GM Case#[XXX] #1 A- On [XXX] vehicle sensors disabled breaks while driving on [XXX]. B- Driver Displayed Alert. a- BRAKE SYSTEM FAILURE 62 MHP TOP SPEED. b- TRAILER BRAKES DISABLED SERVICE REQUIRED. c- SERVICE ESC. C- Concerned by alerts proceeded to exit highway, the breaks where unresponsive. This required pressing break pedal to the floor. It took over 400ft for the vehicle to come to a complet
... e stop. Fortunately no other vehicles were on the exit ramp, a major accident was avoided. D- On 10/30/2024 took vehicle to Bomnin Chevrolet West Kendall and explained break failure and provided pictures of driver alert messages. E- On 11/1/2024, 3 days after, Bomnin Chevrolet West Kendall Technician communicated that vehicle was mechanically fine and were unable to duplicate the problem. I accepted the vehicle back with 5,901 miles of use. #2 A- On [XXX] vehicle sensors disabled breaks again while driving on Miami city street. B- Driver Digital Displayed flashed following Alert Messages. a- SERVICE TRAILER BREAK SYSTEM. b- SERVICE TRACTION CONTROL. c- SERVICE ESC. d- CHECK ENGINE LIGHT WENT ON. C- Having experienced this back in October, I knew what to expect. Unresponsive breaks, fortunately on 2nd occasion I was driving slower on a city street and was able to coast to a stop. D- On 4/10/2025 took vehicle back to Bomnin Chevrolet West Kendall for the same problem, break failure. I once again explained to Technician what happened, showed them pictures of the Alert Messages. E- Vehicle as of today, 5/18/2025, remains at Bomnin Chevrolet West Kendall for a total of 39 days and is yet to be repaired. Given the severity/danger of defective break sensors defects there is “nonconformity” that under the Lemon Law is defined as a defect or condition that substantially impairs use , value, or safety of the vehicle. These 2 incidents make this vehicle unsafe for drivers, passengers, and general public in proximity. On 4 different occasions my vehicle applied the brakes forcefully on its own at high speeds. The dash flashed with the message "check forward collision avoidance assist system." as well as displayed the following icons: master warning light, hill climb assist, traction control, forward collision, and the Auto start/stop was flashing. These continued until I was able to stop and turn the car off. W
... hen I was trying to slow down and stop my brakes were not working correctly as I had to push the pedal all the way to the floor for a long period to stop all the way. When the car was put in park the engine was revving up until it was off. The car was towed to the dealership after first incident and the following codes were in the history: C164202 C161308. Dealership had my car for 6 days, opened a tech line case and without an answer of why this happened nor remedy, my car was released back to me resulting in 3 additional instances. It is now going to another dealership to hopefully get an answer and repair.Read more

I was driving on the Northbound side of the Interstate when my brakes failed when I pressed them to avoid a collision on the highway. The steering wheel malfunctioned when I tried to avoid a head-on collision with another vehicle. I couldn't control or stop the car, causing me to swerve left to right, and I then completely lost control of the vehicle. I crashed into the middle of the highway, but
... because my brakes had failed, the car continued at full speed into the Southbound side of the highway, causing me to wreck into a semi. The Colorado State Patrol investigated the accident. Right before I wrecked into the semi-trailer, I noticed all of the lights on the dashboard were off. My Steering wheel had fallen off completely, but the airbag never deployed even though every other airbag had deployed.Read more

ABS and VSC lights on the dash have been flashing on and off for the last few years. They always went off after flashing. 2 weeks ago they both flashed on and the warning message "Check VSC System" came on with a loud buzzing whine. Now they will not go off. The brake pedal pulses back when applied and felt very hard. It took the car a long time to stop which is a safety danger. Took it to an ind

He also mentioned that Toyota issued several Customer Support Notices to repair or replace the unit but I did not get them even though I am the only owner and still resides at the same address. While driving, my car began to glitch out. The screens started turning on and off, the doors began locking and unlocking, and I lost my ability to steer, brake, and accelerate. I have brought the vehicle into the dealership multiple times due to dangerous behavior involving the Honda Sensing system. The vehicle has attempted to steer out of lanes, and shown repeated safety warnings related to the Collision Mitigation Braking System (CMBS), Lane Departure Warning (LDW), and Road Departure Mitigation (RDM). Despite these serious issues, the dealership repeatedly tells me
... that “no codes were found” and refuses to provide full diagnostic records or confirmation that proper factory scan tools (HDS/i-HDS) were used. I suspect they may be clearing codes or failing to document the issues appropriately. The problem is intermittent but dangerous, and I am very concerned it may lead to a crash or serious injury. Honda has not resolved this, and I believe this is a widespread safety defect that deserves further investigation. Most recently, while driving on the highway, the vehicle suddenly applied the brakes without any obstacle or warning, creating a highly dangerous situation. This incident was alarming and could have resulted in a serious accident.
Since that event, the vehicle has continued, from time to time, to display warning messages related to the Collision Mitigation Braking System (CMBS) and Road Departure Mitigation (RDM). These alerts occur sporadically and do not appear to be triggered by any identifiable driving condition. The dealership does not want to look at the car again for this problem and is trying to tell me I have to deal with Honda directly who is not escalating the issue even after multiple request as I fear for my safety and that of anyone in the vicinity of the vehicle.Read more

On 5/11/2025, my husband and I were returning home from the grocery store. I was operating the vehicle at the time of the incident. Upon arriving at our residence, I pulled into our driveway, which has a slight downward incline. As in our usual practice, I attempted to stop in front of the garage to unload groceries before entering, as the hatch of our vehicle can come in contact with the garage d
... oor when opened inside. While aligning the vehicle with the garage door, I began breaking and turning slightly to the left. However, as I attempted to come to a complete stop in front of the closed garage door, the vehicle unexpectedly accelerated forward. Despite applying the brakes, the vehicle continued moving and crashed through the closed garage door. Due to the steering wheel being slightly turned, the vehicle veered left upon entering the garage, striking the left interior wall and a 32 foot ladder that was leaning against it. The impact with the wall and ladder brought the vehicle to a stop although the engine continued to rev and the vehicle attempted to move forward. My husband instructed me to shut off the engine, which I did immediately. My husband was not injured in the incident. I sustained pain in my left shoulder and upper left back. A medical evaluation indicated that the pain is expected to subside within a few weeks; imaging will be conducted if symptoms persist. I have filed both auto and homeowner's insurance claims regarding the incident. Additionally, I contacted the dealership, which advised me to report the issue to the manufacturers corporate office. They noted that the vehicle is equipped with a fail-safe system designed to prevent simultaneous engagement of the brake and accelerator. This was a very scary incident that I don't want to have happen to someone else.Read more

This Prius Hybrid is my [XXX] mother's car which she got thinking is was a very safe and reliable vehicle. She purchased the vehicle from a Toyota dealership here in Maine on [XXX]. Last week while driving down the road, all of a sudden, every brake warning light lit up the dashboard and the brakes became extremely unresponsive. It scared us both to death but thank goodness, I was driving when
... this happened. After doing some research, I have discovered that this a very well known brake issue with the 2010-2015 Prius cars. I cannot for the life of me understand how this major safety defect has not been made into an actual safety recall. I did contact Toyota customer service and they will not help us at all. Even though my mother has only had the car for just under 9 years and is well under the allotted mileage, they said their Customer Support Program (ZJB) only covers 10 years from the day the car was first driven after production. Her Prius now is inoperable and the repair costs are in the thousands. We are so incredibly disappointed with Toyota and the safety board for allowing this to happen to all of the defective Prius owners. I hope you will reconsider and do the right thing and recall these cars before someone does get into an accident where they are injured or killed. I am reporting a sudden and dangerous failure of the brake booster in my 2012 Toyota Prius. While driving, I experienced a loss of braking power, accompanied by several warning lights including ABS, Brake, and the Traction Control system. The brake pedal became extremely stiff and unresponsive, significantly increasing the stopping distance. This failure nearly caused an accident, as I was unable
... to stop quickly enough when traffic slowed unexpectedly. This issue happened without warning and is a serious safety risk. I later discovered this is a common and widely reported defect in 2010–2015 Toyota Prius models. Toyota issued an extended warranty (Customer Support Program ZE3) to cover this known brake booster defect — but only for vehicles within 10 years or 150,000 miles. While my Prius has well under 150,000 miles, it is just over 10 years old, which disqualifies me from coverage, leaving me with a repair estimate over $4,000 for a well-documented manufacturer defect. This seems grossly unfair, especially for responsible owners with low-mileage, well-maintained vehicles. Toyota’s refusal to issue a formal recall and instead offering only limited support puts millions of Prius drivers at risk. A brake system failure of this kind should qualify as a safety defect and trigger a mandatory recall, not a quietly limited program with arbitrary expiration dates. I urge NHTSA to investigate this issue further and consider a recall of the defective brake booster system in these Prius models to protect public safety and ensure fair treatment for affected owners. Thank you.Read more

This is a second report of an issue that first happened in February 2025, the dealer replaced the battery and called it fixed. On Friday 5-2-2025 I started the vehicle and as I was driving the dash lit up with these warnings: Service Trailer brake system, Service ESC, Brake System Failure 62 mph top speed, Steering assist reduced drive with care, The vehicle steering got stiff and brake pedal went
... to the floor board with a little braking available if I stood very hard on the brake pedal. I got off to the side of the road, cycled the truck on and off and was able to drive off. It did this three times before I could get it to the dealer the next day, If my daughter was driving I would have expected that two of the three would have resulted in a wreck as she would not have been strong enough to steer or stop the vehicle. The dealer had it three days and replaced the brake master cylinder and called it fixed, I picked the vehicle up and on the way home I stopped for gas, after the gas station the dash displayed: Service brake assist, I turned right around and took it back to the dealer, they checked the code and said the brake assist module was drawing too much current, I picked the vehicle back up three days later, they said they bleed the air out of the brake system and called it fixed. No one wants to ride with me and my daughter is begging me to trade the truck, they call it a death trap waiting to happen. GM says it is all normal issues.Read more

May 14, 2025 Dear NHTSA, I am submitting this complaint regarding my 2017 Jeep Wrangler Unlimited Sahara, which has only 60,000 miles and has already required several major repairs that raise serious safety concerns. To date, I have had to replace three brake calipers and the emergency brake, at a cost of over $2,500. The calipers were seizing, and the emergency brake would not release proper
... ly, both of which posed clear risks while driving. I understand through forums and peer discussions that other owners have faced similar issues. In addition, my vehicle now has an oil leak that will cost over $700 to repair, and the windshield washer fluid hose burst, requiring another replacement. These repeated failures on a relatively low-mileage vehicle suggest underlying quality and reliability issues. I believe these concerns warrant review, particularly the safety-related brake defects. I respectfully ask NHTSA to investigate whether these failures are part of a broader pattern that may justify further scrutiny or a formal recall. Thank you for your attention.Read more

My rotors warped with 5,260 miles and dealership would not cover the issue stating it is wear and tear. The brembo brake calipers are large and creates to much heat and causes the rotors to overheat and warp.

I am filing a complaint about my 2024 GMC Acadia AT4, leased brand new from Frank's GMC in August 2024, now with a little over 8,000 miles. The brake pads are worn to 35% life, far below the expected 30,000-70,000 miles, with loud squealing, fluid leakage, and odors. This poses a severe safety risk to my family and others, as compromised brakes could fail. In October 2024, before my 3,000-mile oi
... l change, I reported brake concerns to Frank’s GMC service manager, Javier, who advised waiting. After the November 2024 oil change and tire rotation, only three months after obtaining my vehicle, Javier claimed no issues, despite my prior warnings. The problems worsened, with squealing, leakage, and odors persisting. My March 19, 2025, service appointment was delayed, calls ignored, and follow-ups never occurred. Sales manager Khaled stated that owner Frank confronted Javier about avoiding service issues, yet no action was taken. I contacted ten GMC service departments in PA, CT, and MD; they reported similar AT4 brake issues, some citing total brake failure. I spoke with GM corporate supervisors, who referred me back to the dealer. Frank’s GMC inspections overlooked my concerns. A local police department safety inspection unit confirmed the issues, but no manufacturer or insurance inspections occurred. The vehicle is available for inspection. I urge NHTSA to investigate this premature brake failure and ensure the 2024 Acadia AT4’s safety. The email screenshot I included was sent in April. I contacted the same GMC dealerships that previously reported no issues to check for updates, and they confirmed a change. All of them now report the same problem with brake pads and/or brake failure. This issue is covered under warranty and should be replaced without issue, but dealers are not complying. If overlooked, this could become a national concernRead more
