Three incidents happened with this vehicle. When I put the car on reverse and reversed and stopped, the gear was then put on drive and before the accelerator was stepped on the vehicle propelled itself at a very high speed and I had to emergently step on the brakes before it hit anything. The the front collision warning did not come on and the vehicle did not brake on its own. The same thing happe
... ned in our driveway and before we could apply the brakes the vehicle crashed into a tree. This time also the front collision warning did not come on and the vehicle did not brake and went and hit a tree. There was a video that captured this incident and was shown to our Ford dealership and they did not do anything about it. We told them that the vehicle accelerates by itself and is a safety hazard. Since this did not involve any other vehicle or pedestrian the hazard was limited only to ourselves, The insurance was charged more than $10,000 to fix the damages and we also had to pay a deductible of $1000. More importantly we are unaware if this has been reported by others and can cause serious injuries to the driver, occupants of the vehicle and other who may be involved inadvertenly. Any death caused will become the responsibility of the driver even though this was not the mistake of the driver.Read more

My 2018 Mazda CX-9 experienced catastrophic engine damage after a radiator failure allowed coolant to enter the engine. The vehicle provided no dashboard warning, overheating alert, or coolant warning before the failure occurred. Modern vehicles are designed to alert drivers to overheating or cooling system failures. The lack of warning prevented me from addressing the issue and resulted in catast

Vehicle shut-off while driving on the expressway, around 70-75mph. Upon vehicle turning off, I had no option but to "coast" off to the left side, which thankfully, freed me from traffic for the most part. If anyone who have been a foot over, they would've hit me. I have had numerous electrical issues with this vehicle and have taken it to the dealership but they have been "unable" to duplicate the
... issue. Vehicle was immediately taken to the dealership (and they still have as of this incident report submission). They told me last night that they will keep it again today and test drive it again to see if they can get it to do anything but if it doesn't show anything new, they have nothing to diagnose and will give me my car back. After requesting a copy of repair orders and a list of the history codes in my car, I am beside myself that anyone thinks giving me this vehicle back to drive is ok. See the history code report in documents uploaded. These are MAJOR safety failures in a large vehicle.Read more

I am writing to formally report repeated and serious defects with my 2024 Volkswagen Atlas, VIN [XXX] , which I leased in California. These defects occurred well within the vehicle’s first year and while under factory warranty. Brake System Defect At 8,405 miles, the vehicle received its 10,000-mile maintenance. At 9,340 miles, brake pads measured 10 mm front / 8 mm rear, reflecting nearly ne
... w condition. By 13,175 miles, the pads had separated from their backing plates, causing scoring of the front rotors. Told they crumbled by the dealership. Dealer performed a “one-time goodwill repair” on the 1-year in-service date, replacing pads and rotors, but refused to document the failure as a warranty-covered defect. This premature failure at 13k miles is not normal wear and reflects a serious manufacturing/material defect that created a safety hazard. Engine and Oil Issues At 13,175 miles, the oil light came on and the vehicle was found to be low on oil despite proper service at 8,405 miles. At 8,405 miles the oil light also came on showing low to no oil. Intermittent Warning and Electrical Issues Throughout this period, the vehicle has experienced recurring warning messages and system malfunctions, including: “Error: Dynamic Road Sign Display” “Lane Assist Not Available” “Front Assist Has Limited Availability” TPMS issues, where displayed tire pressure did not match actual pressure, and the light remained on for days despite correct tire pressures being confirmed. Additional Warranty Repairs At 9,943 miles, the radio required a software repair under warranty. Paint defect required repair under warranty. Summary: In less than 14,000 miles, this vehicle has experienced: Critical brake system failure (pads separating, rotor damage). Engine oil loss triggering warning light. Multiple electrical and safety system errors affecting driver-assist features. Radio malfunction requiring repair. Paint defect requiring repair. While traveling from Los Angeles, California to Tucson, Arizona with one passenger, the vehicle experienced a sudden and dangerous loss of multiple critical systems while driving at highway speed. Earlier during the trip the vehicle stopped in Quartzsite, Arizona. When attempting to re-enter the vehicle, the key initially did not unlock the vehicle. The driver contacted Range Rover roadside assist
... ance and asked for guidance, but no explanation or solution was provided. After several attempts the vehicle eventually unlocked and started. Approximately one hour later, while driving eastbound on Interstate 10 at highway speed after dark, a yellow battery warning light appeared on the dashboard. Shortly afterward a red battery warning and a red triangle warning appeared. Within minutes of these warnings, the vehicle suddenly lost propulsion and the transmission shifted into neutral. Immediately afterward, the vehicle lost electrical power and the dashboard and exterior lights went dark. At the same time, steering assistance and brake assist were lost, making it extremely difficult to control the vehicle. The driver had difficulty steering the vehicle and slowing it safely while traveling on a busy highway with heavy truck traffic behind. Because steering assistance and brake assist were lost, the driver had very limited ability to maneuver the vehicle safely. The driver was able to slowly reduce speed and move across several lanes of traffic to the right shoulder of the highway, stopping near Exit 109 (Sun Valley Parkway) on Interstate 10. The vehicle stopped approximately 300 feet before the exit ramp. Because the vehicle had lost electrical power and exterior lighting, the vehicle was difficult for other drivers to see in the dark. Several large trucks passed close to the stopped vehicle. The driver used a mobile phone light to signal the vehicle’s presence and reduce the risk of being struck. The vehicle was later towed to an authorized Range Rover dealership in TucsRead more

Vehicle: Lincoln MKZ 2013 Dealer: Auto Quest Inc. Description of the Safety Issue: I am submitting this report regarding a vehicle safety concern involving a 2013 Lincoln MKZ that I purchased from Auto Quest Inc. in Renton, Washington. Shortly after purchasing the vehicle, within the first few days of ownership, the vehicle began experiencing significant mechanical problems including transmissi
... on issues and persistent brake noise that required multiple repair attempts. These problems appeared almost immediately after the purchase, which raises concerns that the vehicle may have had pre-existing mechanical or safety-related defects that were not disclosed at the time of sale. Since purchasing the vehicle I have had to spend thousands of dollars on repairs in order to keep the vehicle operational. Despite these repairs, the brake system has continued to produce abnormal noise and the vehicle has experienced ongoing mechanical issues. These types of issues may represent potential safety risks to drivers and passengers if the braking system or transmission were to fail while the vehicle is in operation. I am reporting this issue so that the National Highway Traffic Safety Administration can evaluate whether this may represent a broader safety defect affecting this vehicle model or vehicles sold under similar circumstances.Read more

On August 15, 2025, I was backing my vehicle when the rear view camera screen went blank, providing no image of what was behind me. With no warning and no image, I collided with a rock and landscape, causing damage to the rear of my vehicle. At the time of this incident, my vehicle had an open, unrepaired safety defect under Safety Recall 25TA13, which specifically addresses the condition that th
... e rear view camera image may freeze or not display while reversing. I was not aware the recall existed at the time of my accident, and the recall remedy was not completed on my vehicle until February 12, 2026 — nearly six months after the incident. I filed a claim with Toyota Motor North America. They inspected the vehicle on February 11, 2026, found the camera functioning at that time (after the recall had since been remedied), and denied my claim. They cited owner's manual language advising drivers not to rely solely on the backup camera, but did not address the fact that the recall was unrepaired at the time of the accident. Additionally the parking collision warning and automatic braking should have engaged. The rock impacted the car right near one of the car cameras....so the collision warning and braking should have engaged. I am filing this complaint because I believe the unrepaired recall directly caused my inability to see behind my vehicle, and that Toyota's delay in notifying me and completing the remedy contributed to this incident. I am requesting that NHTSA review whether Toyota's notification and remedy timeline for Recall 25TA13 was adequate, and that this incident be added to the record of complaints associated with that recall.Read more

The rear brakes on my 2018 Honda Accord Sport 1.5T (Six Speed Manual Transmission) have failed prematurely. This failure of the pads and rotors was confirmed, at the request of American Honda, by me at my local Honda dealer, and was documented in a response to American Honda. This occurred at 46,150 miles which i believe indicates a defective design in that the brake calipers, pads and rotors
... as a system were not built to the capacity needed to support Honda's design of this car that greatly increases the workload on the rear brakes. I am a very careful driver and minimize the use of brakes as a result. My calculation is that these brakes have failed at 1/4th the life that it would be reasonable to expect. The result is increased safety risk to car owners and drivers who do not act on this premature failure due to it not being normal or expected or affordable. Honda has effectively, through this brake design, implemented a need for car owners to enact a $750 repair on a frequency close to routine maintenance such as oil changes.Read more

I have just purchased this truck to replace an old truck lost due to a total loss . I picked up the truck in South dakota and drove back to Pennsylvania. When setting the parking brakes parked on a small incline with the truck empty the parking brake hold on the front drive axle and it skids those tires when rolling away. However the rear rear drive axle is not equipped with parking brake chambers
... which results in insufficient braking force to hold the truck in place. The truck has rolled away on me twice since I have purchased it . This is a huge saftey concern due to the fact it will certainly cause property damage or death . I have alerted Volvo Trucks and have a case number but they are really no help in resolving this. I have done research and found that this is already an issue with 2020-2027 trucks where the rear rear drive axle is not properly equipped with parking brake chambers.Read more
