My 2017 Ram 2500, 6.4L Hemi experiences warning lights which I'm told is caused by intermittent PCM failure. I'm told this can cause the truck to stall while in motion, refuse to start, or enter "limp mode". This amounts to a possible unsafe condition that can put myself and passengers at risk of property damage, bodily injury or worse. I brought the truck to Priority Automotive of Westerly,
... LLC on March 9, 2026 for diagnosis. Their diagnosis (attached) recommended replacing the PCM to correct (possibly), "...intermittent internal short(s)". Cost of the repair is $2,012.42. Subsequently, I contacted customer care at Stellantis (866-726-4636) and was given Case #[XXX]. I was told to make an appointment at the aforementioned dealership and call them the day before the appointment, so they could send a representative to the dealership for examination. I did exactly that to no avail. No one from Stellantis even contacted the dealership on the appointment date. I called them to find out what happened, only to find out (from a supervisor) that they are not offering assistance in the matter. Additionally, the truck is currently under recall #C4B/NHTSA 24V-896 (PCM related) and, to-date, I'm told the part is still not available after more than a year since the recall. I've owned Dodge Rams for over 27 years and find the situation unsafe and totally unacceptable. I'm also aware that many other owners of the 2017 Ram 2500 6.4L experience PCM/ECM issues, causing no-start conditions or stalling, and have reported them alongside braking system complaints. While driving at approximately 60 mph on an open road with no obstacles present and no braking input, the rear wheels spontaneously locked up without warning. No brake pedal input. No warning light prior to the event. No obstruction in the road. The vehicle has been subject to an active manufacturer recall for rear wheel lockup due to transmission control valve failure. The dealer's own diagnostic
... system generated a report showing six active fault codes in the following days including lost communication with the Brake System Control Module and Transmission Control Module. These codes were not addressed on the repair order. The dealer attributed the lockup to an Automatic Emergency Braking activation without diagnostic confirmation and performed no repair. The manufacturer's own internal bulletin dated January 22, 2026 acknowledges the exact fault codes present on this vehicle as a known defect. The dealer cited this bulletin to dismiss the concern while stating no problems exist. A persistent brake noise has been confirmed by two separate authorized dealers across three visits with no repair performed at any visit. The vehicle has required five dealer visits in 77 days of ownership with 1,729 miles. The defect remains unresolved and poses serious risk of loss of vehicle control at highway speed.Read more

Component: Electrical System / Forward Collision-Avoidance Assist / Speedometer Incident Summary: I am reporting a catastrophic and uncorrected safety defect in my 2021 Kia Sorento (VIN: [XXX] ). Pre-Accident Attempts: Before April 2024, I repeatedly sought diagnosis for instrument cluster 'glitches.' The Kia dealer misdiagnosed this as a battery failure, charging me out-of-pocket for a repl
... acement that did not resolve the issue. Collision (April 22, 2024): While driving, the instrument cluster suffered a total system crash into 'Factory Mode,' losing all telemetry. Simultaneously, the vehicle's Forward Collision-Avoidance Assist (FCA) failed to engage. Despite the vehicle being equipped with automatic emergency braking designed to detect and mitigate collisions, the system provided no warning and no autonomous braking prior to striking a deer. Post-Accident Failures: Following the crash, Geico Insurance funded a wiring harness replacement at a Kia facility (backed by a Geico Lifetime Warranty). However, the glitches and 'Factory Mode' errors have returned, proving the vehicle's core safety systems remain compromised. Safety Risk: This vehicle is non-compliant with FMVSS 101 and 127. The failure of both the speedometer and the autonomous emergency braking creates an extreme risk of further injury or death. Kia Corporate has failed to address this documented accident history. Vehicle: 2013 Kia Optima SXL VIN: [XXX] While driving on a highway at approximately 55–58 mph, the rear driver-side electronic parking brake (EPB) motor housing failed with a loud bang. Immediately after, there was heavy smoke and a strong burning smell. The vehicle became very difficult to drive, and the wheel began screeching and dragging as if the brake had locked. The failure appears to
... have caused the rear brake caliper to seize while the vehicle was in motion. This created a dangerous driving situation at highway speed and could have caused a crash. The brake system component that failed appears to be the electronic parking brake actuator/motor assembly attached to the rear caliper. I am reporting a repeated brake system issue on my 2024 Hyundai Tucson involving premature rotor warping and brake failure at very low mileage. On April 30, 2025 (11,007 miles), I first noticed significant vibration and shaking when braking. On May 29, 2025, Colonial Hyundai diagnosed the issue and resurfaced the brakes under warranty (Hyundai case #40826529). The issue persisted immediately af
... ter repair, and on June 13, 2025, I was informed that both front and rear brakes and rotors needed replacement. On June 19, 2025, all brakes and rotors were replaced under warranty, resolving the issue temporarily. Less than 8 months later, on February 12, 2026 (under 10,000 additional miles), the same braking vibration returned. The original dealer attributed the issue to driving behavior. At Hyundai’s direction, I sought a second opinion on February 26, 2026 at Piazza Hyundai of West Chester. They confirmed that the brakes and rotors again required replacement and indicated this was unusual for such low mileage. On March 16, 2026, despite the dealer’s findings, Hyundai Corporate declined to cover the repair under warranty. This vehicle has now required three brake-related repairs within a short period and low mileage, including resurfacing and full rotor replacement, followed by another failure. This pattern suggests a potential defect in the braking system, materials, or related components, rather than normal wear. Component/System Failed: The failure involves the braking system, specifically repeated warping/failure of front and rear brake rotors and premature brake wear. The vehicle and components are available for inspection upon request. When braking, the vehicle experiences significant vibration and shaking in the steering wheel and chassis, which affects braking smoothness and control. This condition can reduce driver confidence, increase stopping distance, and may lead to loss of control during braking, particularly at highway speeds or in emergency stop.Read more

Vehicle: 2025 Volkswagen Tiguan Incident Description: While driving at approximately 45 mph, about 5 minutes into the drive, my vehicle displayed a Lane Assist warning and a message to place my hands on the steering wheel. Immediately after, multiple warning lights and messages appeared across the dashboard. Followed my emergency warning message. The vehicle then suddenly applied the brakes on
... its own without any obstacle or hazard present. The brakes locked up, and the vehicle would not move. At the same time, the vehicle began honking and exhibited abnormal alarm/locking behavior. I had to restart the vehicle to regain normal operation. This sudden and unintended braking could have resulted in a rear-end collision, serious injury, or loss of control of the vehicle, especially at highway speeds or in traffic. The vehicle was taken to a Volkswagen dealership. The dealership documented that the brakes locked up and the vehicle would not move, but stated they could not replicate the issue and did not identify a cause or provide a repair. I do not feel safe operating the vehicle due to unintended braking and system malfunction. Since this event the issue has occurred twice more on two brand new batteries on no incline, and I now fully disable the auto-stop feature before driving to avoid an accident. The car starts properly with no battery damage if started from the full ignition off position, even if auto-stop originally stopped the engine, pointing to a failure of the auto-stop system specifically. What You’re Noticing (and Why It Matters) 1. Engine Mounts Wearing Out Engine mounts can wear over time, but premature failure (early loosening, vibration, clunking) can point to: Defective parts Poor design for that model year This is a common complaint in some BMW models, especially as mileage increases. 2. Brake System & Sensor Issues Warning lights staying on even after replacing senso
... rs can mean: Faulty wiring or control module (not just the sensor itself) Ongoing system miscommunication BMW braking systems rely heavily on electronics, so one issue can trigger repeated alerts. 3. Oil Gasket / Housing Leaks You mentioned oil leaking from a plastic housing—this is important. BMW engines (including the X1) often use: Plastic oil filter housings Plastic cooling components These are known to: Crack from heat over time Cause oil leaks and coolant loss This isn’t always clearly explained in sales descriptions. 4. Plastic Engine Components Many modern engines use plastic parts for: Weight reduction Fuel efficiency But downside: More prone to cracking, warping, or leaking Especially under high heat conditions ⚠️ Should There Be a Recall? For something to become a recall through the National Highway Traffic Safety Administration (NHTSA), it must: Be a widespread defect Pose a safety risk (not just wear and tear) 👉 Right now: Some of these issues (like oil leaks or mounts) are often treated as maintenance or design concerns, not recalls Brake system failures could qualify if they affect stopping safety—but only if widely reported and proven defectiveRead more

In June 2025, while driving on the highway, the vehicle suddenly displayed multiple brake warning lights and warning messages, including the red brake warning light, ABS light, brake system warning light, and traction/slip indicator. At the same time, the brake pedal became stiff and braking effort increased suddenly, creating a near-crash situation because the vehicle did not slow normally. Befo
... re that incident, the vehicle had already been making abnormal brake-related noises, including a humming/pumping noise when the brake pedal was applied, and there were intermittent episodes where braking felt abnormal. In June 2025, the vehicle was taken to a Toyota dealership during Toyota’s brake booster support program period. The dealership documented customer complaints of “humming noise in cabin when holding foot on the brakes,” a request to “scan for codes for ZJB enhancement,” and that the “vehicle hard to accelerate and brakes got stiff,” but stated that no qualifying ABS codes were present at that time. In March 2026, the same condition returned and worsened. The vehicle again displayed the red brake warning light, ABS light, brake system warning light, and traction/slip light, and braking performance was reduced. An OBD scan of the ABS control unit showed code C1391, which is associated with brake booster / ABS actuator failure. The Toyota dealership has now diagnosed the vehicle with ABS actuator and brake booster failure and quoted repair. This is a serious safety issue because braking became more difficult while driving, including on the highway, with sudden warning lights and degraded stopping ability. The failed components are available for inspection. The problem has now been confirmed by a Toyota dealership. The issue was reported to a Toyota dealership during the prior support-program period but was not confirmed then.Read more

The engine actively fights the braking system, requiring extreme physical force to stop the car and prevent front-end collisions. This lunging has repeatedly triggered the Pre-Collision System, causing the seatbelts to lock and tension against the driver’s body. Additionally, the vehicle exhibits steering resistance and feedback during turns, making it difficult to maneuver safely. The local dealership (AutoNation Toyota Las Vegas) inspected the vehicle and claimed these life-threatening behaviors are "normal" for this model.Read more

Breaks suddenly went out and I couldn't stop. I had our kids in the car.
Incident date: Mar 6, 2026
I was backing out of a parking spot in a strip mall, when I changed the gear to Drive, and moved forward by a foot. I felt a bump, as I appeared to have hit some obstruction on the left underside of the vehicle (likely a low curb/island). After that, I lost control of the vehicle - the car accelerated very hard, and I tried to stop the car, but it was not responding to my brake input. I crashed
... into a stationary vehicle, which deflected my car to a storefront wall. The total distance travelled from when the sudden acceleration started to the crash against the wall, was probably 20 - 30 feet (and not in a straight line). But the impact was severe enough to have deployed the air bags and the car is considered totalled. Besides the sudden acceleration which did not happen on my input, and the inability of the car to stop based on my brake input, neither did the Forward Collision Warning go off, nor did the Automatic Emergency Braking turn on, to prevent the crash.Read more

I am reporting multiple life-threatening safety defects with my 2025 Kia Telluride. The vehicle has experienced Sudden Unintended Acceleration, where it accelerates to excessive speeds without driver input. During these episodes, the braking system fails to respond, only allowing the vehicle to stop once its internal safety systems intervene. Additionally, the vehicle suffers from unpredictable st
... alling and power loss; the engine frequently shuts off upon parking—followed by the electrical system cycling back on without the motor—and the vehicle consistently struggles to start in the morning. These recurring mechanical and electrical failures pose an immediate danger to myself and others on the road.Read more

My Truck has less than 60,000 miles have paid for the extended warranty and now have a infamous ABS module failure which is very dangerous because now My Truck is my only source of transportation and being a senior citizen. This is a hazard for my only vehicle that I have. I have done my research and there are tons of complaints for this issue and it has not became a recall, which is very alarming
... and to top it off. I have always taken in the truck for Service to have it checked, and the dealer has never checked it and now that there’s no warranty now they say it is an issue so I need this to be fixed . This does also affect the driving of the vehicle and the stability and top of all the alarming issues on my dashboard. I am also the original owner of this vehicle.Read more

I WAS GOING UP A STEEP NARROW ROAD, A BIG TRUCK WAS COMING DOWN SO I STOPPED AND HONKED SO HE WOULD BACK UP. WHEN HE DID, I WAS UNABLE TO MOVE MY SUV. IT FROOZE ENTIRELY, WOULDNT GO UP, WOULDNT GO IN REVERSE, JUST STAYED ON BUT WOULD NOT MOVE. AFTER SEVERAL ATTEMPTS I WAS ABLE TO GO BACK A LITTLE AND MOVE TO A SIDE AND THEN I WAS ABLE TO DRIVE UP THE ROAD. THIS WAS THE 2ND TIME THIS HAS HAPPENED
... SO I ASKED THE SERVICE SHOP AT THE DEALER TO CHECK IN DRIVING THE SAME ROAD. THE SITUATION DID NOT REPLICATE, THEREFORE THEY SAY THERES NOTHING THEY CAN DO OR CHECK, ALTHOUGH I HAVE READ THAT THIS VEHICLE HAS BEEN KNOW TO DO THIS, BUT THEY DID NOT EVEN LOOK AT THE PRINTED EVIDENCE I TOOK TO THEM.. THERE WERE SUGGESTIONS ON WHAT HAD TO BE CHECKED BUT THE MANAGER REFUSED TO EVEN READ THE INFORMATION. THE MECHANIC SUGGESTED I STEP ON THE GAS UP TO THE END, ALTHOUGH THE MANUAL SAYS YOU SHOULD HIT GAS SLOWLY WHEN STOPING ON A HILL. EITHER WAY I TRIED BOTH WAYS....IT STILL FREEZES.Read more

Incident date: Dec 9, 2023
On December 9, 2023, while driving my brand new 2023 Honda Accord Hybrid Touring HPD package on the highway during snowy conditions at less than 50 mph less than 8000 miles on the odometer after my first oil change the automatic emergency braking (AEB) system suddenly and unexpectedly engaged with no obstacle, vehicle, or pedestrian ahead, causing the vehicle to skid uncontrollably and slam into a
... concrete highway barrier. The car was towed from the scene, and I was hospitalized for injuries sustained in the high-speed impact. The AEB component or system malfunctioned by activating without cause; it was inspected by police at the scene and I contacted Honda’s safety team to conduct an investigation. This unintended braking put my safety at extreme risk by inducing sudden deceleration and loss of control on a slippery highway, which could have endangered following traffic as well. The problem has not been reproduced or confirmed by a Honda dealer or independent service center since the vehicle was not repaired. Police inspected it at the crash scene and issued me a ticket for “driving too fast for conditions” despite my explanation of the sudden braking; no manufacturer inspection of the AEB occurred, though I contacted Honda’s safety team and they attributed it to not following the owner’s guidebook without further review. Prior to driving, all dashboard warning lights illuminated unexpectedly; I turned the vehicle completely off, restarted it, and the lights disappeared with no further symptoms until the AEB engaged abruptly on the highway. This matches ongoing NHTSA investigations into Honda Accord and CR-V vehicles for braking problems, including false AEB activations.Read more

Multiple safety systems on my 2019 Honda Odyssey Elite have simultaneously failed, including LKAS, ACC, Collision Mitigation Braking System, Auto High-Beam Headlights, and other driver-assist features. The dashboard displays warnings for nearly every safety-related system, and the vehicle disables these features without warning. **Detailed Description of the Problem:** Whenever I start my vehicle,
... multiple error messages are displayed related to the Lane Keeping Assist System (LKAS), Adaptive Cruise Control (ACC), Collision Mitigation Braking System (CMBS), Road Departure Mitigation, Auto Headlights, and other safety features. These warnings appear together and persist even after restarting the vehicle. Apparently, this issue is widespread among 2018–2020 Honda Odyssey owners, with many reporting the same cluster of system failures. The problem appears to be related to a malfunction in the vehicle’s electrical system, sensors/ cameras, or control modules. In my case, the failures occur without any external damage, weather event, or driver action that would explain the sudden loss of safety functionality. When these systems fail, the vehicle disables critical safety features such as automatic emergency braking, lane keeping, adaptive cruise control, and automatic headlights. This creates a significant safety risk, especially at highway speeds or in low‑visibility conditions. **Frequency:** The issue occurs whenever the vehicle is started, affecting my driving everyday.Read more
