There has not been an incident report, however, my brakes failed while driving and I pulled over. This is the fault the SBC System which has an Extended 25 year warranty. I brought it into Mercedes prior to complete failure, but they only replace the Pressure Reservoir. It must be returned to Mercedes for the SBC UNIT replacement.

However, about a week later, the same critical brake failure occurred again. The car would not brake, forcing me to request another tow back to Honda Carland. This time, the dealership claimed the issue was due to a faulty brake master unit, which they replaced—a far more serious defect than initially diagnosed. Given the life-threatening nature of repeated total brake failure, I refused to drive the vehicle and attempted to return it. In researching this issue, I have found similar complaints in online searches, including the NHTSA database, suggesting that this may be a much broader issue affecting Honda Prologue vehicles manufactured in 2024. This is not a minor issue—it is a safety hazard that puts my life and the lives of others at risk. When I was driving and had been relying on the blindspot monitoring and notification and wanted to change my lane there were no notifications, because the blind spot monitoring had stopped working, and I encountered another vehicle in the lane that I was trying to merge; it became very dangerous and now I had to take evasive action to avoid a collision. The driver's door with NFC does not work anymore. He also confirmed that even though my car said it was on 2.2 Software, it was not even partially installed. I called the approved service center in Newport, CA and they informed me they would not be able to work on my car. So, I had to tow my car to Riverside, CA as it's the only other repair shop in Southern California. This incident occurred after I replaced my water pump (24V499000) in Riverside and had to pay $150 out of pocket. I Had My Farmer's Insurance Pay over $19,000 to Have My Car Fixed- it Took 4 mos due to Backordered Parts. Needless to Say, My Car Insurance has Skyrocketed as this is Labeled "At Fault". I Noted the Same Scenario on a NHTSA Complaint #11527971 dated 6/20/23. 2nd Complaint is for the Electrical System and a Complete Lockout on [XXX]. Went to Unlock my car and it wouldn't Unlock. Tried Multiple times, then popped off Key Cover on Handle and used the Valet key to unlock the Door. Once inside, The Car I left the Door open and when I pressed Power, it Wouldn't Start, Nor Shift, but the Steering wheel Would Turn Still. Had to have the Car Towed to Lexus. Moreover, I've noted that partial updates are expected, potentially extending this downtime by another 48 hours. I have had a partial update that has left my vehicle in worse shape than before, what could happen if the process is tried again? This inconsistent performance is not only frustrating, but also poses a serious inconvenience to scheduling and executing such a process. Owner surveys suggest that comprehensive updates, which should resolve these issues, have been successfully completed in under 30% of vehicles which were attempted. Additionally, the alarming figures shared indicate that only around 10% of the fleet has been updated in over five weeks, despite submitting over 800 Vehicle Identification Numbers (VINs). This is regarding an incident with my 2019 Honda CRV, on 2/12/2025. Conditions were dry, clean and dark with street lighting at approximately 6:15 pm Pacific time. I was driving northward uphill on a paved street and approximately half way up the hill, my car came to an abrupt stop and shut down. ... taken to a local repair shop which informed me the vehicle was unsafe to drive. I contacted the dealer who stated the repair in 2013 was on warrantied for one year. I then called Lexus corporation under Case # 250212001465 and spoke to several Lexus corporate consumer advocates in their resolution department, who essentially stated Lexus cannot assist with a failed ABS Brake Booster component as it was once repaired and the responsibility to repair the ABS brake system pump falls on me the customer. I informed Lexus this was not a broken headlight, this is a safety issue, an abilty to stop the car braking issue for us and others on the road as the car is unsafe to drive at any speed. ... ve taken it to Firestone to get the brakes checked and for routine oil maintenance and a leak in the windshield reservoir. After the oil change, a Firestone service tech recommended maintenance and repairs including an alignment, new brake pads and rotors, so I took it back to Enterprise, the car dealership where I bought this vehicle on December 5, 2024. Enterprise reluctantly took it back to their service department and checked the vehicle's steering and brakes, denying there was anything wrong with either. The manager at Enterprise stated that the sound I was hearing was normal wear and tear on the vehicle but I am still hearing the clicking and squeaking sound and when I turn the wheel it still feels sticky. There have been no warning lights flashing on the dash, but again, the wheel continuously feels sticky, and I have to overcorrect my turns. The brakes on this Prius started to make weird noises. Because it is a hybrid, I wasn't sure if it was normal. After searching online, it is a known issue. The car has just under 76,000 miles on it. It might be worth $7000 on a good day. Toyota wants to replace two brake parts, numbered 47050-52020 and 04006-74152 to a tune of $2700. According to a Toyota bulletin, this job should take 2.3 hours.
... For that amount of work, my local Toyota shop wants almost $2000 in labor. My car was seen by the local shop (being on an island, it is literally the only Toyota shop that this car can be seen) when the "customer support program" 21TE01 was in place and they did nothing. I accidentally slid out into traffic as my brakes failed as I pulled up to stop at the edge of the parking lot. I was nearly hit driver's side on a pickup. ... I was informed that the brake booster/accumulator had failed and that this was a known issue on my vehicle. Toyota extended a special warranty on these parts several times but did not notify owners of the safety risk. That warranty coverage has expired for my vehicle. I believe it is an inherent safety risk and they obscured it by covering the parts under this extended warranty. I believe they should be recalled and at the least owners should be warned of the risks. All communications were done over the phone or in person, I have no electronic records. I submitted an identical request on my 2011 Prius by accident, as I used it's VIN when I meant to use the VIN on my 2012 Prius. Regardless, I am now concerned for my safety in both vehicles.Read more

The contact owns a 2019 GMC Sierra 1500. The contact stated while driving approximately 20 MPH, the vehicle came to a screeching stop. There were no warning lights illuminated. The vehicle was towed to the local dealer, where it was diagnosed that the electronic brake control module had short-circuited and there was a miscommunication, which engaged the parking brake and locked up the rear wheels.

The vehicle was previously taken to the same dealer on two occasions and repaired under NHTSA Campaign Number: 19V889000 (SERVICE BRAKES, HYDRAULIC, SERVICE BRAKES, ELECTRONIC STABILITY CONTROL (ESC); however, the failure recurred. When cruise control or self-driving systems are engaged, there have been numerous times when the automatic emergency braking system mistakenly slowed the car suddenly. At 70+ mph, this is incredibly dangerous. The AEBS apparently sees shadows as dangers. Fortunately, there has never been an instance when another vehicle has been close behind.
