The dealership technicians subsequently inspected the vehicle and reported that the entire electronic braking system had suffered a total failure and is completely non-functional. Despite multiple technical service bulletins and recalls issued for similar vehicle builds regarding electronic brake boost interruptions and mechanical brake vacuum pump failures, I am being forced to pay for these critical safety repairs out of pocket. A sudden loss of braking power poses an immediate crash hazard, and I am filing this report to urge the NHTSA to investigate why this manufacturing defect is not covered under an active safety recall for this specific vehicle configuration.Read more

I had pickup my vehicle from the dealership where it had been since the 29th of May due to a previous incident exactly the same as what occurred this time. ... l propulsion. After about 60 seconds the vehicle recovered and I turned to go back to the dealership. At the aforementioned intersection (crossing over I35) it again lost propulsion, only this time for a longer period of time. I was in heavy traffic and the car behind me was honking even though I had my hazard warning flashers operating! I made it to the dealership where the service manager agreed to do a ride along. We were about to the end of the ride along when the vehicle lost propulsion power. It wasn’t as severe and recovered quickly. The service manager said he had noticed the brake pedal move at the time I accelerated. His theory was that the brake being applied at the same time as the acceleration cause the computer to get confused and it’s reaction was to remove propulsion. No offer was given to take the vehicle back for further testing. I dropped the service manager off and proceeded to return home, being cognizant of my foot placement and insuring it was well away from the brake when accelerating. KIA Care has so far refused to get involved. While I recognize braking at the time of acceleration is counterproductive, it should not cause a catastrophic propulsion failure. Plus the system sets no flags that it had this event! I have tried recreating this failure by using my left foot on the brake while accelerating with my right. This doesn’t recreate the problem so it must be a timing issue. I do not know if this issue is unique to my vehicle or is shared with other hybrids! Since acquiring the vehicle, I have experienced persistent mechanical issues that I believe may pose a safety risk to myself, my child, and others on the road. The most concerning issue involves the

I have had the brakes replaced and serviced multiple times, yet the brakes begin grinding again within a very short period, including an instance where the grinding returned approximately one month after replacement. Despite spending money on repairs, the problem continues to reoccur. I am concerned that there may be an underlying defect affecting the braking system that is not being properly identified or corrected through normal maintenance. In addition, the vehicle consumes an excessive amount of oil, requiring frequent monitoring and replacement between oil changes. I am concerned that this issue could potentially lead to engine failure or other dangerous mechanical problems while the vehicle is being operated. As a single mother, I rely on this vehicle daily for transportation to and from work and for caring for my family. I have experienced significant financial hardship attempting to keep the vehicle operational and safe. Despite my efforts to maintain the vehicle, these issues continue to occur. I respectfully request that the National Highway Traffic Safety Administration review this complaint and determine whether these issues may be related to known defects, recalls, investigations, or patterns reported by other owners of 2013 Hyundai Sonata vehicles. I have been informed that replacement ABS modules for this vehicle have been unavailable for approximately nine months and remain unavailable as of today. This appears to be a widespread issue affecting Jeep Wrangler JK vehicles, with many owners reportedly unable to obtain replacement safety-critical ABS modules through Mopar or dealership networks. Because the ABS and traction/stability systems are malfunctioning, the vehicle cannot pass inspection and is unsafe for my family of four, especially during rain or emergency braking situations. This issue affects federally mandated vehicle safety systems, including anti-lock braking and electronic stability control. I drove around 2 hours after leaving and my engine light came on and my brakes failed again going 70 miles an hour surrounded by semis. I called Jim Ellis and they told me to tow it to Walter Jackson who told me it was a vacuum pump that they would have had to remove during the repairs. In addition there is a class action against GM about these pumps for my year and make of car. I have asked for a refund from Jim Ellis contacting ownership, GM, Service Team and no one will respond to my calls or emails. I filed a customer complaint with GM and they are accepting zero responsibility as well. I paid thousands of dollars between both repairs and rental cars and the least I would like for them putting my life and others around me at risk on a freeway is my money back.Read more

He stated that the brake fluid was unable to properly bleed through the brake line restricting air to fully purge from the hydraulic system, which can result in incomplete bleeding, reduced braking performance, uneven pad wear, brake drag, or brake fade and is part of the reason for burning smell near right front tire. He added that car isn’t safe to drive in this condition due to safety concerns of accidents due to inability to properly brake or even a fire. Vehicle hasn’t been repaired due to cost and I will attempt to contact manufacturer. The initial mileage reading when this issue occurred in 2024 was 90,000 miles. ... ied pumping the brake pedal with no effect. No collosion avoidance alert To try and slow the vehicle ran the wheel against the curb of the median which slowed the car down a little . I pressed the parking brake button with no effect. The collision avoidance system did not engage. I clipped the rear of the Toyota Corolla as I jumped the curb to avoid cars using the friction against the curb and move further left when oncoming cars were clear and was able to stop the car. (The car may have stopped because the front control arm bent which made the car drag. ) I am unsure whether the master cylinder failure or ABS overrode braking, but the car would not stop. I do not know if the black box captured the data as the there was no airbag deployment. The collision avoidance system did not engage to slow the car any time. I did not see any lights light on the dashboard did not check specifically as I was busy avoiding a collision other cars . Basically, pedal went down not all the way to the floor but car was not slowing down. I checked the master cylinder reservoir, and it still had fluid. I have a Lincoln MKZ Hybrid 2012 with 88k miles on it. ... f the vehicle - following turning it back on, the dashboard warning disappeared, I was able to brake again, and the warning has not come back since. My vehicle just passed inspection earlier in the month, and nothing with the brakes came up. This is a critical safety issue! It could have been potentially fatal to myself, my passenger, or other drivers if I could not stop before the intersection, or if this had happened while I was on a main road or highway. I took it into the Ford Lincoln dealership. They cited a faulty HCU/ABS module and recommended replacement, but all parts are after-market at this point and would not come with a warranty. OEM replacements are also extremely difficult to come by. The repairs would take around $3.8k to complete, which is close to the current value of the vehicle. Ford issued Safety Recall 19S54 in December 2019, covering 600,166 model year 2006–2010 Ford Fusion, Mercury Milan, Lincoln Zephyr, and Lincoln MKZ vehicles for the exact same defect. No recall exists for anything in 2012 despite at least 17 NHTSA complaints have been filed for the 2012 Lincoln MKZ alone for this specific brake failure. The 2012 MKZ and Fusion use the same braking system architecture as the recalled vehicles. The failure mechanism is identical. The safety risk is identical.Read more

The loss of these safety systems significantly increases the risk of an accident, particularly during emergency braking situations, wet road conditions, loose gravel, or any circumstance where wheel lockup or loss of traction may occur. Without ABS, the wheels can lock during hard braking, increasing stopping distance and reducing steering control. Without traction control and stability control, the vehicle is more susceptible to skidding, fishtailing, and loss of control. I have contacted multiple Jeep dealerships seeking repairs. I have consistently been informed that the ABS module required to repair the vehicle is unavailable and has been on indefinite backorder. Dealership personnel have been unable to provide any estimated availability date or permanent repair solution. While researching this issue, I discovered that numerous Jeep Wrangler owners have reported the same ABS module failure. Online forums, dealership service departments, and consumer complaint websites indicate this is a widespread issue affecting many Jeep Wrangler vehicles. Owners have reported waiting several years for replacement modules, with some being told the part is discontinued or unavailable indefinitely. Dealership personnel have been unable to provide any estimated availability date or permanent repair solution. I believe NHTSA should investigate this issue as a potential safety defect affecting Jeep Wrangler models. UNKNOWN whether the manufacturer's engineering team, police, or insurance representatives have inspected the vehicle—to my knowledge, they have not. Warning lamps, messages, or symptoms prior to failure: UNKNOWN / none noticed. No warning lamps or dashboard messages were observed before or during these events. Parking Brake The Sticker on this U-Haul 15 foot Truck says, "Engage the Parking Brake before turning off the Engine, otherwise UNCONTROLLED OPERATION OF VEHICLE may occur.". Later that Night it happened again at about 25 -30 MPH and pulled to the right and almost into the right side ditch, on a two lane Highway. My Safety was at risk because I could have hit a Telephone Pole or have been steered into the right side ditch. IF it had happened, on a Multi-Lane Highway, I could have Hit a Vehicle in the Right Lane. Others are put at Risk, because anyone on a Sidewalk near the Right side Curb, Could have been Hit, by this Uncontrolled Steering. The U-Haul Regional Manager, Emily Noe said it had not been reported by anyone else. This U-Haul Truck had about 14K miles on it and is fairly new. I assume this U-Haul Truck had been Inspected, before being placed on the Road. There is the Yellow Sticker inside the Truck, with Black Letters that warn about pulling On the Parking Brake before Parking and shutting down the engine....which I did!!! I drive a Standard ‘22 Bronco. Only E-brake works for parking brake when in parking lots and such. My Bronco rolls into oncoming traffic and pedestrians when the e-brake releases. I am former Homeland Security and this unacceptable, as this if my 10th issue with the Ford Bronco! Safety First!

My 2021 Toyota Sienna Hybrid has an intermittent and dangerous brake failure where the brakes suddenly release pressure during normal braking. ... r vehicle. The vehicle as a result can stop up to six feet past the intended stopping point in some instances. This issue is unpredictable and has occurred multiple times. It creates a serious safety risk. Toyota performed T‑SB‑0047‑24 (Brake ECU Reprogramming) on my vehicle, whereafter the brakes seemed to function normally for several months, but the problem resurfaced and occurred threee time in one week in May of 2026, which prompted me to bring it backk to Toyota for another look. The dealer has been unable to reproduce the issue and therefore has not repaired it. The veicle has been returned to me, and continues to have the problem intermittently at a random frequency. Toyota corporate Brand Engagement Center has been notified, and a case was opened (Ref # 260528002416). This case was closed however after telling me that the dealership connot reproduce the problem, therefore they cannot repair it. This appears to be a brake control defect involving the hybrid brake‑by‑wire system. Because the failure is intermittent and leaves no diagnostic codes, it is extremely difficult for the dealer to diagnose, but the safety risk is severe. I'd like to advise that I have owned and driven earlier hybrid models (2012-14) that never exhibited this type of brake failure. I'd also like to point out... an internet search of this problem reveals that this problem seems to be prevelant in newer Toyota hybrid models. The brake pump and actuator on my 2015 prius has been faulty and the repair was apparently covered under the customer support program but it expired in December. I had no knowledge of it and when I took my car in to a toyota dealership, no one told me about about the profram but told me that they needed to be replaced. The One Pedal Drive function (regenerative braking) intermittently deactivates without warning after startup. This issue has occurred multiple times under normal driving conditions. When the accelerator is released, expected deceleration does not occur, resulting in reduced braking response. The manufacturer is unable to properly provide a fix for Recall 24V-896 and my vehicle ABS and Traction Control is intermittent. This recall was issues in November of 2024. According to FCA's website, parts are still not available to repair this.

The OTA is installed; vehicle available for inspection on request. FORD'S OWN ADMISSION (verbatim, from Ford's published OTA release note): "In some instances after this upd

You may also receive a Ford App notification indicating service is needed on the vehicle, which you can disregard. Please be assured this is a visual notification only..." SAFETY RISK: The same wrench-light is the documented symptom of NHTSA Safety Recall 25V863 / Ford 25C69 (Park Pawl engagement failure) - active and unrepaired on this VIN per NHTSA's June 2, 2026 refresh (see complaint 11741722). Ford's 25V863 NHTSA filing states a Park Pawl failure displays "A WRENCH LIGHT AND SHIFT SYSTEM FAULT MESSAGE... IN THE INSTRUMENT PANEL CLUSTER" with automatic EPB application and roll-in-park crash risk. Ford has instructed me in writing to disregard the same notification surface that would warn of a genuine 25C69 failure. The two are visually indistinguishable without dealer diagnosis. REPRODUCED/CONFIRMED: Ford itself confirms the defect in its written release note. INSPECTED: No dealer or independent inspection of this OTA-induced false-positive has occurred; Ford preemptively instructed disregard. WARNING LAMPS / FIRST APPEARANCE: The wrench-light icon and Ford App service-needed notification ARE the symptoms. They began with the May 10, 2026 OTA install per Ford's published note. Both are also the warning channels for the unrepaired 25C69 safety condition.
