Our office has received several notices for a recall on this vehicle regarding the Anti-lock Brake system module. The first notice was post marked 6/3/24 with recall code Z48/NHTSA 22V-426. The second notice was for a 94B/NHTSA 24V-838 recall postmarked 12-4-2024 and another employee took it in to a dealership on 2-12-2025 to be repaired. We have received two more notices, one in January 2026 and
... another in April 2026 with the same recall code. I called the dealership both times and they claimed both times, to not have parts for the recall. This is a federal vehicle used for public service work. We would like to get this resolved so that we do not have any safety issues in the workplace.Read more

This is a supplemental filing to my existing NHTSA complaint #11715308. I am requesting that the Office of Defects Investigation open a formal Preliminary Evaluation into the 2024 Ford Maverick Hybrid braking system. Since my original complaint, the following has occurred: Ford Motor Company denied my claim on April 2, 2026 through a three-sentence letter from a paralegal at Erskine Law PC in Mi
... chigan. The denial addressed none of the evidence submitted including repair orders, fault codes from Ford's own OASIS system, a recorded call proving warranty claim suppression, and a notarized sworn statement. The vehicle now has 107,982 miles. The ABS defect remains unrepaired after four attempts and over 50 days out of service. Active fault codes C052E and U0418 were confirmed in Ford's OASIS system when the vehicle was returned unrepaired on January 28, 2026. Ford has already issued Recall 25V787000 for brake booster ECU overheating causing loss of brake power assist in Maverick vehicles, demonstrating prior corporate knowledge of braking defects in this model line. There are currently 149 brake-related complaints in the NHTSA database for the 2024 Ford Maverick with no open ODI investigation. On [XXX], the braking defect caused a collision (APD Crash Report [XXX], no driver error assigned). I have a notarized sworn statement linking the collision to the unrepaired defect. I have concurrent active complaints with the NM Department of Justice (XXX), BBB AUTO LINE (XXX), FTC (#XXX), CFPB (#XXX), and Ford's internal SpeakUp ethics program (#XXX). I am formally requesting ODI review of the 2024 Maverick Hybrid braking system given the volume of complaints, the existing recall on related models, and Ford's documented refusal to repair my vehicle despite confirmed fault codes in their own diagnostic system. The contact owns a 2019 Chevrolet Equinox. The contact stated that while her husband was driving approximately 45 MPH and exiting the parkway, the brake pedal was depressed but remained depressed to the floorboard. The contact's husband attempted to pump the brake pedal, but the vehicle was unresponsive and failed to slow down as intended. The contact's husband then shifted to neutral(N), and the

The vehicle was pulled over to the side of the road and towed to an independent mechanic, where it was diagnosed that the brake booster pump or vacuum pump had failed and needed to be replaced. I am reporting a suspected brake system malfunction. About one block before my destination, I stopped at a traffic light and applied light pressure to the brake pedal. The vehicle slammed and jerked to a stop unexpectedly. Moments later, while turning into a parking space, I pressed the brake pedal again and the vehicle did not stop as expected. The SUV went over the cement parking post and struck
... a tall sign about 10 to 12 feet ahead. A large bush around the sign slowed the vehicle before it could continue into a tree. This created a serious safety risk to me and my passengers, who included one adult and two young children. Thankfully, no one was seriously injured. The suspected failed system is the brake system. The vehicle is available for inspection upon request. It was towed after the incident because I did not feel it was safe to drive. At this time, the problem has not been reproduced or confirmed by a dealer or independent repair facility because the vehicle is pending inspection. The vehicle has been reported to the manufacturer, and an insurance claim has been opened. UNKNOWN whether any electronic braking or driver-assistance system also contributed. I did not observe any warning lamps or messages before the incident. The main symptom before the crash was abnormal brake behavior at the traffic light, when light brake pressure caused the vehicle to slam and jerk to a stop unexpectedly.Read more

He stated they would do the recalls and then, if needed, they would perform the diagnostic. Later that day Raul called me and stated the recalls were completed and no issues were found. Several items were found from Kia's point inspection, but none related to the issue at hand. I then asked Raul if they could perform the diagnostic to know what was wrong with my car and he stated that they could do the diagnostic on Monday, 4/13/26. Raul stated he would give me a call around 10 to 11 am, to discuss any issues. Raul called at 10:49 am, and stated no lights on the dashboard have come on and he didn't want to do the diagnostic because the lights weren't on. I have a 2024 Mazda CX-90. I want to report that about two years ago I was contacted regarding a recall (or possibly multiple recalls). The steering would become stiff while turning on curves. I took the vehicle to the dealer, and they reprogrammed something, which fixed the issue for a few months (I don’t remember exactly how long). However, the same problem happened again about three times. I
... brought the vehicle back to the dealer on January 23, 2026, and they did not have the necessary parts to repair it. I told them I was very afraid to drive the vehicle in that condition because the steering would get stuck when turning on curves, and I was worried that one day it could cause a serious or even fatal accident. The dealer told me they could not do anything at that time. It took 65 days for them to replace the parts, and it was very stressful driving under those conditions. Even after the repair, the vehicle does not feel right. I feel that the steering still moves abnormally, and I am very afraid to drive it like this. Additionally, the brakes sometimes make a strange noise. I don’t know what to do about this situation. I would like to return the vehicle, but I don’t know how, especially since the dealer is no longer responsible. I need help. I am reporting a safety defect on a 2022 Forest River Rockwood Mini Lite travel trailer, model 2516S, This RV is equipped with Dexter axle #1 and #9 from a total batch of #288 axles that Forest River recalled on July 27, 2022 under NHTSA recall 22V-558. The RV was purchased on July 15, 2022, twelve days before the recall. Of the 244 recalled RVs with those axles, only the single-axle tent campers
... were recalled because the defect was visually obvious on those units. On tandem-axle RVs such as this one, the incorrect installation was not visually apparent. This travel trailer was manufactured with brake axle defects and buyers of these units were never contacted. It does not take an engineering analysis to conclude that if axle #1 and #9 from this batch are defective and 244 RVs were recalled, then all 288 axles should have been inspected and included in the recall. The brakes are significantly ineffective in forward motion, causing complete brake failure under normal braking. This is an active safety hazard. There is serious concern that the remaining 44 axles from this batch were placed on tandem-axle RVs and released into commerce without notification, despite the fact that I have not changed my phone number, address, or email since purchasing this trailer. This is a one-owner, low-mileage unit that has never been serviced or altered. Attempts to resolve this issue through Forest River representative dealerships, Dexter Axles, and Forest River have been ongoing for three months with no resolution. No repairs have been made. The trailer is currently being towed over 1,300 miles to Montana for work with defective brakes. Multiple photos have been attached clearly showing the incorrect brake installation. Please investigate this latent manufacturing defect and consider expanding the recall to include these excluded axles.Read more

... t’s an 80 degree day no rain. After 3 minutes the lights go away, I am driving very cautious, I’m not sure why the both lights came on. The lights come on a second time. When those lights come on the car did not want to drive. It felt as if the breaking system either kicked in or thinks it should. I started looking into what it could be with the help of father. We changed speed sensor and wheel bearing of the front right wheel tire. I had a friend run his own OBE reader and it had signaled the issues was coming from that. Took it for a test drive and at a green light both traction and ABS lights come on and I stuck there unable to accelerate. At this point traction control and ABS light come on and stay on for half the day. I’m no longer driving the car. I finally come to terms that I have to take it to a mechanic. They replaced a module. The night I picked up my vehicle from the Dodge dealership in Bessemer, Alabama, after the Door Control Module was replaced, the ABS and traction control lights activated. I called the shop the next morning, and they instructed me to continue driving they should go off. The lights went off briefly, but soon returned, prompting me to contact Fiat Chrysler corporate. When cor
... porate directed me back to the dealership, the shop stated they could not see the vehicle for a significant period. I attempted to take the truck to other repair facilities, but they refused to work on it, stating they would not touch another shop's unresolved electrical issues. Operating a commercial vehicle while the transmission and braking systems receive invalid signals—especially after being told to keep driving and being unable to find a shop willing to fix the dealership's error—presents a severe safety risk. The truck is sending "invalid data" to the transmission, confirming a network communication failure that began exactly when the door module was installed. This conflict causes shorts in the ABS pumps and modules. Other Dodge dealerships will not touch the unresolved electrical problems. I demand that Fiat Chrysler resolve these recurring electrical failures i am spending alot of money repairing something that start after they did work to it. Even after i reported it to them. They have several tickets created for this issue. I am writing to formally submit a complaint regarding my 2010 E350 (VIN # [XXX] ). I have taken great pride in owning and maintaining this vehicle, and I want to share my concerns based on my recent experience. I have consistently kept this car well-maintained, following all recommended service schedules and ensuring that any necessary repairs were handled promptly. Up until now, I have not exper
... ienced any significant issues with the vehicle. It has been reliable, well cared for, and in excellent condition. Recently, when the brake line indicator came on, I immediately took the vehicle to a mechanic to have it inspected. I was informed that the brake fluid reservoir was empty and that there was a leak present. Upon further inspection, the mechanic identified that the leak was coming from the rear of the vehicle. It was then determined that the issue involved the rear brake line located near the gas tank. This is extremely concerning, not only because it directly impacts the safety of the vehicle, but also because, based on the research I have conducted, this appears to be a common issue with this particular model. Given that this is a critical safety component, it is troubling that such a failure would occur despite the vehicle being properly maintained. Based on what I have researched, I feel that I have a valid complaint. This does not seem to be the result of neglect or lack of care, but rather a potential defect or known issue associated with this vehicle. As an owner who has taken every step to properly maintain and care for this car, it is both frustrating and disappointing to now be dealing with such a serious problem. I am requesting that this matter be reviewed thoroughly and that an appropriate resolution be provided. I believe it is reasonable to expect accountability for an issue that appears to be both common and safety-related. Thank you for your time and attention to this matter. I look forward to your response and a fair resolution. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more
