A Lexus field inspector has been scheduled for early May 2025. Safety was put at serious risk. A child was in the vehicle. The lockup was sudden and without warning, with no ability to anticipate or respond. This incident was preceded by two prior issues with the same vehicle: (1) approximately one month prior, a transmission concern was reported to the dealer, who attributed it to abnormal driving habits and took no corrective action; (2) on April 7, 2025, brake rotors were replaced under a recall for rotors that squeak when exposed to moisture. The problem has been confirmed by the dealership, which has retained the vehicle since April 17, 2025. I am aware of at least 8 to 10 other reported incidents involving the same model vehicle with the same lockup failure. No warning lamps or messages appeared prior to the failure.Read more

... to a recall about a month before on the issue with the engine being turned off and it thinking it was still hot and still running the fan (even though the car was not running). Knudtsen fixed that issue. Low battery voltage light did not come on however battery was older so had battery installed at park to ensure the issue was not due to the battery. The oil was fresh and showed no issues, changed 2 weeks before fresh oil no issues found during service. When tried to restart the car it would think it would try to start and then sputter out and not stay in idle. Les Schwab left and called a mobile mechanic to bring the computer. Computer was hooked up and it would try to start, however, it would not know it was already running and try to start again on it's own...the codes that came up were the PO101 code came up for Mass Air Flow issue...and then the codes would go in a wild sequence of fail.....mechanic checked for loose connections around air filter, took air filter out and it was clean no issues, checked pipe no visible issues. We walked away and walked around the park to give it some time to sit and then we tried it again and it tried and sputtered out and died again would not stay idling. Then all of a sudden we heard it try to start itself without us trying to start it and it clanked. We hauled it to Knudtsen for a diagnosis. They stated "the motor will not turn this way or that way and it is seized or locked up". This is a 2021 with only 89,000 miles it should have a longer life than this. During the second repair attempt, dealership staff personally experienced the same malfunction while test driving the vehicle. I paid approximately $2,078 cash for repairs after being told the issue was fixed, but the same dangerous defect happened again afterward.The vehicle has remained at the dealership for 15+ consecutive days, causing loss of transportation and work hardship. I am also concerned by inconsistent repair documentation and possible repairs performed that were not clearly disclosed or authorized. This vehicle remains unsafe to drive, and the defect has not been corrected despite multiple repair attempts. The brakes aren’t working properly due to loss of vacuum. This happens anytime I’m stopping at a red light or parking which can result in me hitting someone or another vehicle. This has been confirmed by Firestone Compete Auto Care but they were unable to fix it. Ford says this TSB applies to the same year but not to the plant this car was manufactured. There are also body and interior fitment defects. The front passenger door does not sit flush with the rear door panel and appears slightly open when closed. The dealership service manager stated that correcting this condition would require removing and pulling the rear door panel outward so both doors align. The glove box door also does not sit flush when closed, and the service manager advised that replacement might make the condition worse. These concerns were reported to an authorized Chevrolet dealer. The dealer confirmed that General Motors has acknowledged the acceleration vibration condition in a Preliminary Information service bulletin (PIT6466) affecting 2025–2026 Equinox vehicles and advised that GM engineering is investigating the issue and that no repair is currently available. The combination of drivability issues, abnormal noises during braking, underbody movement sensations, and body misalignment affects the use, value, and safety perception of the vehicle. The vehicle has very low mileage, and several of these issues have been present since early ownership. The brake pedal is extremely soft and routinely travels to the floor during normal driving conditons. It also catches at various levels. It is impossible to tap the brakes to slow the vehicle down when rounding a corner which has led to several scary incidents and near misses. I was also told by a service manager that this is due to the electronic braking system and they are reluctant to spend time on troubleshooting because Ford cut their renumeration. A service Field Engineer stated that the brakes were in spec and traveled 2 3/4" toward the floor. This is a subjective observation as every person uses different foot pressure. The gas pedal is at 3" off the floor and the brakes are 5". The brake pedal passes the gas pedal and many times, my foot hits both pedals. My inspection with a ruler shows the brake pedal going 1 1/2" past the gas pedal. There were no warnings the first time this happened. I have taken the vehicle to an independent mechanic who bled air out of both rear brake lines which helped. This repair lasted 3 months and now the issue is back. I've been told by independent mechanics that the problem is in the ABS module (which pulls the pedal to the floor when activated).Read more

This can be extremely dangerous in city driving. We have four of these F-650s in our fleet, and all four of these trucks exhibit the same poor braking. Trucks are all new with hydraulic brakes.

Tow informed us that there should be a manual release under the shifting column, however, this design safety feature had been removed in the 2024 iteration. The car was stuck on the road behind a blind turn on a somewhat busy road with vehicles typically traveling between 25-50 mph. The failure of a manual override for neutral and braking is a critical design failure that can lead to major accidents and death in the event of engine or transmission failure. The dashboard displayed multiple warnings and errors, including: * “Steering assist reduced – Steering may require increased effort” * “Center console buttons to select gears unavailable – Use touchscreen to select gears – Schedule service” * Repeated alerts indicating braking and system limitations * Conflicting and inaccurate readings, including the speedometer displaying “0 mph” while the vehicle was still moving at highway speed At the same time: * Braking response was severely compromised despite full pedal application * Hazard lights and turn signals were non-functional * Gear selection controls became unavailable or unreliable * Driver assistance and safety systems were disabled * The system issued instructions to “press and hold brake” while braking performance was inconsistent This resulted in a complete loss of situational awareness and control while I was in the car with my older parents and my child. l. I was unable to signal to surrounding vehicles while attempting to maneuver into a narrow emergency lane at highway speed. My family and I had fear for our lives. Critically, the vehicle instructed to exit immediately while we were still in a dangerous roadside position without functioning hazard lights. I have asked my insurance company to inspect the car for any malfunctions but they won't. They deem it totaled and say I am at fault because it's my responsibility to control my car. The police arrived but they did not inspect the car either. I have contacted the dealership (Parkway Hyundai), but they are of no help either. There were not warning lamps, messages or other symptoms of the problem prior to the failure. However, I did notice recently that when I put my foot on the break to start the car that sometimes the break was very stiff and other times it was not. I thought it strange but thought it was just the way my car was. My insurance is Progressive. I am filing a safety complaint regarding my 2025 Chevrolet Tahoe Premier for repeated brake assist/braking failures. Most recent incident: While driving, the check engine light came on. I turned the vehicle off for approximately 10 minutes and restarted it, but the check engine light remained on. Approximately 20 minutes later, multiple warnings appeared on the dash including: “Brake Assist Fai

I intended to roll through the stop sign because the brakes were not functioning normally, but another vehicle suddenly approached. I pressed the brake pedal with both feet. The brakes were stiff and hard to press. The vehicle still did not stop properly and came to a stop in the middle of the road past the stop sign. After arriving at my destination, the vehicle sat for approximately 1–2 hours. When restarted, the brakes appeared normal again, but the check engine light remained on. In the MyChevrolet diagnostics app, it then showed warnings for Engine and Transmission System, Transmission System, and Four Wheel Drive System. While driving to the dealership afterward, the vehicle also began shaking. This same issue previously occurred on April 16, 2026. I was driving on the interstate when the brakes lost assist and the dash displayed “Service ESC,” “Brake Assist Failure Press Hard to Brake,” and “Speed Limited.” After reaching my destination, I was told by someone at the dealership to turn the vehicle off for 10 minutes to see if that would fix things, so I turned the vehicle off for about 10 minutes. When restarted, all warning lights were gone and braking returned to normal. I still drove the vehicle to the dealership that same day for service. The dealership has now indicated a potential wiring harness issue but claims this is not related to the recall. They have requested approximately $1,200 for further diagnostic work, which we declined. The manufacturer, Stellantis, has stated that the recall for this VIN is closed and has declined to intervene, characterizing the situation as a dispute between the customer and dealership. As a result of these unresolved issues: * The vehicle cannot pass inspection in Texas * The vehicle cannot be registered or legally operated * Warning lights related to safety systems remain active We believe this represents either an incomplete recall repair or a condition introduced during the recall service process. We are requesting that this matter be reviewed as a failed recall remedy, as the safety issue has not been properly resolved and the vehicle remains in a compromised condition.
