What component or system failed or malfunctioned, and is it available for inspection upon request? Answer: The fuel pump assembly failed. The component is still in the vehicle and is available for inspection upon request. 2. How was your safety or the safety of others put at risk? Answer: The engine stalls or loses power unexpectedly while driving at highway speeds. This creates a high risk of a r
... ear-end collision, as the vehicle becomes a stationary or slow-moving hazard in active traffic without warning. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Answer: I have experienced the failure repeatedly. Although I contacted a Nissan dealer, they stated they could not verify or perform the repair because my VIN is not currently listed in the R21B8 recall database, despite the vehicle exhibiting the exact symptoms described in that campaign. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Answer: No, it has not been officially inspected by the manufacturer yet because they claim the vehicle is not part of the current recall population. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? While traveling on the freeway after approximately 45 minutes of driving at 65 mph, the car suddenly felt like it was down shifting causing it to slow substantially. When I tried to accelerate to avoid cars behind me from hitting me, the car prevented me from speeding up for approximately five seconds. RPM is over 5000 b4 it allowed speed up. This happened on two separate occasions,. The car woul
... d not start after the second trip and was towed to a Ford dealer for repair. I was told I needed a new alternator and battery and what I experienced on the freeway was the alternator impacting the transmission. I picked up what I thought was a repaired vehicle, traveled to one of the same places I had gone previously and approximately 45 minutes into my drive, the same event took place with the car. Slowing severely and not allowing me to accelerate to return to freeway speed for approximately five seconds. It was taken back for repairs shortly thereafter. I was told it wasn't the transmission. It was a sensor in the brake system that causes the brakes to be applied just enough to create friction that heats up two of the wheels to such a high temp., the brake pads melted onto the rotors. The master cylinder, pads, rotors, sensors all needed replacement. The car is a 2019, purchased new, out of warranty with only 26,000 miles. Ford has offered to pay part of the repairs but expects me to pay the other portion. This car has been well cared for. Nothing I have done while driving it has caused their sensor to create such a dangerous situation on the freeway. It also started happening before it was driven to the dealership by refusing to move for five seconds every time the car came to a stop. I have read other accounts of the same situation happening to people who own the same model and year. This is a dangerous situation created by Ford's faulty equipment. The car is at Ford dealership waiting to be repaired for the 2nd time.Read more

I am reporting a serious and potentially dangerous braking issue with my 2007 Lexus RX400h. The vehicle currently has approximately 126,296 miles. While driving, the vehicle emits a continuous and intermittent warning beeping sound. At the same time, the braking system becomes unreliable. The brake pedal becomes extremely difficult to press, requiring excessive force, and the vehicle does not res
... pond consistently when braking. In addition, there are instances where the vehicle appears to brake unpredictably or inconsistently, which creates a significant safety hazard for myself, passengers, and other drivers on the road. Based on my research, this issue appears to be related to a known defect involving the ABS brake actuator assembly. Lexus previously issued a warranty enhancement program for this component due to internal malfunctions that affect brake pressure and system performance. However, this issue was not issued as a formal recall tied to VIN eligibility. Despite my vehicle being under 150,000 miles, I have been informed that I may be outside the time-based warranty window, and no active recall is listed for my VIN. This defect directly impacts the braking system, which is a critical safety component. Failures in the brake actuator can lead to improper brake pressure, reduced stopping ability, and increased risk of a crash. Given the safety implications and known history of this defect, I believe this issue warrants further investigation and potential recall consideration. I am submitting this complaint to request that the NHTSA review this matter for safety risk and determine whether broader action is necessary to protect consumers.Read more

The vehicle is registered to me; my mother was the driver at the time of the failure. At ~2,963 miles, the truck developed intermittent check engine and ABS warning lights. The dealer pulled DTC C058A (brake system control module) and P25A2. Per GM bulletin PIT6012A, the battery was replaced and a brake plunger motor sensor learn was performed. The same codes returned within weeks. The dealer then
... diagnosed internal failure of the brake master cylinder and reservoir, documented the vehicle as "unsafe to drive," and replaced both under warranty. The truck sat at the dealer over six weeks due to a nationwide parts backorder. Repairs were completed April 4, 2025. On May 2, 2025, at ~6,500 miles, the driver was traveling downhill on a city street in the rain at roughly 20 mph. The brake pedal suddenly became hard and resistant. Despite full-force pedal pressure, the vehicle could not be stopped. It entered an intersection, struck another vehicle, then struck a tree. Front and side airbags deployed; the vehicle was totaled. The driver sustained a concussion, broken finger, and severe back and knee injuries, was transported by ambulance, and required months of concussion treatment including vestibular therapy. The hard, resistant pedal is consistent with failure of the electronic brake boost assist (eBoost) system, leaving only unassisted hydraulic braking (roughly 4–5x normal pedal force). This matches other owner reports and the class action in Barron et al. v. GM LLC, which cites defective master cylinders with a missing internal cartridge seal on the same platform. The vehicle is no longer available for inspection. The driver was concussed with significant short-term memory loss at the scene; statements in the police report may not reflect the mechanical failure. Upon removing my foot from the brake pedal, the vehicle suddenly and unexpectedly accelerated without any input . The vehicle proceeded forward and collided with my garage door. Notably, no collision warning alerts were issued, and no automatic emergency braking system engaged prior to or during the impact. Following the initial collision with garage door, the vehicle failed to stop and instead continued to accelerate into the garage, striking a refrigerator. The impact resulted in substantial damage to the refrigerator and caused extensive structural harm to the property, affecting the garage, adjacent living space, floors, and interior walls. The severity of the collision triggered deployment of the front airbags. At the time, my wife was seated in the front passenger seat, and my [XXX] daughter, along with her friend, was seated in the rear of the vehicle. We think there’s an issue with the abs module and wheel speed sensor because we started having a problem where the brakes wouldn’t work or would stutter. The car would shake real bad it would make an awful grinding sound but normally only when trying to stop at higher speeds or long braking. The only thing that made it stop was to unplug the abs but then other parts of the car stopped working
... like the speedometer and the sensors around the car. So we got the abs replaced but the new one started doing the same thing. We did some research and it sounds like the same issue Hyundai is having with the palisade. The incident that made us get it replaced to begin with was were driving down the road and we got to a traffic light but when we tried to brake they just didn’t work so we drove through the red light luckily the one person who was supposed to be going saw us and didn’t pull out in front of us so we didn’t get hit. We haven’t taken it to the dealership yet but my sister who teaches automotive repair at the community college is the one who diagnosed it so she said we should report it here so maybe it won’t happen to anyone else. Before we had the brake issue we were having problems with basically everything on the dash not working randomly. We’d be driving down the road and the speedometer and the outside sensors and the gas gauge and the traction control all would go out at the same time it’s the same stuff that went out when we had the abs unplugged so it seems that might be related.Read more

Vehicle: 2022 Mercedes-Benz S580 The vehicle is experiencing a severe and violent drivetrain "jolt" or "push" during gear transitions, specifically when shifting from 5th to 6th gear at cruising speeds. On multiple occasions, the jolt was so forceful that passengers believed the vehicle had been rear-ended by another car. This behavior is unpredictable and creates a significant safety hazard, as
... it causes a sudden change in vehicle momentum while driving in traffic. The issue has been reported to the authorized dealer (Fields Mercedes Lakeland) multiple times. The dealer kept the vehicle for over 40 cumulative days and performed motor/transmission mount replacements and "software calibrations," yet the violent shifting persists. Additionally, the engine produces an excessive vibration and loud noise during extended operation in high temperatures, which ceases only when the ECO/Auto-stop mode engages. I am concerned that the transmission or torque converter may fail abruptly while at highway speeds, potentially leading to a loss of propulsion or a rear-end collision due to the unexpected deceleration/jolt behavior. The vehicle is suffering from chronic oil consumption issues. The 'Check Engine Oil Level (Add 1 Quart)' message has appeared twice: once on 10/31/2025 and again on 04/17/2026. This is despite the vehicle being exclusively serviced by Mercedes-Benz Lakeland. This recurring engine issue, combined with the violent drivetrain jolts and shifting failures, significantly compromises the vehicle's safety and reliability."Read more

I was driving on the highway in heavy traffic going about 65 mph. An unusual symbol appeared on the dash in red that I don't know what It was or what it meant but I thought it must be something serious. Then a warning came up saying for me to take a break. I can't even imagine what would have happened if I was in the middle or far left lane. I was not unobservant and both of my hands were on the steering wheel when this happened. Once the vehicle stopped, the symbol on the dash disappeared and the brakes released. I was able to resume driving. Although there was no accident, injury or fire, and no police report needed, this does not discount the seriousness of this event. This could have been life-threatening if I had been in a different lane or had panicked, causing a multi car pileup in the heavy traffic. This is not a failed component, the issue is a design interaction issue. This truck when it gets stuck as it unfortunately very easily does even on wet grass goes into a traction control mode where the throttle is VERY damped but the issue is not only is the opening very dampened the closing is also VERY damped. This is made significantly worse by the fact that traction control cannot actually
... be disabled. There is a switch and it indicates it is disabled but the functionality remains the same. This combination means whenever you're working to get the truck unstuck, like yesterday when it was on wet grass you have to expect that the vehicle will suddenly accelerate as soon as it finds traction. normally you can counteract this by expecting to apply significant brakes as soon as it finds traction but if the front wheels are still on a slippery surface the ABS system will lose it's mind and essentially disable brakes as the vehicle accelerates violently backwards with the new found traction. This combination of software decisions makes it so you can very easily replicate an unintended acceleration scenario where the vehicle jumps back a solid 20' when this happens, not respecting any brake input given by the driver and certainly not respecting that the throttle was released long ago. The throttle closing should not be damped when traction control is running and the ABS system should not be behaving the way it does. I understand getting the vehicle unstuck from wet grass is not normal operation but that's what the traction control disable button should be for and Ford makes that button useless also. When taken together, all these engineering decisions make for a very unsafe situation that has zero reason to exist. I've attached pictures that show the overview of the situation as well as the characteristic ABS marks showing that it was disabling brakes.Read more

The vehicle exhibits unintended acceleration behavior when the brake pedal is applied. Instead of slowing down, the engine revs and the vehicle does not decelerate as expected. On March 16, 2026, while merging onto a highway and applying the brakes, the engine suddenly revved to approximately 3,500 RPMs while the vehicle failed to slow down. There have been previous issues with the vehicle not sl
... owing when breaks are being applied, nut this is the first instance when it was noticed that the RPMs increased while breaking. This issue has also been reported to the dealership, with service documentation noting that when applying the brakes, the engine revs and the vehicle does not slow or accelerate properly. This creates a dangerous condition where braking input does not result in expected deceleration and may instead cause acceleration-related behavior. The issue has not been resolved. This issue significantly increases the risk of collision and loss of vehicle control.Read more

While driving at various speeds, including highway speeds, the vehicle intermittently fails to slow down when the brake pedal is applied. On multiple occasions, the brake pedal was pressed but the vehicle did not respond appropriately and continued moving forward. On December 1, 2025, while traveling at approximately 65 mph on the interstate in rainy conditions, the vehicle began making a grindin

On December 22, 2025, the issue occurred twice in one day. While attempting to brake at a stop sign and in traffic, the vehicle did not slow down, nearly causing collisions with vehicles in front and adjacent lanes. In the instance where approaching the stop sign where a vehicle was stopped, the dashboard flashed “break” while I was breaking as hard as I could. Warning lights and sensor-related alerts have appeared intermittently prior to and during some incidents. The issue has continued intermittently, including additional incidents where the brakes were either unresponsive or overly sensitive. They do not respond in a consistent or predictable manner, making safe driving nearly impossible. This problem places myself and others at significant risk of collision due to loss of braking control. The vehicle has been taken to multiple dealerships for inspection. The problem has not been successfully replicated, and no repairs have been performed to resolve the issue. Vehicle Information Vehicle: 2024 Toyota Tacoma VIN: [XXX] ate: January 26, 2025 Repair Completed: October 1, 2025 Safety Complaint Description Beginning on or about February 5, 2025, my 2024 Toyota Tacoma developed a serious braking-related safety issue that substantially affected the safe operation of the vehicle. When I brought the vehicle to the dealership, I reported braking concerns and
... was advised by dealership personnel that the vehicle may not be safe to drive due to concerns that the rear brakes might not function properly under certain conditions. I was specifically told by dealership staff that they would not recommend driving the vehicle due to safety risks. Despite this warning, I was forced to continue operating the vehicle because no repair remedy was available at that time and no equivalent loaner vehicle or safe alternative transportation was provided. Over the course of several months, I repeatedly contacted the dealership regarding the issue. Each time, I was told that no repair remedy was available and that nothing could be done until Toyota released a repair solution. I was also advised that my only option was to leave the vehicle at the dealership indefinitely, which was not practical because the vehicle was my only means of transportation. As a result, I was forced to continue driving a vehicle that had been identified as unsafe. The defect remained unresolved for approximately eight months. During this time, I experienced significant safety concerns and loss of reliable vehicle use. Because of the prolonged delay in repair and lack of alternative transportation, I was ultimately forced to purchase another vehicle in order to maintain necessary transportation. The front rotors need machining every 10,000 to 15,000 miles. The rotors experience exceeding distortion due heating by the brake pads. While driving approximately 35 MPH on a 4 lane major roadway, the vehicle suddenly shut off in motion, resulting in a complete loss of power. This included loss of power steering and the ABS system. The vehicle effectively locked up, and a message appeared on the display instructing me to place the vehicle in park. As a result of the shutdown, the vehicle lost all propulsion and came to a complete
... stop in an active travel lane of a high-speed roadway. There was no ability to maneuver the vehicle out of traffic. I could not steer it to safety, restart it, or shift it into neutral. The vehicle was fully immobilized in the roadway. This left me and my family stopped in an active lane of traffic, exposed to oncoming vehicles traveling at highway speeds, creating an immediate and serious risk of collision. At the time of the incident, I had two children in the vehicle. We remained in this dangerous position until police arrived and an hour after. The vehicle ultimately had to be removed from the roadway with assistance, as it was not operable. During the event, there was a total loss of electrical power. The critical safety systems, including brake lights, were not functioning during the shutdown. After attempting to reset the battery, the vehicle continued to malfunction. It would not properly turn on or off, wouldn't shift into neutral and remained unable to be moved. This is the third occurrence of this; once was while driving at highway speeds. All warning lights, including the battery light, were illuminated. The vehicle is subject to an active recall; however, I have been informed that there is currently no available remedy . Despite this, the dealership and Stellantis advised that the vehicle was safe to operate. Given the repeated loss of power while driving, the inability to move the vehicle out of an active roadway, and the absence of an available repair, this vehicle presents a serious safety risk and I should be able to turn this vehicle in with refund.Read more

The car started locking up my steering wheel. I took it to the mechanic and they ran the codes and said it is the ABS control system module. I called the dealer to get the part and they said it is on back order nationally and it may be months before they can get the part. The 2018 model for Chevrolet traverse has been recalled for this exact thing but my model has not even though this is a known i

If it is on back order it is obvious that it is a big issue nationally yet they have not submitted a recall. The wheel keeps locking up every time I drive it for more than 5 minutesRead more

I am reporting a serious safety concern involving my Tesla Model Y. While driving, the vehicle’s main center screen (MCU) suddenly failed and entered a continuous reboot loop. This resulted in the loss of critical driving information and controls, including navigation, camera display, and vehicle system status. The issue occurred without any warning and nearly caused a serious accident, as I wa
... s unable to safely monitor essential driving information. This failure made the vehicle extremely unsafe to operate. The problem appears to be related to a known MCU system failure affecting some Tesla vehicles. I am concerned that this issue may present a broader safety risk for other drivers. Please investigate this matter for potential safety defects.Read more

In August of 2025 I purchased a Chevy MD 5500 from Paul Masse, Inc., in RI. From the time of delivery the truck had a serious shake while operating. Paul Masse, Inc., told me that the tires had flat spots from sitting on the lot prior to purchase. As time went on the dealership placed two new sets of tires on the truck. Neither set fixed the shaking. Paul Masse, Inc., told me that this was how the
... truck operates and that there was nothing that they could do about it. I continued to contact the dealership for further examination of the vehicle for months with no response. I eventually sent a claim into the RI Dealer Commissioners' office. The commission acknowledged my claim and reached out to Paul Masse, Inc. Paul Masse, Inc., agreed to have their Executive VP, Bob Masse take the vehicle for a ride with me to see what was going on. After driving the truck with Bob Masse, he agreed to bring the truck in for further review. I dropped the truck off a day later, Bob Masse called me after his mechanics looked the vehicle over and told me that they would not be able to fix the truck, but they would offer a buyback if I agreed to take a new truck. I agreed to take the new truck. I had to finance a whole new truck, paying for more insurance, sales and registration taxes. The day I took delivery of the new truck it drove exactly as the old truck drove. In addition to the truck shaking like the old truck, the check engine light was on. I immediately called the Bob Masse, Bob told me to drop the truck off and they would have a look at it. After I dropped the truck off to the dealership, they had it for five days. Bob Masse had called me to say that the check engine light had been repaired, but they were waiting on new tires to come in, as they thought the shaking was once again from "Flat Spots" on the tires. Two days later I was notified from the service department that the truck was ready for pickup. There were no new tires on the truck. Bob Masse said no new tires.Read more

My Volvo's XC90 brakes have failed twice. At around 95,000 miles, my brakes stopped working, and then I had them repair it. They charged me close to $3,500, which they said they were able to cover under warranty, and I only had to pay the deductible, which I don't remember. In less than nine months, at about 104,000 miles, the same problem persists. Both times I was driving the car, by God's grac

On Sept.17, 2021 my 2018 Ford Escape rolled away. When I was leaving a driveway, I got in car, buckled my seatbelt, started the engine and put the vehicle in Reverse. I immediately realized I had not paid for the item and shifted into Park. I unbuckled my seatbelt, opened the door, got out of my vehicle, walked fully past the door. At that point, I noticed the vehicle rolling backward on the flat
... driveway. I ran after my car to try to get my foot on brake because I was in a neighborhood with children. I nearly got my foot on it when I was knocked down and the driver-side front wheel ran over me, crushing my pelvis and sacrum and causing other injuries to my abdomen and legs. -I believe my car was in False Park. That is why it didn’t move as soon as I took my foot off the brake and was able to clear the open door. Both my husband and I have been able to get the car back to that false park position where it might stick for 5 seconds or several minutes. I have several videos I can share. - Automatic Electronic Park Brake would have prevented this accident. - Inadequate Warnings 1.The chime for Door Open when in Park is almost identical to the Door is Open not in Park. 2.The warning on the dash is in white and Orange for Door Open In Park and All WHITE for the more dangerous Door Open Not in Park. 3. A warning in the magnitude and attention-grabbing of the Seatbelt off or Collision warning would be more appropriate. 4.Ford did not warn customers until July of 2022 that there was a transmission issue recall. However, Ford sent a technical Bulletin to their Dealer Repair shops that there was a transmission issue in March 2021. I always used my Parking brake on hills but not on flat ground. Had Ford sent me a warning in March 2021, I most certainly would have used it that day and every day. -Quick Reference Guide should include Parking Brake Information and warning not just in the long owners manual. 2023 Toyota Sienna occasionally, without warning, "lurches" forward at low speeds when braking. While putting steady pressure on the brake when coming to a stop at a stop sign or traffic light etc. it feels as if the brakes stop applying pressure and if we wouldn't have quick reflexes to brake harder, we would roll out into the road or into the car in front of us. I believe it is something to do w
... ith the transition between regenerative braking (vehicle is a hybrid) and the hydraulic braking. I have spoken to Toyota, and they don't have any recall. People posting on online forums seem to believe there is a software update (T-SB-0047-24) that helps address the concern. Toyota charges to diagnose and apply software updates as the vehicle is out of warranty. I believe this to be a safety issue and something Toyota should cover. Based on my online search, there seems to be a lot of people with the same issue. This has been an ongoing issue the last couple of years and I'm thinking it should be reported. I have not brought this to a dealer to have diagnosed. These are largely family vehicles and my wife, and I don't feel very comfortable every time it happens.Read more
