A few weeks ago, while waiting at a traffic light, the brake warning lights (ABS, Emergency brake, Traction control) lights on my 2013 Prius lit up and the brake pedal suddenly becomes much less responsive. I drove carefully back home, and researched online what the problem could be. To my surprise, this seems to be a known problem to Toyota, who had issued a campaign in the past to address this
... problem (ABS actuator, pump) and to replace the known faulty parts for free, but only for owners who were experiencing a failure at that time. Now the recall has expired, and owners like me who weren't having a problem at the time are stuck in a situation with cars that have a known, dangerous fault, while Toyota refuses to help with the problem anymore. The approximate cost for replace/repair is over $4000. All owners should have been included in the previous campaign, whether they were experiencing a failure or not, during that time period since Toyota knew that these parts are faulty.Read more

Dear Sir or Madam, I am writing to file a formal complaint regarding a critical safety failure of my Fisker Ocean One. This incident involved a sudden and total loss of power while driving, which created a severe safety hazard for myself and other motorists. Date of Incident: [Date the incident occurred] Location of Incident: [Name of Highway, City, State, e.g., Interstate 35, Austin, TX] Detai
... led Description of the Incident: On [Date of Incident], while I was operating my Fisker Ocean One on the highway, the vehicle experienced a sudden and complete loss of motive power without any prior warning. The vehicle decelerated and came to a stop in a live traffic lane, rendering it a dangerous obstruction. Immediately following the power loss, I made multiple attempts to restart the vehicle and engage "Drive." The vehicle's systems were unresponsive. Despite repeated efforts, the gear selector would not shift into drive, and the vehicle remained immobile. Shortly after these failed attempts, the vehicle's entire electrical system powered off, leaving the car completely disabled on the highway. This catastrophic failure created an extremely dangerous situation, leaving me stranded in a vulnerable position and at high risk of being struck by other vehicles. The inability to move the car, even to the shoulder, constitutes a grave design or manufacturing defect. Given that the manufacturer, Fisker Inc., has declared bankruptcy, I am not filing this complaint with the expectation of a warranty repair or corporate response. Instead, I am submitting this report to formally document this dangerous defect for public safety records and to urge your agency to investigate this issue. The sudden and complete failure of the powertrain and electrical systems in the Fisker Ocean presents a significant risk to all owners and the publicRead more

There is a recall on this motorcycle's front brake switch. Supposedly it was repaired before it was delivered to me in 2021. I am reporting a critical safety defect involving my brand-new 2025 Hyundai Santa Fe. While driving on the freeway, the vehicle experienced a complete brake system failure, nearly causing a serious accident. My boyfriend was driving at the time, and when he tried to brake, the vehicle did not stop. He had to take immediate evasive action to avoid crashing, which put both of us — and nearby drive
... rs — in immediate danger. The vehicle was purchased about a month ago and had never been serviced or modified. After the failure, it was towed to Hanlees Auto Mall Hyundai in Richmond, California, where the service department inspected it and confirmed the problem was caused by a missing clamp, which is a critical component of the braking system. This appears to be a manufacturing or dealership oversight. They took photos, documented the issue, and made the repair — but they also told me they cannot guarantee this won’t happen again. There were no warning lights, messages, or symptoms before the failure — the brakes simply stopped working during operation on the freeway. As of now: •The issue has been confirmed by Hanlees Hyundai in Richmond. •I have not yet received a loaner vehicle. •The car is currently in my possession, but I do not feel safe driving it. •I have been in contact with Supervisor Steve at Hyundai of Vacaville, who has offered to pick up the vehicle and provide a loaner, but that has not yet occurred. This is an extremely dangerous failure that occurred on a brand-new vehicle. I am requesting a formal investigation into this issue to determine if it’s isolated or part of a broader manufacturing defect with the 2025 Hyundai Santa Fe. A missing brake component that results in total failure at freeway speeds is a life-threatening safety hazard and should be taken seriously by the manufacturer and federal safety officials.Read more

After researching, I found there was a recall (NHTSA 15V759000) for a nearly identical issue involving the brake light switch and stopper pad. Vehicle: 2021 Toyota Highlander XSE Approximate Mileage: 92,000 Location: Boerne, TX (Texas Hill Country) ⸻ 🔧 Summary of the Problem: My vehicle experienced serious electrical failure after heavy rainfall in the Texas Hill Country, where catastrophic flooding recently occurred. At a red light, the dashboard began flashing, power windows stopped working, and the electronic parking brake wou
... ld not disengage. I was nearly rear-ended with my children in the car because the vehicle temporarily lost responsiveness. ⸻ 💧 Pattern of Issue: This problem only occurs after rain. Once the vehicle dries out, the systems return to normal. Toyota tested the battery and alternator and found no faults. The dealership and Toyota Corporate have been notified, but they have so far refused to approve a Goodwill Repair or perform deeper inspection for water intrusion into the fuse box, firewall harness, or Body Control Module (BCM). ⸻ 🧩 Suspected Cause: I believe there is water intrusion due to a design flaw, likely through the windshield cowl or firewall. Other Highlander XSE owners have reported nearly identical symptoms following rain exposure. Toyota has issued service bulletins in the past about water reaching electrical components in other models. ⸻ 📣 Safety Concern: This is a serious safety issue. A near-crash involving my children occurred. If Toyota does not resolve this issue and it leads to an accident, I will hold them fully accountable. I am reporting this so it can be formally investigated and linked with similar complaints.Read more

On [XXX] at [XXX] I dropped my wife and 2 children off at the Funland Arcade in Deep Creek, Maryland. The parking lot was full from the holiday weekend and I was planning on taking the Lightning to a nearby charger while the family gamed so I double parked perpendicular to the arcade entrance, put the hazards on, and let them out. After the 3 of them got out, I placed the Lightning in reverse to
... back up 5-10 feet so that I could exit the parking lot through the same way I came in. Once I had enough space from reversing so I could make the turn to exit I placed the Lighting in drive. The moment the Lightning switched to drive it accelerated at a very high speed as if I was flooring the gas pedal, which I was not. The lightning was in one-petal mode which I always use unless I’m on a highway. I immediately began pumping the brakes but the truck would not stop, I probably slammed on the brake petal 4-5 times to no avail. I even tried shifting the truck back into park but it would not shift out of drive. I was able to luckily steer the truck into a hillside approximately 150 feet away. The truck ran over numerous small trees and shrubs before crashing to a stop approximately 16 feet up the hill. Shortly after the shock wore off, I slowly reversed the truck down the hill, exited the truck, and informed dozens of onlookers who came out of the arcade to see what the noise in the parking lot was from, that the truck just took off on its own but that I was fine. I called 911 and the police came. Luckily no people or vehicles were between the arcade entrance and the hillside or the outcome would have been significantly worse. This crash was witnessed first hand by wife and 2 children. I have grave safety concerns about this truck now and will not put my family or self back in it until Ford fixes this problem. I am making this complaint in case this information can help prevent future crashes of this vehicle.
