Possible malfunction involves vehicle speed control and braking during a low-speed parking maneuver in my 2024 Lexus NX350h. While slowly pulling into a parking space, I was actively applying the brake and creeping forward. My safety and the safety of others were put at serious risk because this occurred unexpectedly while parking. If a pedestrian, fixed structure, or another vehicle had been in front of me, this could have caused severe injury or worse. Thankfully, I was not physically injured, but this was a traumatic event, and I was terrified. The problem has not been reproduced by me after the incident. At the time of this report, it has not yet been confirmed by the dealer or an independent repair facility. I have requested a full diagnostic inspection and review of all available stored data before any codes are cleared. The vehicle has been delivered to the Lexus dealer for inspection. The incident was also immediately reported to my insurance company. I do not know if Lexus manufacturer representatives or other parties will inspect it. I did not observe any warning lamps, messages, or unusual symptoms before the event, and this had never happened before.Read more

From this point forward the car drives in “low engine power mode” even with repairs completed and codes cleared. 2- 2 years later the same part “brake vacuum pump” exploded again. Another $1000 repair. At this time we discovered that the fine particles that were not removed from motor has now knocked the timing off by 8%. Also it has caused a knick in a seal causing an oil leak under 100k miles $1800 repair. After these extensive repairs…… The car emits gas fumes into cabin. The smell is so strong it can’t be parked in the garage because it circulates through the house. Even when car is not running. The Car can’t be driven because risk of fire or explosion from gas. Fuel system gets cleaned. Injectors replaced. The car temporarily runs without the gas smell. 2 weeks later the gas smell returns Come to find out the fuel pump failed. Search online and find there is a recall for fuel pumps in this same make model and year. But not this vin number. The car has been a headache from day 1. Recall the whole car at this point, to have so many faulty parts in such major systems of a car is crazy.Read more

They claim our warranty is expired, but our vehicle only has 37,638 miles and should be covered under the 8 year, 120,000 mile warranty that covers a major malfunction like this. Diagnostic code U1126 was found — indicating the left rear short range radar sensor had lost communication on CAN Bus 8. The sensor module was on national backorder, keeping the vehicle out of service for approximately two weeks. The module was replaced and programmed on March 7, 2026. Safety concern: Unexpected activation of automatic emergency braking with no objects present creates a serious collision risk from following traffic. This failure occurred on public roads during normal driving. Additionally, the U1126 CAN bus communication fault is the same vehicle communication architecture implicated in the battery/electrical failure documented in Repair Orders #558923 and #559167, suggesting these failures share a common root cause in the vehicle’s electrical communication network rather than representing isolated component failures.Read more

While driving at highway speeds, I needed to accelerate hard in order to pass another vehicle and while accelerating there was a downshift and increase in RPM. I pulled over and initially shifted to neutral, but the engine was still revving high (4000 rpm observed). Then I placed the car in park which did not change the high RPM. These issues make us feel the vehicle is unsafe and not reliable to drive. The recalls and issues have also affected our ability to use the vehicle as intended and have reduced the value of the vehicle. We are concerned that this vehicle may have a safety defect related to the engine or hybrid/electrical system that could cause loss of power, steering, braking, or vehicle control. We have been told there is currently no fix for these issues and that we should continue driving the vehicle, to park outside and not charge it. This makes us feel the vehicle may be unsafe and unreliable to drive. We are making monthly payments for a vehicle that does not behave as intended.Read more

On 2/25/26, in rush hour traffic we were driving on a NJ highway. We happened to be in the left-most of 3 lanes with a concrete divider (no shoulder – there was about 3 ft between the concrete divider and the left side of the left lane). ... not engage; they locked. The car skidded; it felt like we were on ice, but the road was clear. We were upon the car in front of us and had no place to go. I held the steering tight and tried pumping the brakes to disengage the lock. To no avail. I was planning on scraping the divider on my side to slow the car down. I steered the car into the tight space next to the divider thinking it would cause less injuries as opposed to plowing into the back of the stopped car in front of me. We slid at least 4 car lengths before traveling past 3 stopped cars in succession, I just barely missed hitting the concrete divider and/or the cars in the lane in front/beside me. On the left side, I could see that my mirror was over the divider and on right side we were within an inch or two of a hit. Realizing that I miraculously missed hitting anything, I tested the brakes going only a couple of inches and the brakes did engage. I slowly moved across the lanes to get to the right shoulder. Through all of this, the car indicated “Everything OK”. On 2/26 the car was towed to the dealership. The dealership technician and BMW representative easily replicated the brake failure. Through their testing they determined that the Integrated Brake Module, the same part that had be replaced in Nov 2025 for a recall, had failed again. Before, during, and after the incident, the car indicated “Everything OK”. After repeated requests, neither the dealership or BMW rep gave us any information about why the car failed to warn us of the problem or assurance us of its safety.Read more

Despite that, the dealer refused meaningful diagnostic work and later returned the vehicle by flatbed. Since that return, the front brakes appear seized/engaged. If the vehicle is placed in drive, it remains stationary. If throttle is applied, it moves only slightly while squealing as though the brakes are locked on. The vehicle is not safely drivable and has remained inoperable since 01/08/2026. This created a safety risk because the vehicle was returned for use on public roads despite violent shaking, burning odor, and what appears to be brake seizure/drag. As several brake recalls have recently been fixed inclding replacing leaking brake lines and brake lights. While there are currently known recalls on certain Ford Fusion brake systems (such as brake hoses, brake lights etc), these recalls do not appear to cover critical components such as the vacuum pump, brake booster, hard brake lines, and camshaft. These omissions leave a gap in addressing a potentially life-threatening safety risk. The dealer’s incomplete diagnostic leaves me and others exposed to a dangerous failure. I urge NHTSA to open an investigation into these brake system components to ensure that a comprehensive recall is considered. The repeated failure, despite previous repair attempts, makes it imperative that all related brake components be thoroughly examined, as this is a life safety risk. On [XXX], while driving my 2023 Jeep Grand Cherokee L on the [XXX] , at approximately 65 mph, a ring (like a washer) detached from the brake pedal. This caused the vehicle to suddenly lose braking capability. ... ard, and the vehicle required immediate assistance. The brake pedal did not respond normally to pressure. I had to downshift rapidly to slow down and exit the freeway safely. Once in a safer area, I was finally able to stop the vehicle by applying a lot of pressure to the pedal. This dangerous failure put myself and other drivers at serious risk of an accident. Due to the severity of the issue, I immediately called a tow truck and had the vehicle taken to the authorized Jeep dealership in Tustin for repairs. I had previously experienced another major repair (rear axle joint binding) that took approximately 64 days when the vehicle had around 30,000 miles. The current incident occurred at approximately 52,000 miles. I reported this safety issue to Jeep Customer Service and received Reference Number [XXX]. The WABCO EBS Trailer Modulator (Part No. 4801070010), a 12-volt RSC valve installed on dry van trailers, intermittently retains residual pneumatic pressure in one delivery port without any active brake command. This causes a single wheel-end brake chamber to remain partially applied during highway operation, creating a sustained brake drag condition that is completely invisible to the driver —
... no warning lamps, no fault codes, no dash alerts, no detectable cab vibration or smell during early heat buildup. Over the course of normal highway operation, frictional heat accumulates at the affected wheel end beyond the thermal capacity of the drum and brake shoe assembly. The result is brake-shoe ignition and a trailer fire at highway speed, with no opportunity for the driver to detect or respond before flames are established. To date, 17 trailer fires have been reported as a result of this defect, resulting in loss of the trailer unit and sometimes full cargo. Approximately 1,200 trailers are believed to be equipped with this valve. All affected trailers have exhibited excessive brake wear consistent with chronic brake drag caused by the same pressure retention fault. Separate incidents have also resulted in severe wheel-end overheating, destroyed brake drums, seized components, and damage to axle hardware without progressing to a full fire. Third-party brake timing tests confirmed the anomalous pressure retention is isolated to the valve delivery port — not downstream brake hardware — and changes when electrical power is applied to the valve, implicating the internal solenoid or ECU mechanism. Independent inspections confirmed all other brake components were in proper condition, isolating the defect to this valve. The valve manufacturer has been contacted. Furthermore, when I contacted Tesla Service, they demonstrated extreme negligence by instructing me to drive the vehicle in this lethal condition to their service center. They also initially refused to cover the towing and repair costs, claiming it was not a factory defect despite the clear evidence of fastener failure (improper torque or thread locker application during manufacturing). This is a clear manufacturing defect that poses an immediate threat to the driver, passengers, and other road users. At around 9000 miles, the dealership had to replace the front rotors and brakes due to rotors cracking and warping. Now at 22,000 miles I’m having the same issue and I only do normal driving. There is obviously something wrong with the breaking system in the front of this vehicle. when I press the brakes, the steering wheel shakes.

Received an error on the dash for Service Electronic Braking System and ABS System. Took to a mechanic and it is a failure of the Anti-Lock Brake Module. This is the same failure they are recalling other Ram trucks for already. I am not sure why the Ram 1500 is not included in the recall also.
