On 11 Apr 2025, I was driving my 2023 Honda Passport at night on a toll road in clear weather when my infant son began screaming in distress. I pulled over onto the shoulder, partially onto a dirt area with bushes for more space. I pressed the “Park” button, unbuckled my seatbelt, and opened the driver door to step out. As I did, the vehicle suddenly began rolling forward despite being in Park
... with the door open and the seatbelt off. I fell trying to exit and had to roll away from the vehicle as it continued moving. A passenger jumped into the front seat and stopped it. This wasn’t a lurch or slight movement the car rolled enough to put me in danger. It behaved as if it were in Neutral, not Park. I later tested the same conditions (engine running, door open, seatbelt off), and the vehicle auto-shifted to Park as designed. This confirms the safety feature failed in that moment. No warning lights, alerts, or error messages appeared before or during the incident. I reported the issue to Honda and asked the dealer to inspect the transmission and interlock systems. No faults or diagnostic codes were found. The service advisor said Park can take a second to engage, but the movement I experienced was excessive and unsafe. This is a serious safety concern, especially with children in the car. I’ve also seen other reports of similar issues in Passport and Pilot forums. I have manageable scrapes and a joint effusion in my kneeRead more

The first incidence occurred around [XXX]. I was traveling [XXX] on [XXX] in North Carolina about an hour north of Charlotte. I pulled into a rest area and, upon pressing the brake pedal, heard a bit of a hissing noise and suddenly the brake pedal was extremely hard and the car was very difficult to bring to a stop. I did an online search for what might be wrong and discovered it was likely a brak
... e booster failure. I drove the car onto Charlotte, carefully and with extreme difficulty, and took it to the Swedish Connection, a local Volvo specialist. They confirm the diagnosis and replaced the brake booster. Last night, [XXX], I drove the car down to Pickens South Carolina for work. When I got in to return to Charlotte late at night, I turned the ignition, put the car in reverse, and, with my foot on the brake and waiting to back out of the parking space, I heard that hissing noise again. Sure enough, the brake booster had evidently failed again. I drove the car back to Charlotte, mostly on [XXX], and the electronic braking seemed to work OK under cruise control maintaining speed and distance between other cars in traffic. Of course, driving the vehicle on local roads, through stoplights, and turns was extremely difficult and dangerous. I have returned the car to the same mechanic that repaired the last brake booster. I will be reporting this to Volvo as well, as this is a serious safety issue and defect with my vehicle. The mechanic has confirmed that this has happened in other Volvo XC 90s. The vehicle has NOT been inspected by the manufacturer. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On three separate occasions while driving in moderate stop-and-go traffic, I experienced an intermittent issue with my braking system. I was unable to fully depress the brake pedal. I am submitting this complaint to report a significant safety system failure in my 2025 Cadillac XT4 during an accident that occurred on April 12, 2025, in Dallas, Texas. During the incident: •The Anti-lock Braking System (ABS) did not activate, and I was unable to stop in time to avoid a collision. •Neither the driver nor passenger airbags deployed, despite sufficient impact that should have
... triggered deployment. •OnStar Automatic Crash Response did not activate, nor was I contacted by any emergency response service through the vehicle. None of the vehicle’s safety systems functioned during this event, despite being standard factory-installed features. My XT4 had no warning lights or diagnostic messages indicating any malfunction prior to the crash. This failure closely resembles previous complaints noted in NHTSA filings (e.g., ABS sensor failures, OnStar module failures, and the 2025 XT4 passenger airbag recall). I am extremely concerned that this may point to a broader defect within the model line’s safety infrastructure, including collision sensors, electronic communication systems, or airbag deployment logic. I respectfully request that NHTSA investigate whether a defect exists in the braking or airbag systems, particularly where no safety features activate in a crash. I would be willing to provide repair records, crash photos, and allow the vehicle to be inspected if needed. Thank you for your attention and for helping to ensure driver safety through proper oversight.Read more

Brake Boost issue with my 2013 Ford Edge Sport. When attempting to brake, there is very little stopping resistance. Even at slow speeds, it takes a dangerous amount of time and distance to bring the vehicle to a stop. While braking, a hissing noise can be heard from the brake pedal area, and the brake pedal presses easily to the floor with minimal resistance. Upon researching the issue, I discover
... ed that this is a very common problem with this vehicle. A certified mechanic diagnosed the issue and stated that this is a known problem and may be covered under Ford's Customer Satisfaction Program 13N02. While this is a known and common issue that is covered under warranty when the vehicle is under 10 years from warranty purchase date or under 150,000 miles, the Ford Dealership is fighting me on this and is refusing to assist in correcting this safety issue. If a vehicle has already exceeded the time or mileage limit and is part of the incremental launch population, this coverage will last through November 30, 2025. Coverage is automatically transferred to subsequent owners. Brakes began failing around July/August of 2024. Due to financial hardship, I cannot afford this repair on my own. Driving this vehicle in this condition is extremely dangerous for myself and other drivers on the road. I've had to veer off of the road or into other lanes in order to avoid a collision with vehicles in front of me because it takes an extremely unreasonable distance to stop the vehicle with a bad brake booster many times already; It's only a matter of time before I encounter an unavoidable collision due to bad brakes. I am filing a complaint regarding a serious safety issue with my 2017 GMC Yukon XL related to the brake vacuum pump. According to recall information (NHTSA Recall ID: N192268490), certain 2015-2017 GMC Yukon and Yukon XL vehicles are equipped with a defective vacuum pump that may decrease in output over time. I am speaking on behalf of my [XXX] mother who owns the car. Summarily, we took the car to two Lexus Service Repair shops (i.e. one in Glendale and one in Longo, El Monte). On [XXX], we the brake and tire sensor lights came on. We first took it to Lexus of Glendale. The diagnosis and estimate was to cover these items: tire sensors, brake sensor, brake pads, air filters, oil change, and nav system
... to fix the defrost buttons, the DVD software for the nav system, and anything that would cause the brake/ABS emergency light and tire pressure lights to come on. Prior to that, we replaced all 4 tires, upper and lower suspension arms, air filters, fuses, alignment, etc. Both lights (tire sensors and brake lights in the dashboard came on the last week of January 2025. The total cost was $4200.00. Then the same day my mom rolled over a speed bump, and the tire sensor lights came on. She went back and they did not replace the tire sensor lights and we opened up a case on this issue. Then a day later, the brake light and ABS light came on. After a couple of days of that, the brakes would lock up before she turned on the ignition. We filed a complaint with Department of Consumer Affairs, BAR. We scheduled an appointment with Lexus of Longo. Mr. Lam said it could be the actuator and it would cost $4200.00 because we missed this 2017 warranty. I told him we never received any letter from Lexus's CSP warranty program on the brake actuator. And instead, we only received leather interiors or glove compartment interiors. I asked if I could take a photo of this warranty. He allowed me to. Today, after the diagnosis, he said he would charge us $3000.00 for the actuator. I am wondering if you could intervene and have Lexus cover this warranty for my senior mother.
