It is my understanding Toyota had a service bulletin to fix the issue under T-SB-0046-25 which was issued in July 2025, however to date, Toyota has not repaired my vehicle. It is my understanding that some vehicles were repaired under the bulletin but the fix did not correct the problem. I was told by my local Toyota dealer this week that Toyota told them to stop all repairs because the TSB was not correcting the issue and they were waiting for Toyota engineers to find a new solution. Request your assistance in getting Toyota to repair all the vehicles affected by this problem before a serious accident occurs because of brake failure or another driver taking evasive action when they hear this noise.Read more

A message briefly appears on the vehicle information screen each time this inadvertent braking occurs. It reads, “Brake Failure System”. These brake lockdowns occurred when driving on streets and in the driveway. No accident resulted but could have easily occurred. This is dangerous for a driver and occupants of a vehicle with this safety flaw as well as for other people and vehicles in the vicinity. This has happened five times. The last two instances resulted in the vehicle being loaded onto a tow truck and taken to a dealer of the vehicle’s manufacture. ... 5 times. After the 4th incident, a new pop up warning appeared: "Collision Mitigation braking system problem. See your dealer. " I took a photo of this warning light and drove to the closest dealership. The unwarranted braking problem happened again while on the freeway. When I arrived at the dealership, at first they said "there's no problem. " I showed them the photo, they reluctantly checked in my vehicle for diagnostics and informed me that I would have to pay for the diagnostician. The safety of myself and other California drivers was put at risk by this malfunctioning braking system. I was unable to over ride a warning, the vehicle braked unexpectedly in the middle of the freeway. Mossy Honda dealership did not offer me a loaner vehicle nor a ride home. When I asked, I was told that they might be able to rent a vehicle to me if someone was still around to do so. Cost =$50/ day. I lost a day of work to deal with this issue. I had to call the dealership for an update. The vehicle was inspected by a certified Honda technician at Mossy Honda of Lemon Grove. The technician informed me that Honda's policy is to realign the system. The honda technician asked if the vehicle was recently hauling a heavy load that could have ruined the alignment. I have not. It seems the dealership wants to blame me for this safety issue. If there are follow up issues, I am advised to return to the dealership for assistance. The dealership did not offer a ride, nor apologies. They have not provided an updated Cost estimate for this alignment or any needed follow up repairs.Read more

Check engine light illuminated; dealership advised vehicle was safe to drive. Vehicle retained again without repair completion. No technician availability; recall repair not completed before crash. I relied on dealership assurances that the vehicle was safe. This defect creates a serious risk of unintended acceleration and loss of braking control. The brakes went completely to the floor for 3–5 seconds before the engine restarted—just before I nearly collided with another vehicle at a red light. I reported this immediately, and the dealership kept the car for further testing. On Thursday, 10/2, they informed me they found no additional issues and believed the camera was the root cause. However, the problem persisted. On Sunday, 10/5, while stopped at a red light at Hawthorn St. and Memorial Dr., the engine shut off again as I began to accelerate. The brakes sank to the floor, and I had no steering control for approximately 5–7 seconds before the vehicle restarted. My wife and I were extremely shaken, especially with our children in the car. On Monday, 10/6, we contacted roadside assistance to tow the vehicle back to the service center. After additional testing, we were informed that the battery had failed this time and was replaced. However, replacing the battery does not reassure us that the engine and braking system will not fail again while driving, or the brake failure would happen again when the battery is low - which poses a serious safety risk to our family. When my wife called GM to report this issue, all they offered was 200 points worth of rewards....We originally bought this car for safety, but not this has been compromised and we no longer feel safe. Inferior brake system on 2015 Prius master brake cylinder and vacuum booster. Extended warranty issues identified by Toyota for 10 years or 150, 000 miles. I bought my Prius used in 2021 with 92,000 miles at age [XXX] old intending it to last as long as I did. I am now [XXX] old and essentially totally independent. On 2/6/26, I took it to Goettle Toyota, Ocala FL (mileage. 139,160) knowing there was an extended warranty program available that was admitting the parts were defective but not worthy of recall. I was advised the warranty for my vehicle expired 10/26/2025 (3 months and 8 days prior) and was now invalid. Brake replacement cost is $3675.00 + tax🥴. I was provided warranty information and instructed to call Toyota. I called, spoke with Brook and was advised the warranty was invalid due to date even though the car has less than the 150, 000 mile covered and nothing could be done. I was never notified of this issue so I could monitor for problems before the expiration. Average life for these items on a Prius is 250,000 miles, not 139, 000. I am on limited, fixed income and cannot afford this unnecessary repair. I am already gladly driving a 10 year old car to be independent. Loss of transportation changes my entire lifestyle. I cannot afford another car (if I could, I would be fixing this one). I would be reliant FOR THE REST OF MY LIFE, on others for all needs outside of my home. I live in a very rural area. I feel since I was NEVER notified of any issues and the car is LOW mileage and there should be another 100,000 miles on this car (I have driven it less than 50,000 miles in 5 years) that Toyota should honor the extended warranty based on the proximity of the malfunction to the ending of the warranty period and the LOW MILEAGE of the vehicle. Travelling with my young daughter on the Freeway at Freeway speeds at 9pm (nighttime). The entire vehicle just shut down. EVERYTHING - completely dark. Steering went out. Brakes went out. This has been a recall for almost 2 yrs now and the dealer is saying no parts available. There are 1000s of RAM trucks driving around waiting for parts. A simple google search shows there are parts available on line. What is taking so long. This should have been resolved a year ago.

1) I was struck by a vehicle that ran the stop sign at 50mph. After the impact my vehicle continued rolling forward and wouldn’t stop. I ended up a block away from the accident and my emergency brake, brake pedal and center console brake would not engage. My vehicle was stopped after a car pulled up infront of me for me to rear end it to stop. Vehicle is a total loss and sitting in a tow yard.

To avoid a high-speed rear-end collision, I was forced to perform an emergency steering maneuver toward the curb and into roadside bushes to stop the vehicle. This maneuver resulted in severe mechanical failure, including a broken driver-side axle, damaged rims, and broken brake pads. I have about 6,000 miles on the vehicle which was purchased new. There were 2 incidents prior to 2/15/26 (the 3rd incident), but it was only this 3rd incident whereby I could identify the problem. The 2 early incidents I only experienced the smell of burning rubber when driving and exiting the vehicle. Which, i couldn't fathom where it was coming from. And, there was probably a month between

This latest 2/15/26 incident, I was driving and started smelling rubber again, but now I happened to have my fuel mileage indicator on and saw my fuel indicator drop from it's normal average of 17mpg to about 6 mpg. I immediately suspected the parking brake had independently engaged but there was no light or any indication of that on my dashboard screen. I pulled over, turned the engine off, got out and smelt strong scent of rubber burning. While standing next to the vehicle (the engine is off mind you) and making a note of this event on my phone, I heard hydraulics start and stop underneath the truck . That made me suspect the parking brakes even more. I got in, started up, and drove. All was back to normal, no more burning rubber or bad mileage. ... und me. I was towed to Hanania Subaru where they identified the "brake booster" failed. My vehicle is ready today, 2/18/2026 to pick up and I'll return the Subaru loaner vehicle. Apparently the problem was an electronic failure code and replacing the brake booster was all that was needed. Fortunately, I was not driving in traffic when it failed and I'm told this type of failure is extremely rare. I do appreciate the ability to document this potential hazard to NHTSA.Read more

I am filing this complaint due to repeated and dangerous electrical and hybrid system failures in my 2024 Dodge Hornet, which have resulted in sudden loss of acceleration, loss of braking performance, dashboard warning lights, and system malfunctions while driving. The vehicle has been serviced multiple times at Meadowland of Carmel Chrysler/Dodge/Jeep/Ram with the following documented incidents:
... • Check engine light and loss of acceleration on highway – Vehicle would not accelerate while driving on the highway. • Repeated highway loss of power – Vehicle drops speed while driving and will not accelerate, requiring me to pull over and shut the vehicle off to regain acceleration. • Battery/Hybrid system malfunction – Vehicle experienced abnormal battery percentage drops and required multiple software flashes and reprogramming of PCM, hybrid control processor, and transmission control modules. • Grinding noise from engine with complete loss of acceleration – Vehicle had to be towed. Dealer found active hybrid generator performance code (P0A92). The motor generator unit (MGU) was found to have failed and required replacement. • All dashboard warning lights illuminated / Drive mode unavailable message – Vehicle would not accelerate and multiple control modules required flashing and updates. • Brake pedal went soft – Loss of normal brake pedal feel was reported. • Electrical communication faults – Stored codes for lost communication with AC compressor control module and cabin air heater module. • Frozen backup camera (intermittent issue not always replicated at dealer) – Rearview camera screen has frozen while reversing, creating risk of collision. The most serious safety concern is the repeated loss of acceleration while driving at highway speeds, forcing me to pull over in active traffic. Additionally, the soft brake pedal and electronic failures raise concerns about braking reliability. These failures create a serious risk of crash or injury to myself and others.Read more

I have had 22 recalls to date on this vehicle and received 1 recall notice. Engine light comes on alot, makes noises and then goes away after a couple days. Windshield washer stopped working on front windshield, now back stopped working. Tire pressure monitor & sensor default stays on (previous recall) Window on driver side stopped working, back passenger door on drivers side don't shut correctly
... like the weld come loose. Back brakes make a crazy noise when backing up. Back up camera warning comes up. I've only had this vehicle since 09-2023 and purchased from Ford as a Ford Blue Certified and have nothing but issues. Most of these recalls risk is a fire. If I'm involved and live through a fire I will make sure FORD is held accountable. The open recall with risk of fire currently has been open since July 2025 and no repair yet.
