The issue is my brakes failed on me all of a sudden. My abs light, traction control light, and brake light came on out of nowhere. Suddenly didn't have brake pressure. Had to pump the brakes a few times before the car would stop. Luckily I didn't hit anything or anyone. I brought the car to Prius specialist and they diagnosed it saying it was the ABS Actuator Pump Assembly and Master Cylinder. The
... y said it is quite common in the 3rd generation of Prius'. I contacted Toyota North America and they said they had a recall on the parts but it ended Dec 31st, 2021. I bought the car in 2022 unknowingly. Was quoted around $4000 to repair it parts and labor. This is a safety issue that should not be limited to time. Luckily I was not on a Freeway or anywhere where I could have done damage. This issue is a hazard to myself and other people on the road. Toyota needs to resolve this.Read more

In January, 2024, my vehicle (2023 Hyundai Santa Fe) started shuddering when brakes were applied at high speed, on quick or hard stops, in stop and go traffic, and especially going downhill. I had only been driving the vehicle for about 5 months. I took the vehicle to an independent mechanic, who explained that my front rotors had burn spots. He recommended that I have them resurfaced. I did not w
... ant to pay for the problem to be fixed when I believed it was a factory defect and should be covered by warranty. I asked the Mission Hills Hyundai dealership Service Department to investigate and correct the problem, because I felt that my family's safety was jeopardized by faulty brakes. They showed me the burn spots on the front rotors and recommended resurfacing. They said I could wait for the pads to wear down and do the resurfacing at the same time as the pad replacement. Their estimate for resurfacing was higher than the one from my mechanic. I found a Hyundai Technical Service Bulletin, Number 23-BR-004H, which described the exact defects of my vehicle. I returned to the dealership with the TSB in February, 2024. They replaced the two defective front rotors and front disc pads at no charge. The shuddering was no longer evident, and I thought the problem was resolved. However, 10 months later in December, 2024, I once again experienced the shuddering while braking on the freeway while going downhill and when slowing for stops from high speed. Since I had been told that the new rotors and pads were covered by warranty for 5 years or 60,000 miles, I returned to the dealership on 1/16/25 and asked them to evaluate the brakes again. They said that the front rotors again had burn spots. They once again recommended resurfacing at a cost of $249. I reminded them that there was a warranty on the new parts, they claimed that the only warranty was for the original parts that had been replaced. They agreed to resurface no charge. Please investigate and respond to my complaint.Read more

The issue involved multiple systems, including the ABS system, brake system, and electronic stability system (ESG), along with a significant loss of braking power. These components may still be available for inspection. My safety and that of others were seriously at risk during my morning commute in the Atlanta metropolitan area. The truck lost braking capability, nearly causing a collision with t
... he vehicle in front of me. Later, as I attempted to park, it again lost braking force, and I narrowly avoided crashing into bushes. The collision prevention system also failed to activate during these incidents. The problem was inspected and diagnosed by the dealership after being towed via roadside assistance. They confirmed that the battery caused the failures across multiple systems. This was not the first issue with the battery. About a month ago, the battery suddenly died, preventing the truck from starting. At that time, Chevrolet sent me a message stating that the battery status was critical. After recharging it, the vehicle functioned normally until this recent incident. The first symptoms of the latest issue appeared while I was stopped at a red light. The truck suddenly shut off and restarted on its own. Warning lights for the ABS system and brake system appeared on the dashboard. Once the light turned green and I started to accelerate, an ESG message appeared, followed by another message limiting the maximum speed to 43 mph. When I tried to brake to park the truck, the brakes were slow to respond, making it feel like the truck had completely lost braking power. The collision prevention system failed to activate, increasing the danger. Fortunately, my two babies were not in the truck at the time of the incident. This was an extremely frightening situation for me as a driver. The truck is only six months old, has 37,000 miles, and was purchased new. I believe many of these failures could have been prevented with proper safeguards. While I was lucky to avoid anRead more

Took delivery of vehicle 11/19/24. Had the vehicle 10 days. Let another vehicle out of the driveway & upon pulling back in got "brake system failure" alert and the car would not stop rolling not matter how hard I hit the brake. I had to push the brake button. The "one pedal driving unavailable" alert also came on. Towed to dealership same day(11/29/24). They tell me it's a software update, but the

Brake system failure, one pedal driving & service stabilitrak alerts come on while going to leave my house. Towed again the next morning(12/16/24). Brake booster ordered 12/17 with no ETA. 2 days go by with no communication from them. I ask what's up. Still no ETA. I mention lawyer on Monday the 22nd & wow my part is there on Tuesday! Tells me car will be fixed by 12/27. That day comes only for them to tell me their system isn't working again to program the car. I file a case with Honda. I hear nothing from the dealership again until 1/8/25. I filed lemon law on 1/7. I hear nothing from service again until 1/13 just more excuses & about engineers coming to look at the car and ordering a new module. This is how becoming highly reported failure among GMC/Chevy vehicles with shaft driven vacuum pump design between 2019-2022. GM has stopped using this design on 23 and newer models. I brought the vehicle in to have recalls addressed on 12/30/24. Starting the next day we noticed the car was weaving without any steering, but thought it might have been icy road conditions and waited to see if it reoccured. The car has since weaved intermittently, but has righted itself after releasing the accelerator. However, on 1/15/25 while traveling about 50mph, the car started weaving on it
... s own and when the accelerator was released the car started to wildly fishtail. Called dealer and they agreed this was a serious issue and agreed to see car the next morning. Service tech said when they drove the car it did the same thing and that the troubleshooting recommended I change the regenerative braking setting to "smooth" and sent me on my way. I did not get even a mile before the car began weaving rapidly again on its own. Went straight back to dealership where manager wanted me to drive the vehicle again with the tech in car which I refused as it is not safe to drive. So he said if I was ok with it tech would take car out again but if it was just acting how regenerative brakes work, there wasn't much else they could do. When manager returned to talk to me, he said he ended up going with the tech and it did replicate the weaving action two times during that drive. The car is steering on its own weaving wildly left and right and imposes a great danger and no longer feels like a safe vehicle to drive.Read more

Driving down CR 230 in Wyoming, the situation started if I have to estimate, 7 miles west of Woods Landing, I heard a metal on metal grinding noise from the engine bay, and a loss of power on the throttle. I look at my RPM gauge and notice it is a zero. No check engine light, all electronics working as normal along with the brakes, and I slow the vehicle down to 30 MPH. Shortly after noticing, I g
... et a "Check Brake System" flash on my instrument panel cluster, also the light which illuminates for my parking brake turns on, and I lose power brakes, the brake pedal turned extremely stiff. I immediately look for a pull off zone, but because I had lost brakes I did not want to risk keeping the vehicle moving. I get the vehicle stopped on the side of the road, and instantly put it in park along with the parking brake and shut the vehicle off. The vehicle would not start or crank, just a flash across all of the electronics, the lights go dark, then illuminate again as if normal. This is an extremely dangerous situation, the vehicle had not notified me that the motor had stopped turning; not a check engine light or ring at all, and shortly after that the power brakes stop working. This is very dangerous for motorists because of the lack of notification from the vehicle, and the incredibly weak breaking ability. If the vehicle had been moving at highway speeds, any sort of traffic, I could've ended up injured or dead, and even worse, killed another driver. The vehicle was towed to the nearest town where it would be inspected by a dealership; they reported to have found coolant in the oil and that the vehicle needed a new motor. No other details have been confirmed, and the problem has not been reproduced by the dealership. It is unknown if the dealership inspected the braking system.Read more

Yes, my vehicle is available for inspection upon request. When I go to put my 2019 GMC Acadia in "park" the shifter states it is in park & the dashboard shows it in "P." However, when I turn the vehicle off, an error message comes up reading "shift to park" and my car wont fully shut off. You can feel the car move forward or backwards a tiny bit (depending where vehicle is parked). This makes me
... have to turn the vehicle back on, shift into "drive," back into "park," and then shut the engine off and pray the "shift to park" display doesn't come up. This process was repeated this morning when I arrived at Walmart 3x before it finally shut off, then when I got home it happened at least 8x before I could safely leave my car in my garage without the worry of it not truly being in park. This has gotten to the point I have bought wheel chalks to put around my car tires because I am afraid this vehicle could roll down my driveway into my neighbor's house or kill some kid playing in our culdesac. I let the last place I was at know & they told me GMC was aware of the issue, but that there was no way to fix it (Auto Revival in Justin, TX). Hello. I have a 20/20 Nissan Leaf SV Plus. This morning I had a total brake failure for lack of a better term. The E-Pedal system, which I always use, suddenly failed while I was driving, showing a warning on screen that said E-Pedal system malfunction... Please use pedal to break. When I went to use the pedal to break nothing happened except for this horrible sound and feel in the pedal. It barel
... y just finally grabbed a little bit enough to stop before I hit the car ahead of me. But it was nothing short of a total brake failure, both E-Pedal and manual pedal. Extremely distressing. What I want to know is how on earth when the E-Pedal fails, the normal manual brake pedal doesn't work either. One thing to note, I let it sit almost all day while I was at work, went out and tried it, and it suddenly was working. I drove around the parking lot, turned it off...back on ..and then The E-Pedal warning came up again. I then went and purchased a 12 volt battery, as I read in some forums that it's possibly due to that. That also did nothing. Got the same E-pedal malfunction. The main reason I'm letting your agency know, is because I would consider this a total break failure, and completely unacceptable. By sheer luck I was only going about 5 mph when it first happened, and I could still barely break in time. Any help on the Federal level...would be great. Thank you so much for your time.Read more

I was able to look up online and see that there is or was a class action lawsuit and that it's a known problem with many Nissan armadas being the master solenoid. This seems to be a continuing safety issue and if someone were to have to come to a quick stop there's no warning that this is going to happen beforehand and would negate any kind of safety stop. This has been going on for many years and is extremely expensive to fix. Which puts it right out for most families to be able to afford fixing this and just having to lope along hoping that it isn't one of those times when the brakes are going to give out in an emergency. Given that Nissan contacted me with a lawyer shows me that they are afraid of what could happen but not yet concerned enough that enough people have died for it to be worth their while cost-wise.Read more

... me because I thought I was going to be pushed into my garage wall. These incidences do not happen every time but they shouldn't be happening at all. I understand the hybrid aspect of it with the brakes and expect a small sensation but to be lurched forward so much is completely unsafe. I initially told the dealership about the brake sensation shortly after punching the car in 2021 and was told there was nothing wrong. I now see hundreds of people posting on social media with the same problem. Toyota needs to do a recall and have this fixed. There are no warning lights or messages that appear when the problem takes place.Read more

I am hesitant to drive it to an independent service center cause I don't know if the brake system will completely fail. In researching the warnings I found this to be a common issue with these trucks and found an NTSA recall for the same warnings for the truck but for different years.Read more

Location: [XXX] Description of Incident: On a rainy morning, while driving to work, I encountered a sudden lane cut-off by a speeding truck as I exited a school zone. I applied the brakes to avoid a forward collision, but the brake system failed entirely. This resulted in the loss of vehicle traction, causing the car to spin out uncontrollably. The vehicle collided with an island, bounced into
... oncoming traffic, and sustained severe damage, including: •Tires ripped off the rims •Front axle damage Injuries: I sustained a lightly sprained neck as a result of the incident. Vehicle History: The vehicle had been in the custody of the Hyundai dealership just days prior to this incident for an engine/powertrain recall and replacement. The dealership held the car for over a week to address the issue. However, shortly after retrieving the vehicle, the brake failure occurred. Observations and Concerns: •The brake failure occurred after recent service by the manufacturer dealership. •Hyundai vehicles are known to have documented brake system issues, which may have contributed to this incident. •This failure nearly resulted in a life-threatening accident. Action Requested: I am requesting an investigation into the service performed during the recall and the brake system failure. It is imperative to determine accountability and address any potential defects to prevent future occurrences. [XXX] [XXX] - Victim and incident reporter. When pressing the accelerator from a full stop, the car would either not move at all, idle/roll forward, or unsteadily jerk forward. In addition, the car would brake while driving despite not applying any pressure to the brake pedal. This issue persisted regardless of "drive mode" and no cruise control was in use.

The contact owns a 2019 Jeep Cherokee. The contact stated that while driving at various speeds, the brakes would activate causing the vehicle to stall and shut down immediately without any warning lights illuminated. The vehicle was taken to the dealer who determined that oil was leaking from the vehicle. The oil level sensor had been replaced but the failure persisted. The contact stated that whi
... le reversing, the vehicle made a bumping and thumping sound underneath the vehicle. Additionally, the brakes were squealing while the brake pedal was depressed. The contact also stated that several attempts were needed while attempting to start the vehicle. The vehicle was returned to the dealer, and the contact was informed that the brakes might not have been properly installed on the vehicle. The brakes and the battery had been replaced; however, the failures persisted. The contact stated that on another occasion while driving approximately 25 MPH, the brakes unintendedly activated, causing the vehicle to stall. There was no warning light illuminated. The contact became aware that antifreeze was leaking from the vehicle. The contact called AAA and the vehicle was jumpstarted after several attempts. The contact pulled over to the side of the road and had the vehicle towed to the residence. The contact stated that the failure was persistent, and the vehicle needed to be jumpstarted regularly. The contact stated that the vehicle would only drive for short distances before stalling and shutting off. The failure had occurred 3 times. The vehicle had not been diagnosed or repaired. The manufacturer was made aware of the failure. The failure mileage was unknown.Read more

The issue appears to be related to the vehicles electronic parking brake for manual transmission vehicles only. When stopping the car and with the car in neutral and engine running, the parking brake is applied per standard operating procedures. However sometimes the parking brake will disengage itself, seemingly at random. This caused a rollaway issue for me twice thinking that the car was in "p
... ark" when it wasn't. This was also verified by a Toyota technician during a routine service appointment who documented the issue. There appears to be some chatter about this online with other owners complaining about the issue here: [XXX] Another person on YouTube also documented the issue here: [XXX] I believe this is a very serious issue as a rollaway vehicle can be extremely dangerous for pedestrians.
