on any of the other 5 new cars that I have owned. Highly variable brake biting point 4. Requirement to pump the brake pedal to obtain normal braking 5. Unpredictable brake engagement during normal driving These symptoms are intermittent and occur without warning, significantly increasing stopping distance and reducing driver confidence. The behavior is consistent with internal hydraulic or HECU malfunction, which the original recall attributes to corrosion caused by improper brake fluid formulation. I am particularly concerned because I am a disabled driver, and any reduction in braking reliability presents an elevated safety risk. In June 2019, while driving my 2018 Nissan Maxima, the vehicle suffered a sudden and violent mechanical failure. Without warning, the steering wheel jerked forcefully to the side, and the vehicle became uncomtrollabel leading to an approx 100-foot spin across a median.
