My 2024 Toyota Land Cruiser has exhibited persistent brake squealing since the day it was purchased. The noise occurs during normal braking conditions and has been present from the very first use of the vehicle. This is not a break-in issue, as it has never resolved on its own. Despite initially denying the issue, Toyota and the dealership have since acknowledged the problem and confirmed the exis
... tence of a Technical Service Bulletin (TSB) addressing brake noise on this vehicle. However, despite multiple contacts with both the dealership and Toyota Motor North America directly, no corrective action has been taken, and the defect remains unresolved. This constitutes a known manufacturing defect acknowledged by Toyota themselves. The failure to remedy a documented TSB issue in a timely manner is unacceptable and may constitute a violation of warranty obligations.Read more

To Whom It May Concern, I am submitting a formal safety complaint regarding my Kia Sorento and a recurring brake light failure that poses a serious risk to my safety and the safety of my children. After every single oil change, without exception, my brake lights stop functioning properly. This has happened repeatedly and consistently. Most recently, immediately after an oil change, both brake li
... ghts went out. I replaced the bulbs, but the issue did not resolve. Instead, the lights are now stuck on continuously, which strongly suggests a failure related to the brake light switch or associated electrical components—not normal wear and tear. This pattern is extremely concerning. The fact that the failure occurs every time the vehicle is serviced indicates either a defect in the design or a component that is highly susceptible to failure during routine maintenance. Brake lights are a fundamental safety feature. When they fail, other drivers are not alerted when I am slowing down or stopping, putting me, my children, and everyone around me at risk of a serious rear-end collision. I understand that brake light switch failures have been a known issue in Kia vehicles in the past, including recalls for similar defects. Given the repeated nature of this problem and the safety implications, I do not believe I should be responsible for the cost of repairs for what appears to be a known or recurring defect. I am requesting that this issue be formally investigated and that Kia be required to repair or replace the faulty brake light switch and any related components at no cost to me. Additionally, I ask that this matter be reviewed to determine whether further action, including a broader recall or service campaign, is necessary to protect other drivers. This is an ongoing and dangerous issue that has now occurred multiple times. I should not have to worry that a routine oil change will leave my vehicle unsafe to drive. I urge you to take this complaint seriously and act quicRead more

I own a 2020 Tahoe, 160,000 miles. While driving, the vehicle experienced a sudden loss of brake assist due to a failure of the vacuum brake pump. The brake pedal became extremely hard, and the vehicle would not slow or stop as expected, requiring me to put vehicle in neutral and frantically find a way to the shoulder to force a stop (pulling the emergency brake). This created a dangerous condit
... ion and significantly increased stopping distance, posing a serious safety risk to myself, and others on the road. There was little to no warning prior to the failure. This issue appears to be consistent with known complaints involving vacuum pump failure and loss of brake assist in similar vehicles. A failure of the braking assist system is NOT a normal wear condition and presents a critical safety hazard. I am reporting this incident due to the safety risk associated with sudden brake assist failure and the potential for accidents or injury. Vehicle issue has been confirmed by an independent service center. There was a warning light that appeared at the same time the brake stopped working leaving me no time to plan or know ahead not to drive.Read more

My 2019 Ford EcoSport 1.0L EcoBoost engine began making loud noise consistent with oil pressure failure. I brought the vehicle to an authorized Ford dealership. The dealership performed a teardown and confirmed the oil pump belt tensioner had failed — the exact defect described in Safety Recall 23S64. The dealer replaced the oil pump belt, tensioner, and oil pump under Recall 23S64. However, the
... dealer then stated that the resulting engine damage — severe scoring of connecting rod bearings and crankshaft journals caused by oil starvation — is “not related to the recall” and refused to replace the engine under Recall 23S64. Ford’s own recall documentation states: if engine failure is related to a broken oil pump belt or tensioner, a long block engine replacement is authorized as an interim repair at no charge. The updated Service Action also states the oil pump belt and tensioner no longer need to be replaced before long block installation. The causal chain is direct: tensioner failed (recall defect) → oil pressure lost → engine destroyed. The dealer’s refusal to replace the engine appears to violate Ford’s own Recall 23S64 service procedure. The vehicle has been inoperable for weeks. I continue making monthly loan and insurance payments on a vehicle I cannot drive. My extended warranty claim was also denied because the warranty company requires written confirmation from Ford that the engine damage is not recall-related — a document the dealer says they cannot provide. I am requesting NHTSA to investigate whether Ford dealers are systematically denying recall-related engine replacements under 23S64 that they are obligated to perform per Ford’s own recall service procedures. Additionally, prior to the complete engine failure, the vehicle stalled unexpectedly while stopped at a red light in active traffic without any warning. The engine shut off suddenly, consistent with sudden loss of oil pressure caused by the recall defect. This created a dangerous safety situationRead more

Summary of the Event: The driver of a 2025 BMW X1 M35i experienced an issue where the vehicle did not respond as expected when braking. The incident occurred in Gainesville on [XXX] at approximately [XXX]. After proceeding through a traffic light, the roadway transitioned from two lanes into one. The driver was in the right lane and needed to merge left. As the driver approached a turn while me
... rging, she attempted to slow the vehicle but reported that it felt as though it was accelerating and that braking was ineffective or unresponsive. When the brake pedal was pressed, it felt unusually firm with very little travel and was difficult to depress, and the vehicle did not slow as expected. The driver was unable to reduce speed sufficiently to complete the turn, and the vehicle continued forward, striking a brick wall at the entrance of a residential subdivision at an estimated speed of 35–45 mph. Airbags deployed. No direct head impact occurred. Following the incident, the driver experienced headache and nausea and was evaluated in the emergency room approximately 10 days later, where she was diagnosed with a concussion. No prior braking issues were observed. Insurance will deem the car a total loss and not repairable. However, the vehicle is currently under manufacturer inspection with diagnostic data being preserved. The driver was cited for careless driving; however, this is being contested based on the reported braking issue. First Incident- volume kept rising on its own. Would turn it down, it would go right back up. Would turn the radio off, it would turn on by itself and would keep rising until full blast. Vehicle was parked, came back from work, turned vehicle on and the volume was fine on it. Second Incident- (this has happened twice) about a month after the volume incident. This time the vehicle would accelerate
... by itself. No cruise control was on, vehicle would keep going faster and faster. First time this happened the vehicle needed to be towed, cops were involved. I finally was able to stop the vehicle at a red light (where it eventually broke down and would not start). Extended warranty covered the tow. There was no check engine light on. Replaced the battery as I was told the errors were pointing back to low voltage. After battery was replaced, maybe three weeks later the vehicle did the same thing on the highway again. Car speed kept increasing on its own, no cruise control or pedal input. Vehicle kept speeding. I took this vehicle to Moon Township, PA Nissan Cochran dealership and they could not replicate the issue. I have video proof the first time this happened where the vehicle kept increasing in speed without pedal input. I have two kids under the age of [XXX] , This is an unsafe vehicle and it has lost my trust completely. The extended warranty company for this vehicle said they can’t do anything since the dealership says no check engine light is on. Third incident- got into vehicle, turned the vehicle on and the parking brake would not release. The parking brake was never applied in the first place so I have no clue how or why it was on when I turned the vehicle on. I just recently called the dealership on this issue now (Moon Township, Nissan Cochran) and will see what they say. Again, no check engine light is present. I am at a loss on this vehicle. 2022 with only 45k miles on it and have never experienced this many issues with a vehicle before! It is unsafe. I am reporting a safety issue involving a 2013 Ford Edge Sport AWD related to progressive brake failure. The affected components include the brake booster and master cylinder. The vehicle remains available for inspection upon request. In mid-2025, the vehicle developed a gradual loss of braking capability. The brake pedal required increasing force over time to bring the vehicle to a stop. Because
... the failure developed progressively rather than suddenly, the severity of the issue was not immediately apparent. At the time, the vehicle was primarily driven by my teenage son, a new driver, who continued operating the vehicle by pressing the brake pedal harder to compensate. This created a significant safety risk, as the vehicle did not reliably stop under normal braking conditions, increasing the risk of collision. The issue was confirmed by an independent mechanic, and repairs required replacement of the brake booster, master cylinder, and related components. The vehicle was later discussed with a Ford dealership; however, no warning of coverage or recall applicability was identified at that time. There were no clear warning lights or dashboard messages indicating brake system failure prior to the issue. The primary symptom was a progressive increase in required pedal force. After the repair, I learned that brake booster failure is a known issue for this vehicle platform and has been the subject of prior manufacturer service campaigns. I am concerned that this type of gradual brake degradation may not be recognized by drivers, particularly inexperienced drivers, and could result in unsafe operating conditions. The vehicle has also experienced repeated rear differential failures. Replacement parts are no longer manufactured by Ford, requiring use of used components with unknown condition and lifespan, further affecting vehicle reliability and safe operation. I have attached a receipt for the second time purchasing and replacing the rear differential.Read more

The breaks on these vehicles are extremely dangerous . Before I leased this vehicle test drove 3 vehicles. The salesman said the breaks will adjust as you drive. They have not. ... it takes for the car to respond is a significant distance before the car comes to a full stop. I have brought back the car to my dealership three times expressing my displeasure and each time I m told there is nothing they can do unless a fix comes from the manufacture. URGENT SAFETY DEFECT - Repeated front hub overheating/fire risk (2025 Western Star 47x) I am submitting a formal safety complaint regarding my 2025 Western Star 47x The front wheel hubs have over heated to the point of failure on 6 separate occasions, resulting in a fire on two of those occasions. The issues have been documented with Freightliner and accompanied by photo/video evidence. Each inc
... ident required the vehicle to be towed to an authorized dealership, and repairs were performed under warranty. Despite these repeated repairs, the issue has not yet been resolved. The hubs are currently overheating again, indicating a persistent and unresolved defect. This is no longer a routine maintenance or warranty issue. This is an ongoing safety defect that presents a serious fire hazard and risk of injury, property damage, or loss of life. Key Concerns - - Six separate over heating and/or fire incidents involving the front hubs - Multiple warranty repairs with no permanent resolution - Recurrence of the same failure condition - Within days or weeks of warranty repair, the vehicles becomes unsafe to operate under normal working conditions due to over heating hubs, which break down the lubricating gear oil, furthering the problem, causing more heat with thinner oil, which eventually leaks onto the brakes, causing a fire. During the times where I was fortunate enough to catch it before a fire started, the fluid was so hot that it melted the plastic hub cap to the point of blowing up like a balloon before eventually rupturing. These instances were video documentedRead more

I am submitting a formal consumer complaint regarding a persistent brake issue—specifically a loud squeal when coming to a stop in a 2022 Ford Expedition Max Limited purchased as a Certified Pre-Owned vehicle through Ford’s Blue Advantage Program. At the time of sale, I was assured the vehicle met certification standards, had no unresolved safety concerns, and that any recalls had been addres
... sed. However, I later learned that Safety Recall 25S47 (NHTSA Campaign 25V-314) had been issued in May 2025—one month prior to purchase—for a front brake line defect that may cause brake fluid leaks, reduced braking performance, and increased crash risk. Despite this, the vehicle was certified and sold with representation that the recall showed “no concern.” Since purchase, the vehicle has exhibited constant brake squealing, worsening stopping performance, and reduced responsiveness. The issue occurs at every stop and persists despite replacement of front and rear brake pads and rotors. Timeline: • Apr 2024 – Prior brake-related service • May 9, 2025 – Recall issued • June 4, 2025 – Brake service prior to sale • June 14, 2025 – Purchased as certified • Within 1 week – Issue reported • June 25, 2025 – Inspection found “no concern” • July 2025 – Pads/rotors replaced • Within 2–3 months – Issue returned • Ongoing – Condition deemed “within guidelines” Multiple attempts to resolve this through the dealer, a consumer complaint organization, a state regulatory complaint, and a manufacturer repair case have not resulted in a fix. This raises concern that a vehicle with a known safety-related defect was certified and sold without proper resolution. I request investigation into whether certification standards were properly followed and whether consumer protection requirements were violated. I am seeking a permanent manufacturer-backed repair, repurchase, or other appropriate relief.Read more

I was driving my car home from work and many warning lights lit up on my car dashboard (brake problem, engine problem, uphill start, steering problem) etc.. I got off the freeway and my car dashboard lit up and notified me “to pull over to a safe location because my car needle (temperature) was on high. I pulled over to a safe location and opened the hood and noticed that the antifreeze coolant
... compartment was empty) my car could have caught on fire because there was no coolant inside the compartment. After I got home and filled the car with coolant I placed cardboard under the car engine to see if the coolant would leak. There are no signs of leakage. I took my car to my mechanic and he told me that he couldn’t fix the problem and suggested I call Honda USA customer service. The mechanic told me that the problem is with the “head gasket” and that the coolant does leak into other parts of the engine. He didn’t charge me anything for the inspection and added coolant to my car. I called Honda USA customer service and they told me that there are “no recalls” regarding coolant leaks and head gasket problems. I googled problems with “Honda 2016 civics and learned that there is a problem that exists and it’s very costly to fix. I also learned that Honda was sued in Canada and Honda settled with customers regarding this problem. Honda USA simply told me to take my car to a Honda dealer and ask for an “inspection” I asked if they would cover the cost and they didn’t not say they would pay for the inspection. This is a manufacture problem that occurs after a Honda Civic has gone past 70,0000 miles. People still complain about this issue and wonder where the coolant in the car is leaking and where is the coolant going into since the coolant is not visibly leaking. The coolant is making its way within the engine and causing more problems in my car. I have never had a problem like this one. Vehicle has two open unrepaired recalls: T59 (NHTSA 17V-572, brake booster water shield) and V62 (NHTSA 19V-813, fuel pump relay). The brake booster is currently symptomatic — an audible humming noise is present when the vehicle is stopped and idling, consistent with vacuum loss in the brake booster system. The dealer never followed up regarding the rental. Instead, the dealer contacted me to appraise my current vehicle and discuss new vehicle inventory. The recall work was never discussed. When I did not appear for the appointment, the dealer did not follow up or offer to reschedule. I then contacted Stellantis customer service directly and explained the situation, including the active brake booster symptom. The Stellantis representative connected me with a second dealership, Bill Snethkamp Chrysler Dodge Jeep Ram on Woodward Avenue in Detroit, Michigan, then disconnected from the call without notice. I was left explaining the situation a second time to a dealership that informed me they do not provide loaner vehicles. The rental option requires me to pay out of pocket upfront and then seek reimbursement from the manufacturer in the form of a check. This is not a viable option. Additionally, this dealership is located approximately one hour from my residence in Flint, making it logistically impractical for recall service. The part on my truck that is currently waiting to be repaired is the exact same part number as the part number listed on the active recall. My extended warranty company doesn't want to cover the repairs since its a manufacture's defect/ issue, and the dealership won't cover it since the vehicle isn't currently covered by the recall. I know I am not alone in this, I live in a small town and all the repair shops in town (2 smaller repair shops and 2 dealerships) are all starting to have more Dodge Ram 1500's showing up daily with the same issues. 3 new trucks arrived at the same shop I took mine to in the last week alone. Can someone please help us with getting our vehicles fixed since we all have the same issues and the exact same parts as the one listed on the active recall?Read more

Attempted to have my local Ford dealership, Sam Galloway Ford, Ft. Myers, schedule the recall on 4/14. Was told they parts are on a back order. Contacted FORD Corp. via chat with AI, then an agent asking in the Brake Master Cylinder replacement parts were on a national backorder. I was told no and they requested the name of the dealership. Pressed the dealership for an appointment. This was grante
... d 4/23, 7:30am EST. Arrived and was checked in. At 10am was told they do not have the parts. A supervisor explained that on Monday they had to return 32 parts packages to FORD because these were defective. Contacted FORD again, they denied that the replacement parts are defective. Who is one to believe at this point?Read more

This is the third time my brakes have failed while driving, which seems to have something to do with the 12-volt battery. At least at first, I disconnected my 12-volt battery and reconnected it; this fixed the issue, at least for a while. The other day, I was driving down a road near my house and the brakes failed right before I came around a bend. I had to quickly press the e-brake to slow down,
... or I would have run right off the road. This is definitely a dangerous issue. Please issue a recall for this because I've noticed many other people complaining about the same issue—the same sound, feeling, and symptoms. It appears the brakes are malfunctioning due to a communication error, or a low voltage issue to the brake computer systems?Read more

The ABS/Brake warning lights have lighted up on my instrument panel. It appears that the ABS/VSA module is bad. I do not see a recall for this from Honda but after doing further research, I have learned that this is a known problem for 9th generation Honda Accords. This leaves the vehicle without the anti-lock brake system working. Brakes still engage, but no ABS. This puts driver and any passenge
... rs at risk. The dealer acknowledges the known issue but wants over $3k for the repair. Faulty ABS modules are a great liability and put drivers and their passenger at risk of injury and/or death. I am asking that the NHTSA seriously look into this matter and make Honda take accountability for this know issue they are fully aware of.Read more

2024 Mercedes-Benz Sprinter 2500 experienced unusually rapid brake, tire, and battery wear at low mileage. Front brakes required replacement at approximately 29,000 miles. Rear brakes were already worn low by approximately 37,000 miles. Rear tires required replacement by approximately 37,000 miles. Vehicle battery also failed and required replacement before approximately 37,000 miles. Vehicle had
... earlier emissions/sensor related repairs as well. This creates safety concerns because premature brake wear can reduce stopping ability and increase stopping distance. Premature tire wear can reduce traction, especially in rain, and increase risk of tire failure or blowout. Unexpected battery failure can disable the vehicle without warning in traffic or roadside conditions. Vehicle is used in normal commercial delivery service and is generally not heavily loaded. I have owned and operated multiple prior Sprinter vans over many years, and previous model years achieved substantially longer brake, tire, and battery life under similar or heavier use. A newer comparable fleet vehicle with similar mileage has not required these repairs. The issue appears abnormal compared with prior ownership experience and may indicate brake system drag, an alignment issue, defective components, battery charging issue, or another unresolved defect. Vehicle has been serviced by dealer and components were replaced, but premature wear remains the concern. In addition, rear brake wear occurring this early suggests possible imbalance or excessive brake application that should be investigated. Rapid tire wear combined with brake wear may indicate alignment or braking system issues affecting vehicle control and safety. I am submitting this complaint so the condition can be reviewed for potential defect trends affecting other similar vehicles. Defective parts may be available from prior repairs depending on service center retention.Read more

There is a recall on my 2015 GMC Yukon, that I am not the original owner of. The recall is for the brake booster and says that it expired. I have had the car checked by two different shops (both confirmed it was this), and one of them was the local GMC dealership by my house. I called to make an appointment for the recall to be fixed, to which they said they needed to order parts and it would take
... a few weeks. I dropped the car off three weeks later per their request for then another person to call me and say they won't fix it because it is expired. It is a safety matter that the breaks don't work occasionally.Read more

In the event of having to panic stop, slamming on the brakes doesn't stop the car. The brake pedal goes all the way to the floor and I have to pump the brake before the car stops. This has happened multiple times with my 2016 dodge dart the last time was 2 months ago. This needs to be a recall.

Brakes do not have proper stopping power. The brakes are very slow to react, and the brake pedal has a long travel before the truck starts to slow down. This can be extremely dangerous in city driving. We have four of these F-650s in our fleet, and all four of these trucks exhibit the same poor braking. Trucks are all new with hydraulic brakes.
