On Saturday, February 7th, at approximately 1:00 PM, I was involved in a high-impact rear-end collision while driving cautiously with Adaptive Cruise Control and driver assistance systems activated, maintaining the distance set by the vehicle itself. When the vehicles ahead of me suddenly collided, my Nissan Armada did not brake properly. I am not stating that the braking response was insufficien
... t — I am stating that the braking system failed to function correctly. Despite fully depressing the brake pedal, the vehicle did not decelerate in a normal or expected manner. Most importantly, there were no visible skid marks or braking marks on the pavement after the accident, which strongly indicates that the braking system did not engage as it should have. Any properly functioning vehicle under full brake application would normally leave visible braking evidence on the roadway. This did not occur. The impact was severe and resulted in extensive frontal damage, yet no airbags deployed, which further intensifies the concern regarding multiple safety system failures. This incident placed my life, the lives of my family members traveling with me, and the lives of others on the road at extreme risk. While there were no fatalities, there were minor injuries and significant emotional trauma, particularly for my [XXX] daughter, who is now fearful of riding in a vehicle. Component or system that malfunctioned: The braking system on my 2022 Toyota Highlander Hybrid, specifically the transition between regenerative braking and hydraulic friction braking, appears to malfunction intermittently. During braking, the vehicle sometimes lurches or surges forward instead of slowing smoothly, especially at low speeds or when braking downhill. The vehicle is available for ins
... pection upon request. How safety was put at risk: The unexpected forward surge while braking increases stopping distance and creates uncertainty about braking performance, particularly in traffic or parking situations. This creates a risk of rear-end or low-speed collisions and reduces my confidence in the braking system. Problem reproduced or confirmed by dealer or service center: The vehicle was inspected by a Toyota dealership, and a Technical Service Bulletin (TSB) software update intended to address braking behavior was performed at approximately 50,000 miles. However, the braking problem continues unchanged after the repair. I had to pay the dealer to perform this TSB update. The dealer was initially reluctant to apply the TSB and did not associate my braking complaint with the known issue. I had to research the TSB myself and request that it be applied. The invoice notes that the dealer reproduced the issue. Vehicle or component inspected by manufacturer or others: The vehicle has been inspected by a Toyota dealership service department. It has not been inspected by police or insurance representatives, as no accident has occurred. Warning lamps or prior symptoms: No warning lights or messages appear when the problem occurs. The issue happens intermittently without warning and has occurred since the vehicle was new. The condition appears to be getting progressively worse. The issue seems more likely to occur when braking while the hybrid battery is near or at full charge, suggesting a possible problem in the transition between regenerative and friction braking.Read more

To NHTSA, I am reporting a serious safety defect in my 2023 Chevrolet Tahoe purchased from Criswell Chevrolet (Gaithersburg, MD). Defects include a documented brake system failure while driving and repeated electrical/shift faults preventing shifting out of Park. The issue is ongoing even after the dealer/manufacturer stated it was fixed. Key events: Feb 3, ~12:45 pm: While traveling ~35–39 m

I have video of the incident and two other times this has happened with pictures as the car was in park. Three prior start-up incidents: All dash lights illuminated (including brake failure and Forward Collison System Unavailable); vehicle stuck in Park. First required towing; dealer could not replicate despite reported stored codes. Subsequent event stranded my family. I have notified Chevrolet and the dealer; their offer does not address safety or recurrence. Please record this complaint, assess broader risk, and advise on related recalls/investigations. On February 14th while operating the vehicle I experienced a braking failure where the brake pedal went to the floor and braking distance was increased. I had the vehicle towed to Ford of Columbus in Columbus, IN on the morning of Feb. 16th where it was confirmed that the rear brake hose had blown due to the associated recall. The failure is exactly due to the recall as acknowledged by both Ford M
... otor Company and the dealership. Ford Corporate acknowledged (case number XXX) that the repair should not be my financial responsibility yet they continue to demand that I pay out of pocket now for an interim repair because the "official" fix is not ready. By refusing to authorize a no cost repair for a vehicle that has already failed due to their recall, Ford is denying a safety remedy and forcing me to bear the financial burden of a known defect. I do not believe it is my responsibility to pay now and then have to jump through hoops to seek a refund at a "later date" determined by Ford. This is a deceptive and bad faith practice by Ford. I have called Ford Motor Company twice and have utilized the chat once. I have not heard back from the dealership in nearly 24 hours since asking for an update on the status of the repair that I have since approved. ABS Failure cause loss of power and ability to accelerate, dealer states part under engineering review for 2 years
Incident date: Jan 8, 2026
While towing 10,000 lb trailer vehicle displayed indicators that traction control and ABS were failing. I've taken vehicle to dealer 5 times since recall and it's now over 1 year and Chrysler failed to remedy recalled defective part 68143491AA. Chrysler complaint case no. 95672952 filed on 2/16/26. Chrysler HAS in fact produced the replacement part No. 68143491AB - supersedes failed part no.
... above. Local dealers have qty 9 of part locally in stock and sell to public for $500. After calling many parts dept sources, I learned dealers are lying and telling current owners no remedy available under recall so that Chrysler can prioritize replacing Ram trucks traded in to dealers so they can fix under recall and make those vehicles sellable by dealers as highest priority for parts. However, dealers are also willing to sell the new part to consumers at full msrp at parts counter + labor in service center. So they make eligible customers pay full price up front and fight to get reimbursed later after being out nearly $1000 to fix Chrysler's problem. I get that there may not be enough parts on hand for every recalled unit - yet. But there are enough now at dealers that when a truck comes in to dealer service center with proven HCU failure indicators (traction control and ABS warning lights) those customers should be given the recall part with labor as required of Chrysler. In my case, brake failure warning lights were documented 7 months ago at North Fort Worth Auto Nation dealer service center under invoice #187541. So, there is no excuse why I'm now being lied to and told they don't have new part 68143491AB when parts mgr told me himself they have 8 parts in stock at that dealership alone as of 2/16/26. NHTSA is allowing Chrysler to self-regulate on this without sufficient oversight. After a year plus since first recall notice there is NO excuse why current owners should be denied available parts to fix this known major safety issue.Read more

I have a 2022 Ford Maverick XL Hybrid and the system that failed is the rear lighting system, specifically the tail lamps, brake lamps, and rear turn signals. This appears to be related to the Body Control Module (BCM) software, which was the subject of Ford Recall 24S27 affecting many 2022–2024 Ford Mavericks. The failure began on my vehicle with the left rear tail lamp assembly (running light,
... brake light, and turn signal), and within approximately two minutes the right rear tail lamp assembly also stopped functioning. Since that time, the entire rear lighting system has been inoperative while driving. This issue creates an extremely serious safety hazard. While driving, my vehicle has no functioning rear running lights, brake lights, or turn signals. This means other drivers cannot see when I am braking, signaling a turn, or even driving at night. This significantly increases the risk of rear-end collisions or other traffic accidents, especially in low visibility conditions. I was unaware of the issue until visually noticing it when unlocking the vehicle, and there were no dashboard alerts to warn me prior to that either. Loveland Ford in Loveland, Colorado inspected the vehicle and confirmed that the rear lighting system is not functioning. However, instead of addressing the known Recall 24S27 Body Control Module software issue, they stated that LED bulbs were installed where halogen bulbs should be and issued an $816 estimate to replace the bulbs. I have never performed any aftermarket modifications, and the vehicle has only been serviced by Ford dealerships. The vehicle has been inspected by Loveland Ford, an authorized Ford dealership. Ford customer service confirmed that Recall 24S27 exists for this exact issue in many 2022–2024 Ford Mavericks but stated there is no record of my VIN receiving the recall remedy, which could mean the recall was never accomplished. No warning lamps, dashboard messages, or other alerts appeared prior to the failure.Read more

Incident date: Feb 13, 2026
The vehicle experienced repeated electrical wiring harness damage in the lighting, tire and connector areas due to exposed and insufficiently protected wiring near socket terminations. On two separate service visits, authorized dealership technicians confirmed that rodents chewed through multiple exposed wires. The affected wiring involves critical electrical systems. The connectors lack adequate
... shielding, loom, or protective insulation at exposed termination points. The insulation ends before the connector, leaving conductors vulnerable to environmental exposure and rodent access. Warning indicators illuminated on the dashboard, and the vehicle required dealer repair on both occasions. The damaged wiring was inspected and confirmed by certified technicians and is available for inspection upon request. This condition presents a significant safety risk. If a rodent partially damages a wire connected to braking systems, steering systems, high-voltage components, sensors, or engine control systems, failure could occur while the vehicle is in motion. A sudden loss of power, interruption of propulsion, or malfunction of safety systems at highway speeds could increase the risk of a crash. Because this is an electric vehicle, sudden electrical interruption may result in complete loss of propulsion or charging capability. If such failure occurs in remote areas, at night, or in extreme weather conditions, occupants could be left stranded in potentially hazardous situations. This issue has occurred more than once, and no permanent preventive solution has been provided. The design appears to leave critical wiring insufficiently protected from foreseeable environmental and rodent exposure, creating an ongoing safety hazard. I request review of whether the wiring design provides adequate protection for safety-related electrical systems.Read more
