Hydraulic brake booster pump abs actuator has been on a service bulletin around 2015 with a definitive timetable. I have had numerous notifications with warning lights that are indicative of the booster pump failure for more than 10 years and every time I bring it to the Lexus dealer they said it is my battery. They replace the battery and reset the computer and it repeats itself over and over. M
... y main concern as this should have been a recall and not a service bulletin; an abs brake failure is a significant defect with possible fatal outcome. The problem has been well documented with numerous Lexus and Toyota customers hybrid vehicles going back to 2005. My vehicle is 2007 Lexus RX 400h with 78,000 miles. I am afraid to drive the vehicle with the issue unresolved.Read more

I am continuing to have warning lights telling me to service Anti Lock brake system and also the Electronic brake system. I took it to a dealership and they told me that the hydraulic control unit and the ABS module was faulty but made no attempt to resolve the problem unless I paid over $6000 for the repairs. My truck only has 122300 miles on it and it has been a constant problem.

On December 19th at about 5pm, I was pulling into a gas station driving at about 10-15 MPH when my brakes locked up and the car started shaking. It felt like the anti lock braking system was activated and I did not have full braking control of the vehicle. I was driving slow enough that the vehicle did finally come to a complete stop. The gas station was flat and the road appeared in good conditio
... n, but is was dark outside and about 40 degrees F. No warnings were observed on the dash or heads up display. No one was hurt in or outside of the vehicle. We have only had the vehicle for a few months (purchased used from Carvana with only 6k miles) and only 70 hours of drive time where the issue has never been observed or reoccurred. Hyundai has not yet been notified but I will file a report with them and ask for an inspection although I’ve read in Hyuandai forums that they are aware of this issue and have no root cause or solution.Read more

Breaks squeal while in reverse.
Incident date: Sep 27, 2024
The car experienced a sudden loss of boost power and, at the same time, and MUCH more concerning and dangerous, a total loss of brake power assist. The brakes were firm (hard), but had ZERO power assist, and I could barely stop the car at idle in my driveway, much less on a highway or open road. I thank God that my son, who was driving at the time, was not in a dangerous situation when this failur
... e occurred. It turns out, a single vacuum pump, mated directly to the camshaft, applies vacuum boost to the turbo (power loss) and brakes (catastrophic brake failure). I replaced the vacuum pump, but the problem persisted, so I had to take it in to the dealership for evaluation. I was told the problem is the bolt that mates the vacuum pump to the cam shears off when the vacuum pump seizes. I subsequently read of many cases of this problem on the Escape, but also on the F-150 (for which there is apparently a recall???) -- really, any Ford with an Ecoboost turbo engine. Additionally, the bolt shearing and vacuum pump disintegrating, leads to metal shavings that destroy the turbo. How this VERY serious road hazard caused by a terrible engineering design flaw (single point of catastrophic failure) hasn't resulted in a MAJOR safety recall for all these Ford Ecoboost engines is totally shocking and beyond me. This is coming from an engineer who has spent 50+ years working on cars. NHTSA has absolutely failed in not addressing this terrible safety and design problem.Read more

Incident date: Sep 1, 2024
Entering a freeway off-ramp at about 65 mph, I depressed the brakes and the brake pedal went to the floor with little resistance. Two error messages were displayed on the dashboard: "Brake System Failure - 62 MPH Top Speed" and "Service ESC". The following dashboard lights came on: Check Engine, Brake, ESC, ABS. I held the brake pedal to the floor and coasted to a complete stop at the intersection
... , then proceeded slowly into a nearby parking lot and stopped. Braking effort did not become difficult, rather it was reduced to a small fraction of normal braking power. An online search indicated that disconnecting/reconnecting the negative battery terminal might resolve the problem, and it did, allowing me to carefully drive home with full braking. The local Chevrolet dealer replaced the brake system control module based on GM document 5081450.Read more

The following information refers to recall notice 19V544000 "SERVICE BRAKES" and "ELECTRONIC STABILITY CONTROL" (recall number same for both notices) by Toyota for the 2019 Rav 4 Hybrid: "In 2019, Toyota issued a recall for certain 2019 Toyota RAV4 Hybrid vehicles due to a faulty brake assist system, specifically related to a potential issue with the brake booster pump which could lead to brake f
... ailure; the 2019 Toyota Prius was also included in this recall." The recall covered the manufacturing period 4/26/2019 - 6/3/2019 and a recall notice was issued on 7/24/2019 for the affected vehicles and owners were notified on 9/20/2019. We purchased our vehicle on 9/21/2019 with manufacture date August 2019 and on 12/19/2024 we suffered a brake failure causing damage to the vehicle and our garage. The Rav Hybrid Limited comes with sensors that will auto brake if it believes the speed/distance is insufficient and the brakes have not been applied. The brakes were self-applied at the time and no sensor action occurred. I believe the above recall should be extended to include August 2019 manufacture and should also include any sensors that involve the braking system and/or stability control system. Currently we are dealing with insurance and Toyota, no appraisal costs as yetRead more

The contact owns a 2018 Ford F-150. The contact stated while depressing the accelerator pedal, the vehicle accelerated at excessive speeds. The contact aggressively depressed the brake pedal and shifted into neutral to decelerate and slow the vehicle to a stop. The failure was intermittent and occurred at various speeds. The contact’s son inspected the vehicle and determined that a fuse related
... to the cruise control might be defective. The contact’s son had replaced the fuses. While driving approximately 10 MPH and coming to a stop sign, the contact decided to come closer to the stop sign however, the vehicle accelerated past the stop sign. The contact attempted to depress the brake pedal several times however, the vehicle failed to stop until approximately a block away from the stop sign. There was a certified mechanic nearby who inspected the vehicle but found no cause for the failure. The vehicle was taken to the dealer who diagnosed that the cruise control fuse had failed. The dealer replaced the fuse. The contact was uncomfortable with the fuse being in the vehicle, so the dealer removed the fuse to deactivate the cruise control. The manufacturer was made aware of the failure and a complaint was filed. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was 90,000.Read more

Multiple systems failed on this unit. Trailer Brake failure—The rear trailer brake wires were discovered to be severed inside the wheel drum from the factory. For the first year of use they were inoperable. This was suspected and confirmed with a thermal camera. The tow vehicle also alarmed “Check trailer brakes” while driving. A dealer inspected them and found the wires severed. This was
... repaired. Suspension failure—the rear driver side leaf spring broke on a paved highway 9 months after purchase. Pulled over immediately to inspect the issue and repair it. On 2 other separate occasions, we found broken wet bolts (after the unit was sitting in one place, unmoved for weeks). Also a cracked equalizer was replaced under warranty within the first year. The front passenger side leafspring also broke on an empty unit. Frame Failure & opening baggage compartment doors, slide out backing out in transit—symptoms of excessive upperdeck flex and frame failure began immediately upon purchase. Baggage compartment doors frequently open in transit. This has been a significant safety risk to the operator as well as other drivers. On more than one occasion the doors open in dense traffic in the middle of a city with nowhere to safely pull over. None of the contents of the compartment has fallen out to our knowledge, but could very easily fall out since none of the doors can be adequately secured. One door was replaced, but the issue not resolved. Upon further inspection, there were severely broken welds in the frame of the unit. The unit has sustained over 22 hours of welding on it to repair and reinforce the damaged frame. However, this has been repaired a total of 3 separate times. There is no guarantee that the repairs will hold moving forward. Also a result of the broken frame, the slide out began backing out in transport. Before leaving, the slide out was fully retracted and measured. Upon arrival the slide would be backed out 0.5-1” from the pre-departure measuremenRead more

I was cough of guard and almost hit a car in front of me at the traffic light. afterwards I just drove very slowly to my house. I checked the errors on my car and did research (I did not go to the dealership because they just extort money from you). I found out that the auxiliary battery failed. I replaced the battery and reset the system and the new battery information and that fixed the issue. I read on the Land Rover forums similar experience from many owners of this car. I believe there is a serious design flow because the low voltage of the auxiliary battery should not have caused the brake booster to malfunction while I drove. Just to mention that this auxiliary battery is an additional battery, (it looks like a motorcycle battery) and the main one is still healthy. And this is a hybrid (PHEV) car. Lots of source of energy to power important modules in the car which should not depend on the small auxiliary battery. Plus, the engine was running, meaning the alternator should be enough to power the modules that failed. I was very scared and happy that it was not my wife who drove the car. While driving in the rain, the vehicle failed to decelerate and brake efficiently. There were two event this morning (12/18/24). 1. Quickly i began applying braking force with the brake pedal, but there was no change in the vehicles speed. Finally I firmly forced the brake pe

It wasn't firm, nor soft just "regular". Knowing that i had distance to avoid collision, i let off the brake then applied again. This time-still nothing. ISSUE TYPE: Safety Concern “Brakes Auto Engage When Driving At ANY Speed and Parking Brake Lever Is Raised” DESCRIPTION OF THE PROBLEM: I inadvertently lifted up on the Parking Brake switch in center console while driving about 30mph, trying to turn off a different switch on the console. This caused the vehicle to slam-on the brakes, narrowly avoiding a rear-end collision. It is labeled as a
... Parking Brake but is also an Emergency Brake (I discovered) that can be triggered by anyone who has their hand around the center console and accidentally raises this switch. This poses a serious safety risk to passengers and causing rear-end collisions! STEPS TO REPRODUCE: 1Drive the vehicle at any speed (cruise, increasing/decreasing) 2Lift up the Parking Brake lever only momentarily and release 3The vehicle immediately slams-on full rear-brakes then releases 4Hold the switch UP for longer period - experience a full-on emergency stop. Poor excuse for Honda's Parking Brake system. Emergency Brake should be located elsewhere on the dash away from console. DEALER SERVICE: Told me this is normal, as lifting the Parking Brake "up" will engage the rear wheel calipers full-on REGARDLESS of speed! This is not normal Parking Brake action and very dangerous. The manual does not point out this is a FULL-ON braking. They say it "will assist in braking". This is just wrong & needs a software correction now. The Parking Brake should never be able to engage when the wheels are turning at ANY speed. We don't need a 2 wheel Emergency Brake for a super-obscure, super-rare brake failure (if ever). CORRECTIVE ACTION: Simple software modification needed... if the Parking Brake lever is lifted (accidentally) AND the wheels are turning it should INHIBIT the brakes from being slammed-on. An Emergency Brake should be in a separate place on the dash. Honda should fix this very dangerous condition to make it a Parking Brake ONLY. It should only engage when stopped (like in a parking spot).Read more

Please read the attached file "prius complaint signed" for a full description. I am the owner of a 2010 Toyota Prius, 2ZR-FXE. The warning light came up on my dashboard of my vehicle, so I took my 2010 prius to AutoZone, to get a free diagnostic. The diagnostic indicated the problem code of “C1256” “Accumulator Low Pressure” for the “Anti-Lock Brake System.” See Ex. A (AutoZone Dia
... gnostic). It is available for inspection. According to the safety recall number “D0H”, 2010 prius model “2ZR-FXE” has a “Brake Booster Pump Assembly which can develop a crack inside the accumulator housing. If this occurs nitrogen gas could leak into the brake fluid and gradually cause a loss of power assist. Under certain circumstances, THIS COULD AFFECT STOPPING DISTANCE AND INCREASE THE RISK OF A CRASH.” (Emphasis added). I have noticed that when I brake hard, IT TAKES LONGER TO COME TO A STOP, WHICH THREATENS NOT ONLY MY LIFE, BUT THE LIVES OF MY WIFE AND TWO [XXX] DAUGHTERS WHO ALSO RIDE IN THIS VEHICLE. According to the safety recall, “Toyota dealers will perform an inspection and, if necessary, replace the Brake Booster Pump Assembly at NO CHARGE to the vehicle owner . . .” See id. The safety recall does not explicitly permit dealers or servicers to charge for an inspection with regards to this defect, according to the information provided me. The estimate, provided by DARCARS recommending replacement of the “BRAKE BOOSTER WITH MASTER CYLINDER WITH PUMP[,]” confirms that the issue appears to be that which is covered by the safety recall. Ex. F.; see also Exhibit B Safety Recall (The vehicle has a “BRAKE BOOSTER PUMP ASSEMBLY WHICH CAN DEVELOP A CRACK INSIDE THE ACCUMULATOR HOUSING . . .”). See also Exhibit E Error Code from DARCARS (“C1256 Accumulator Low Pressure”). Nevertheless, DARCARS refused to waive the diagnostic fee of $150 and would not repair the vehicle unless I paid them “$5,150.” See Exhibit F.
